Malgré une prise en charge optimisée lors de la grossesse chez les patientes DT1, certaines complications ne sont pas contrôlées comme l’excès de croissance fœtale (EPF) qui est associé à d’autres complications comme l’hypoglycémie néonatale, la détresse respiratoire, le transfert en réanimation. L’objectif était de déterminer l’impact du poids de naissance (PN) sur les complications néonatales et d’identifier des facteurs maternels prédictifs. Étude observationnelle, rétrospective, monocentrique entre 1997 et 2022. Nous avons recueilli les données métaboliques maternelles, obstétricales et pédiatriques. La macrosomie (LGA) est définie par un PN≥90e percentile (AUDIPOG). Les complications néonatales comme l’hypoglycémie néonatale, la détresse respiratoire et les transferts néonataux ont été analysés en catégories de percentiles de naissance. Sept cent quatre-vingt-dix-sept grossesses de patientes DT1 ont été analysées. Le PN moyen était de 3520 grammes avec 52,8 % de LGA. 30,9 % des nouveau-nés ont présenté une hypoglycémie, 8,4 % une détresse respiratoire, et 12,4 % ont été transférés en néonatalogie. Nous avons montré un risque significativement accru d’hypoglycémie néonatale (OR=1,67 ; IC95 %=1,17–2,36), de détresse respiratoire (OR=3,08 ; IC95 %=1,60–5,93) et de transferts néonataux (OR=3,32 ; IC95 %=1,19–5,60) en cas de PN>97e percentile. Nous n’avons pas montré d’augmentation significative du risque de ces complications selon la durée de diabète, la présence d’une rétinopathie ou d’une prééclampsie. L’EPF est associé aux hypoglycémies néonatales et à la détresse respiratoire. Ces données sont intéressantes dans la prise en charge clinique de ces patientes notamment en ce qui concerne les décisions de fin de grossesse et la surveillance qui doit être accrue en raison du risque néonatal.
Contexte Le devenir maternofœtal peut varier selon le profil glycémique de l’HGPO lors du diagnostic de DG. L’objectif était de déterminer l’impact des anomalies glycémiques sur le risque de LGA lors de DG tardif. Méthodes Inclusion de 2173 patientes DG tardif dans 4 groupes : 1. Hyperglycémie à jeun (HAJ) (FM) ; 2. Une glycémie postprandiale (HPP) élevée (1PM) ; 3. Deux valeurs PP élevées (2PM) ; 4. HAJ et une ou deux valeur PP élevées (FM+PM). Évaluation du risque de « Large for Gestational Age » (LGA) en régression logistique après ajustement sur l’âge, l’IMC, traitement, antécédents familiaux de DT2, antécédents de DG, macrosomie et multiparité. Résultats L’IMC était plus élevé dans les groupes FM et FM+PM versus 1PM et 2PM (p<0,001). Les patientes multipares étaient majoritaires dans le groupe FM et FM+PM vs 2PM (p<0,001). Les patientes insulinées étaient majoritaires dans le groupe FM+PM (40,6 %, p<0,001) et FM (26,4 %) vs 1PM (16,1 %) et 2PM (17,7 %) p<0,001, ainsi que les enfants LGA (groupe FM [16,7 %] et FM+PM [17,4 %] que dans 1PM [11,6 %], p=0,01). Après ajustement, le groupe FM+PM présente un risque de LGA 1,46 fois (IC95 % : 1,02–2,09) plus élevée que 1PM. Conclusion Les femmes avec HAJ avaient un IMC plus élevé. En cas d’HAJ isolée ou combinée avec HPP, le risque de LGA est plus important malgré un taux d’insulinothérapie plus élevé, suggérant un risque résiduel de LGA. Ces résultats sont en faveur de phénotypes différents en fonction du profil glycémique.
Chez les femmes porteuses de Streptococcus agalactiae au niveau vaginal qui se disent allergiques aux béta-lactamines, la prévention de l'infection néonatale bactérienne précoce (INBP) repose principalement sur une antibioprophylaxie maternelle par macrolides pendant le travail. Environ un quart des souches étant résistantes aux macrolides, cette antibioprophylaxie risque d'être inadaptée à la souche, avec un risque d'INBP. Posséder un antibiogramme et en tenir compte pourrait permettre de diminuer ce risque. Parmi les femmes porteuses de S. agalactiae se disant allergiques aux bêta-lactamines, notre travail vise à mesurer la proportion d'antibioprophylaxies inadaptées à l'accouchement, malgré la réalisation d'un antibiogramme, et à évaluer le risque induit d'INBP chez les nouveau-nés. Entre 2012 et 2021, étude rétrospective observationnelle auprès des femmes enceintes allergiques aux bêta-lactamines et porteuses de S. agalactiae pour lequel un antibiogramme avait été demandé au moment du prélèvement. Les INBP ont été définies par l'existence d'une CRP > 20 mg/l ou d'un prélèvement central positif ou de signes cliniques évocateurs s'améliorant après 24 heures d'antibiothérapie. Parmi 138 femmes enceintes allergiques aux bêta-lactamines, 30 portaient une souche de S. agalactiae résistante à la clindamycine (21,7%). Huit ont accouché avant qu'une première injection d'antibiotique puisse être administrée (travail ≤ 2 heures). Parmi les 130 femmes éligibles à une antibiothérapie (travail > 2 heures) et malgré l'existence d'un antibiogramme, 17 n'ont pas reçu de traitement antibiotique adapté (13,1%). Finalement, 8 nouveau-nés ont présenté une INBP, dont deux avaient une antibioprophylaxie inadaptée. Le risque d'INBP était plus élevé chez les femmes ayant reçu une antibioprophylaxie inadaptée, mais de manière non significative (OR 2.9 [0,5; 16,2], p = 0,23). En cas d'allergie aux béta-lactamines, tenir compte de l'antibiogramme de S. agalactiae est susceptible de diminuer le risque d'INBP. Aucun lien d'intérêt

MATERIAL AND METHODS:This is a retrospective monocentric study (Lille, France), according to a "before"/"after" design. The inclusion criteria were a singleton pregnancy, with a fetus in cephalic presentation, a term >37 WA, with a cervix having a Bishop score lower than 6 after a first line of maturation by cervical balloon catheter. Two groups were formed: "before" corresponding to the continuation of maturation by vaginal prostaglandins, from March 2019 to November 2019, and "after": corresponding to the continuation of maturation by oral misoprostol, from June 2020 to December 2020. The primary outcome was vaginal delivery rate. RESULTS:One hundred women were included in each group. The rate of vaginal delivery was similar between the 2 groups (76% vs 81%, p=0.39), as were the times between the start of induction and the birth and between the start of induction and the transition to birth room. There was no difference in the indication for caesarean section, with in particular an identical rate of caesarean sections for induction failure (p=0.52). Subgroup analysis in obese women showed a significantly higher rate of vaginal delivery in the "after" group (OR=4.17;95% CI [1.02;17.07]). CONCLUSION:The vaginal delivery rate is similar when using vaginal prostaglandins or oral misoprostol as second line cervical ripening after use of a cervical balloon catheter.

Less than half of the seniors surveyed had received education in maneuvering. Of those who did not practice the maneuver, the majority did not consider its systematic practice to be useful (19/26, 73.1%), and one third considered it potentially harmful (8/26, 30.8%). Theoretical and practical training of the juniors significantly improved their knowledge.There are great differences in the practice of the Couder's maneuver. It deserves to be better known, practiced and evaluated.
"Health capital" is influenced by the environment during critical periods of fetal life or early childhood. This concept is known as "developmental origins of health and disease" (DOHaD). An exposure to toxic substances, such as tobacco smoke, alcohol or endocrine disruptors, to poor life style, to "toxic" stress, or an imbalance intestinal microbiota, in particular through inappropriate perinatal antibiotherapy, are associated with the occurrence of metabolic diseases (obesity, hypertension, type 2 diabetes), development of cancers, inflammatory diseases or allergies later in life, and are key determinants of psychosocial competences. Other factors of the fetal environment have protective eff ects, such as physical activity and diversified food intake. The mechanisms involve epigenetic modifications that may alter genes expression over the long term, even from one generation to the next. This article will describe some potential ways to reduce risks to the unborn child by preserving maternal health and its environment.
Comparer les prostaglandines vaginales au misoprostol oral en deuxième ligne de maturation cervicale après recours à un ballonnet de dilatation cervicale. Il s'agit d'une étude rétrospective monocentrique (Lille, France) de type « avant » / « après ». Les critères d'inclusion étaient une grossesse monofœtale, avec un fœtus en présentation céphalique, à terme (>37 SA), avec un col ayant un score de Bishop inférieur à 6 après une première ligne de maturation par ballonnet de dilatation cervicale. Deux groupes ont été constitués : « avant » correspondant à la poursuite de la maturation par prostaglandines vaginales, de mars 2019 à novembre 2019, et « après » correspondant à la poursuite de la maturation par misoprostol oral, de Juin 2020 à décembre 2020. Le critère de jugement principal était le taux d'accouchement par voie basse. Cent femmes ont été incluses dans chaque groupe. Le taux d'accouchement par voie basse était semblable entre les 2 groupes (76 % vs 81 %, p = 0,39), tout comme les délais entre le début du déclenchement et la naissance et entre le début du déclenchement et le passage en salle de naissance. Il n'y avait pas de différence concernant l'indication de la césarienne, avec notamment un taux identique de césariennes pour échec de déclenchement (p = 0,52). L'analyse en sous-groupes chez les femmes obèses montrait un taux d'accouchement par voie basse significativement plus élevé dans le groupe « après » (OR = 4,17; IC95 % [1,02;17,07]). Le taux d'accouchement par voie basse est similaire lorsqu'on utilise les prostaglandines vaginales ou le misoprostol oral en tant que deuxième ligne de maturation cervicale après utilisation d'un ballonnet de dilatation. Il existe peu de données publiées sur l’association présumée entre le groupe sanguin ABO/le système Rhésus (Rh) et le risque de développer un diabète gestationnel (GDM). Toutes les femmes ayant accouché au Centre hospitalier régional universitaire de Lille (Lille, France), entre le 1er août 2017 et le 28 février 2018, ont bénéficié d’un dépistage du diabète gestationnel, en utilisant la méthode de référence selon les directives nationales françaises. Le risque de diabète gestationnel a été évalué pour chaque groupe sanguin ABO, chaque phénotype Rh et leurs combinaisons, en utilisant des modèles de régression logistique. Au total, 1194 femmes présentaient au moins un facteur de risque de GDM. Le pourcentage de diabète gestationnel varie en fonction du groupe ABO (p = 0,013). Par rapport aux femmes du groupe O, les femmes du groupe AB étaient plus susceptibles de développer un diabète gestationnel (OR = 2,50, IC 95 % [1,43 à 4,36], p = 0,001). Par rapport au groupe O rhésus positif, seul le groupe AB rhésus positif présentait un risque élevé de développer un diabète gestationnel (OR = 3,02, IC à 95 % = 1,69 à 5,39, p < 0,001). Nos résultats ont montré que les femmes du groupe AB rhésus positif ont un risque plus élevé de diabète gestationnel. Dans le but de prévenir le DG, les individus à risque pourraient être identifiés en considérant le phénotype du groupe sanguin ABO comme un facteur de risque unique ou en combinaison avec d’autres facteurs de risque.
OBJECTIVE:Breech presentation at term accounts for around 5% of births. Few studies have evaluated the effectiveness of different induction methods for breech presentations. We aim to compare the mode of delivery after induction by intra cervical dilatation balloon compared to prostaglandin (PGE2) in breech presentation. We also evaluated the risk factors associated with a failure of induction. METHODS:Single-center retrospective study from January 2000 to December 2020 including all women induced from 36 weeks, breech presentation, with a single pregnancy without contraindication to vaginal delivery and with an unfavorable cervix (Bishop<6). The primary endpoint was the mode of delivery. Failure of induction was defined as the need of a cesarean section. RESULTS:One hundred seventy six patients were included, 96 in the balloon group and 80 in the prostaglandin group. The cesarean section rate in the balloon group was not significantly different from the prostaglandin group (34.4% vs. 26.3%, p= 0.24). Fifty percent of the patients in the balloon group required additional maturation with prostaglandins after the balloon was dropped or removed. In our overall population, the factors associated with induction failure were nulliparity (OR= 3.144; CI95%: (1.496-6.661)) and BMI > 30 kg/m2 (OR= 3.15 CI95%: (1,374 - 7,224)). CONCLUSION:Mode of delivery after mechanical methods in breech delivery induction appears similar to prostaglandins. However, it should be noted that in half of the cases, additional maturation with prostaglandin was necessary, calling into question the value of the mechanical methods. Factors associated with cesarean were maternal characteristics (nulliparity and BMI > 30 kg/m2) but not induction method.
Many countries with a high perinatal level have started a policy of vaccination of pregnant women against pertussis. To date, France has not chosen this policy. The objective was to review knowledge on pertussis mortality in infants. Compare the strategies available to protect the infant before his first vaccination, scheduled for two months of age. We proceeded to a litterature analysis, from January 1998 to 2021. Search by the following keywords used ; "Whooping cough, vaccination, pregnancy, strategy, cocooning", on the scientific basis of "Pubmed", as well as French and foreign vaccination recommendations. Currently 90% of whooping cough deaths are concerning infants under six months of age and this mortality represents 2% of mortality in the first year of life. Vaccination at birth is not effective. The cocooning strategy, which consists of vaccinating those around the child, is expensive and difficult to implement. A systematic vaccination policy for pregnant women is effective and reasonably expensive when compared to the cocooning strategy. In England, it was recently accompanied by a 78% reduction in confirmed cases of pertussis in infants under six months of age. In conclusion, compared to cocooning strategy, pertussis vaccination of pregnant women appears more effective and cost-effective, and this with each pregnancy.
OBJECTIVE:To evaluate whether ultrasound assessment of fetal head position and station though head perineum distance (HPD), is more predictive of a difficult operative vaginal delivery (OVD) than digital examination. METHODS:Retrospective, monocentric case control study including all singleton OVD at ≥34 weeks gestation. The principal criteria for a difficult OVD were based on a composite criterion of: an OVD considered "difficult" by the birth attendant, and/or two vacuum device detachments if a vacuum was used, and/or change of instrument, and/or a cesarean delivery for OVD failure. RESULTS:Two hundred eighty-six OVDs were included, among which 65 (22.7%) were difficult. The area under the curve (AUC) for predicting difficult OVD according to fetal position from digital examination or ultrasound was 0.62 (95% CI: 0.54-0.70) and 0.66 (95% CI: 0.58-0.73), respectively. Regarding fetal station, the AUCs of HPD without and with pressure were 0.59 (95% CI: 0.51-0.66) and 0.60 (95% CI: 0.51-0.68), respectively. Factors associated with difficult OVD were posterior and transverse positions (OR: 2.931, 95% CI: 1.640-5.239; p= 0.0003), HPD without pressure (threshold of 37 mm, OR: 2.327, 95% CI: 1.247-4.245; p= 0.0080), and HPD with pressure (threshold of 17 mm, OR: 2.594, 95% CI: 1.230-5.429; p= 0.0114). CONCLUSION:Ultrasound assessment of fetal head position and station before OVD moderately predicts difficult OVD. Ultrasound assessment of posterior or transverse positions and HPD with a threshold of 37 mm (without compression of soft tissue) and 17 mm (with compression) were factors associated with difficult OVD.
Objective. - To recommend the most appropriate biometric charts for the detection of antenatal growth abnormalities and postnatal growth surveillance. Methods. - Elaboration of specific questions and selection of experts by the organizing committee to answer these questions; analysis of the literature by experts and drafting conclusions by assigning a recommendation (strong or weak) and a quality of evidence (high, moderate, low, very low) and for each question; all these recommendations have been subject to multidisciplinary external review (obstetrician gynecologists, pediatricians). The objective for the reviewers was to verify the completeness of the literature review, to verify the levels of evidence established and the consistency and applicability of the resulting recommendations. The overall review of the literature, quality of evidence and recommendations were revised to take into consideration comments from external reviewers. Results. - Antenatally, it is recommended to use all WHO fetal growth charts for EFW and common ultrasound biometric measurements (strong recommendation; low quality of evidence). Indeed, in comparison with other prescriptive curves and descriptive curves, the WHO prescriptive charts show better performance for the screening of SGA (Small for Gestational Age) and LGA (Large for Gestational Age) with adequate proportions of fetuses screened at extreme percentiles in the French population. It also has the advantages of having EFW charts by sex and biometric parameters obtained from the same perspective cohort of women screened by qualified sonographers who measured the biometric parameters according to international standards. Postnatally, it is recommended to use the updated Fenton charts for the assessment of birth measurements and for growth monitoring in preterm infants (strong recommendation; moderate quality of evidence) and for the assessment of birth measurements in term newborn (expert opinion). Conclusion. - It is recommended to use WHO fetal growth charts for antenatal growth monitoring and Fenton charts for the newborn. (C) 2022 CNGOF. Published par Elsevier Masson. All rights reserved.
Les cardiopathies congénitales (CC) concernent 0,9 % des naissances en France. Notre objectif était d’explorer la base de données nationale PMSI-MCO (Programme français du système d’information médicale en médecine, chirurgie et obstétrique), pour estimer l’incidence des CC chez les descendants de mères avec diabète préexistants. De 2013 à 2019, toutes les naissances vivantes ont été incluses. La présence de CC et de diabète maternel selon la classification internationale des maladies, 10th [révision], a été dénombré. Au total, 6 076 251 couples mère-enfant ont été inclus. Le taux de malformations congénitales était plus élevé dans le groupe DT2 (8,4 %) vs DT1 (8,0 %) avec un taux plus élevé de CC dans le groupe DT1 (3,0 %) vs DT2 (2,7 %). L’incidence des CC était de 8,01/1000 naissances en France métropolitaine : 29,6/1000 et 27,4/1000 respectivement dans les groupes DT1 et DT2 (DT1 vs DT2 < 0,05). Par rapport à la population non diabétique, les DT1 avaient un risque 2,50 fois plus élevé (IC à 95 % 2,30 à 2,21 ; p < 0,01) de présenter un enfant atteint de CC. Ce risque a été estimé à 2,34 fois plus élevé chez les mères DT2 (IC à 95 % 2,10 à 2,61 ; p < 0,01). La césarienne, la macrosomie, le SGA et la prématurité sont d’autres facteurs de risque de CC. Le diabète prégestationnel est un facteur de risque de CC. Le déséquilibre glycémique pourrait être un des mécanismes impliqués, mais également un facteur de risque modifiable afin de restreindre l’incidence des CC.
BACKGROUND:Breech presentation of the first twin occurs in 20% of twin pregnancies. However, the impact of delivery mode on morbimortality in breech fetuses remains controversial in the literature generally, and has been infrequently studied in twin pregnancies specifically. The aim herein was to evaluate neonatal and maternal outcomes according to delivery mode when the first twin was in breech presentation, and to compare these results with those in the current literature. MATERIAL AND METHODS:This was a single-center, retrospective study in Lille, France, from January 2010 to December 2017, including twin pregnancies in which the first twin was in breech presentation and delivery was after 32 weeks of amenorrhea. Two groups were defined: planned vaginal delivery (PVD) and planned cesarean delivery (PCD). The primary outcome was neonatal morbidities, defined as a 5-minute Apgar score < 7, cord pH < 7.10 at birth, sepsis, and acute respiratory distress syndrome. RESULTS:Among the 184 patients included, 116 attempted a vaginal delivery (63%). Morbidity did not differ between PVD and PCD for the first twin (12/116 (10.3%) versus 7/68 (10.3%), respectively, p = 0.99), the second twin (18/116 (15.5%) versus 7/68 (10.3%), respectively, p = 0.31), or either twin (27/116 (23.2%) versus 11/68 (16.2%), respectively, p = 0.25). The rate of postpartum hemorrhage was significantly lower in the PVD group (PVD 36/116 (31%) versus PCD 41/68 (58.8%), p = 0.001). CONCLUSION:PVD is a reasonable option when the first twin is in breech presentation with probably no higher neonatal mortality and morbidity and less risk of maternal severe postpartum hemorrhage.
