Les cas d'atteintes humaines à corynébactéries étaient jusqu'à récemment exceptionnellement rapportés en Guyane. Depuis 2016, des cas sont signalés régulièrement. L'objectif de cette étude était de décrire cette émergence. Il s'agissait d'une étude épidémiologique, rétrospective, multicentrique. Tous les patients chez qui une atteinte à corynébactérie du complexe diphtheriae a été diagnostiquée entre le 1er janvier 2016 et le 31 décembre 2021 en Guyane ont été inclus et décrits. Les caractéristiques bactériologiques et pheno-génotypiques des souches ont été analysées et décrites. Soixante-quatre atteintes à corynebactéries ont été diagnostiquées, 61 dus à C. diphtheriae, trois à C. ulcerans, correspondant à 60 patients inclus. L'incidence estimée était de 0,7 cas pour 100 000 habitants en 2016, 8,0 en 2021. L'âge moyen était de 29,2 ans (ET = 22,8), le sex-ratio de 0,6 (F / M = 22 / 38). La plupart des patients (80% ; n = 48 / 60) ont été diagnostiqués dans les territoires de l'intérieur de la Guyane. Parmi eux, 10% présentaient un ou plusieurs antécédents d'immunodépression relative (n = 6 / 58, 2 NR), 14% une pathologie cutanée chronique préexistante (n = 8 / 58, 2 NR), 66% des patients avaient un statut vaccinal antidiphtérique à jour (n = 29 / 44, 16 NR). Cinq souches de C. diphtheriae étaient porteur du gène tox, les tests d'Elek étaient négatifs. Parmi les 61 cas de C. diphtheriae, pour 58 il s'agissait de formes cutanées (95%, n = 58 / 61), pour trois autres il s'agissait d'une atteinte pulmonaire, systémique ou muqueuse. Les trois souches à C. ulcerans étaient cutanées. Nous avons observé trois atteintes sévères. L'évolution était bonne dans 58% (n = 37 / 64 cas, 24 NA). Aucun isolat n'était résistant aux molécules de première ligne (amoxicilline et érythromycine). Les principaux gènes de C. diphtheriae résistant étaient sul1 (résistance au sulfamethoxazole) dans 18% des isolats (n = 11 / 61, 1 NA) et 7%, (n = 4 / 61) isolats portaient le gène tetW (résistance à la tétracycline) et le gène cmx (résistance au chloramphénicol). Il y avait 16 lignées cgMLST (< 500 différences cgMLST) qui sont quasiment toutes observées uniquement en Guyane. Les cas d'atteinte à C. diphtheriae non-toxinogène, à prédominance cutanée, dont des formes sévères, sont rapportés en Guyane. La bactérie parait circuler de manière endémique. Alors que la couverture vaccinale est insuffisante, l'émergence de souches toxinogènes pourrait y trouver une niche favorable. Il paraît essentiel de sensibiliser les acteurs de première ligne et d'améliorer les conditions de prise en charge. Aucun lien d'intérêt
La diphtérie à Corynebacterium diphtheriae (C. diphteriae) est une infection bactérienne à déclaration obligatoire (DO) d'expression clinique ORL ou cutanée. A l'été 2022, une augmentation du nombre de cas de diphtérie chez des personnes migrantes était identifiée en France et dans d'autres pays européens. Nous présentons un bilan de la situation épidémiologique et biologique de la diphtérie à C. diphteriae en France métropolitaine. Les données présentées sont issues de la DO, des investigations menées autour des cas ainsi que des analyses microbiologiques et de génotypage, définissant les 'sublineage' (SL) et 'genomic cluster' (GC) par séquençage génomique. En 2022, 35 cas de diphtérie à C. diphteriae tox-positifs ont été rapportés en France métropolitaine contre 3,4 cas en moyenne annuelle sur les 5 dernières années. La majorité des cas était rapportée chez des personnes migrantes (n=27) ou des voyageurs (6). Pour ces cas, la notion d'un séjour dans un ou plusieurs des pays suivants a été retrouvée notamment Afghanistan (24), Turquie (6), Serbie (6), Italie (5), Autriche (4), Tunisie (3). Un cas a probablement été infecté en métropole par le contact d'un voyageur. Un cas n'était pas interrogeable. Le délai entre l'arrivée en France et la notification était généralement de moins de 15 jours (9/12). Les cas étaient majoritairement des hommes (32), de moyenne d'âge de 23 ans (min : 11 ; max : 59), 11 résidaient en foyer d'hébergement pour migrants et 3 étaient à jour de leur vaccination. Les cas ont été diagnostiqués dans 9 régions. La région Normandie était la plus touchée avec 7 cas. Cliniquement, 27 cas présentaient une forme cutanée, 4 une forme ORL classique (migrants), 1 une forme ORL peu symptomatique (cas autochtone), 3 étaient asymptomatiques (sphère ORL) et 1 cas est décédé. Le génotypage de 34 isolats a montré 12 groupes génétiques différents dont 5 parmi les migrants en provenance d'Afghanistan: SL377-GC817 (n=8), SL466-GC823 (1), SL384-GC805 (7), SL698-GC795 (7) et SL698-GC804 (2). Ces groupes ont également été identifiés chez des migrants d'autres pays européens. Une augmentation importante du nombre de cas de diphtérie à C. diphteriae a été observée en métropole en 2022. La couverture vaccinale contre la diphtérie en métropole est très élevée : 96% pour le rappel à 11 mois chez les nourrissons en 2019. Le risque d'apparition de cas graves ou de clusters en population générale en métropole est donc extrêmement faible. Cependant, la survenue de cas de diphtérie chez des personnes migrantes, très majoritairement non à jour de leurs vaccinations, fait craindre l'apparition de cas groupés dans les lieux d'hébergement pour migrants. Il convient que les professionnels prenant en charge ces populations soient particulièrement vigilants au repérage des signes cliniques de diphtérie et à la mise à jour des vaccinations de ces populations. Le nombre croissant d'isolement de Corynebacterium diphtheriae relevé sur le territoire étudié, la potentielle gravité de cette infection et la répercussion en terme d'actions de santé publique à mettre en place, nous ont incités à nous intéresser plus particulièrement à cette espèce bactérienne et à son épidémiologie locale. Toutes les souches de C. diphtheriae isolées, identifiées par MALDI-TOF et conservées durant la période de mai 2015 à mai 2019, sur le territoire concerné, ont été inclues dans notre étude : 58 épisodes ont été rapportées. Pour chaque cas, une étude clinique rétrospective a été réalisée par consultation des dossiers patients, les antibiogrammes ont été retestés, le gène tox recherché, le biovar déterminé, les souches séquencées et un arbre phylogénétique a été réalisé en se basant sur le core génome de notre cohorte. Dans notre cohorte, le sexe ratio était de 1,07, le patient le plus jeune avait 9 jours et le plus âgé 78 ans. Nous notons une concentration des cas en particulier autour de la ville principale (24 %, 14/58) et d'une commune peu peuplée (26 %, 15/58) Cette répartition géographique inégale est en partie expliquée par la répartition des MALDI-TOF sur le territoire et par des pratiques de prélèvement différentes d'un centre à un autre. Les prélèvements ont été réalisés pour 88 % (51/58) au niveau cutané. La souche a été retrouvée en association avec S. aureus et/ou S. pyogenes dans 74 % (43/58) des cas. Deux cas d'endocardites chez des patientes jeunes ayant des antécédents de rhumatisme articulaire aigu ont été inclus et concernent des souches tox-. Les souches présentes sur le territoire sont globalement sensibles à la majorité des antibiotiques couramment utilisés, en particulier aucune souche n'est résistante à l'amoxicilline ou à l'erythromycine, antibiotiques de première intention. On note cependant 29 % (17/58) de souches résistantes à la tétracycline, chez toutes ces souches sauf une, la résistance est corrélée à la présence du gène tetO. Nos souches appartiennent à 13 sublineages différents, dont deux peu retrouvés ailleurs dans le monde, sont particulièrement représentés et réunissent 64 % (37/58) de nos souches. Les souches ne possèdent pas le gène de la toxine diphtérique pour 90 % (52/58) des cas. Les cas tox+ sont répartis en deux épisodes, de 1 cas et 5 cas, tous importés d'un pays voisin. Tous les isolats de l'étude possédant le gène spuA sont de phénotype Gravis, et seulement 4 isolats de phénotype Gravis ne possèdent pas le gène spuA. L'évolution des référentiels utilisés pour la réalisation des antibiogrammes tend à rendre les souches plus souvent résistantes sans réalité biologique lorsque l'on applique la même technique à toutes les souches. Des portages asymptomatiques et découvertes fortuites font penser qu'il y a un nombre important de souches tox- sur le territoire, qui circulent largement silencieusement. Quelques cas d'introduction de souches tox+ via un pays voisin indiquent qu'une attention particulière doit être portée en ce qui concerne la couverture vaccinale de la population. Introduction: Diphtheria due to Corynebacterium diphtheriae (C. diphtheriae) has become rare in developed countries. In France only 10 cases of toxigenic diphtheria have been reported since 1989, in all cases causing pharyngitis and all emanating from endemic countries with exception of one contact case. We report herein 13 cases with cutaneous diphtheria, in 5 of which diphtheria toxin was produced, and all imported into France between 2015 and 2018. Observations: Thirteen patients aged 4 to 77 years presented painful and rapidly progressive round ulcerations of the legs, that were superficial and in some cases purulent, with an erythematous-purple border covered with greyish membrane. Bacteriological sampling of ulcers revealed the presence of C. diphtheriae. Only 6 patients had been properly immunized over the preceding 5 years. Discussion: These cases underline the resurgence of cutaneous diphtheria and the circulation of toxigenic strains in France following importation from Indian Ocean countries. This may constitute an important reservoir for ongoing transmission of the disease. Re-emergence of this pathogen stems from the current migratory flow and decreased adult booster coverage. Conclusion: Cutaneous diphtheria should be considered in cases of rapidly developing painful skin ulcers with greyish membrane, especially among patients returning from endemic areas, regardless of their vaccination status. The clinician should order specific screening for C. diphtheriae from the bacteriologist, since with routine swabbing Corynebacteriaceae may be reported simply as normal skin flora. Vaccination protects against toxigenic manifestations but not against actual bacterial infection. Early recognition and treatment of cutaneous diphtheria and up-to-date vaccination are mandatory to avoid further transmission and spread of both cutaneous and pharyngeal diphtheria. Background. - Diphtheria is re-emerging in Europe. A total of 36 cases were reported in Europe in 2015 versus 53 cases between 2000 and 2009. Patients. - We report two cases of Corynebacterium diphtheriae infection in a French hospital in 2016: a cutaneous infection with negative toxin testing in a French traveller, and a respiratory diphtheria carriage with positive toxin testing in an Afghan refugee diagnosed with pulmonary tuberculosis. Mayotte est un département d’outre-mer géographiquement isolé, exposé au risque de maladies infectieuses et soumis à une forte migration sanitaire en provenance des îles voisines. La diphtérie illustre l’importance d’un diagnostic microbiologique de proximité. En 2007, le dernier cas de diphtérie ORL à Corynebacterium diphtheriae porteur du gène codant la toxine (tox +) est notifié chez un nourrisson de 2 mois, non vacciné, qui en est décédé. L’année suivante, le laboratoire de l’hôpital met en place une procédure pour le diagnostic rapide des suspicions cliniques de diphtérie. L’analyse porte sur les isolats de C. diphtheriae tox +identifiés de 2008 à 2015 en associant microbiologie conventionnelle (milieu de culture spécifique de Tinsdal) et biologie moléculaire (amplification du gène tox) avec confirmation secondaire du diagnostic par le CNR. Sur cette période, 13 isolats de C. diphtheriae tox+ sont identifiés au niveau de lésions cutanées chroniques évoluant depuis des semaines, voire des mois, souvent polymicrobiennes. Parmi ces cas : 10 hommes et 3 femmes, âge médian de 11 ans (12 mois-39 ans), 8 cas importés et 7 sans antécédent vaccinal. Les patients ont reçu une antibiothérapie adaptée, associée ou non à un geste chirurgical, sans sérothérapie en absence de manifestations toxiniques. Le diagnostic par le laboratoire a permis une prise en charge adaptée et la mise en œuvre précoce des mesures de gestion préconisées par les autorités sanitaires pour les contacts (écouvillonnage nasopharyngé, antibioprophylaxie, vaccination ou rattrapage vaccinal). Parmi 168 contacts identifiés, l’analyse a permis le diagnostic de 8 cas de portage pharyngé asymptomatique dans l’entourage de 4 patients. Il est noté que 25 isolats tox− ont également été collectés de lésions cutanées chroniques. Des cas de diphtérie cutanée tox + surviennent épisodiquement à Mayotte. L’absence d’une circulation plus importante de ces bactéries ne peut être écartée du fait de la fréquence de plaies cutanées surinfectées et des difficultés d’accès aux soins d’une partie de la population. Face à l’insuffisance de couverture vaccinale chez l’adolescent et l’adulte, la poursuite de la surveillance microbiologique de la circulation de C. diphtheriae demeure une priorité.
The objectives of this study were to undertake the microbiological and molecular characterization of Corynebacterium diphtheriae isolates collected in Algeria during epidemic and post-epidemic periods between 1992 and 2015. Microbiological characterization includes the determination of biotype and toxigenicity status using phenotypic and genotypic methods. Antimicrobial susceptibility was determined by the E-test method. Molecular characterization was performed by multi-locus sequence typing. In total, there were 157 cases of C. diphtheriae isolates, 127 in patients with respiratory diphtheria and 30 with ozena. Isolates with a mitis biotype were predominant (122 out of 157; 77.7%) followed by belfanti (28 out of 157; 17.8%) and gravis biotype (seven out of 157; 4.5%). Toxigenic isolates were predominant in the period 1992-2006 (74 out of 134) whereas in the period 2007-2015, only non-toxigenic isolates circulated (23 out of 23). All 157 isolates were susceptible to erythromycin, gentamicin, vancomycin and cotrimoxazole. Reduced susceptibility to penicillin G, cefotaxime, tetracycline and chloramphenicol was detected in 90 (57.3%), 88 (56.1%), 112 (71.3%) and 90 (57.3%) isolates, respectively. Multi-locus sequence typing analysis indicates that sequence type 116 (ST-116) was the most frequent, with 65 out of 100 isolates analysed, in particular during the epidemic period 1992-1999 (57 out of 65 isolates). In the post-epidemic period, 2000-2015, 13 different sequence types were isolated. All belfanti isolates (ten out of 100 isolates) belonged to closely related sequence types grouped in a phylogenetically distinct eBurst group and were collected exclusively in ozena cases.In conclusion, the epidemic period was associated with ST-116 while the post-epidemic period was characterized by more diversity. In March 2014, a person in their eighties who was diagnosed with extensive cellulitis due to toxigenic Corynebacterium ulcerans died from multiple organ failure. Environmental investigation also isolated C. ulcerans in biological samples from two stray cats in contact with the case. This finding provides further evidence that pets can carry toxigenic C. ulcerans and may be a source of the infection in humans.
A 9-year-old boy presented to the pediatric emergency service for multiple skin lesions of the upper and lower limbs ([Fig. 1A][1] and [B][1]). These lesions had been present for 1 month. The patient had no prior medical history and had been correctly vaccinated. He lives in mainland France but had
Answer: Imported cutaneous diphtheria. The suspected colonies were rapidly identified as Corynebacterium diphtheriae by use of a Microflex LT matrix-assisted laser desorption ionization–time of flight (MALDI-TOF) mass spectrometer (Bruker Daltonics, Bremen, Germany) and subsequently characterized as nontoxigenic Corynebacterium diphtheriae biotype mitis by the National Reference Center for Corynebacterium of the diphtheriae Complex (Institut Pasteur, Paris, France). Treatment with antibiotics was carried on for a total duration of 14 days and allowed for a complete recovery. Nasopharyngeal swabs were taken for all members of the household (i.e., the patient's sister, father, and mother) in order to assess the presence of asymptomatic carriers. All samples were negative for C. diphtheriae growth. Multilocus sequence typing of the isolate generated a new allelic profile, assigned as sequence type 251 (1). Diphtheria is usually considered a life-threatening pharyngitis or laryngitis, and C. diphtheriae is generally not regarded as a skin pathogen. In vaccinated countries, cutaneous diphtheria is discovered as imported cases in travelers coming from areas where the disease is endemic or as autochthonous cases in socioeconomically disadvantaged populations (2,–4). Clinical presentation of skin diphtheria is nonspecific, and most commonly, the organs involved are the lower and upper limbs. Clinical symptoms typically begin as a vesicle, which rapidly progresses to form a punch-out ulcer. At first, these skin lesions may be painful, but they rapidly become insensitive to pain. Diphtheria ulcers generally evolve chronically, and spontaneous recovery can take several weeks. Interestingly, in the present case, the lesions were pruriginous, and scratching them could therefore enhance the diffusion of the bacterium. Definitive diagnosis is generally provided by culture of lesion swabs, but the specific Tinsdale medium is generally not available, as it is expensive and has a shelf life of 1 week. C. diphtheriae colonies are morphologically indistinguishable from those of other Corynebacterium species and thus are likely to be confused with skin flora. Furthermore, most ulcers yield growth of associated pathogens such as Staphylococcus aureus and Streptococcus pyogenes, therefore implying the existence of a causal relationship between the latter pathogens and the skin lesions (5, 6). In fact, these pathogens are usually involved in acute skin infections (e.g., erysipelas or impetigo), while cutaneous diphtheria is chronic and painless. C. diphtheriae identification is challenging and time-consuming with the use of conventional methods, but as it proved to be of valuable assistance in this investigation, MALDI-TOF mass spectrometry would probably simplify and expedite the identification of this pathogen (7, 8). Recommended treatment of skin diphtheria includes thorough cleaning of the lesion, penicillin or macrolide therapy, and, in cases of toxigenic symptoms, antitoxin (2, 9, 10). In conclusion, chronic cutaneous ulcers in specific epidemiological situations should be tested for diphtheria, independent of vaccination status. Their singular clinical and microbiological manifestations probably explain why cutaneous lesions evolve chronically and are more contagious than respiratory infections (3). Selection of appropriate typing method depends on a number of factors, including the scale of the investigation, the rapidity required of the results and the financial and technical resources available. Several typing methods have been applied to Corynebacterium diphtheriae genotyping, but most are laborious and time-consuming or require expensive equipment. We report an evaluation of the utility of the PCR melting profile technique for simple and easy-to-perform genotyping of C. diphtheriae. We compared the method with ribotyping-the 'gold standard' for C. diphtheriae typing-and PFGE, MLST, AFLP, RAPD and spoligotyping.
Autochtonous toxigenic Corynebacterium diphtheriae have disappeared in mainland France, but non-toxigenic C. diphtheriae are still circulating. Using phenotypic and molecular tools, we retrospectively characterized 103 non-toxigenic C. diphtheriae collected in mainland France and highlight several changes. The proportion of C. diphtheriae belfanti increased between 1977 and 2011 and it is the most frequent biotype recovered in recent years. Resistance to ciprofloxacin has increased and most isolates with decreased sensitivity belong to the belfanti biotype. Using multilocus sequence typing, we demonstrate that French isolates are distributed in a large number of sequence types and identify three distinct lineages. C. diphtheriae mitis and gravis form lineage I while C. diphtheriae belfanti forms lineages II and III. Almost all isolates of lineage II are part of a unique clonal complex or are very close to it. Most French isolates have a dtxR sequence homologous to that of toxigenic isolates, suggesting that if lyzogenised by a corynephage, they can express diphtheria toxin.
ABSTRACT Corynebacterium diphtheriae, the agent of diphtheria, is rarely responsible for bacteremia. However, high numbers of bacteremia have been reported in countries with extensive immunization coverage. Here, we used molecular and phenotypic tools to characterize and compare 42 invasive isolates collected in France (including New Caledonia) and Poland over a 23-year period.
ABSTRACT Matrix-associated laser desorption ionization–time of flight mass spectrometry (MALDI-TOF MS) is a rapid and simple microbial identification method. Previous reports using the Biotyper system suggested that this technique requires a preliminary extraction step to identify Gram-positive rods (GPRs), a technical issue that may limit the routine use of this technique to identify pathogenic GPRs in the clinical setting. We tested the accuracy of the MALDI-TOF MS Andromas strategy to identify a set of 659 GPR isolates representing 16 bacterial genera and 72 species by the direct colony method. This bacterial collection included 40 C. diphtheriae, 13 C. pseudotuberculosis, 19 C. ulcerans, and 270 other Corynebacterium isolates, 32 L. monocytogenes and 24 other Listeria isolates, 46 Nocardia, 75 Actinomyces, 18 Actinobaculum, 11 Propionibacterium acnes, 18 Propionibacterium avidum, 30 Lactobacillus, 21 Bacillus, 2 Rhodococcus equi, 2 Erysipelothrix rhusiopathiae, and 38 other GPR isolates, all identified by reference techniques. Totals of 98.5% and 1.2% of non-Listeria GPR isolates were identified to the species or genus level, respectively. Except for L. grayi isolates that were identified to the species level, all other Listeria isolates were identified to the genus level because of highly similar spectra. These data demonstrate that rapid identification of pathogenic GPRs can be obtained without an extraction step by MALDI-TOF mass spectrometry.
In March 2011, a 40 year-old French man was diagnosed with diphtheria caused by toxigenic Corynebacterium diphtheriae. Fifty-three close contacts were identified from whom throat samples were analysed. C. diphtheriae was found only in the asymptomatic partner of the index case. The two cases had travelled in Spain during the incubation period of the index case. Investigation around the second case identified 13 new close contacts. None of them was found to be infected.
