In a classic tree of the model CART, knots used a unique attribute. Moreover, decision trees are hierarchical method, the pathway between the root and leaves are one-way and the classification of the observations in a leaf is definitive. We propose a new concept of decision tree, including multivariate knots and non-hierarchical pathway. A risk score, including all factors, is calculated for each knot, using a combinatorial approach. Terminal leaves are exclusively constituted by negative observations ("not at risk") and by the last positive leaf ("at risk") of the tree. An algorithm based on 5 criterions, selects one combination among those maximize the sum of sensitivity + specificity: identical observations, the minimization of the sum of the absolute values of coefficients, the maximization of the AUC, the minimization of the sum of the absolute values of coefficients, and random selection. The data of the sinking of Titanica (n = 1316) were used to compare the performance and the internal validation (bootstrap, n = 1000) obtained with this new tree and the logistic regression. The sensibilities estimated were equal to 84.75% for the logistic model versus 84.76% for the new tree. The specificities estimated were equal to 67.72% for the logistic model versus 67.69% for the new tree. The new tree model was slightly better (21.62% versus 21.69%) than the logistic regression for the criteria of proportion of misclassifications estimated. We described a new method of regression-classification including several methodological innovations in methods of classification trees. Applications of this new tree were already published. The research is continuing on the development of random forest built with these trees. The first results are very promising. The first version of the program will be available as a package for R and could be requested.
En astronomie, on peut identifier le nombre de planètes et leurs orbites autour d’un soleil par l’observation de l’irrégularité de la lumière perçue venant de l’astre. En appliquant ce raisonnement aux données biomédicales, nous pouvons identifier des facteurs d’intérêt, inconnus et absents de nos bases de données. À travers trois exemples, nous proposons de décrire la stratégie et la méthode d’identification des trous de données, appelés « Facteurs Fantômes » (FF). ROP (Régression Optimisée) est une classification-régression dans laquelle chaque nœud est géré un score de risque incluant l’ensemble des prédicateurs. L’analyse de la variation des coefficients des prédicateurs des scores de risque permet d’identifier des observations pour lesquelles un prédicateur change complètement le sens de son effet. Nous faisons l’hypothèse que ces observations ont une information différente des autres observations, que nous appelons FF, qui expliquerait le changement radical du sens du prédicateur. Identifier ce FF revient à modéliser des trous d’informations. L’intérêt statistique de ce FF est vérifié par un modèle multivarié classique en l’ajustant sur l’ensemble des variables. L’identification de la nature de ce FF est réalisée avec des données indépendantes et non impliquées dans sa construction. Il n’était pas possible de démontrer de manière expérimentale que les délais opératoires pour le curage ganglionnaire des mélanomes augmentaient le risque de récidive. Nous avons modélisé la prise de décision des chirurgiens par un FF et le modèle de Cox ajusté sur ce FF a pu démontrer l’augmentation du risque de récidive avec l’augmentation des délais opératoires. L’effraction capsulaire ganglionnaire d’un mélanome augmente le risque de sa récidive. Cependant cette effraction permet aussi de présenter les antigènes tumoraux au système immunitaire. Nous avons démontré que l’effet paradoxal « protecteur » de l’effraction capsulaire serait positivement lié à l’antigène de Melan-A et négativement à anti-PDL1 exprimé par les cellules mélanique et qui inhibe l’activité des lymphocytes T spécifiques de la tumeur. Nous avons ôté de la base analysée un facteur de pronostic significatif. Nous avons ensuite démontré que le FF que nous avons créé était très corrélé à la variable initialement ôtée. Dans le modèle ROP, le changement de signe de régression d’un prédicateur dans un cluster permet de déduire la présence d’un effet spécifique et indépendant des autres prédicateurs analysés, puisque tous les prédicateurs étaient déjà inclus. Le modèle ROP ouvre de nouvelles perspectives pour identifier de nouvelles pistes.
Nous définissons une donnée fantôme comme étant une variable non colligée et dont la nature et l’identité sont inconnues. Il n’existe pas de méthode permettant d’imputer une information totalement inconnue. Notre hypothèse est que dans un système biologique, toutes les variables sont reliées entre-elles, mais cette relation n’est pas forcément linéaire. Notre objectif est de démontrer qu’un modèle non linéaire comme le modèle régression optimisée (ROP) est capable de reconstruire une information inconnue à partir des autres variables existantes, sans utiliser des variables de confusion. Comme il semble impossible de trouver une information qui n’existe pas, la démonstration nécessite de retirer une variable pertinente pour simuler cette donnée fantôme, de modéliser ce trou d’information par le modèle ROP, d’évaluer la pertinence de cette nouvelle information par la régression logistique. Les données publiques ICU.dat seront utilisées. La variable « TYP » sera supprimée et simulera la donnée fantôme qui sera modélisée par le modèle ROP. Le modèle de régression logistique incluant les variables PO2 et « TYP » ainsi que leur interaction montre un effet significatif de la variable PO2 (p = 0,0189), alors que le modèle excluant la variable « TYP » confirme que la variable PO2 n’est plus significative (p = 0,168). Le modèle ROP identifie un cluster de 117 patients pour lesquels le coefficient de régression du modèle ROP pour la variable PO2 est négative, alors que l’effet moyen de cette variable augment le risque de l’état Y. Ces 117 patients sont donc porteurs d’une information expliquant la variabilité de l’effet de la variable PO2 et donc en lien avec la variable « TYP » absente des données analysées. Une nouvelle information dichotomique est crée pour traduire l’information portée par le cluster des 117 patients et introduite dans le modèle de régression logistique qui montre un effet significatif de la variable « PO2 » (p = 0,020). De plus, il existe une relation significative entre la nouvelle information créée par ROP et la variable « TYP » initialement ôtée des données analysées (p = 0,003). La nouvelle information crée par le modèle ROP a permis de retrouver l’effet significatif de la variable PO2. Cette information est corrélée à la variable ôtée. Tout se passe comme si ROP était capable de créer une nouvelle information pertinente en lien avec un facteur inconnu et non colligé.
Pour les mélanomes de stade IIIB, l'effraction capsulaire est un facteur de pronostic péjoratif. Cependant, dans certaines situations, l'effet de ce facteur peut apparaître paradoxal, en diminuant le risque de récidive. Le modèle ROP est une classification–régression non paramétrique et linéaire par morceaux (J.-M. Nguyen, A. Gaultier, D. Antonioli, J Health Edu Res Dev. 2015 ;3(141) :2). Il permet d'identifier directement les observations à l'origine de cette variabilité. Notre hypothèse est que cette variabilité serait associée à des facteurs immunologiques. L'objectif de ce travail consistait à identifier les patients porteurs de la source de cette variabilité et d'étudier la corrélation de cette variabilité avec des facteurs immunologiques. Quarante-quatre patients atteints de mélanome de stade IIIB dont le curage ganglionnaire a été réalisé entre le 01/01/2013 et le 31/01/2016 ont été inclus. Des mesures semi-quantitatives par QPCR de 15 données immunologiques (CD4,CD8,Foxp3,CD272 [BTLA],CD152 [CTLA-4], PD-1, PD-L1, PD-L2, IDO-1, Melan-A, gp100, NYESO-1, Tyrosinase, CD80, CD86) ont été réalisées. Un modèle ROP a été développé pour expliquer la récidive 6 mois après le curage ganglionnaire et pour identifier les patients porteurs de cette source de variabilité. La régression logistique (RL) a été utilisée pour identifier les facteurs immunologiques associés à la variabilité de l'effet de l'effraction capsulaire. ROP a identifié 17 patients chez lesquels la diminution du risque de récidive à 6 mois était paradoxalement liée à une effraction capsulaire (Fig. 1A). ROP a aussi identifié 4 patients chez lesquels il n'existait aucune corrélation entre la diminution du risque de récidive en cas d'effraction capsulaire et 11 des 15 variables immunologiques étudiées (Fig. 1B). La RL réalisée sur les 36 observations restantes, montre que la diminution du risque de récidive est corrélée positivement à l'expression de Melan-A avec un OR = 1,86 [1,14–3,34] (p = 0,021) et négativement à l'expression de PD-L1 avec un OR = 0,41 [0,14–0,80] (p = 0,021). L'AUC-ROC de ce modèle logistique est de 77,65 %. Selon le modèle ROP, le groupe des 4 observations est corrélé positivement à l'expression de GP100 et de PD-L1 et négativement à l'expression de CD272 et de PD-L2. La RL ne permet pas d'établir des corrélations significatives à partir de l'ensemble des patients, du fait d'un mélange de plusieurs populations. ROP a permis d'identifier des populations concernées pour lesquelles la RL peut être efficace pour démontrer des relations significatives. Ces résultats suggèrent que l'expression de Melan-A et de PDL-1 sont associées à la diminution du risque de récidive en cas d'effraction capsulaire. Le faible nombre d'observations concernant l'hypothèse d'expression de gp100 ne permet pas de la valider statistiquement.
On désigne par syndrome de Diogène un comportement acquis du sujet âgé, marqué par une extrême négligence de l'hygiène corporelle et de l'habitat, un repli sur soi avec refus de l'aide extérieure, et par une tendance à l'accumulation d'objets hétéroclites au domicile. Ce syndrome représente une entité clinique gériatrique particulière et de description relativement récente, puisque les deux articles princeps, par 2 psychiatres puis 2 gériatres, remontent à 1966 et 1975. Sa rareté contraste avec une notoriété qui doit autant à l'emprunt du nom du célèbre philosophe qu'à son caractère hors du commun. Il suscite toujours une fascination pour le clinicien confronté à ces patients vivant dans des conditions aberrantes, en marge de la société et en dehors du temps, n'ayant aucune critique de ce mode de vie et aucune demande d'aide. Ces patients sont découverts le plus souvent de façon fortuite, soit au décours d'une pathologie somatique, soit suite à une intervention sociale secondaire à leur comportement. La prise en charge de ces patients qui ne demandent rien est difficile et soulève des questions éthiques. De plus, l'évolution est grevée d'une mortalité extrêmement importante, estimée à 46 % dans les 5 ans. En dehors des situations mettant en jeu le pronostic vital, l'hospitalisation doit être évitée autant que possible au profit de solutions ambulatoires et de mesures sociales. La prescription de traitements psychotropes peut être nécessaire et dépend du tableau clinique et de la pathologie psychiatrique éventuellement retrouvée. Bien que plusieurs hypothèses cliniques aient été émises, la question d'un substrat étiopathogénique à ce trouble reste ouverte. La plupart des auteurs s'accordent pour dire que ce comportement n'est pas le reflet de l'expression du libre arbitre et par conséquent n'aurait aucun rapport avec le philosophe grec, Diogène de Sinope, qui enseignait le cynisme et le retour à la vie naturelle. Les critères diagnostiques développés n'ont pas fait l'objet de consensus, ils reprennent les principaux éléments qui définissent le syndrome et excluent pratiquement tout syndrome psychiatrique. Clark et Mankikar, les auteurs qui ont dénommé ce syndrome, pensent qu'il s'agit soit d'un mode de défense du sujet âgé en réaction à un stress, soit d'une évolution naturelle liée au vieillissement. Cependant, diverses pathologies psychiatriques et neurologiques ont été retrouvées associées au syndrome de Diogène. Dans la littérature, il est ainsi décrit des cas de psychoses paranoïdes, de psychoses paranoïaques, de troubles de l'humeur et de troubles obsessionnels compulsifs. L'addiction à l'alcool semble davantage un facteur aggravant qu'un facteur précipitant. La présence d'un syndrome de Diogène doit également faire rechercher une pathologie démentielle et notamment une démence fronto-temporale. Finalement, le rôle de ces pathologies dans la genèse de ce trouble reste indéterminé : s'agit-il de facteurs déclenchants, comorbides ou étiologiques ? Doit-on parler de ce syndrome en tant que « maladie » ou en tant que « symptôme » ? Nous proposons d'aborder le syndrome de Diogène comme un trouble du comportement et d'en distinguer 2 types : le syndrome de Diogène « actif » et « passif ». Les patients « actifs » entassent dedans ce qu'ils récoltent dehors et remplissent leur intérieur pour combler le vide d'une existence qui se dégrade et qui perd son attrait narcissique. Les patients « passifs » se font envahir passivement par leurs déchets et se laissent déborder par les accumulations qui s'entassent par défaut, par manque. Enfin, dans une démarche de compréhension psychopathologique de ce comportement, nous nous référons aux théories psychanalytiques du Moi-peau décrites par Anzieu. Le Moi-peau représente une structuration de l'appareil psychique fondée sur le principe que « toute fonction psychique se développe par appui sur une fonction corporelle dont elle transpose le fonctionnement sur le plan mental ». La peau recouvre alors 3 fonctions : celle d'enveloppe contenant, celle de barrière protectrice du psychisme et celle de lieu d'échanges. Le Moi-peau est organisé en « double paroi », à la fois barrière (mécanismes de défense psychiques) et filtre (entre le psychisme et le monde extérieur) et préserve ainsi la relation et la cohésion « contenant-contenu ». Lorsqu'une blessure narcissique intervient, le Moi-peau s'altère et perd sa fonction de contenant. Dans le syndrome de Diogène, les objets accumulés viennent « suturer » ce Moi-peau et le logement devient de nouveau « étanche », et par la même occasion s'oppose à toute intervention extérieure. Ce comportement revêt donc une fonction de suture psychique, de colmatage mais aussi et surtout une fonction vitale, comme un « aménagement pour la survie ».Diogenes syndrome is a behavioural disorder of the elderly. Symptoms include living in extreme squalor, a neglected physical state and unhygienic conditions. This is accompanied by a self-imposed isolation, the refusal of external help and a tendency to accumulate heteroclite objects. This particular geriatric syndrome has been described for the first time only quite recently, as the 2 primary descriptions by geriatricians and psychiatrists date from 1966 and 1975 respectively. Its rare occurrence contrasts with the fact that it is well-known, partly due to it being named after the Greek philosopher « Diogène de Sinope », who taught cynicism philosophy and a return to a natural way of life, and partly because of its rare characteristics. The Diogenes syndrome is a fascinating object of study for the clinician who takes care of patients living in uncommon conditions, on the edge of society and unaware of the particularity of their lifestyles. Patients suffering from Diogenes syndrome are usually discovered by chance, either because of a somatic illness, or as a result of social intervention related to their behavioural problems. Management of the syndrome is difficult and ethically challenging, as the patient does not seek help. Moreover, 46 % of patients have a 5 year mortality rate. Hospitalisation has to be avoided whenever possible and ambulatory treatment and social measures should be favoured. Psychotropic treatment prescription may be necessary, depending on clinical features and the possible underlying psychiatric disease. Although several clinical hypotheses have been suggested, the true ethiopathogeny of the syndrome remains unclear. Most authors agree that this behaviour does not reflect free will and has consequently no theoretical relationship to the Greek philosopher. There is no true consensus about diagnostic criteria. They include the main features of the syndrome and exclude known psychiatric syndromes. Clark and Mankikar, who named this syndrome, reckon it may represent stress-related defence mechanisms of the elderly or may be related to natural ageing process. However, psychiatric pathologies as paranoid and paranoiac psychoses, mood disorders and obsessive and compulsive disorders have been described to be associated with it in the literature. Dementia, in particular temporo-frontal dementia, should be looked for and excluded clinically. Alcohol abuse seems to be an aggravating rather than a precipitating factor. Finally, the link between these pathologies and Diogenes syndrome is not yet determined : are they triggering, co-morbid or etiological factors ? Should this syndrome be considered as a true illness or as a symptom ? This paper presents Diogenes syndrome as a behavioural disorder and distinguishes 2 types : the « active type » - patients who collect from outside to clutter inside - and the « passive type » - patients who passively become invaded by their rubbish. Active type patients fill their home to fill in the vacuum of their life, as it deteriorates and looses its narcissical appeal. Passive type patients accumulate by default and emptiness. A psychopathological understanding is presented here, referring to psychoanalytical theories of the Moi-peau (ego-skin) described by Anzieu. The Moi-peau represents a structure of the psyche founded on the following principle : any psychic function develops itself according to a bodily function from which it transposes its functioning at a mental level. The skin has three functions : the containing shell, the protective barrier of the psyche, and a medium of exchange. The Moi-peau is organised as a double-wall acting both as a defence mechanism and as a filter between the psyche and the external world. It preserves the relationship and the cohesion « container-content ». As a result of a narcissical wound, the Moi-peau is damaged and looses its function of a container. In the case of Diogenes Syndrome, the accumulated items repair the Moi-peau and the home becomes an « exterior-proof », thus playing the role of the Moi-peau. This behaviour therefore plays a repairing role for psychic functioning, allowing psychic survival.
Summary Objectives: To propose an objective approach in order to determine the number of beds required for a hospital department by considering how recruitment fluctuates over time. To compare this approach with classical bed capacity planning techniques. Methods: Asimulated data-based evaluation of the impact that the variability in hospital department activity produces upon the performance of methods used for determining the number of beds required. The evaluation criteria included productive efficiency measured bythe bed occupancy rate, accessibility measured by the transfer rate of patients due to lack of available beds and a proxy of clinical effectiveness, by the proportion of days during which there is no possibility forunscheduled admission. Results: When the variability of the number of daily patients increases, the Target Occupancy Rate favors productive efficiency at the expense of accessibility and proxy clinical effectiveness. On the contrary, when the variability of the department activity is marginal, the Target Activity Rate penalizes the proxy of clinical effectiveness, and the Target Occupancy Rate under-optimizes productive efficiency.The method we propose led to a superior performance in terms of accessibility and proxy of clinical effectiveness at the expense of productive efficiency. Such a situation is suitable for intensive care units. In the case of other departments, a weighting procedure should be used to improve productive efficiency. Conclusions: This approach could be considered as the first step of a family of methods for quantitative healthcare planning.
Objective: The number of acute hospital beds is determined by health authorities using methods based on ratios and/or target bed occupancy rates. These methods fait to consider the variability in hospitalization demands over time. On the other hand, the implementation of sophisticated models requires the decision concerning the number of beds to be made by an expert. Our aim is to develop a new method that is as simple to use as the ratio method while minimizing the roundabout approaches of these methods.Method: A score was constructed with three parameters: number of transfers due to Lack of space, number of days with no possibility for S unscheduled admissions and number of days with at least a threshold of U unoccupied beds. The optimal number of beds is the number for which both the mean and the standard deviation of the score reach their minimum. We applied this method to two internal medicine departments and one urological surgery department and we compared the solutions proposed by this method with those put forward by the ratio method.Results: The solutions proposed by this method were intermediate to those calculated by the local and national length of Stays ratio methods. Simulating an unusual increase in admission requests had no consequence on the bed number selected, indicating that the method was robust.Conclusion: Our tool represents a real alternative to the ratio methods. Even though many scales for the assessment of sexual dysfunction have been recently developed, most of them are suitable rather for the research purpose in clinical trials than to routine interviews in a daily, private practice. We report here the first experience with a simple, semi-quantitative scale for parallel assessment of sexual dysfunction and lifesatisfaction (considered to globally reflect the quality of life), which was tested in depressed patients treated in the psychiatric, private practice setting. A combined Sexual Dysfunction(SD-S) and Life-Satisfaction Scale (LS-S), was constructed based on previous interviews with patients. Both consisted of 4-items, assumed to represent core elements of sexual function and individual well-being. The scales were applied to depressed patients treated with any of the SSRIs or with moclobemide, a reversible and selective MAO-A inhibitor. These two treatments were selected for testing the scales because it is known that SSRIs can induce or exacerbate them and moclobemide does not seem to affect them. The selection of treatment modality in this study was, however, entirely at the discretion of the physician. The assessments were done during 3 visits (at baseline, after 2 months and after 4 months). The results of this exploratory trial, testing the applicability, acceptance and utility of a combined mini- SD-S- and LS-S- scale, in 62 depressed patients, showed that the scale: a) was simple to use and well accepted by physicians and patients, b) was a suitable instrument for the practicing physician to control the success of the treatment and c) was sensitively assessing the presence and severity of sexual dysfunction.
Objective: Most methods used to estimate ICU bed needs rely either on simple formulas that do not consider the actual needs of the population or on simulations that are too specific to be applicable to all hospitals. We sought to develop a universally applicable nonparametric method. Design and setting: For each day, the number of immediate patient transfers to other ICUs because of a full unit and the number of patients treated in the ICU were collected. The number of beds needed was selected according to the minimization of both the mean and the variance of three parameters (accessibility, safety, and efficiency). This method was applied to the ICU of a general hospital. Robustness of the model was assessed using outliers. Main results: During the 5-month study period, 215 ICU stays were collected. The method selected a ten-bed model whereas length-of-stay ratio and case-mix methods selected a twelve- and height-bed models respectively. An unusual increase in admission requests had no consequence on the bed number selected, indicating that the method was robust. None of the parameters were dependent on specific ICU characteristics, establishing that this method is applicable to any type of hospital ward. Conclusion: Our model is reliable for determining the number of beds needed in any type of ICU and can be used by all ICU managers. The software is available.
The 17-item Hamilton depression rating scale (HAMD17) is the standard efficacy outcome in antidepressant clinical trials. It is criticized for multidimensionality and poorly discriminating treatment from placebo. HAMD subscales may overcome these limitations and reduce the sample size of clinical trials. This study compared the discriminative performance of the HAMD17 and three established HAMD subscales (Bech, Maier-Philipp, Gibbons) across a range of antidepressants with different mechanisms of action.We analyzed data from 24 clinical trials including 3692 patients randomized to tricyclic or tetracyclic antidepressants (TCAs or TeCAs), selective serotonin reuptake inhibitors (SSRIs) or placebo. Data were analyzed using a mixed model for repeated measurements (MMRM). Standardized effect sizes for the HAMD17 and subscales were derived for every time-point, and their effect on sample size was evaluated.For TCAs and TeCAs vs. placebo, the HAMD17 consistently provided the highest standardized effects. The sample size to establish efficacy at week six was >25 percent smaller than for any of the subscales. However, for SSRIs vs. placebo, the HAMD17 provided slightly smaller standardized effects and was the least efficient outcome. There were no relevant differences between the subscales.Data were derived exclusively from mirtazapine trials. Conclusions are restricted to clinical trial settings.Comparative performance of the HAMD17 and various subscales strongly depends on type of antidepressant. Results support using HAMD17 as primary endpoint in clinical trials, but it will be beneficial to pro-actively include subscales as additional endpoints to successfully establish treatment effects of new antidepressants.
Objective: Periodic endoscopic surveillance is generally recommended for patients with Barrett's esophagus. The optimal follow-up strategy for uncomplicated Barrett's esophagus is controversial, in part because of limited data on the rate of neoplastic progression (through the sequence of metaplasia-dysplasia-carcinoma) during endoscopic surveillance. This study aims to quantify the development of dysplasia in patients with uncomplicated Barrett's esophagus and to explore clinical risk factors associated with the development of dysplastic lesions. Methods: We identified 102 patients with endoscopic evidence of Barrett's esophagus and the presence of specialized columnar epithelium who had received endoscopic surveillance for adenocarcinoma at our medical center between 1970 and 1994. We abstracted endoscopic and histologic data from the medical record. All specimens that showed any degree of atypia (per report) were reexamined in blinded fashion by a team of study pathologists who indicated the grade of dysplasia. Time to first diagnosis of dysplasia was plotted using Kaplan-Meier survival curves, and risk factors for development of dysplasia were assessed using Cox regression. Results: During 563 patient-yr of endoscopic follow-up, three patients developed adenocarcinoma at least 4 yr after initial diagnosis (one developed adenocarcinoma of the cardia, which was incidentally detected during surveillance for Barrett's esophagus). At some point during follow-up, 19 patients developed new onset, low grade dysplasia and four developed high grade dysplasia. None of the patients who had received antireflux surgery developed dysplasia. Conclusion: If confirmed by larger follow-up studies, our results suggest that surveillance endoscopy can be safely deferred for at least 2 yr following an initial biopsy that is negative or indeterminate for dysplasia. Adoption of this approach would substantially reduce the cost of surveillance for adenocarcinoma. Future trials should explore the role of antireflux surgery in protecting against neoplastic transformation of Barrett's esophagus.
PURPOSE The purpose of this study was to determine the utility of spiral CT in the diagnosis of brain death. METHODS Spiral CT was evaluated prospectively in 14 brain-dead patients and in 11 healthy subjects. A two-phase protocol was used. Twenty seconds after intravenous injection of a nonionic iodinized contrast medium, the CT table was drawn through the gantry at a rate of 10 mm/s while scanning was in progress. The second scanning phase was started automatically a mean of 54 seconds later, using the same parameters. Opacification or absence of opacification of carotid, vertebral, and basilar arteries and intracerebral veins was ascertained for each image in both phases. The diagnosis of brain death was confirmed by elecroencephalography (n = 7), angiography (n = 5), or both (n = 2). Statistical analysis with the Fisher exact test enabled us to compare the brain-dead patients with the healthy control subjects. RESULTS In brain death, the pericallosal and terminal arteries of the cortex did not opacify during the two phases of spiral CT, whereas the superficial temporal arteries were always visible. The internal cerebral veins, the great cerebral vein, and the straight sinus did not opacify, whereas the superior ophthalmic veins were visible on both sides 13 times. For each vessel type, specificity was 100% for nonvascular opacification criteria on the right and left sides. CONCLUSION Two-phase spiral CT can demonstrate the absence of intracerebral blood flow in brain death.
PURPOSE:This study was undertaken to assess the electron microscopic and microbiologic findings in tissue biopsy samples from patients with pouchitis and to compare them with findings in patients with normal pouches, conventional ileostomies, and normal ileum.METHODS:Tissue samples were obtained from 78 patients: 23 patients with normal pouches endoscopically and histologically (Group 1), 12 patients with endoscopic and histologic evidence of inflammation (pouchitis) (Group 2), 14 patients who had either endoscopic or histologic evidence of inflammation but not both (Group 3), 20 patients with conventional ileostomies (Group 4), and 9 patients without ileostomies from whom biopsy samples of normal ileum were obtained (Group 5).RESULTS:The mean total aerobic facultative counts in the biopsy samples from the pouchitis patients were significantly higher when compared with biopsy samples from Groups 4 and 5 (P < 0.05). There were no significant differences in the mean anaerobic counts among the five groups. Positive cultures were obtained in 90 percent of patients with pouches compared with 69 percent of patients with conventional ileostomies or normal ileum (P < 0.05). Intramural bacteria were observed on electron microscopy in biopsy specimens of 47 percent patients with pouches compared with 14 percent of patients with conventional ileostomies or normal ileum (P < 0.05). However, the proportion of patients with positive cultures or intramural bacteria was not increased in the pouchitis group compared with the normal pouch group.CONCLUSION:These data suggest that intramural aerobic facultative bacterial counts are elevated in patients with pouchitis and may play a role in the pathogenesis of pouchitis.
