Platinum-etoposide combination chemotherapy is the first-line standard-of-care for pts with advanced, poorly-differentiated, gastroenteropancreatic (GEP) NEC. However, nearly all pts will develop resistance and there is no standard second-line treatment. PRODIGE 41 – BEVANEC (NCT02820857) is an academic, non-comparative, phase 2 trial. Main inclusion criteria were histologically proven, grade 3, locally advanced or metastatic GEP-NEC or NEC of unknown primary, and documented progressive disease during or after first-line therapy. Pts were randomized 1:1 to receive 5 mg/kg Bevacizumab (B) with FOLFIRI, or FOLFIRI alone, every 14 days until progression or unacceptable toxicity. The primary objective was to demonstrate a 6-month overall survival (OS) rate of ≥50% in the experimental arm (efficacy: if at least 26 of 52pts alive at 6 months), power, 85%; one-sided alpha risk, 10%). Other endpoints included progression-free survival (PFS), objective response rate (ORR), response duration, biochemical response and safety. From Sep 2017 to Feb 2022, 133 pts were randomized in 26 centers; 126 pts received at least one course of chemotherapy and were evaluable for the primary endpoint. Median age was 67 years (range 26–85), 66% males, 90% with ECOG PS 0-1. The primary tumor was mainly colorectal (n=38), pancreas (n=33), oesogastric (n=22) and unknown (=23). The primary objective was met (30 pts alive at 6 months ± 5 pts still in follow-up less than 6 months) with Folfiri-B. Median PFS and OS were 3.7 months (95%CI [1.9-5.6]) and 7.0 months (95%CI [4.6-11.5]) with Folfiri-B, and 3.5 months (95%CI [1.9-5.1]) and 8.9 months (95%CI [5.7-10.7]) with Folfiri. Biochemical response, ORR (25.5% vs 18.3%) and response duration were numerically higher in Folfiri-B vs Folfiri. Three patients stopped B because of toxicity; one treatment-related death occurred in Folfiri-B and most frequent grade ≥ 3 AEs were neutropenia (12%), asthenia (10%) and diarrhoea (10%). Folfiri-B reached the primary endpoint in 2nd line NECs pts.
We report the results of the first multicentric randomized open-label non-comparative phase II study assessing PRRT-177Lu-Octreotate antitumor efficacy (OCLU, OCLURANDOM trial: EudraCT N o : 2013-004032-30). Patients (pts) with progressive advanced PaNET according to RECIST1.1 were randomized 1:1 to OCLU (7.4 GBqX4/8w) or sunitinib (SUN) 37.5 mg/d. Primary endpoint was: progression-free survival (PFS) rate at 12 months according to RECIST 1.1 real-time central review. The sample size calculation of the OCLU arm assumed a 25% increase (from 35% to 60%) of the 12-m PFS rate. 40 pts had to be included in a single stage Fleming design with type I error = type II error = 5%. The SUN group served as an internal control to validate the null hypothesis with a 12 months PFS rate of 35%. 84 pts were enrolled. Main characteristics were well balanced. The primary endpoint was met with a 12m-PFS rate at 80.5% in the OCLU arm (IC90%: 67.5–89.9, n = 33 pts without progression at 12 months/41) vs. 41.9% in the SUN arm (IC90%: 29.1–55.5,including 35% the null hypothesis; n = 18/43). Median PFS was 20.7 in the OCLU arm (90CI: 17.2–23.7) vs. 11 months in the SUN arm (90CI: 8.8–12.4). The OCLURANDOM study met its primary endpoint.
La lutathérapie (RIV) a l’AMM depuis 2017 pour le traitement des TNE bien différenciées de grade 1 ou 2, gastro-entéro-pancréatiques, métastatiques, exprimant les récepteurs à la somatostatine. Le temps à progression (TTP) optimal pour sélectionner les patients à traiter par RIV n’est pas précisément défini. Elle permet généralement une stabilisation de la maladie, voire une réponse partielle mais peut être inefficace. L’objectif était de caractériser rétrospectivement les patients qui ont progressé selon RECIST pendant ou dans les 4 mois après la RIV. 28 patients issus de 6 centres ont été traités entre 2015 et 2021 et inclus. Les primitifs étaient grêliques, pancréatiques ou autres chez 50 %, 39 % et 11 % des patients avec un grade 1, 2 ou 3 chez 21 %, 47 % et 25 %. Ki médian à 10 % (2–61). 50 % étaient sécrétantes. La maladie était métastatique au niveau hépatique (97 %), ganglionnaire (64 %), péritonéal (39 %) et osseux (43 %). L’envahissement hépatique moyen était de 37 %. Le TTP moyen avant RIV était de 7,2 mois. Toutes les tumeurs exprimaient les récepteurs à la somatostatine. Elles étaient hypermétaboliques au TEP-FDG chez 4 des 6 patients ayant eu l’examen. La RIV était en 2e, 3e ou ≥ 4e ligne chez 46 %, 32 % et 22 % des patients. La PFS médiane après début de la RIV était de 3 mois (0–8). 60 % des patients étaient décédés 8 mois après la RIV. Les patients avec échec de la RIV présentent un profil évolutif initial rapide pour des TNE bien différenciées. Le TTP doit être précisé pour toute décision thérapeutique.
