Background: Patients with Cushing syndrome (CS) are at increased risk of venous thromboembolism (VTE). Objective: The aim was to evaluate the current management of new cases of CS with a focus on VTE and thromboprophylaxis. Design and methods: A survey was conducted within those that report in the electronic reporting tool (e-REC) of the European Registries for Rare Endocrine Conditions (EuRRECa) and the involved main thematic groups (MTG’s) of the European Reference Networks for Rare Endocrine Disorders (Endo-ERN) on new patients with CS from January 2021 to July 2022. Results: Of 222 patients (mean age 44 years, 165 females), 141 patients had Cushing disease (64%), 69 adrenal CS (31%), and 12 patients with ectopic CS (5.4%). The mean follow-up period post-CS diagnosis was 15 months (range 3–30). Cortisol-lowering medications were initiated in 38% of patients. One hundred fifty-four patients (69%) received thromboprophylaxis (including patients on chronic anticoagulant treatment), of which low-molecular-weight heparins were used in 96% of cases. VTE was reported in six patients (2.7%), of which one was fatal: two long before CS diagnosis, two between diagnosis and surgery, and two postoperatively. Three patients were using thromboprophylaxis at time of the VTE diagnosis. The incidence rate of VTE in patients after Cushing syndrome diagnosis in our study cohort was 14.6 (95% CI 5.5; 38.6) per 1000 person-years. Conclusion: Thirty percent of patients with CS did not receive preoperative thromboprophylaxis during their active disease stage, and half of the VTE cases even occurred during this stage despite thromboprophylaxis. Prospective trials to establish the optimal thromboprophylaxis strategy in CS patients are highly needed. Significance statement The incidence rate of venous thromboembolism in our study cohort was 14.6 (95% CI 5.5; 38.6) per 1000 person-years. Notably, this survey showed that there is great heterogeneity regarding time of initiation and duration of thromboprophylaxis in expert centers throughout Europe.

L’épaississement de la tige pituitaire associée à un diabète insipide peut être observé en cas de pathologie tumorale, granulomateuse ou inflammatoire. Un homme de 82 ans, aux antécédents de syndrome de Tolosa Hunt (ophtalmoplégie d’origine inflammatoire), est traité par corticothérapie pour une suspicion clinique d’artérite de Horton. Sous Médrol, il développe une asthénie, un amaigrissement et des transpirations profuses. L’échographie abdominale montre un épaississement de la surrénale gauche confirmé au scanner et suspect d’être d’origine tumorale. La biopsie de la lésion surrénalienne initialement envisagée n’est finalement pas réalisée en raison de sa « diminution spontanée ». Cette évolution radiologique évoquant une relative cortico-sensibilité de l’affection sous-jacente. Deux mois après le sevrage progressif de la corticothérapie, apparaissent un diabète insipide et un panhypopituitarisme. Un nouveau bilan iconographique révèle des masses surrénaliennes bilatérales supra-centimétriques (30 UH à blanc), une lésion hypothalamique et un épaississement de la tige pituitaire hyper métaboliques au Pet scan FDG. Le patient bénéficie d’une ponction, par écho-endoscopie, de la lésion surrénalienne gauche. L’examen anatomopathologique conclut au diagnostic de lymphome de Burkitt. Les métastases hypophysaires sont rarement observées dans le lymphome (0,5 %). Elles s’observent plus fréquemment dans le lymphome B diffus à grandes cellules (DLCBL) que dans le lymphome de Burkitt. Si trois cas de patients âgés atteints de DLCBL, avec une atteinte surrénalienne bilatérale et une atteinte hypophysaire ont déjà été rapportés, c’est à notre connaissance le premier cas décrit d’atteinte simultanée de ces deux glandes endocrines en présence d’un lymphome de Burkitt.
INTRODUCTION:Nephrolithiasis is a frequent disease observed in 1 to 20 % of the general population. This disease predominates in male patients (2:1) and is characterized by a high rate of recurrences (about 50 %).CASE REPORT:We report the case of a 45-year old male patient who experienced during about ten years recurrent bilateral renal colic episodes due to brushite lithiasis. These stones were treated with multiple extracorporeal shock wave lithotripsy sessions. A pyeloureteral junction syndrome predisposing to bulky stones formation has been put in evidence and required a pyeloplasty. After more than ten years of disease activity, a biochemical screening diagnosed primary hyperparathyroidism (PHPT). Radiological assessment identified a parathyroid gland adenoma. Successful surgical removal of this lesion was followed by resolution of the symptomatic kidney stones formation.DISCUSSION:PHPT is associated with kidney stones in about 20 % of the patients. Hypercalciuria is the main risk factor of stones formation but other predisposing factors are also probably involved. Patients carrying a polymorphism located in the coding sequence of the calcium-sensing receptor gene or in the regulatory region of this gene seem to experience an increased occurrence of urinary lithiasis.CONCLUSION:The present case stresses the importance of a metabolic assessment in all patients with recurrent nephrolithiasis, especially in case of bilateral episodes.
Acromegaly is a rare hormonal disease related to excessive growth hormone secretion. It can result in a range of complications, including cardiovascular, respiratory, metabolic, articular and neoplastic disorders. Among patients with the condition, obstructive sleep apnea syndrome occurs frequently and the effect of treatment is inconstant: improvement, statu quo or deterioration can be observed. L’épaississement de la tige pituitaire associée à un diabète insipide peut être observé en cas de pathologie tumorale, granulomateuse ou inflammatoire. Quel que soit l’âge du patient, une tumeur germinale doit être prioritairement recherchée. Une jeune femme de 18 ans est prise en charge pour l’évaluation d’un diabète insipide et d’un hypogonadisme hypogonadotrope. La seule anomalie décrite, sur les IRM cérébrales et hypophysaires, en dehors de la perte de l’hypersignal de la post-hypophyse est un épaississement de la tige pituitaire. Les marqueurs tumoraux (HCG et AFP) sont normaux (sang et LCR). Dès lors, la patiente est suivie régulièrement pour « grosse tige isolée ». Des troubles cognitifs apparaissent deux ans plus tard. L’IRM concomitante révèle, d’une part, une nette régression de l’épaississement de la tige pituitaire, d’autre part, l’apparition de lésions bilatérales périventriculaires. Les marqueurs tumoraux restent inchangés. On retient le diagnostic de neurosarcoïdose. Sous corticothérapie, l’évolution clinique et radiologique est initialement favorable. Malheureusement, 6 mois après le début du traitement, une lésion hétérogène partiellement kystique apparaît, centrée sur le corps calleux. L’examen anatomo-pathologique de la tumeur réséquée conclut au diagnostic de germinome. Le diagnostic de germinome peut être posé plusieurs années après l’installation d’un DI et la découverte d’un épaississement de la tige. Chez l’enfant et l’adulte jeune, l’augmentation progressive de l’épaisseur de la tige est hautement suspecte de germinome. Mais, même sa régression spontanée, telle qu’observée chez notre patiente ne permet pas d’exclure ce diagnostic.
Le but de cette étude est de comparer la réponse des adénomes hypophysaires à GH au traitement en première ligne par analogues de la somatostatine(SSA) en fonction de l’intensité du signal IRM des adénomes en séquence T2. Au total, 95 patients acromégales traités par SSA pour une période entre 3–12 mois ont été inclus dans cette étude multicentrique de manière rétro- et prospective. Le volume tumoral, l’intensité du signal T2 sur l’IRM de diagnostic, aussi bien que des données biologiques (valeurs de GH et IGF1) avant et après traitement par SSA ont été retenus. Au diagnostic, il y avait 70 % d’adénomes T2-hypointenses, 14 % hyperintenses et 16 % isointenses. Les adénomes T2-hypointenses ont présenté une diminution volumique médiane sous traitement de 32,8 %, ceux T2-hyperintenses de 22,4 % et ceux isointenses de 12,7 % (p = 0,0009). Concernant la réponse biologique aux SSA, la diminution médiane du taux d’IGF1 sous traitement a été plus importante pour les adénomes T2-hypointenses (59 %) versus T2-hyperintenses (33 %) et T2-isointenses (30 %)(p = 0,001). Les premièrs résultats de notre étude soutiennent le rôle du signal T2 des adénomes à GH comme facteur prédictif de la réponse biologique et tumorale au traitement par SSA, avec une réponse plus importante des adénomes T2-hypointenses. Une analyse de ces données sur une série encore plus nombreuse serait intéressante, ainsi que la corrélation entre le signal T2 et l’aspect histologique de ces adénomes.
En présence d’un syndrome d’hypersécrétion d’une hormone hypophysaire, le but de l’imagerie par résonance magnétique (IRM) est de mettre en évidence l’adénome hypophysaire responsable qui est souvent de petite taille (microadénome hypophysaire).Lors de l’exploration des adénomes hypophysaires à extension suprasellaire, le but de l’IRM est de préciser l’extension tumorale par rapport aux structures voisines : chiasma optique, artère carotide interne, sinus sphénoïdal, etc.L’IRM permet le plus souvent d’affirmer ou d’éliminer un envahissement du sinus caverneux par l’adénome de l’hypophyse, cette donnée étant capitale pour la décision thérapeutique.Après chirurgie des adénomes de l’hypophyse, l’IRM est à nouveau essentielle pour visualiser, le cas échéant, un résidu adénomateux et surtout, à condition de disposer d’examens rigoureusement reproductibles, d’apprécier l’évolution de ce résidu dans le temps.In cases of pituitary hormone hypersecretion, magnetic resonance imaging (MRI) can demonstrate the causal lesion, which is often small (microadenoma).In pituitary adenomas with suprasellar extension, MRI shows the tumor’s relation to the surrounding structures: the optic chiasm, the internal carotid artery, the sphenoidal sinus, etc.MRI usually makes it possible confirm or rule out any cavernous sinus invasion by the pituitary adenoma, a determination essential for planning treatment.After pituitary surgery, MR imaging is again essential for visualizing any residual tumor tissue and — if the MRI is carefully performed to be reproducible — for assessing its development over time.
One technique used frequently among quality practitioners seeking solutions to multi-response optimization problems is the desirability function approach. The technique involves modeling each characteristic using response surface designs and then transforming the characteristics into a single performance measure. The traditional procedure, however, calls for estimating only the mean response; the variability among the characteristics is not considered. Furthermore, the approach typically relies on the accuracy of second-order polynomials in its estimation, which are not always suitable. This paper, in contrast, proposes a methodology that utilizes higher-order estimation techniques and incorporates the concepts of robust design to account for process variability. Several examples are provided to illustrate the effectiveness of the proposed methodology.
Nasal dermal sinus in children (NDSC) is a rare malformation (1/20,000 to 1/40,000). Apart from local infection, they present as median nasal lump or pit on the dorsum and their diagnosis and treatment are often delayed. Consequences of untreated NDSC are: local infection, meningitis, and empyema, due to their frequent intracranial extension.Six cases of NDSC were retrospectively reviewed (5 boys and one girl), all treated between 2006 and 2012 in our institution (Nancy University Hospital). All patients underwent a brain and facial CT-scan and MR imaging to check any bony lesions, skull base extension by foramen cæcum, course of the sinus and the possible associated brain malformations. Evolution, treatment and follow-up (FU) were conducted by the same multidisciplinary team (neurosurgeon, ENT surgeon, and plastic surgeon). Children were operated on by a conjoined approach (cranial and facial) for removal of the sinus and its intracranial extension.Mean age at diagnosis was 12 months (birth–36 months). Initial presentation consisted of three local infections, one dorsum nasal lump, one CSF leakage, and one asymptomatic child. Five children presented with a skull base extension. There were no associated brain malformations. We observed only one surgical complication (bleeding from the anterior part of the superior sagittal sinus during dissection) leading to blood transfusion. Pathology results confirmed three dermoid cysts, one epidermoid cyst, one cyst with granulation tissue, and negative in one case. Average FU was 30.8 months (4–84 months). Two recurrences (same child) occurred, leading to two re-operations. There were no recurrences or complications at the end of FU.NDSC are rare malformations, mostly diagnosed before the age of three years, due to an infectious complication. The aim of the treatment is complete removal to avoid recurrence, and a multidisciplinary strategy is required.Les fistules du dorsum nasal de l’enfant (FDNE) sont des malformations rares (1/20 000 à 1/40 000). En dehors d’épisodes infectieux, elles se présentent sous forme d’une tuméfaction ou d’un petit pertuis médian, sur le dorsum nasal et leur diagnostic et prise en charge sont souvent retardés. Les conséquences d’une FDNE non traitée sont : surinfections locales, méningites, voire empyème cérébral en raison de leur très fréquent prolongement intracrânien.Notre étude est rétrospective et concerne 6 enfants (5 garçons et une fille), traités pour FDNE entre 2006 et 2012 au CHU de Nancy. Tous les enfants ont bénéficié d’un scanner et d’une IRM cérébrale et faciale pour préciser l’atteinte osseuse, l’extension au niveau de la base du crâne par l’intermédiaire du foramen cæcum, le trajet fistuleux et les éventuelles malformations cérébrales associées. L’évaluation, le traitement et le suivi sont réalisé en triple équipe : neurochirurgicale, ORL et maxillo-faciale. Les enfants ont tous été opérés (même équipe), par un abord mixte (facial et endocrânien) pour exérèse du trajet fistuleux et de son prolongement intracrânien.L’âge au diagnostic était en moyenne de 12 mois (naissance–36 mois). La présentation clinique initiale comprenait 3 infections locales, une tuméfaction du dorsum nasal, un écoulement de liquide cérébro-spinal et un enfant asymptomatique. Cinq enfants présentaient une extension au niveau de la base du crâne. Aucune malformation cérébrale associée n’était retrouvée. Nous notions une complication du geste opératoire (saignement provenant de la partie antérieure du sinus sagittal supérieur pendant la dissection) avec nécessité de transfusion. L’histologie de l’extension intracrânienne avait pu conclure à 3 kystes dermoïdes, un kyste épidermique, un tissu de granulations et cette histologie n’a pu être retrouvée dans un cas. Le recul moyen était de 30,8 mois (4 mois–84 mois). Deux récidives étaient notées chez un même enfant, ayant conduit à deux réinterventions. On ne retrouvait aucune récidive ni complications au terme du suivi.Les FDNE sont des malformations rares, dont le diagnostic est souvent fait avant l’âge de trois ans, à l’occasion d’une complication infectieuse. Elles imposent une exérèse chirurgicale complète pour éviter la récidive et une stratégie thérapeutique multidisciplinaire.
This article deals with traffic data assimilation and algorithms that are able to predict the traffic flow on a road section. The traffic flow is modellized by the Aw–Rascle hyperbolic system. We have to minimize a functional whose optimization variables are initial condition. We use the Roe method to compute the solution to the traffic flow modelling system. Then we compute the gradient of the functional by an adjoint method. This gradient will be used to optimize the functional. Copyright © 2008 John Wiley & Sons, Ltd.
In this Note, we present a new method for the computation of the eigenmodes of some elliptic operators. This technique is based on the concept of component mode analysis but uses overlapping domain decompositions. This allows us to achieve a better method than the previous ones based on nonoverlapping decompositions both from the theoretical and the numerical point of view. in particular, we are able to derive infinite order of convergence for this method.
Through a formalism similar to a kinetic model, an entropy fix Sor Roe's scheme is proposed. A flow splitting is applied to the characteristic variables associated to the genuinely nonlinear fields. The model introduces nonlinearities where the linear approximation violates the entropy property. The numerical diffusion is controlled in an optimal way, in, the sense that it boils down to zero across nonlinear waves. From a numerical point of view, the resulting scheme brings both entropy and robustness; it is an advantage compared to the classical Roe scheme.
We present a new first order accurate scheme for solving the Euler equations in one space dimension. The numerical fluxes are, according to the conditions, either those of Godunov's scheme [1] or those of a flux vector splitting based on a kinetic interpretation of the equations. Several properties are described. They point out the originality and the qualities of this scheme.
