Introduction: Cervical locations of malignant peripheral nerve sheath tumor (MPNST) are rare, at less than 1% of malignant tumors of this region.Case report: A 53-year-old woman presented with a lateral cervical swelling involving the parotid region. Histology was in favor of MPNST. Adjuvant radiotherapy was indicated because of the infiltrating nature of the tumor. At 2 years' follow-up, there was no recurrence.Discussion: Clinical diagnosis is difficult in cervical MPNST. Only histology with immunohistochemistry can establish the correct diagnosis. La localisation cervicale du schwannome malin est rare, elle représente moins de 1 % des tumeurs malignes de cette région. Une femme âgée de 53 ans a présenté une tuméfaction latérocervicale étendue à la région parotidienne. L’histologie a révélé un schwannome malin. Une radiothérapie adjuvante a été indiquée en raison du caractère infiltrant de cette tumeur. Aucune récidive n’est constatée après deux ans. Le diagnostic clinique est difficile dans les cas d’une localisation cervicale du schwannome malin. Seul l’examen anatomopathologique et l’immunohistochimie peuvent confirmer le diagnostic. Le traitement impose une exérèse chirurgicale complète et une surveillance clinique régulière.
La fente cervicale médiane est une anomalie congénitale rare de la partie antérieure du cou.Les auteurs rapportent le cas d’un nourrisson de sexe féminin, âgé de neuf jours, présentant une bride cervicale médiane congénitale et ayant bénéficié d’une intervention chirurgicale à l’âge de 7 mois. L’évolution a été très satisfaisante sans récidive, à deux ans post-opératoires.Les auteurs discutent la présentation clinique de cette anomalie, l’embryologie, la prise en charge thérapeutique et l’évolution. Cette malformation rare se traduit dans sa forme complète par une bride cervicale verticale médiane sous-cutanée mento-sternale, associée à une fente cutanée. Cette fente cutanée consiste en une ulcération surmontée d’une protubérance cutanée fibromateuse et prolongée vers le bas par un trajet fistuleux borgne. Le mécanisme embryologique de formation n’est pas formellement établi, mais la plupart des auteurs s’accordent sur le défaut de fusion des arcs brachiaux sur la ligne médiane par anomalie de mésodermisation. Le traitement est chirurgical et doit être réalisé dans les premiers mois de la vie afin d’éviter les troubles fonctionnels et morphologiques sur le cou et la mandibule. La chirurgie doit comporter l’exérèse complète de l’ulcération cutanée et du tissu fibreux sous-jacent, suivie par la fermeture du site à l’aide de plasties en « Z » multiples. La fréquence des récidives par rétraction fibreuse impose une surveillance post-opératoire rigoureuse, à long terme.The midline cervical cleft is a rare congenital anomaly of the anterior part of the neck.We report the case of a 9-year old girl, presenting a congenital midline cervical cleft who underwent corrective surgery at the age of 7 months. Currently, outome is quite satisfactory two years post-operatively; no recurrence has developed.We discuss the clinical manifestation of this anomaly, the embryology, the surgical treatment and the clinical course. In its complete form, this rare disorder presents as a subcutaneous midline vertical cervical cord, associated with a cutaneous cleft. The cutaneous cleft consists of an ulceration with an overhanging cutaneous fibromatous protuberance which prolongs downwards to a blind-ended sinus tract. The embryologic mechanism is not formally established, but the majority of the authors agree on the imperfect midline fusion of the paired branchial arch tissue by anomaly of mesodermisation. Surgical treatment is required and must be carried out in the first month of the life in order to avoid neck and mandible functional and morphological disorders. Surgery involves complete excision of the cutaneous ulceration and subjacent fibrous cord followed by site closure using multiples “Z” plasties. Long postoperative monitoring is needed due to the frequency of the recurrence.
Purpose. The frequency of maxillary sinusitis of dental origin (MSDO) is under estimated. The medical and surgical treatment has improved thanks to endoscopic guidance. We report our experience and strategy in the treatment of MSDO.Patients and methods. Twenty-two patients presenting with MSDO were treated between 1998 and 2008. The results were evaluated on clinical, functional, sinusal, and odontologic signs.Results. MSDO accounted for 16% of surgically managed sinusitis. CT was performed in 95% of cases. The etiologies were apical leakage in seven patients, migration of a tooth or root during extraction, or presence of ectopic tooth in the sinus in nine patients, a cyst in three patients, and oroantral communication in three patients. Surgery was performed after antibiotic and NSAID treatment. The first surgical step was the treatment of the odontogenic source. The second step was sinus drainage by endoscopic treatment in 64%, Caldwell-Luc in 23%, and drainage by oroantral communication enlarged then closed in the same operative time in 13%. The follow up ranged from 3 months to 10 years. Early postoperative superinfection was observed in two patients. Two patients presented with recurrent sinusitis. The postoperative sequels were hyposmia in three patients, dental pulpotomy, and trigeminal neuralgia in five patients treated by Caldwell-Luc surgery.Discussion. Nasal endoscopy has improved the surgical management of MSDO. It makes curettage and exclusion of sinus cavities obsolete. It is reliable and has a low rate of complications. A seventy-year-old woman consulted for a mass in the left hemi-face having evolved over the last 20 years. The physical examination revealed a hard and large tumor invading all the palate. Computed tomography revealed a heterogeneous 8.5 cm long maxillary mass. The diagnosis of CRAP was made oil a biopsy. A histological study confirmed the diagnosis after surgical resection of the tumor, specifying its noninvasive character.Discussion. CRAP is generally located in the parotid gland; it is very rarely located in the palate. The degenerated epithelial component generally corresponds to an adenocarcinoma or all undifferentiated carcinoma: squamous-cell carcinoma is more rarely reported. Le kératokyste (KK) est une tumeur bénigne rare caractérisée par son agressivité locale et une forte tendance à la récidive. Notre objectif était d’étudier les facteurs de risque de récidive des KK et de confronter nos résultats à ceux de la littérature. Trente-six cas de KK ont été traités chez 32 patients entre 1996 et 2006. Nous avons étudié rétrospectivement les récidives selon les différents paramètres radiocliniques, histologiques et pronostiques. L’âge moyen était de 41 ans avec une légère prédominance masculine (sex-ratio de 1,3). Trente localisations étaient mandibulaires. Une ostéolyse uniloculaire radiologique a été retrouvée dans 75 % des cas. Le traitement primaire a toujours été chirurgical et conservateur. Douze récidives sont survenues chez huit patients, deux à dix ans plus tard. Toutes les récidives étaient mandibulaires, 41 % au niveau de l’angle et du ramus. Les kystes multiloculaires ont récidivé dans 55 % des cas, les uniloculaires dans 11 %. Les récidives concernaient 40 % des formes orthokératosiques. La présence de kystes satellites au niveau de l’épithélium de surface et l’extension aux parties molles ont conduit à 60 % de récidives. Soixante-six pour cent des kératokystes infectés ont récidivé. Parmi les 12 kystes récidivés, huit ont été énucléés en plusieurs fragments. Le risque de récidive des KK est d’autant plus grand que la localisation est postérieure (angle, ramus) et l’architecture multiloculaire. Cela pourrait s’expliquer par la difficulté d’accès à ces régions. Le type histologique orthokératosique et la présence de kystes satellites semblent aussi être associés à un taux élevé de récidive. Dans notre étude, il existait une relation entre le taux de récidive, l’infection du kyste et les conditions d’exérèse chirurgicale. The odontogenic keratocyst is a benign tumor with a high recurrence rate and aggressive behavior. We analyzed the risk factors for recurrence of odontogenic keratocysts and compared our results to published ones. Thirty-two patients (36 odontogenic keratocysts) were treated in our department between 1996 and 2006. We retrospectively analyzed recurrence according to anatomoclinic, histopathological, and prognostic parameters. The patients’ mean age was 41 years. The sex ratio was 1.28 with male predominance. The mandible was the most common site (30 cases). The most common radiological finding was a unilocular cyst in 75 % of cases. The primary treatment was conservative surgery for all patients. Twelve recurrences were observed in eight patients between 2 and 10 years of follow-up. All recurrences occurred in the mandible with 41 % in the angle and ramus. Multilocular cysts recurred in 55 % of cases, unilocular in 11 %. The recurrence rate of orthokeratocysts was 40 %. The presence of satellite cysts and extension to soft tissues were associated with a high rate of recurrence (60 %). Sixty-six percent of infected cysts recurred and among these, eight were enucleated in several fragments. The recurrence rate of odontogenic keratocysts is higher in case of mandibular posterior region localization and multilocular keratocysts. This might be explained by the difficult surgical accessibility. Some histological findings including the presence of satellite cysts and orthokeratocysts are associated with a higher recurrence rate. Cyst infection and treatment modalities influence the recurrence rate.
Décrire les aspects radiologiques (radiographie standard, TDM et IRM) des sarcomes des maxillaires. Série rétrospective portant sur 14 cas de sarcomes des maxillaires : 10 femmes et 4 hommes d’âge variant entre 4 et 73 ans. Le motif de consultation était une tuméfaction génienne rapidement évolutive avec asymétrie faciale. Tous les patients ont été explorés par une radiographie panoramique et une TDM. Deux patients ont bénéficié d’une IRM. Les localisations mandibulaires ont été retrouvées dans 9 cas et maxillaires dans 5 cas. La radiographie standard et la TDM avait mis en évidence : une lésion lytique irrégulière (n = 5), expansive (n = 4) et mixte (n = 2), une réaction périostée spiculée (n = 2), une masse des tissus mous dans tous les cas avec extension intraorbitaire dans 2 cas. Le diagnostic de sarcome a été confirmé dans tous les cas par l’examen anatomopathologique : sarcome ostéogénique (n = 4), sarcome d’Ewing (n = 1), chondrosarcome (n = 1), fibrosarcome (n = 1), hématosarcome : myélome (n = 2) et lymphome de Burkit (n = 3), rhabdomyosarcome (n = 2). Les sarcomes des maxillaires sont rares. L’imagerie évoque le diagnostic et permet un bilan d’extension locale précis.

Introduction. The desmoplastic fibroma is a rare bone tumor, characterized by aggressive local infiltration, with frequent recurrence. The most common site is the mandible. Radioclinical signs are not specific and the histological diagnosis may be difficult.Case. A 16 year-old male patient consulted for a painless and hard left mandibular swelling, without inferior alveolar nerve disorders. The tumor extended from tooth 31 to tooth 35, the mucosa was healthy. The panoramic view showed a multilocular osteolytic lesion with denial root resorption. CT scan showed expansion of bony cortex with rupture of the outer cortical. The biopsy indicated a desmoplastic fibroma or a fibrosarcoma. A second histological analysis combined with an immuno-histochemical study proved the diagnosis of desmoplastic fibroma.Discussion. The desmoplastic fibroma has a polymorphous symptomatology. Radiological signs are unspecific. Anatomopathology combined with immuno-histochemistry can prove the diagnosis and guide the treatment. Les fractures du plancher de l'orbite de type « blow-out » pur correspondent à l'irruption de structures intra orbitaires dans le sinus maxillaire à travers une déhiscence de la paroi orbitaire inférieure qui est très mince et fragile. Il existe trois types anatomopathologiques : les fractures « effondrement », les fractures « en trappe » et les fractures « en clapet ». Le but de notre travail est de déterminer les caractéristiques cliniques, radiologiques, thérapeutiques et évolutives de cette entité traumatologique. Il s'agit d'une étude rétrospective à propos de 11 cas de fracture du plancher de l'orbite type « blow-out » pur, colligés au service de chirurgie maxillo-faciale sur une période de 10 ans, allant de 1995 jusqu'au 2005. L'âge moyen des patients était de 14 ans. Les accidents domestiques représentaient l'étiologie la plus fréquente, suivis des accidents sportifs, les agressions et les accidents de la voie publique. La diplopie était le signe clinique le plus fréquent (7 cas). L'examen tomodensitométrique a permis de préciser le siège, l'étendue et la forme anatomopathologique pour chaque cas ; on a trouvé une fracture en clapet chez 9 cas et une fracture en trappe chez 2 cas ; 6 cas ont présenté une fracture postérieure, 3 cas ont présenté une fracture antérieure et 2 cas ont présenté une fracture moyenne du cadre orbitaire. Les patients ont été opérés par abord transconjonctival dans 4 cas ; un abord sous ciliaire chez 6 cas et une voie médiale palpébrale chez 1 cas. La réparation du plancher était assurée par une greffe autologue dans 6 cas et inerte dans 5 cas. Un suivi post opératoire clinique et avec un test de Lancaster a été réalisé. L'évolution était bonne chez 10 cas et persistence de la diplopie chez 1 cas. Les fractures « blow-out » pures du plancher de l'orbite représentent une forme particulière des fractures de l'orbite. Elles sont caractérisées par un risque de séquelles à type de troubles oculomoteurs ou d'énophtalmie en cas de défaut de diagnostic. Une connaissance des éléments cliniques et radiologiques essentiellement par la TDM permet de comprendre les différentes formes de ces fractures et d'adapter la prise en charge adéquate afin d'éviter les séquelles.
Introduction: The giant cell reparative granuloma is a benign lesion of bone usually located in the maxillary. Localization in the temporal bone is exceptional. We report an observation.Observation: A 45- year-old man was referred to our department for a buzz of the left ear with hypoacousia evolving for 3 years. ORL examination objectified a lesion of the external auditory. Computed tomography scan as well as magnetic resonance imaging examination revealed an osteolytic lesion of the left temporal bone. The biopsy of the tumor through the auditory meatus concluded with an aspect in favour of giant cell reparative granuloma.The resection of tumour was almost complete. Postoperative histologic examination showed findings consistent with a giant cell reparative granuloma. No recurrence is noted one year after surgery.Discussion: Localization in the temporal bone is scarcely noted, our case is the 22(nd) reported in the international medical literature. The characteristics of this rare localization of the giant cell reparative granuloma will be developed through this new observation and the review of the literature.
Objectif de l’étude. – Nous avons examiné chez des patients atteints de carcinome de la cavité buccale ou de l’oropharynx la relation entre le nombre de ganglions envahis dans la pièce de curage cervical et l’évolution de la maladie évaluée par les probabilités de survie et de contrôle locorégional.Patients et méthodes. – Il s’agit d’une étude rétrospective qui a concerné 183 patients traités de 1976 à 1993, 79 (43 %) pour une tumeur primitive de l’oropharynx et 104 (57 %) pour une tumeur de la cavité buccale. Tous les patients ont eu un curage ganglionnaire cervical emportant au moins six ganglions, 159 (87 %) ont eu une chirurgie d’exérèse de la tumeur primitive, 158 (86 %) ont eu une radiothérapie postopératoire de dose médiane de 60 Gy (écart-type de 10 Gy). Les patients atteints de tumeur métastatique ou d’une tumeur synchrone n’ont pas été inclus dans l’étude. Le recul d’observation moyen était de 52 mois.Résultats. – Le taux de survie globale de la population à cinq ans était de 42,6 %. Les taux de survie globale à cinq ans des patients sans envahissement (pN0), ou avec la présence d’un, de deux et plus de deux ganglions envahis étaient respectivement de 60,0 %, de 39,5 %, de 28,0 % et de 24,4 % (p = 0,000 4). Il n’y avait pas de corrélation entre la survie spécifique sans maladie, le contrôle locorégional et le degré d’envahissement ganglionnaire.Conclusion. – Un envahissement même limité à un seul ganglion est un facteur de pronostic défavorable qui doit inciter à la pratique d’un traitement complémentaire de la chirurgie.Purpose. – To evaluate the relationship between the number of positive nodes and probabilities of locoregional control and survival in patients with invasive squamous cell carcinomas of the oral cavity and oropharynx.Material and methods. – Between 1976 and 1993, we treated with curative intent 183 patients (median age: 56 years; standard deviation: 10 years). Seventy-nine patients (43%) had oropharyngeal primary invasive carcinoma and 104 (57%) had oral cavity (excluding the lip) primary invasive carcinoma. Patients with simultaneous primary lesion or visceral metastases were excluded from the analysis. All the patients had neck dissection with at least six nodes to analyse. One-hundred fifty-nine patients (87%) underwent resection of the primary lesion and 158 (86%) were treated postoperatively with external beam irradiation alone or combined with interstitial implant (median dose: 60 Gy; standard deviation: 10 Gy). Average follow-up was 52 months.Results. – The overall 5-year survival rate using the Kaplan-Meier method was 42.6%. The 5-year survival rates were 60.0% when lymph nodes were histologically negative, 39.5% when one lymph node was positive, 28.0% when two lymph nodes were positive and 24.4% when three or more lymph nodes were positive (P = 0.0004). The number of positive nodes did not significantly influence the specific disease-free survival and locoregional control rates.Conclusion. – Patients with one or more positive neck nodes must have postoperative treatment.
L'orbite anophtalme est différente de l'orbite nor-male ; les signes habituels de tumeur orbitaire en sont donc modifiés.Nous rapportons le cas d'une patiente de 70 ans, énucléée, chez qui est survenu un adénocarcinome anaplasique lacrymal.Le patient énucléé pose classiquement le problème du syndrome de l'orbite anophtalme : tolérance et bascule de l'implant, énophtalmie, approfondissement du creux sus palpébral, ptôsis ou rétraction de la paupière supérieure, voire ectropion palpébral inférieur, mais très rarement celui de tumeur intra-orbitaire. Un épisode de dacryomégalie doit faire évoquer ce diagnostic et illustre l'importance de la surveillance clinique et radiologique (TDM, IRM) de ces patients.La démarche diagnostique est discutée ainsi que l'indication thérapeutique : type d'exentération, radiothérapie et problème de reconstruction.The classical symptoms of orbital tumors are modified in the event of an anophthalmic socket.We report a case of primary malignant neoplasm of the lacrimal gland in an enucleated 70-year-old women.Patients with an anophthalmic orbit often present postenucleation socket syndrome: problems of tolerance and rocking of the ocular prothesis, enophthalmos, ptosis, lower eyelid ectropion. Development of a malignant tumor of the lacrimal gland is however exceptional. An oversized lacrimal gland is suggestive of tumor development. Rigorous clinical and radiological follow-up (CT, MRI) is essential. We discuss diagnostic and therapeutic aspects including the type of exenteration, radiotherapy, and reconstruction technique.
The vascular anatomy of the pectoralis major muscle and overlying skin and subcutaneous tissue was investigated in seventeen cadaver specimens by cannulation of the arterial supply and injection of contrast medium. A rich anastomosis between the internal mammary artery and the pectoral branch of the thoraco-acromial artery and between the pectoral branch and the lateral thoracic artery was demonstrated. In 36% of cases a major axial cutaneous vessel, originating close to the thoraco-acromial artery, was shown to extend caudad.The lateral thoracic artery, in addition to the pectoral branch, contributed significantly to the vascularity of the pectoralis major muscle and was the main blood supply to the female breast.The skin overlying the muscle was abundantly supplied from the muscle vasculature. Both the major arteries were enclosed within the muscle's fascial envelope.A technique for developing a standard pectoralis major myocutaneous island flap is described. Our studies show that a hemichest flap incorporating all the skin of the chest over the muscle is feasible.
