Background. A combat gunshot wound is significantly different from a civilian trauma. It is characterized by the prevalence of penetrating injuries, which increases the volume of blood loss at the pre-hospital stage, and the destruction of large masses of muscle tissue (rhabdomyolysis), which leads to acute kidney injury. Moreover, combat trauma occurs in conditions of chronic background stress as a result of severe emotional and physical strain, uncomfortable weather conditions, and deprivation of sleep, drinking and food. So, such a phenomenon as voluntary dehydration is common among soldiers in combat conditions. In wounded, oliguria is often considered a result of acute kidney injury, but it can also be a symptom of severe dehydration. The purpose of our work was to analyze three clinical cases of oliguria caused by dehydration in wounded with combat trauma to better understand the severity of the condition of such victims and to improve medical aid for them. Materials and methods. The article describes three cases of men aged 35, 50 and 44 years with combat gunshot wounds to the extremities, who were admitted to the tertiary care hospital on the second day after the injury with oliguria (0.18–0.19 ml/kg/hr) and high creatinine (333 to 457 μmol/L). Results. All three patients were conscious, breathing spontaneously, had stable hemodynamics, and moderate anemia after pre-hospital blood transfusions. Focused ultrasound study revealed hyperdynamic left ventricle and small inferior vena cava with complete inspiratory collapse, which suggested hypovolemia. Upon further investigation of the medical history, patients admitted not drinking any liquid for one to two days prior to injury. Tissue hydrophilicity test was conducted which showed severe dehydration in all three cases. Infusion volume was calculated using P.I. Shelestiuk nomogram (modified by O.V. Kravets et al.) and amounted to 60 ml/kg of balanced crystalloid solutions. Upon starting rehydration, diuresis was restored within two hours and amounted to 0.7–2.1 ml/kg/h in all three patients. Creatinine levels normalized in 2–4 days. Patients were transferred to another hospital in a moderate condition in 4–5 days. Conclusions. Oliguria is a frequent complication of combat gunshot injury. Although it is most often associated with acute kidney injury from rhabdomyolysis, it should also be considered that in a combat environment, soldiers’ access to water may be limited and the injury may be accompanied by dehydration. In the cases presented, the differential diagnosis of the causes of oliguria in the wounded made it possible to detect signs of severe dehydration, abstain from the inappropriate use of saluretics, quickly compensate for the fluid deficit, and to avoid the development of kidney damage and the need for renal replacement therapy.
ОСОБЛИВОСТІ АНЕСТЕЗІОЛОГІЧНОГО ЗАБЕЗПЕЧЕННЯ
Recently discovered specific markers open up new possibilities for the diagnosis of acute kidney injury (AKI) in burn disease in order to optimize the treatment of such patients. Early diagnosis with the involvement of biomarkers prevents the sudden death of burn patients and allows predicting the course of the pathological condition. There are several characteristics that an “ideal” AKI biomarker should conform to: being non-invasive, locally specific, highly sensitive, being a stable molecule at different temperatures and pH values, having the ability to rapidly increase in response to kidney injury (quantify it), remaining at high levels during the episode and decreasing during the recovery period. There is a difference between the biomarkers that can be freely filtered in the glomerulus, so any increase in their plasma concentration (due to damage to other renal tissues) can lead to a high concentration of indicators in the urine (loss of specificity), and high-molecular-weight markers that are not freely filtered and therefore are more specific when measured in urine. Renal function in burn patients is usually determined by blood and urine tests, as biopsy can cause iatrogenic damage and is not commonly used in this cohort. After the onset of AKI, the level of biomarkers remains elevated for a certain period. None of the described indicators is monospecific for AKI; this makes estimating the time of AKI quite difficult. It has been proven that the combination of three biomarkers at two different time points in adults and the combination of two indicators at two time intervals in children allows to increase the reliability of determining AKI up to 0.78
Досліджено динаміку емпатійних здібностей лікарів-інтернів за спеціальністю "анестезіологія" в залежності від періодів локального військового конфлікту, пандемії COVID-19 та повномасштабної війни в Україні. Для проведення таких досліджень найбільш валідним та доступним є опитувальник міжособистісної реактивності М. Девіса (IRI). Було проаналізовано 240 анонімних анкет лікарів-інтернів за фахом "анестезіологія". Вони показали більш високі емпатійні можливості, ніж молоді колеги інших спеціальностей. Найзначніше підвищення показників спостерігалося за шкалою "емпатичний дистрес", що є психологічно-захисною реакцією. Найбільше зниження емпатійних характеристик було задокументовано за шкалою "фантазійне співпереживання", що поясняється більш реальним сприйняттям дійсності під час соціальних катастроф. В умовах пандемії та воєнного стану інтерни-анестезіологи показали прогресивне підвищення когнітивно-продуктивних емпатійних здібностей супроти зниження емоційної реактивності (афективності).
Pain remains an important problem after radical prostatectomy, leading to discomfort and sometimes prolonged hospital stays. Despite the fact that laparoscopic procedures are less invasive surgical interventions, they can still be challenging in terms of postoperative pain, as both somatic and visceral pain pathways are involved. To alleviate pain and optimize improved recovery after laparoscopic prostatectomy, regional anesthesia techniques have been used to avoid or reduce the need for opioids. The aim of our study was to investigate the postoperative recovery of patients after laparoscopic robotic-assisted radical prostatectomy, depending on the method of postoperative analgesia and in the context of the peculiarities of anesthesia in robotic surgery. To achieve this goal, the “Medical Plaza” Medical Center examined 49 patients who underwent radical prostatectomy using a robotic system. Patients were divided into 2 groups. Group 1 (n=25) – combined intraoperative anesthesia with epidural analgesia with 0.125% bupivacaine. Patients in group 2 (n=24) underwent TAP-block with 15 ml of 0.25% bupivacaine immediately after the last suture was placed on the skin both sides of the abdomen. Patients in the groups did not differ in age, height, body weight and physiological status (р>0.05) according to the American Society of Anesthesiologists (ASA) scale. When analyzing the statistical data it was found that the volume of the prostate did not differ in the study groups, as well as the duration of the operation and the amount of blood loss (р>0.05). Blood pressure and heart rate fluctuations were similar between the groups. The amount of muscle relaxants used had no statistical difference in the study groups (р>0.05). The amount of opiates used during anesthesia did not differ (р>0.05). Mobilization of patients in both groups occurred in 8 hours without statistical difference (p=0.094). A direct medium strength significant correlation was found between the method of analgesia and weakness in one of the lower limbs at the time of mobilization (r=0.69; p=0.039), a direct medium strength significant correlation was found between the size of the prostate and the time to tracheal extubation (r=0.39; p=0.041). So, the level of intraoperative blood loss did not depend on the variants of perioperative analgesia. Haemodynamics and heart rate did not decrease with the addition of intraoperative epidural analgesia. Intraoperative initiation of epidural analgesia with a low concentration of local anesthetic does not affect the rate of postoperative mobilization. Pain after robotic radical prostatectomy is moderate, but requires multimodal treatment for faster mobilization of the patient, adaptation to the existing urinary catheter. Both epidural analgesia and TAP block have shown sufficient safety profile and efficacy in postoperative pain management. After radical prostatectomy, the TAP-block is an effective method of analgesia, while not interfering with the timely full mobilization of the patient. Epidural analgesia has a high analgesic profile, but is associated with certain risks of catheter migration and interference with full mobilization of the patient.
Nowadays, rewarming of the affected tissues is the primary method of treatment for patients with cold injuries. But the warming manipulation has its own characteristics and limitations, depending on specific circumstances. Untimely and incorrectly performed rewarming can lead to a significant increase in the level of dangerous complications, mortality, and disability. The rewarming strategy is implemented according to one of the two scenarios. If there is a risk of freezing again, the injured area is not actively rewarmed; it is just immobilized, and thermo‑insulating bandages are applied. Slow warming with body heat is also acceptable. If the frozen area can be warmed and kept warm without refreezing until the evacuation is completed, a quick warming with warm water or special heating blankets is preferable. Recommendations on the ideal water temperature significantly differ among authors and include a wide range between 37 °C and 43 °C. The extent of damage to the tissues becomes obvious only after thawing. The traditional classification system of local cold injuries distinguishes four degrees of frostbite. First‑degree frostbite presents with superficial damage to the skin; second‑degree frostbite involves deep skin damage; third‑degree frostbite results in full‑thickness skin damage, including the subcutaneous and surrounding tissues; and fourth‑degree frostbite causes deep necrosis of the subcutaneous structures. Depending on the extent of damage, patients may experience constant and severe pain during rewarming, so analgesics should be prescribed to relieve it. It is recommended to use topical agents (creams, gels, and ointments) to improve circulation and prevent and treat infection. Tissue necrosis with severe frostbite requires surgical treatment of wounds. The authors hope that the provided information will be useful to doctors‑of‑first‑ contact and in hospital conditions in order to optimize the treatment of local cold injuries.
Acute kidney injury (AKI) is a common complication in critically ill burn patients and is associated with serious adverse outcomes, including increased length of hospital stay, development of chronic kidney disease, and increased risk of mortality. The incidence of AKI among burn patients in the intensive care units is 38 (30–46) %. A high percentage of the total burn surface area and a number of individual predisposing factors are considered to be the leading risk factors for AKI. Pathophysiological and morphological changes in the body under the combination of burn disease and kidney damage have certain discrepancies with the classical course of the pathological process in some nosological forms. Despite significant progress in the technologies of fluid resuscitation, intensive care and renal replacement therapy in recent years, the morbidity and mortality rate in such patients remain quite significant. A better understanding of clinical characteristics, early detection and prevention of risk factors for kidney damage in burns, as well as timely medical intervention can effectively reduce morbidity and progression of the pathological process, and also optimize the prognosis in the long run.

Capnometry/capnography is a method of measuring and displaying the concentration of carbon dioxide (CO2) in respiratory gases. Most often, this term involves measuring the partial pressure of carbon dioxide (PCO2) at the end of exhalation (end-tidal CO2, ETCO2). The curve formed during capnography is called a capnogram, where the inspiratory and expiratory segments are distinguished. The main determinants of ETCO2 are CO2 production, cardiac output, pulmonary perfusion, and alveolar ventilation. Normally, ETCO2 is 2–5 mm Hg lower than arterial blood PCO2. This gradient increases when the ventilation-perfusion ratio is impaired, for example, in pulmonary embolism or pulmonary hypoperfusion during cardiac arrest. The shape of the capnogram, in turn, depends on the concentration of CO2 in the alveoli and the pattern of their emptying, as well as on cardiac output. This allows the use of capnography to register bronchospasm, CO2 circulation in the circuit, spontaneous breathing attempts, etc. In Ukraine, capnometry and capnography are routinely used in the anesthesia during laparoscopic surgeries, which require strict control of ETCO2 due to the insufflation of CO2 into the abdominal cavity and its absorption by the peritoneum. However, the spectrum of capnometry application is actually much wider than just laparoscopic surgery. In the intensive care unit, it includes but is not limited to the following indications: confirmation of the endotracheal tube position during tracheal intubation; monitoring the respiratory circuit integrity, including while turning and transporting a patient; evaluation of the cardiopulmonary resuscitation effectiveness; evaluation of the mask ventilation effectiveness; breathing monitoring during procedural sedation; monitoring during mechanical ventilation; auxiliary monitoring of hemodynamics. This literature review discusses the advantages, disadvantages and methods of using capnometry/capnography for each of the listed indications in detail.
Currently, the problem of chronic alcoholism affects all segments of the population in the whole world. Providing dental care to patients suffering from this pathology is a complex task that requires a comprehensive approach. Alcohol and its metabolites have a negative effect on the condition of the oral cavity. The incidence of leukoplakia, erythroplakia, glossitis, caries, periodontitis and carcinoma of the oral cavity is several times higher in drinkers compared to non-drinkers. This is associated with several factors. Most alcoholic beverages have chemical properties which can be damaging to the tooth enamel and mucous membranes. Vomiting can create a destructive acidic environment in the mouth. Metabolites of ethyl alcohol, like acetaldehyde, can also damage the mucous membranes of the oral cavity and affect its microbiome. Acetaldehyde is also a known carcinogen, and it contributes to the development of oral cancer. Acute alcohol intoxication provokes systemic inflammation response, while the immune system is compromised and cannot react adequately to the infection. Moreover, alcoholism impacts personality, which leads to the changes in eating habits and deterioration of personal hygiene – both are the factors which eventually affect oral health. Somatic diseases often occur in alcohol-dependent persons, and can also manifest in lesions of the oral cavity. Functional and morphological disorders of various organs, systems and metabolism in patients create problems not only in the treatment of dental pathology, but also in anesthesia. Alcohol-induced personality changes hinder the development of positive and healthy doctor-patient relationships. Disorders of the cardiovascular and nervous systems need adjustment of the dosage and selection of drugs. Liver dysfunction may lead to hypocoagulation and facilitate serious peri- and postoperative bleeding and hemorrhage. All these changes require appropriate correction in the perioperative period: such patients need careful monitoring and management of delirium, potential withdrawal syndrome, cardiovascular dysfunction and coagulation disorders. Both acute alcohol intoxication and chronic alcoholism significantly alter the metabolism of inhaled and intravenous anesthetics, opioids, and other drugs used for anesthesia. Doses of anesthesia drugs should be reduced in case of acute alcohol intoxication and adjusted during elective procedures, taking into account the fact that in chronic alcoholism, a cross-tolerance forms between alcohol and most of the drugs used for anesthesia. Acetaminophen dose should be adjusted, considering impaired lived function. Tolerance to hypoxia is usually decreased. Also, such patients are at high risk for regurgitation and aspiration, and have an increased acidity of the gastric contents. Regional anesthesia and analgesia in such patients also have their own features, because such patients tend to have a combination of decreased sensitivity to local anesthetic with a compromised cardiovascular function and increased rate of side effects, which makes choosing a correct dose a challenge. Pre-medication plays a significant role in reducing anxiety and intoxication symptoms. Recovery period may be complicated by alcohol withdrawal syndrome. Healthcare professionals’ awareness of the pathophysiology of chronic alcoholism, timely treatment and promotion of patients' abstinence from alcohol consumption can reduce the number of complications and mortality in dental patients.
Фактори ризику гіпотермії включають умови, за яких знижується термогенез, порушується терморегуляція та зростають втрати тепла. До основних груп факторів ризику гіпотермії відносяться: низька температура зовнішнього середовища; дія холоду, що періодично повторюється; зниження теплопродукції; порушення терморегуляції при тяжких травмах і захворюваннях; підвищення тепловіддачі внаслідок антропологічних особливостей (вік, раса, стать, конституція), патологічних станів (шкірних захворювань, вазодилатації, захитування та інших соматичних та неврологічних захворювань) або умов зовнішнього середовища (високої теплопровідності, вологості повітря, швидкості вітру). Даний літературний перегляд можливих причин загального переохолодження та факторів, що сприяють йому, може стати в нагоді при плануванні співробітниками соціальних та спеціалізованих служб заходів із запобігання ненавмисній гіпотермії у робітників різних галузей господарства, військовослужбовців та соціально незахищених верств населення, а також при ретроспективному аналізі причин холодової травми співробітниками рятівних служб, лікарями першого контакту, комбустіологами та персоналом відділень інтенсивної терапії.
У роботі наведені результати аналізу 53 історій хвороб постраждалих із політравмою мирного та воєнного часу, яким проводили поповнення крововтрати за ліберальною стратегією, та обстеження 16 постраждалих, яким застосовували рестриктивну стратегію поповнення крововтрати. Обмеження інфузійно-трансфузійної терапії до 70 мл/кг не знизило її протишокового ефекту і в той же час дозволило зменшити позитивний водний баланс, скоротити тривалість вазопресорної підтримки та штучної вентиляції легень, зменшити кількість пневмоній та інших ускладнень, уникнути різких коливань гемоглобіну, гематокриту та загального білка в 1-шу добу, знизити кількість повторних трансфузій, зменшити летальність.
Співробітники кафедри анестезіології, інтенсивної терапії та медицини невідкладних станів ФПО ДДМУ протягом 15 років займаються підготовкою лікарів-інтернів різних спеціальностей до ЛІІ «Крок-3», що являє собою складний динамічний процес, який вимагає ретельного дослідження та корекції. Окремо дистанційна форма навчання може використовуватися як тимчасовий захід за умов неможливості проведення очного навчання з наступним переходом до традиційної форми. Методологія навчання лікарів-інтернів різних спеціальностей за модулем «Невідкладні стани» повинна бути гнучкою залежно від динамічних змін соціально-психологічного статусу молодих колег.
Introduction. Rigor mortis is the tension of skeletal and smooth muscles that occurs after death and fixes the body in a certain position. It is one of the signs of biological death and usually develops in 1.5 - 3 hours, starting from the lower jaw. The full development of rigor mortis is observed in 12 - 24 hours after death. But in rare cases, its development can be significantly accelerated. Clinical case. A 34-year-old man was in the department of anesthesiology and intensive care for 4 days with a diagnosis of explosive trauma, neck injury with damage to the right carotid artery, hemispheric ischemic stroke. In the setting of multiple organ failure, moderate coma, central hyperthermia (40.3 °C), resistant to antipyretics, he went into a cardiac arrest. Resuscitation measures (including triple defibrillation) for 50 minutes were without effect. During the ascertainment of biological death, the presence of rigor mortis was noted in the lower jaw, neck and extremities, which was absent during the first 30 minutes of resuscitation and at the beginning of the asystolic rhythm. Discussion. The occurrence of rigor mortis is explained by the release of calcium ions from myocytes and the depletion of muscle adenosine triphosphate, which leads to the formation of a stable bond between actin and myosin. Instant or accelerated rigor mortis is rare. According to the literature, high body temperature, strenuous exercise before death, electric shock, convulsions and muscular dystrophy contribute to the acceleration of rigor mortis. Conclusions. In our case, the patient had severe hyperthermia (40.3 ° C), and defibrillation was performed during resuscitation, ie the body was exposed to electric current. Probably, these factors caused the accelerated development of rigor mortis.
Актуальність. Травматичні ушкодження нерідко призводять до рабдоміолізу — руйнування м’язової тканини, що призводить до витоку внутрішньоклітинного вмісту міоцитів у кровоток. Окрім краш-синдрому, причинами травматичного рабдоміолізу можуть бути пряме пошкодження м’язів при масивних травмах кісток та м’яких тканин, м’язова гіпоксія на тлі краш-синдрому, синдрому системної запальної відповіді або гіповолемічного шоку, а також синдром позиційного стискання при тривалих операціях, агресивна та тривала ретракція м’яких тканин під час операцій, множинні та складні оперативні втручання та тривале використання турнікетів. Найбільш значущими біохімічними маркерами рабдоміолізу є креатинкіназа та міоглобін. Метою нашого дослідження було провести аналіз динаміки біохімічних маркерів рабдоміолізу у постраждалих із мирною та бойовою політравмою. Матеріали та методи. Обстежений 121 постраждалий із політравмою. Досліджувалися потреба в стимуляції діурезу та замісній нирковій терапії, рівні сечовини та креатиніну крові, рівні загальної креатинкінази (КК), КК-МВ та міоглобіну сироватки крові. Результати. Рівень загальної КК понад 5000 Од/л при надходженні корелював із летальністю. У постраждалих, які вижили, рівень загальної КК починав знижуватися через 24 години після травми та на 14-ту добу перевищував норму усього у 2 рази. У постраждалих, які померли, рівні КК залишалися на вихідному рівні або підвищувалися. Рівні КК-МВ не показали вірогідного зв’язку з летальністю, але у постраждалих із тяжкими бойовими травмами кінцівок, перенесеною масивною трансфузією та потребою в замісній нирковій терапії спостерігалося тривале та стійке підвищення цього показника. Вихідний рівень міоглобіну не мав кореляції з летальністю в подальшому, але в пацієнтів, які вижили, рівні сироваткового міоглобіну швидко знижувалися, тоді як у померлих залишалися на сталому рівні або підвищувалися повторно після деякого зниження. Рівні маркерів рабдоміолізу мали чіткий зв’язок із показниками функції нирок та потребою в стимуляції діурезу та/або замісній нирковій терапії, причому у КК та КК-МВ цей зв’язок формувався вже через 6 годин після надходження постраждалого до стаціонару, а в міоглобіну — із 3-ї доби. Склад поповнення травматичної крововтрати також впливав на показники маркерів рабдоміолізу. Об’єм введених у першу добу штучних колоїдів, СЗП та еритроцитів був прямо пов’язаний із рівнями КК, КК-МВ та міоглобіну на усіх етапах дослідження. Висновки. Таким чином, політравма з масивною крововтратою супроводжується рабдоміолізом, вираженість якого залежить від тяжкості травми, та є предиктором гострого пошкодження нирок та летальності. Відсутність зниження рівнів КК та міоглобіну на тлі інтенсивної терапії є несприятливим прогностичним критерієм. У постраждалих із тяжкою політравмою рівні КК, КК-МВ та міоглобіну доцільно досліджувати в динаміці.
Електротравма — це фізичне ушкодження організму внаслідок впливу електричного струму, який перевершує за своїми фізичними характеристиками поріг стійкості організму. Найчастіше страждають особи молодого працездатного віку. У поданій клінічній лекції розглянуті основні чинники, фактори ризику електротравми, сучасна класифікація і патофізіологія. Виділені особливості ураження атмосферною електрикою та електричною зброєю. Наведені ускладнення електротравми й принципи медичної допомоги на догоспітальному й госпітальному етапах, а також заходи профілактики ураження електричним струмом.
Актуальність. Дослідження впливу бойового стресу на вираженість порушення когнітивних функцій у ранньому періоді після перенесеної тяжкої травми має істотне практичне значення в плані підвищення якості життя поранених та їх швидшої і повноцінної адаптації у віддаленому посттравматичному періоді. Мета даного дослідження — визначення відмінностей когнітивних дисфункцій у поранених внаслідок бойових дій і постраждалих із травмами, отриманими поза бойової обстановки, у посттравматичному періоді. Матеріали та методи. Використано шкали RTS, ISS і TRISS, шкалу оцінки впливу травматичної події (IES-R). Соматична реакція на травму і пов’язаний із нею стрес оцінювалися за допомогою індексу Кердо і глюкозо-лейкоцитарного індексу. Оцінка когнітивних функцій проводилася з використанням методики FAB і шкали MMSE. Результати. За шкалою IES-R у 1-шу добу після травми було виявлено статистично значуще перевищення загального значення в групі ІІ за рахунок показників субшкали «вторгнення» і особливо субшкали «фізіологічна збудливість». При цьому показники субшкали «уникнення» у поранених були вірогідно нижчими, ніж у постраждалих від небойових травм. Стан когнітивних функцій під час надходження в лікарню в постраждалих із небойовою травмою і поранених із травмою, отриманою під час бойових дій, характеризується насамперед зниженням уваги та короткочасної пам’яті. У міру віддалення від моменту травми вираженість когнітивних порушень поступово знижувалася. Висновки. Тривалий стрес, пов’язаний із бойовими діями, викликає в поранених більш виражені когнітивні порушення, ніж травма, отримана в небойових умовах. Найбільшою мірою з усіх когнітивних функцій у хворих обох груп страждали увага і короткочасна пам’ять, відзначалися також труднощі, пов’язані з вибором. Найбільший ступінь пригнічення когнітивних функцій спостерігався на 7-му добу після травми.
У 2020–2021 роках в умовах пандемії COVID-19 система післядипломної медичної освіти пішла на вимушені заходи щодо збереження життя та здоров’я суб’єктів навчання та викладацького складу у вигляді тимчасового переходу на дистанційну освіту. Під час проходження лікарями-інтернами різних спеціальностей модуля «Невідкладні стани» було проведено анонімне тестування 342 молодих колег із подальшими певними висновками. Дистанційний андрагогічний процес є прийнятним і корисним лише як складова дуальної форми вищої медичної освіти. Віддалена форма навчання у великих аудиторіях має багато недоліків, тому заняття доцільно вести у малих групах. Такий вид навчання потребує ретельного вивчення, оптимізації й адаптації до сучасних умов.
Утоплення — це процес грубого порушення дихання в результаті повного або часткового занурення в рідке середовище. Розрізняють 5 видів утоплення: справжнє (аспіраційне), асфіктичне, синкопальне, гіпервентиляційне та раптова смерть на воді. У клінічній лекції детально розглядаються патофізіологічні особливості кожного виду утоплення, тактика лікування при кожному із них, заходи порятунку потопаючих та заходи профілактики утоплення.
