Bei einem Teil der Frauen, die eine menopausale Hormontherapie (MHT) wünschen, liegen Vorerkrankungen vor, die für die Indikation, Substanzwahl und Applikationsform relevant sind. Bisher fehlten konkrete Handlungsempfehlungen für diese interdisziplinären Fragestellungen. Das kann einerseits dazu führen, dass keine MHT angewandt wird, obwohl eine Indikation oder ein Behandlungswunsch vorliegt, andererseits dass Risiken übersehen werden. Im Rahmen der Aktualisierung der S3-Leitlinie Peri- und Postmenopause – Diagnostik und Interventionen wurde eine Evidenzrecherche zur MHT bei relevanten Vorerkrankungen wie Migräne, Adipositas, Leber- oder hämatologischen Erkrankungen durchgeführt. Für die meisten Vorerkrankungen sind lokale und transdermale Applikationen der oralen Anwendung überlegen. Lokales vaginales Östriol, das kaum systemische Wirkung hat, kann bei fast allen Vorerkrankungen eingesetzt werden.
Anfang 2026 wurde die S3-Leitlinie „Prophylaxe der venösen Thromboembolie“ nach über 2‑jähriger Arbeit aktualisiert. Nach ausgedehnten Evidenzrecherchen konnten die Vertreter*innen der Deutschen Gesellschaft für Allgemeinmedizin und Familienmedizin (DEGAM) dieses Mal die anderen Fachgesellschaften überzeugen und Sondervoten vermeiden. Bei Immobilisation des Unterschenkels wird keine generelle medikamentöse Thromboseprophylaxe mehr empfohlen, ebenso soll bei arthroskopischen Eingriffen am Knie, bei akuten internistischen Krankheiten, die außerhalb der Klinik behandelt werden, bei ambulant behandelten Krebserkrankungen sowie bei ambulanten Varizen-Operationen nicht generell eine medikamentöse Thromboseprophylaxe erfolgen, sondern nur, wenn das Thromboembolierisiko individuell erhöht ist. Die aktualisierte Fassung der Leitlinie ermöglicht eine Abkehr von einem reflexartigen Griff zur Heparinspritze hin zu einem abwägenden, risikoadaptierten Vorgehen.
Müdigkeit ist in der hausärztlichen Praxis ein häufiges Symptom, das vielfältige, teils auch ungeklärte Ursachen haben kann. Es tritt sowohl im Rahmen psychischer Störungen, wie bei Depression oder Angststörung, auf als auch als Symptom vieler somatischer Erkrankungen wie Anämie, Schilddrüsenfunktionsstörungen, Diabetes mellitus und Schlafapnoe. Seltener liegen Malignome oder myalgische Enzephalopathie/chronisches Fatigue-Syndrom (ME/CFS) als Ursache zugrunde. Ein biopsychosozialer Ansatz in der Diagnostik, der eine sorgfältige Anamnese, körperliche Untersuchung und eine gezielte Basislaboruntersuchung umfasst, ist essenziell. Die Therapie richtet sich nach den individuellen Ursachen und kann von aktivierenden Maßnahmen über Verhaltenstherapie bis hin zur spezifischen Behandlung einer Grunderkrankung reichen. Insbesondere bei ME/CFS ist ein angepasstes Vorgehen wie Pacing wichtig, um eine Überlastung zu vermeiden.

The benefits of physical activity are undisputed. However, adverse events can occur in rare cases, particularly during high-intensity or prolonged exercise. During physical activity, at-risk patients can experience major cardiac events, whereas adverse events affecting the musculoskeletal system are more common but less severe. A sports preparticipation evaluation (PPE) for apparently healthy adults is designed to detect at-risk individuals and prevent potentially fatal events. This guideline for conducting PPEs was developed by consensus among 16 medical societies and sports associations and is based on previously published guidelines and consensus papers. Sports medicine physicians and potential participants were also surveyed to assess the recommendations' content, feasibility, and implementation. On the basis of the 20 recommendations developed and agreed upon by the abovementioned entities, PPE comprises individuals' personal, family, and sports histories, as well as a physical examination. The need for additional examinations (e.g., laboratory parameters, echocardiograms, or stress tests) is determined on the basis of the PPE findings. This approach's feasibility in various regions, including resource-limited settings, and the extent to which it prevents adverse or potentially fatal events, should be examined in future research.

Osteoporotic fractures impair the quality of life and may make independent living impossible. Recommendations on the diagnosis and treatment of osteoporosis are indispensable for the effective care of this large group of patients. Methods: For a thorough updating of the German clinical practice guideline (an evidence-based guideline with recommendations for clinical practice) on osteoporosis, a comprehensive, systematic search for relevant publications was carried out, including guidelines from other countries. The retrieved literature was assessed with standardized (Oxford) criteria, and clinically relevant key questions were answered according to the PICO scheme ("population, intervention, comparison, outcomes"). Results: The assessment of clinical risk factors for osteoporosis is the basis of osteoporosis diagnostics, which should be carried out quickly after a fracture. If risk factors are present in a postmenopausal woman or a man aged 50 or above, bone densitometry testing with dual-energy x-ray absorptiometry (DXA) is recommended. The further diagnostic evaluation should proceed in stepwise fashion depending on the clinical symptoms, the fracture status, and the degree of bone density reduction. Pharmacotherapy should be adapted to the fracture risk. Osteoanabolic treatment is recommended with high priority if the patient is judged to have a very high risk of fracture (10% or more in the next three years). The further course and duration of treatment should be determined individually, depending on the evolution of the patient's clinical state. Conclusion: Prerequisites for the optimal treatment of patients with osteoporosis include a correct diagnosis and interdisciplinary and interprofessional collaboration to determine and provide the proper treatment. 71% of persons with osteoporosis in Germany are still untreated, and this gap must be closed.

The effect of implementing this guideline should be examined in future research.

Schlafstörungen haben Krankheitswert bei resultierender Beeinträchtigung der Leistungsfähigkeit oder des Wohlbefindens am Tag und müssen nach ICD-10 mindestens einen, nach ICD-11 mindestens 3 Monate anhalten. Die S3-Leitlinie dazu wurde kürzlich upgedatet und publiziert. Hier werden die wichtigsten Empfehlungen erläutert. Schlüsselfrage war die Wirksamkeit verschiedener therapeutischer Verfahren oder Interventionen bei Menschen mit Insomnie. Dazu erfolgte eine systematische Literaturrecherche. In einem interdisziplinären Konsens wurden die Empfehlungen konsentiert und die Texte durch die beteiligten Fachgesellschaften und die AWMF autorisiert. Das empfohlene diagnostische Vorgehen blieb weitgehend unverändert, aber die Liste der auslösenden Medikamente und Erkrankungen wurde aktualisiert. Die kognitive Verhaltenstherapie für Insomnie (KVT-I) soll bei allen Betroffenen als erste Behandlungsoption empfohlen werden, präferenziell als Therapie in Präsenz und auch bei komorbiden körperlichen oder psychischen Erkrankungen. Es gibt hier neuerdings auch die digitalen Gesundheitsanwendungen. Eine medikamentöse Therapie kann angeboten werden, wenn die KVT‑I nicht hinreichend effektiv war oder nicht durchführbar ist, mit großer Zurückhaltung bei älteren Personen und nicht als Langzeittherapie. Von einer Behandlung mit Antipsychotika ohne Vorliegen einer entsprechenden psychiatrischen Indikation oder sedierenden Antihistaminika wird explizit abgeraten. Für Phytotherapeutika gibt es eine „Sollte-nicht“-Empfehlung. Das gilt auch für eine über 12 Monate hinausgehende Behandlung mit dem Orexinrezeptorantagonisten Daridorexant. Auch Melatonin soll nicht langfristig eingesetzt werden. Eine Behandlung mit Bewegungs‑, Licht- und künstlerischen Therapien kann erwogen werden.
Patienten mit dem häufigen Symptom Müdigkeit stellen durch die vielfältigen, teilweise verknũpften biologischen, psychologischen, sozidien sowie umweit- und berufsbedingten Krankheitsursachen den Arzt vor besondere Herausforderungen. Ein gleichzeitiges, nicht chronologisches Erfassen aller wichtigen Krankheitsursachen beugt der Komplikation der somatischen Fixierung vor. Eine Überbewertung leichter Normabweichungen birgt die Gefahr von Pseudodiagnosen und Übersehen komplikationsträchtiger gefährlicher Krankheitsverläufe, In der Therapie ist neben der kausalen Behandlung insbesondere eine angepasste Aktivierung erforderlich, um der Dekonditionierungsspirale vorzubeugen. Die besondere Patientengruppe mit schweren organischen Erkrankungen bedarf bei persistierender Müdigkeit ebenfalls dieses bio-psycho-sozialen Betreuungsansatzes, um nicht etwa eindimensional alle Beschwerden auf die Grundkrankheit zurückzuführen und so dem Patienten wichtige therapeutische Möglichkeiten vorzuenthalten.
Abstract Background Antidepressants are first-line medications for many psychiatric disorders. However, their widespread long-term use in some indications (e.g., mild depression and insomnia) is concerning. Particularly in older adults with comorbidities and polypharmacy, who are more susceptible to adverse drug reactions, the risks and benefits of treatment should be regularly reviewed. The aim of this consensus process was to identify explicit criteria of potentially inappropriate antidepressant use (indicators) in order to support primary care clinicians in identifying situations, where deprescribing of antidepressants should be considered. Methods We used the RAND/UCLA Appropriateness Method to identify the indicators of high-risk and overprescribing of antidepressants. We combined a structured literature review with a 3-round expert panel, with results discussed in moderated meetings in between rounds. Each of the 282 candidate indicators was scored on a 9-point Likert scale representing the necessity of a critical review of antidepressant continuation (1–3 = not necessary; 4–6 = uncertain; 7–9 = clearly necessary). Experts rated the indicators for the necessity of review, since decisions to deprescribe require considerations of patient risk/benefit balance and preferences. Indicators with a median necessity rating of ≥ 7 without disagreement after 3 rating rounds were accepted. Results The expert panel comprised 2 general practitioners, 2 clinical pharmacologists, 1 gerontopsychiatrist, 2 psychiatrists, and 3 internists/geriatricians (total N = 10). After 3 assessment rounds, there was consensus for 37 indicators of high-risk and 25 indicators of overprescribing, where critical reviews were felt to be necessary. High-risk prescribing indicators included settings posing risks of drug-drug, drug-disease, and drug-age interactions or the occurrence of adverse drug reactions. Indicators with the highest ratings included those suggesting the possibility of cardiovascular risks (QTc prolongation), delirium, gastrointestinal bleeding, and liver injury in specific patient subgroups with additional risk factors. Overprescribing indicators target patients with long treatment durations for depression, anxiety, and insomnia as well as high doses for pain and insomnia. Conclusions Explicit indicators of antidepressant high-risk and overprescribing may be used directly by patients and health care providers, and integrated within clinical decision support tools, in order to improve the overall risk/benefit balance of this commonly prescribed class of prescription drugs.
In der hausärztlichen Praxis ist Müdigkeit ein häufiger Beratungsanlass, ebenso in der Psychotherapie ein wichtiges Begleitsymptom. 2002 wurde hierzu durch die Deutsche Gesellschaft für Allgemeinmedizin und Familienmedizin (DEGAM) eine Leitlinie erstellt, die kürzlich erneut überarbeitet wurde 1. Hier informieren wir über wichtige Empfehlungen, soweit sie auch für den Bereich der Psychotherapie von Interesse sind.
Zusammenfassung Ziel Im Rahmen der Überarbeitung der 2017er Leitlinie Prophylaxe, Diagnostik und Therapie der Osteoporose bei postmenopausalen Frauen und bei Männern für den Deutschsprachigen Raum sollte das Risiko für osteoporosetypische Frakturen möglichst präzise und unter Einbeziehung aktueller Daten quantifiziert werden. Methoden Anhand einer systematischen Literaturrecherche ermittelte die Autorengruppe Risikofaktoren für osteoporosetypische Frakturen- in diesem Falle explizit hüftnahe und Wirbelkörperfrakturen. Die Ergebnisse wurden zusammengeführt, bewertet und priorisiert in Hinblick auf ihre Relevanz für die Risikokalkulation, die zukünftig elektronisch in Form eines Risikorechners möglich sein soll. Ergebnisse Besonders starke Risikofaktoren für Hüftfrakturen sind vorausgegangene Frakturen verschiedener Lokalisationen sowie Typ 1 Diabetes, Glucocorticoid-Therapie und einige neurologische Erkrankungen. Bei über 75-jährigen bildet ein timed-up-and-go Test >15 sec. sehr gut das erhöhte Risiko durch mehrere mit erhöhter Sturzneigung korrelierenden Erkrankungen ab. Nach Frakturen ist das Risiko weiterer Frakturen im Folgejahr stark erhöht, weshalb zusätzlich das imminente Frakturrisiko zu beachten ist. Hier berichten wir den derzeitigen Stand der Bewertung und Einarbeitung Schlussfolgerung Wegen der Komplexität der einzelnen Einflussfaktoren ist die Risikoabschätzung am besten mit einem validierten, elektronisch verfügbaren Risikokalkulator möglich. Zwischenergebnisse dazu werden hier genannt, wobei für die Übergangsphase auch eine Papierversion im Leitlinienupdate zur Anwendung kommen soll.
