Systemic therapy for advanced hepatocellular carcinoma (HCC) is still challenging. A biomodulatory therapy approach targeting the communicative infrastructure of HCC, including metronomic low-dose chemotherapy with capecitabine, pioglitazone and rofecoxib, has been evaluated in patients with non-curative HCC. Altogether 38 patients were evaluable in this one-arm, multicenter phase II trial. The primary endpoint, median progression-free survival was 2.7 months (95% CI: 1.6–3.79) for all evaluable patients and 8.4 months (95% CI: 0–18.13) for patients ≥ 6 weeks on protocol. Median overall survival (OS) was 6.7 months (95% CI: 4.08–9.31) and 9.4 months (95% CI: 4.82–13.97), respectively. Most common adverse events were edemas grade 3, which were commonly related to the advanced stage, with 66% of the patients suffering from liver cirrhosis. Exploratory data analyses showed significant impact of ECOG performance status grade 0 versus 1 and CLIP score 0/1 versus > 1 on OS, 9.8 months (95% CI: 4.24–15.35) versus 2.7 months (95% CI: 1.03–4.36; P = 0.002), and 9.8 months (95% CI: 3.23–16.37) versus 4.4 months (95% CI: 3.14–5.66; P = 0.009), respectively. Preceding tumor surgery had significant beneficial impact on survival, as well as maximal tumor diameter of < 5 cm. The correlation of C-reactive protein decrease with significantly improved OS underlines the close link between inflammation and tumor control. Biomodulatory therapy in advanced HCC may be a low toxic, efficacious treatment and principally demonstrates that such approaches should be followed further for treatment of advanced HCC.
Background and Aims: Transketolase-like (TKTL) 1 is one of the key enzymes for anaerobic sugar degradation even in the presence of oxygen (aerobic glycolysis). Transketolase-dependent reactions supply malignant tumors with ribose and NADPH. Therefore, TKTL1 activity could be crucial for tumor proliferation and survival. The aim of the study was to evaluate the expression of TKTL1 in colorectal cancer (CRC) and its regulation under hypoxic conditions. Methods: We studied TKTL1 mRNA and protein expression in CRC cell lines and human CRC biopsies by quantitative real-time PCR, Western blotting and immunohistochemistry. Regulation of TKTL1 under oxygen depletion was analyzed by cultivating cells either in a three-dimensional spheroid model or in a hypoxia incubator chamber. Results: TKTL1 mRNA was heterogeneously expressed in monolayers of cells with high levels in HT-29 and SW480. TKTL1 protein was also clearly detectable in HT-29 and SW480. Hypoxia-inducible factor (HIF)-1α protein expression correlated with TKTL1 protein expression in SW480 spheroids over time. On the one hand, induction of hypoxia in T84 spheroids did not induce TKTL1; on the other hand, hypoxia by incubation at 1% O2 in a hypoxia incubator chamber clearly showed an upregulation of TKTL1. In 50% of CRC patients, TKTL1 protein expression was upregulated in tumor compared to non-tumor tissue. The immunohistochemical staining of TKTL1 in CRC patient samples resulted in 14 positive and 30 negative samples. Conclusions: TKTL1 expression correlated with HIF-1α protein expression and was induced upon hypoxic conditions which could facilitate energy supply to tumors under these circumstances.
Background: New therapeutic options for metastatic pancreatic cancer are urgently needed. In pancreatic cancer, overexpression of the epidermal growth factor receptor 2 (HER2) has been reported in up to 45%. This multicentre phase II study investigated the efficacy and toxicity of the HER2 antibody trastuzumab combined with capecitabine in the patients with pancreatic cancer and HER2 overexpression. Methods: Primary endpoint was progression-free survival (PFS) after 12 weeks. A total of 212 patients were screened for HER2 expression. Results: Immunohistochemical (IHC) HER2 expression was: 83 (40%) grade 0, 71 (34%) grade 1, 31 (15%) grade 2, 22 (11%) grade 3. A total of 17 patients with IHC +3 HER2 expression or gene amplification could be assessed for the treatment response. Grade 3/4 treatment toxicities were: each 7% leucopenia, diarrhoea, nausea and hand-foot syndrome. Progression-free survival after 12 weeks was 23.5%, median overall survival (OS) 6.9 months. Conclusion: This study demonstrates +3 HER2 expression or gene amplification in 11% of patients. Contrary to breast and gastric cancer, only 7 out of 11 (64%) patients with IHC +3 HER2 expression showed gene amplification. Although the therapy was well tolerated, PFS and OS did not perform favourably compared with standard chemotherapy. Together, we do not recommend further evaluation of anti-HER2 treatment in patients with metastatic pancreatic cancer.
Background: Genetic alterations within the epidermal growth factor receptor (EGFR) pathway, including KRAS mutations, have been demonstrated to be associated with response to EGFR inhibitors like cetuximab in colorectal cancers. Mutations in the KRAS gene have been found in 70–90% of pancreatic cancers. Unfortunately, the addition of cetuximab to chemotherapy did not increase response or survival in patients with advanced pancreatic cancer in phase II and phase III studies. The aim of this study was to evaluate the relationship between KRAS mutations and response or survival in patients with metastatic pancreatic cancer treated with cetuximab plus chemotherapy. Methods: Within a multicenter phase II trial, 64 patients with metastatic pancreatic cancer were treated with cetuximab in combination with gemcitabine and oxaliplatin until disease progression. Analyses of the EGFR pathway, including KRAS mutations, could be performed in 25 patients. Analyses were carried out following microdissection of the tumor. Results: Fourteen (56%) of the 25 patients examined harbored a point mutation in codon 12 of the KRAS gene. No differences between the groups were noted in median progression-free survival (104 days in KRAS wild-type patients vs. 118 days in patients with KRAS mutations). Overall survival was longer in wild-type patients compared to patients with KRAS mutations (263 vs. 162 days), but the difference did not reach statistical significance. A further analysis of our clinical phase II trial showed that the presence of a rash was significantly correlated with overall survival. Conclusions: KRAS mutation in codon 12 may be associated with reduced survival compared to KRAS wild type. The role of KRAS mutations for cetuximab therapy in pancreatic cancer warrants further investigation in larger trials to exclude an epiphenomenon. Furthermore, the development of a rash is indicative of clinical benefit.
To study the chemical constituents of the roots of Ilex asprella Champ. ex Benth.Compounds were isolated by silica gel, ODS, and Sephadex LH-20 column chromatography, and their structures were elucidated on the basis of physicochemical properties and spectroscopic analysis.Four triterpenoid glycosides were isolated and identified as (3β)-19-hydroxy-28-oxours-12-en-3-yl β-D-glucopyranosiduronic acid n-butyl ester (1), ilexasoside A (2), monepaloside F (3), and ilexoside A (4).Compound 1 is a new triterpenoid glycoside, and compounds 3 and 4 were isolated from this plant for the first time.
Ziele: Die klinische Relevanz einer computertomografisch zufällig diagnostizierten Dickdarmwandverdickung ist unbekannt. Ziel dieser retrospektiven Studie war die Evaluation computertomografisch detektierter Dickdarmwandverdickungen hinsichtlich der aus dem Befund abgeleiteten weiterführenden Diagnostik und des Ergebnisses einer ggf. durchgeführten Endoskopie. Methode: Mittels Datenbank-Abfrage wurden alle CT-Befunde von Juli 2003 bis Juli 2009 identifiziert, in denen eine Verdickung der Dickdarmwand befundet worden war. Patienten mit einer bekannten (z.B. entzündlichen od. malignen) Darmerkrankung wurden ausgeschlossen. Ansonsten wurden die Patienten unabhängig von der Indikation für das CT eingeschlossen. Anhand der Arztbriefe und Endoskopiebefunde wurde der weitere Krankheitsverlauf der Patienten beurteilt. Ergebnis: 62 Patienten erfüllten die Einschlusskriterien. 21,0% (13/62) dieser Patienten zeigten eine generalisierte oder mindestens zwei Segmente betreffende Dickdarmwandverdickung. In 58,1% der Fälle war nur ein (36/62), meist linksseitiges (25/62) Dickdarmsegment betroffen. 43,5% der Patienten (27/62) wurden endoskopiert. 15 dieser Patienten (55,6%) erhielten eine komplette Koloskopie, während 8 bzw. 4 Patienten sigmoidoskopiert bzw. rektoskopiert wurden (29,6% bzw. 14,8%). 14,8% der endoskopierten Patienten (4/27) zeigten einen malignomsuspekten Befund, der sich bei 2 Patienten histologisch bestätigte. 18,5% der endoskopierten Patienten (5/27) hatten einen unauffälligen Befund. Bei den verbleibenden 18/27 endoskopierten Patienten (66,7%) fand man benigne Läsionen, wie z.B. eine infektiöse Kolitis, eine ischämische Kolitis oder eine entzündliche Darmerkrankung. Insgesamt lieferte die Endoskopie bei 22/27 Patienten (81,5%) einen pathologischen Befund. Schlussfolgerung: Aufgrund der erhobenen Daten besteht die Indikation für eine endoskopische Abklärung, wenn im Rahmen einer Computertomographie eine Dickdarmwandverdickung diagnostiziert wird, da die Rate an pathologischen Befunden hoch ist.

Die Achalasie als neuromuskuläre Erkrankung des Ösophagus ist durch eine reduzierte oder aufgehobene Relaxation des unteren ösophagealen Sphinkters (UÖS) und eine Störung der Motilität in der tubulären Speiseröhre gekennzeichnet. Durch diese Veränderungen ist die Entleerung der Speiseröhre hochgradig beeinträchtigt (Internist 1993; 34: 1122–1132). Dabei kann die Motilität des tubulären Ösophagus amotil, hypomotil oder auch hypermotil sein. Die Ösophagusmanometrie ist der Goldstandard in der Achalasiediagnostik. Mit ihr kann der Nachweis der fehlenden Relaxation des unteren Ösophagussphinkters und der Störung der tubulären Ösophagusmotilität erbracht werden. Bei der Sonderform der hypermotilen Achalasie („vigorous achalasia“) bestehen häufig typische krampfartige Thoraxschmerzen mit ausgeprägter Dysphagie. Manometrisch finden sich hier meist verstärkte nicht peristaltische Kontraktionen (> 160–180mmHg). Hier liegt eine Überlappung zum klinischen Bild des diffusen Ösophagospasmus vor. Die Achalasie nimmt normalerweise einen langsam progredienten Verlauf. Ohne Therapie kommt es über zunächst leichte Schluckbeschwerden und aktive Regurgitationen zu einem zwischenzeitlichen Adaptationsstadium mit leichter Besserung der Dysphagie, weil durch die Dilatation der Speiseröhre diese als Reservoir fungieren kann. Schließlich führen passive Regurgitationen und Aspirationen zu Gewichtsabnahme und Aspirationspneumonien. Bei starker Dilatation der Speiseröhre können sich die Schluckbeschwerden wieder bessern. Bei einigen Patienten erreichen die Beschwerden auch langfristig ein Plateau. Normalerweise ist der untere Ösophagussphinkter tonisch kontrahiert. Beim Schluckakt kommt es zu einer Aktivierung inhibitorischer, v. a. nitrerger Neurone in Kombination mit einem Nachlassen der exzitatorischen Innervation, was zu einer Öffnung des unteren Ösophagussphinkters führt. Der Verlust der inhibitorischen Neurone kann derzeit nicht behandelt werden, daher konzentrieren sich die Behandlungsversuche auf die Senkung des Druckes im unteren Ösophagussphinkter (UÖS) mittels medikamentöser, endoskopischer oder operativer Verfahren. Bisher war unklar, ob die Unterscheidung der Achalasie in Abhängigkeit der Motilität der tubulären Speiseröhre von klinischer Relevanz ist. Pandolfino und Mitarbeiter konnten an einem großen Patientenkollektiv durch Einsatz der hochauflösenden (High-Resolution) Manometrie (HRM) zeigen, dass es Unterschiede im klinischen Verlauf in Abhängigkeit der Motilität des tubulären Ösophagus bei der Achalasie geben könnte (Gastroenterology 2008; 135: 1526–1533).
Hintergrund: Die klinische Relevanz einer Darmwandverdickung im CT ist unklar. Empfehlungen zur weiteren klinischen Abklärung fehlen bislang. Ziel dieser retrospektiven Studie war es, zu untersuchen welche Konsequenzen aus einer Darmwandverdickung im CT abgeleitet wurden und welche Befunde sich aus der weiteren Abklärung ergaben.

Hintergrund: Das kolorektale Karzinom ist in Deutschland die zweithäufigste Krebserkrankung sowohl bei Männern als auch bei Frauen. Die Zahl der Neuerkrankungen liegt bei ca. 70.000 pro Jahr. Allein in Deutschland versterben jährlich 31.000 Menschen am KRK. Bisherige Daten begründen den Verdacht, dass sekundäre Pflanzenstoffe eine mögliche Rolle in der Chemoprävention spielen. Ziel der vorliegenden Studie ist es zu untersuchen, ob Anthocyane (Heidelbeerextrakt) einen Einfluss auf die Tumorentstehung haben.
Die Bolusimpaktion in der Speiseröhre ist eine häufige Indikation für eine Notfall-Endoskopie. Ziel der Studie war zu untersuchen, welches die häufigsten Ursachen für eine Bolusimpaktion sind und mit welchem endoskopischen Werkzeug die Boli am erfolgreichsten entfernt werden konnten. Patienten: In einer retrospektiven Studie wurden die Daten von 54 Patienten (41m) mit Bolusimpaktion in der Speiseröhre analysiert. Die Endoskopien wurden von Ärzten/-innen mit einer mindestens einjährigen Endoskopie-Erfahrung durchgeführt. Art und Lokalisation des Bolus sowie das verwendete endoskopische Extraktionswerkzeug wurden ausgewertet. Bei 48 von 54 Patienten wurden Biopsien für eine histologische Untersuchung aus dem Ösophagus entnommen. Ergebnisse: Das mittlere Alter der Patienten lag bei 53±20 Jahren (15–88 Jahren). 14 von 54 Patienten berichteten, dass Sie bereits früher einmal eine Bolusimpaktion hatten. Am häufigsten waren Fleischboli (n=35) in der Speiseröhre zu finden. Während der mittlere Ösophagus selten betroffen war, stecken die Boli entweder im distalen (n=31) oder im proximalen (n=18) Ösophagus fest. Mit dem Endoskop konnte bei 22 Patienten der Bolus in den Magen vorgeschoben werden. Die anderen Boli inklusive Fremdkörper konnten am häufigsten mit der Polypengreifzange oder der Fasszange entfernt werden. Zwei quer in der Schleimhaut steckende Fremdkörper (Gänserippe/Zahnstocher) konnten erst nach Durchtrennung mit dem Laser entfernt werden. Schwerwiegende Komplikationen traten nicht auf. Die zwei häufigsten Ursachen für eine Bolusimpaktierung waren eine eosinophile Ösophagitis (n=10) bzw. eine Refluxösophagitis mit/ohne peptische Stenose (n=10). Schlussfolgerung: Nach Entfernung eines impaktierten Bolus in der Speiseröhre sollten immer Biopsien aus dem Ösophagus entnommen werden, da am häufigsten eine eosinophile Ösophagitis oder eine Refluxösophagitis zugrunde liegen.
Problemstellung: Ziel der Arbeit war die endosonographische Bestimmung der Wanddicke von Ösophagus, Magen und Duodenum von Patienten mit SSc im Vergleich mit Kontrollpatienten.
Targeting the epidermal growth factor receptor pathway in pancreatic cancer seems to be an attractive therapeutic approach. This study assessed the efficacy of cetuximab plus the combination of gemcitabine/oxaliplatin in metastatic pancreatic cancer. Eligible subjects had histological or cytological diagnosis of metastatic pancreatic adenocarcinoma. The primary end point was response according to RECIST. Patients received cetuximab 400 mg m−2 at first infusion followed by weekly 250 mg m−2 combined with gemcitabine 1000 mg m−2 as a 100 min infusion on day 1 and oxaliplatin 100 mg m−2 as a 2-h infusion on day 2 every 2 weeks. Between January 2005 and August 2006, a total of 64 patients (22 women (34%), 42 men (66%); median age 64 years (range 31–78)) were enrolled at seven study centres. On October 2007, a total of 17 patients were alive. Sixty-two patients were evaluable for baseline and 61 for assessment of response to treatment in an intention-to-treat analysis. Six patients had an incomplete drug combination within the first cycle of the treatment plan (n=4 hypersensitivity reactions to the first cetuximab infusion, n=2 refused to continue therapy). Reported grade 3/4 toxicities (% of patients) were leukopaenia 15%, anaemia 8%, thrombocytopaenia 10%, diarrhoea 7%, nausea 18%, infection 18% and allergy 7%. Cetuximab-attributable skin reactions occurred as follows: grade 0: 20%, grade 1: 41%, grade 2: 30% and grade 3: 10%. The intention-to-treat analysis of 61 evaluable patients showed an overall response rate of 33%, including 1 (2%) complete and 19 (31%) partial remissions. There were 31% patients with stable and 36% with progressive disease or discontinuation of the therapy before re-staging. The presence of a grade 2 or higher skin rash was associated with a higher likelihood of achieving objective response. Median time to progression was 118 days, with a median overall survival of 213 days. A clinical benefit response was noted in 24 of the evaluable 61 patients (39%). The addition of cetuximab to the combination of gemcitabine and oxaliplatin is well tolerated but does not increase response or survival in patients with metastatic pancreatic cancer.

Einleitung: Anastomosenstenosen bei End-zu-End-Choledochocholedochostomie nach Lebertransplantation sind ein häufiges Problem. Die endoskopische Bougierungstherapie mit Plastikprothesen ist die einzige nichtoperative Therapiemöglichkeit. Ziele: Wir wollten die Erfolgsrate der endoskopischen Bougierungstherapie an unserem Zentrum ermitteln. Methodik: Bei 45/306 Patienten, die in der Universitätsklinik Regensburg von Oktober 1999 bis März 2009 lebertransplantiert wurden, entwickelte sich eine Anastomosenstenose. Retrospektiv wurden die klinischen Daten dieser Patienten evaluiert. Eine erfolgreiche Bougierungstherapie mit anschließender Stentfreiheit wurde als „primärer Erfolg“ definiert. Die Notwendigkeit einer biliodigestiven Anastomose war ein „Misserfolg“. War nach erfolgreicher Bougierungstherapie mit Stentfreiheit über mehr als 3 Monate wieder eine Stentanlage erforderlich, handelte es sich um einen „Relaps“. Patienten, die nach erfolgter endoskopischer Bougierungstherapie dauerhaft keinen Stent mehr benötigten, galten als „dauerhafter Erfolg“. Ergebnisse: Die Patienten wurden 44±34 Monate (6–176, median 34) beobachtet. Durchschnittlich alle drei Monate wurden die Prothesen gewechselt. Zahl und Durchmesser waren individuell verschieden. Nach Normalisierung der Leberwerte und bei Nachweis eines suffizienten Galleabflusses wurde die Therapie beendet. Die durchschnittliche Therapiedauer betrug 8±8 Monate (1–43, median 6). 9/45 Patienten sind noch unter Therapie. 20/45 Patienten wurden primär erfolgreich behandelt. Dauerhaft erfolgreich behandelt wurden 16/45 Patienten. Ein Relaps trat bei 2/45 Patienten auf. 18/45 Patienten benötigten schließlich eine biliodigestive Anastomose. Damit lag die primäre Erfolgsrate bei 55% (20/36) und die dauerhafte Erfolgsrate bei 44% (16/36). Die Misserfolgsrate betrug 50% (18/36), die Rückfallquote 6% (2/45). Schlussfolgerung: Nach LTX entwickelt sich eher selten eine Anastomosenstenose. Die Erfolgsrate der endoskopischen Therapie liegt bei 50%. Nach erfolgreicher endoskopischer Therapie sind Rückfälle eher selten.
