L’hypotension orthostatique est un facteur de risque de chute fréquemment retrouvé. La bonne connaissance des facteurs qui y sont associés peut permettre de mieux prendre en charge les patients âgés afin de limiter les chutes. L’objectif de cette étude est d’analyser les facteurs associés à une hypotension orthostatique en hôpital de jour chez des patients chuteurs de plus de 75 ans. Il s’agit d’une étude observationnelle analytique transversale monocentrique menée à l’hôpital de jour gériatrique de l’hôpital nord du CHU de Marseille entre septembre 2016 et septembre 2018. Chaque patient a bénéficié d’une évaluation gériatrique standardisée et d’une mesure d’hypotension orthostatique. Parmi les 318 patients inclus dans l’analyse (âge moyen 84,95 ans), 134 présentent une hypotension orthostatique (42,1 %). La mesure d’une pression artérielle est plus élevée en position couchée chez les patients avec une hypotension orthostatique (PAS : 143 vs 137 ; p = 0,007 et PAD : 71 vs 68 ; p = 0,041). Les patients avec une hypotension orthostatique ont plus souvent un antécédent de maladie de Parkinson (10,4 % vs 3,3 % ; p = 0,009), prennent davantage de traitements psychotropes (56,7 % vs 43,5 % ; p = 0,02) et ont plus fréquemment un mini-MNA à risque (51,5 % vs 39,1 % ; p = 0,028). En analyse multivariée, l’hypotension orthostatique est associée à une artériopathie oblitérante des membres inférieurs (OR : 4,193, IC95 % [1,302–13,501] ; p = 0,016). La prise d’un inhibiteur de l’enzyme de conversion (IEC) (18,7 % vs 28,8 % ; p = 0,038), un MMSE entre 24 et 30 (29,9 % vs 46,2 % ; p = 0,003), et un mini-MNA normal (23,9 % vs 37,5 % ; p = 0,01) apparaissent comme protecteurs vis-à-vis de l’hypotension orthostatique. Nous retrouvons comme facteurs protecteurs associés à l’hypotension orthostatique la prise d’un IEC, un MMSE entre 24 et 30 et un mini-MNA normal. Par contre, une pression artérielle plus élevée en position couchée, une maladie de Parkinson, des psychotropes et un mini-MNA à risque semblent plus à risque d’hypotension orthostatique dans cette population.
La diminution des capacités physiques est un facteur de risque de chute. L'un des tests utilisés pour évaluer les performances physiques est le « short physical performance battery » (SPPB). Cependant la capacité de ce test à identifier les patients qui présenteront une chute et une chute grave a été peu étudiée et avec des résultats discordants.L'objectif de cette étude est d'évaluer l'association entre le score total au SPPB et la survenue de chute et de chute grave dans une population à risque. L'objectif secondaire de l'étude est d'établir la capacité du score total au SPPB à identifier les patients qui seront victimes de chutes et de chutes graves Il s'agit d'une étude de cohorte rétrospective sur une population de 90 patients évalués en hôpital de jour gériatrique dédiée aux chutes et aux malaises entre le 1re novembre 2019 et le 30 octobre 2020. L'évaluation initiale comprenait une évaluation gériatrique standardisée associée au SPPB. L'identification d'une chute et de ces complications a été réalisée par un interrogatoire téléphonique à la fin de la période de suivi (juin 2021). Sur les 90 patients analysés, 52 % (47) ont chuté dont 80 % (38) sont des chutes graves. Les patients qui ont présenté une chute durant le suivi ont une plus grande prévalence de polymédication(prise de plus de 4 traitements) (78 % vs 58 %, p = 0.041) et une plus grand prévalence d'atteinte cognitive (MMS < 21) (68,2 % vs 42,9 % p = 0,014). Ils ont également obtenu en moyenne un score total au SPPB inférieur aux patients qui n'ont pas chuté (F [1,87] = [5596], p = [0,020]). À l'analyse ROC, le SPPB est associé à la survenue de chute et de chute grave. Cette analyse nous donne un seuil optimal pour un score total au SPPB à 6 (sensibilité 73,91 % ; spécificité 53,4 % avec un RR = 1,8 pour la chute et RR = 2,35 pour la chute grave). À l'analyse multivariée, le score total au SPPB n'est pas indépendamment corrélé à la survenue de chute et de chute grave, contrairement à l'atteinte cognitive (OR = 3,28 IC95 % 1,17–9,23 p = 0,024) et à la polymédication pour les chutes graves (OR = 3,72 IC95 % 1,02–13,50 P = 0,045). Le SPPB est associé à la survenue de chute et de chute grave. Ce test permet d'identifier les patients les plus susceptibles de présenter une chute et une complication de cette chute avec un seuil optimal fixé à 6. Le SPPB semble un bon test à effectuer en routine avec la polymédication et l'atteinte cognitive.
Acute Graft versus Host Disease (aGvHD) grades 2–4 occurs in 15–60% of pediatric patients undergoing allogeneic haematopoietic stem-cell transplantation (allo-HSCT). The collateral damage to normal tissue by conditioning regimens administered prior to allo-HSCT serve as an initial trigger for aGvHD. DNA-repair mechanisms may play an important role in mitigating this initial damage, and so the variants in corresponding DNA-repair protein-coding genes via affecting their quantity and/or function. We explored 51 variants within 17 DNA-repair genes for their association with aGvHD grades 2–4 in 60 pediatric patients. The cumulative incidence of aGvHD 2–4 was 12% ( n = 7) in the exploratory cohort. MGMT rs10764881 (G>A) and EXO rs9350 (c.2270C>T) variants were associated with aGvHD 2–4 [Odds ratios = 14.8 (0 events out of 40 in rs10764881 GG group) and 11.5 (95% CI: 2.3–191.8), respectively, multiple testing corrected p ≤ 0.001]. Upon evaluation in an extended cohort ( n = 182) with an incidence of aGvHD 2–4 of 22% ( n = 40), only MGMT rs10764881 (G>A) remained significant (adjusted HR = 2.05 [95% CI: 1.06–3.94]; p = 0.03) in the presence of other clinical risk factors. Higher MGMT expression was seen in GG carriers for rs10764881 and was associated with higher IC50 of Busulfan in lymphoblastoid cells. MGMT rs10764881 carrier status could predict aGvHD occurrence in pediatric patients undergoing allo-HSCT.
La maladie de Biermer est une des principales causes d’hypovitaminose B12, qui elle-même peut être responsable d’hyperhomocystéinémie. Si plusieurs études ont confirmé l’association entre l’hyperhomocystéinémie et la survenue de thromboses veineuses, très peu de cas rapportent une maladie de Biermer associée à une thrombose veineuse profonde. Nous rapportons ici un cas de thrombus flottant de la veine iliaque compliquée d’une embolie pulmonaire révélant une maladie de Biermer. Un patient de 50 ans, sans antécédents ni traitement, se présente aux urgences pour une dyspnée brutale associée à des paresthésies des extrémités qui évoluent depuis plusieurs mois. Sur le bilan réalisé aux urgences, il est mis en évidence une élévation D-dimères associé à une bicytopénie. En effet, on notait une anémie macrocytaire arégénérative à 5,4 g/dL avec un VGM à 110 fL, une thrombopénie à 100 G/L, associée à des stigmates d’hémolyse avec une haptoglobine effondrée, des LDH augmentés et l’absence de schizocytes. Dans ce contexte un scanner thoraco-abdomino-pelvien est réalisé, et mettait en évidence une embolie pulmonaire segmentaire droite ainsi qu’un thrombus flottant étendu de la veine iliaque gauche. On notait également de nombreux ganglions médiastino-hilaire. Le patient était ainsi transféré dans le service de cardiologie pour anticoagulation curative par héparine non fractionnée par voie intraveineuse lente et surveillance. Un avis de médecine interne était demandé pour suspicion d’hémopathie devant les cytopénies associées à des adénopathies. Devant la bicytopénie, la macrocytose et le tableau d’hémolyse intramédullaire, une étiologie carentielle aux cytopénies était suspectée. La présence de polynucléaires hypersegmentés sur le frottis a conforté notre hypothèse, qui fut confirmée par le dosage de la vitamine B12 qui était indosable. Un bilan auto-immun est alors réalisé et retrouvait des Ac anti-cellules pariétales gastriques positifs avec un titre à 200, confirmant la maladie de Biermer. Les anticorps anti-facteur intrinsèque étaient négatifs. Parallèlement, un bilan étiologique pour l’embolie pulmonaire est demandé, et mettait en évidence une hyperhomocystéinémie à 184 μmol/L (normes du laboratoire entre 0 et 14 μmol/L). Le reste du bilan de thrombophilie était négatif. Un TEP scanner a été réalisé pour éliminer une cause paranéoplasique et est redevenu normal. Le patient était alors supplémenté par de la vitamine B12 en injection permettant une correction des cytopénies. Concernant les paresthésies des extrémités, une sclérose combinée de la moelle était suspectée, mais non confirmée en IRM. Cependant l’IRM a été réalisée après quelques jours de supplémentation, qui a d’ailleurs permis la régression des paresthésies. Une fibroscopie œsogastroduodénal sera réalisée à distance pour confirmer histologiquement la maladie de Biermer. Il n’a pas encore été recherché de mutation du gène de la méthylènetétrahydrofolate réductase (MTHFR) : l’hyperhomocystéinémie a été attribuée comme cela a déjà été décrit à la carence en vitamine B12 de la maladie de Biermer. Le taux d’hyperhomocystéinémie sera contrôlé à distance de la supplémentation en vitamine B12. La maladie de Biermer est une cause d’hyperhomocystéinémie secondaire à la carence en vitamine B12, et expose ainsi à un sur-risque de thrombose veineuse. Cependant, peu de cas de maladie de Biermer associée à des thromboses veineuses sont décrits. Nous rapportons ici une embolie pulmonaire associée à un thrombus flottant de la veine iliaque ayant fait découvrir une maladie de Biermer. Ce diagnostic doit être évoqué par le clinicien devant toute thrombose veineuse associée à la présence d’une anémie arégénérative et d’une hémolyse.
Background Medication errors are a major source of many risks to patients. The replacement of hand-written orders by the recommended Computerised Provider Order-Entry (CPOE) system1 makes it possible to issue exit prescriptions more safely. Indeed, outpatients can benefit from pharmaceutical opinions reported during their hospitalisations. Purpose Outpatients must have a complete record of medication drugs prescribed by the hospital’s physicians. To improve patients’ clinical care, it is necessary to evaluate the conformity of outpatient medication. Material and methods We conducted a retrospective study in three hospital departments (digestive surgery, internal medicine, geriatrics) over a 6 month period in order to analyse the conformity and errors in the exit prescription related to CPOE using the software Pharma (Computer Engineering, France) and the Electronic Medical Records (EMR) in Axigate (Pharmagest, France). Results One-thousand two-hundred and eighty-nine patients were included, of which 933 (72%) had an exit prescription using the software Pharma. Analysis of conformities showed that 204 patients (16%) had no Pharma exit prescription but exit treatments written in the EMR and 152 patients (12%) had no data either in Pharma nor in Axigate. Among the 933 patients, 348 (37%) had a copy/pasted prescription into their EMR and 585 (63%) presented discrepancies or lack of treatment into their EMR. No patient had the exit prescription scanned into their EMR although the software allows it. Two-hundred and seventy patients (29%) had no bodyweight provided even after the pharmacist notifications. Analysis of errors’ prescriptions: 255 were incorrect (4% of 7258 total number of drugs prescribed) with 36% drug redundancies, 29% incorrect dosage forms, including 7% of excessive dose and refractory period not respected in 25% cases. These errors were formulated daily by hospital pharmacists as a pharmaceutical opinion in Pharma but not applied by physicians in exit prescriptions. Conclusion The exit prescriptions are not always recorded with CPOE Pharma. Several nonconformities and errors in outpatients’ prescriptions, mainly absence of bodyweight and incorrect drug prescriptions are noted. Hospital pharmacists’ initiatives, such as training and communication with physicians, have been set to improve exit prescriptions which will be served by community pharmacies. Introduction. - Proton pump inhibitors (PPI) are widely prescribed for unrecognized indications, at high a dose and for a long duration, in spite of side effects and numerous drug. interactions. In 2009, the HAS (French Health Authority) published recommendations of good prescription but the latter are poorly respected. In this context of over prescription and additional cost for the society, we performed a professional practice evaluation of on the model of the Deming wheel. The objective of this work was to optimize the relevance of the prescriptions of the IPP in two services of internal medicine and geriatrics through an evaluation of the professional practices. All PPI prescriptions introduced in outpatient visits or during hospitalization were analyzed. Patients and methods. - Data collection was prospective, over two periods of 2 months and included 163 (first phase), then 139 patients (second phase). An assessment grid of PPI prescriptions was completed by physicians regarding the active substance, the dose, the duration and the indication of the prescription. The relevance of the prescription corresponded to PPI with a conformed indication and duration and to the prescriptions no recommended stopped. Following the first period of data collection, information was given to medical students and physicians on the relevance of their prescriptions with regard to the current recommendations and informative flyers were offered with the aim of improving the practices before the second period of evaluation (second phase). Results. - During the first phase, only 25% of the pre-hospital prescriptions and 33% of the hospital prescriptions respected the HAS recommendations. The main indication of the PPI was the prevention of peptic ulcers in a context of associated drug estimated at risk. An improvement of the global relevance of prescription was observed after awareness of the physicians: 26% relevance during the first phase and 60% in the second one (P<0.012). During the second phase, the part of PPI prescriptions introduced at hospital decreased from 33 to 17% and the discontinuation of the not corresponding prescriptions increased from 6 to 33%, with an additional information given to the general practitioner (P<0.001). However, during the second phase, 33% of the prescriptions introduced in hospitalization were always not corresponding and 61% of the not corresponding prescriptions begun in outpatient visits were always pursued on discharge, probably due to the lack of sufficient information to stop the prescription. Conclusion. - Our study underlines the frequent disrespect of the indications in the prescription of PPI. Interestingly, a professional practices evaluation improved the relevance of the prescriptions with a more frequent withdrawal of the not corresponding exposure and a decrease in global not corresponding prescriptions. Our study suggests that it is crucial to regularly inform physicians on the good prescription of PPI. Patient information focused on the indications and the limited duration of PPI prescription, potentially severe side effects of chronic exposure and on the risk of drug interactions also remains necessary in order to facilitate the stop of the exposure and restrict self-medication. OBJECTIVE:To describe the outcome and tolerance in patients treated with anti-TNFα in severe and refractory major vessel disease in Behçet's disease (BD). METHODS:A multicenter study evaluating 18 refractory BD patients with major vessel involvement [pulmonary artery (n = 4), aorta (n = 4) or peripheral artery aneurysm (n = 1) and/or pulmonary artery (n = 7), inferior vena cava (n = 5), or intra-cardiac (n = 3) thrombosis or Budd Chiari Syndrome (n = 2)] treated with anti-TNFα agents. RESULTS:Vascular remission was achieved in 16 (89%) patients. The 9 months risk of relapse was significantly higher with conventional immunosuppressants used prior anti-TNFα agents as compared to anti-TNFα therapy [OR = 8.7 (1.42-62.6), p = 0.03]. The median daily dose of corticosteroids significantly decreased at 12 months. Side effects included infection (n = 4) and pulmonary edema (n = 1). CONCLUSION:TNFα-antagonists are safe and might be associated with a decreased risk of relapse at 9 months compared to conventional immunosuppressants in BD patients with major vessels disease.
Introduction Les inhibiteurs de la pompe a protons (IPP) sont largement prescrits pour des indications non reconnues, a dose trop forte, duree trop longue, et ce malgre des effets indesirables et des interactions medicamenteuses nombreux. En 2009, la HAS a publie des recommandations de bonne prescription mais ces dernieres sont peu respectees. Dans ce contexte de surprescription et de surcout pour la societe, nous avons realise une evaluation des pratiques professionnelles sur le modele de la roue de Deming. L’objectif etait l’etude de la pertinence de la prescription des IPP dans deux services de medecine interne et de geriatrie et son evolution apres rappel des bonnes pratiques aux prescripteurs. Tous les traitements par IPP etaient analyses qu’ils aient ete inities en externe ou durant l’hospitalisation. Patients et methodes Le recueil des donnees etait prospectif, sur deux periodes de 2 mois et incluait163 (premiere phase) puis 139 patients (deuxieme phase). Une grille d’evaluation des prescriptions d’IPP etait remplie par les medecins concernant la molecule, la posologie, la duree et l’indication du traitement. A la suite de la premiere periode de recueil des donnees, une information des internes et medecins sur la pertinence de leurs prescriptions au regard des recommandations actuelles et la distribution de plaquettes d’aide a la prescription etait faite dans le but d’ameliorer les pratiques avant la seconde periode d’evaluation (deuxieme phase). Resultats Lors de la premiere phase, seulement 25 % des prescriptions pre-hospitalieres et 33 % des prescriptions hospitalieres respectaient les recommandations de l’HAS. La principale indication des IPP etait la prevention d’ulceres gastroduodenaux dans un contexte de prise medicamenteuse estimee a risque (hors AINS a dose anti-inflammatoire). La pertinence correspondait aux prescriptions d’IPP avec une indication conforme et aux prescriptions sans indication recommandee arretees. Une amelioration de la pertinence globale de prescriptions etait observee apres la sensibilisation des prescripteurs : pertinence de 26 % lors de la premiere phase a 60 % apres rappel aupres des prescripteurs ( p Conclusion Notre etude souligne le non-respect frequent des indications reconnues dans la prescription des IPP. De maniere interessante, une evaluation des pratiques professionnelles a permis une amelioration de la pertinence des prescriptions avec un arret plus frequent des traitements chroniques non conformes et une baisse des prescriptions hors indications. Notre etude suggere qu’il est primordial de poursuivre la sensibilisation des internes et des medecins hospitaliers et de ville dans leur prescription d’IPP. Une information des patients sur les indications et la duree limitee de ce type de traitement, les effets indesirables potentiellement graves d’une prise prolongee et le risque d’interactions medicamenteuses reste egalement necessaire pour faciliter l’arret des traitements et limiter l’automedication.
Objectives. - To assess the rate of anomalies in the etiological evaluation of patients presenting recurrent early miscarriages (RM) according to miscarriage chronology (number of miscarriages, history of live birth and succession of RM). Methods. - Retrospective single centre study including RM, defined as at least 2 miscarriages at less than 14 weeks of gestation (WG) between the 1st January 2012 and the 31st December 2015. Clinical data and etiological evaluation include blood glucose levels, screening for antiphospholipid syndrome (APS), endocrine assessment, vitamin levels, pelvic imaging, karyotyping of both partners, chronic endometritis and thrombophilia screening. Results. - Two hundred and eighty-eight patients were included over this period, 118 (41%) patients had no history of live birth. Two hundred and twenty-three (77%) patients had consecutive RM and 65 (22%) patients had non-consecutive RM. For consecutive RM, 62,8% had thrombophilic disorders versus 69,8% for non-consecutive RM (P > 0,05); 44,7% had endocrine disorders or vitamin deficiencies versus 39,7%; 34,6% of patients with consecutive RM had uterine anomalies versus 45,5% respectively. No difference was found depending on the recurrence of RM or the history of live birth (P > 0.05) apart from the age of the patient. Fifty-nine (17.4%) patients had uterine anomalies. There are 24 chronic endometritis on 31 biospsies performed. Seventy-eight (27%) patients were offered treatment. Ninety-four (90%) patients showed good therapy compliance. Eighty-one (78%) patients became pregnant. Conclusion. - An etiological evaluation provides, for over half of the cases, an etiology or the identification of risk factors responsible for RM, as well as in some cases offering an adapted, efficient, therapeutic approach.
