本研究报告一例晚期贝尔面瘫的老年男性患者,术前体格检查:安静时,右面部松弛下垂,右侧额纹消失,眉位置低于健侧,睑裂增大,下睑袋明显,右鼻唇沟浅,鼻根歪向健侧,口角向健侧歪斜.面部表情活动时,抬眉无力,右眼睑闭合不全,巩膜外露,鼓腮漏气,笑时口角歪斜加重,暂无吞咽困难及流口水表现.术前House-Brackmann量表评分为Ⅴ级.通过SMAS筋膜除皱联合阔筋膜悬吊极大的恢复了患者休息时面部对称性,减少了患者痛苦.
背景:脂肪移植常应用于软组织填充、再造、重建及美容等,但脂肪移植的成活率差异较大,寻找提高脂肪移植成活率的方法成为研究重点.目的:探究自体脂肪移植后给予不同剂量维生素C对脂肪移植成活的影响.方法:20只雌性SD大鼠,随机分成低剂量维生素C组、高剂量维生素C组、超高剂量维生素C组及对照组,每组5只.取出各组大鼠腹腔肠系膜脂肪,适当处理后取0.89颗粒脂肪移植于背部皮下直径1 cm囊袋中,术后连续7d前3组分别腹腔注射50,150,1000 mg/d的维生素C溶液1mL,对照组腹腔注射等量生理盐水.移植后8周将大鼠安乐死,取出移植物,剥除粘连纤维结缔组织后进行大体观察、称移植物留存质量、苏木精-伊红染色、CD34免疫组织化学染色.结果 与结论:①3个维生素C组的脂肪质地较对照组柔软,表面有更多的细小血管,脂肪移植物质量均大于对照组,且以高剂量维生素C组效果最明显(P<0.001):②3个维生素C组均可减少囊泡形成,高剂量维生素C组、超高剂量维生素C组在减轻炎性浸润及纤维化上更显著(P<0.05);③高剂量维生素C组、超高剂量维生素C组的毛细血管密度较大,均高于对照组(P<0.001);④结果表明,高剂量维生素C可以提高大鼠自体脂肪的毛细血管密度及留存质量,减轻炎性浸润及纤维化程度,减少囊泡形成.
目的:研究自体富血小板纤维蛋白(Platelet-rich fibrin,PRF)对兔耳软骨移植后成活的影响,为临床应用提供证据.方法:选取16只新西兰大白兔,于兔双耳对称处设计并剥离出大小、形状相似,重量为80mg的耳软骨块,双侧耳软骨块随机分为对照组与实验组,对照组耳软骨块直接置入兔背部左侧浅筋膜腔隙内,实验组耳软骨块联合PRF共同置入兔背部右侧浅筋膜腔隙内.分别于2周、4周、8周、12周取材进行大体观察、重量比较及统计学分析、染色观察(HE染色、番红〇染色、阿利新蓝染色).结果:通过大体观察和软骨重量可见:2~4周耳软骨块重量增加,8~12周耳软骨块重量减小,各时间点实验组耳软骨块重量均大于对照组,两组间比较差异有统计学意义(P<0.05).染色观察可见:各时间点在相同显微倍数、同一视野下实验组软骨细胞明显多于对照组,其中新生软骨细胞也多于对照组.实验组异染性软骨基质、蓝色的糖胺聚糖均更明显.结论:自体富血小板纤维蛋白能够促进兔耳软骨移植后的存活.
Objective To investigate the effect of adding glutathion( GSH) to tumescent solution on autologous fat graft survival. Methods 14 male and female New Zealand rabbits were divided into experimental group and control group randomly, 7 in each group. Experimental group:The donor areas of the rabbits were injected with 3 ml of tumescent solution with GSH. Control group:The donor areas of the rabbits were injected with 3 ml regular tumescent solution. DCFH-DA probe was used for fluorescent staining of harvested fat cells. Then stained fat cells were measured for the intracellular reactive oxygen species ( ROS ) content by fluorescence microplate. The grafts were harvested at 3 months after transplantation and assessed by general observation, volume measurement, wet weight measurement, HE staining for the number of fat cells, and CD34 immunohistochemical staining for the measurement of micro-vascular density. T test was performed by using SPSS 24. 0. Results The intracellular ROS content of harvested fat cells in experimental group was lower than that in control group, and the difference was statistically significant ( P<0. 05 ) . At 3 months after transplantation, the wet weight of fat grafts in experimental group was (1133. 21±87. 97) mg and that in control group was (718. 79±79. 27) mg. The volume of fat grafts in experimental group was ( 1. 00 ± 0. 04) ml and that in control group was ( 0. 59 ± 0. 03) ml. The number of fat cells in experimental group was (746. 6±15. 7)/10 high magnification vision and that in control group was ( 350. 1 ± 32. 4 )/10 high magnification vision. The density in group experimental was (8. 1±2. 0)/high magnification vision and that in control group was (6. 7±2. 4)/high magnification vision. The grafts'volume, wet weight, and the number of fat cells in experimental group were higher than those in control group, and the differences were statistically significant (P<0. 05). The difference of micro-vascular density between experimental group and control group had no statistical significance ( P>0. 05) . Conclusions The addition of GSH to tumescent solution optimizes the process of autologous fat harvest, thereby improving the survival of fat graft.
目的:探索改良型富血小板纤维蛋白(advance platetel-rich fribin,A-PRF)与普通型富血小板纤维蛋白(platetel-rich fribin,PRF)在脂肪移植中的应用效果,进而为临床应用提供指导.方法:将18只实验兔随机分成三组,实验组(A-PRF组):取兔背部脂肪组织,将2ml A-PRF+1ml颗粒脂肪移植到兔耳分离好的区域;阳性对照组(PRF组):兔耳移植区移入2ml PRF+1ml颗粒脂肪;阴性对照组(NS组):兔耳移植区移入2ml生理盐水+1ml颗粒脂肪.分别于4周、8周、12周取材进行大体观察、称重、HE染色与免疫染色来比较三组间脂肪成活差异.结果:各时间点,A-PRF组的脂肪存活率及新生毛细血管密度明显优于其他两组,PRF组结果优于NS组,且三组间比较差异有统计学意义(P<0.05).结论:A-PRF在增加脂肪细胞成活率、减轻炎症反应及促进毛细血管新生方面优于普通型PRF,这为A-PRF的临床使用提供参考.
Chinese hamster ovary (CHO) cells have become the mainstream in expression systems for producing therapeutic antibodies.The cultivation of CHO cells and expression of antibody are extremely related to culturing environment.We previously generated a fusion antibody named JZB01 targeting vascular endothelial growth factor receptor 2 (VEGFR2) and natural killer cells with significant anti-tumor activity but limited production.In this study,we aim to develop a robust method for fed-batch fermentation using design of experiments (DOE) with Minitabl 6.0,and evaluate the influence of commercial media,supplements and culture conditions on cell viability and antibody production.To improve the yield of fusion protein and establish a strategy for further study,the optimization of the production was undertook with orthogonal experimental analysis for four factors and the level was assessed by the response.Furthermore,in order to sustain the stability and uniformity of fusion protein for further research and diagnosis,the bioprocess was scaled up to a 3-L bio-reactor under the conditions of commercial medium CDM4PerMAb,supplemented with Boost2 + Boost5,12% (V/V) volume,1 × 106 cells/mL of initial density and 2 days of feeding.The final yield was 54.45 mg/L and Western blot was adopted to identificated the affinity of fusion protein preliminarily,which was characterized as a homodimer with binding ability to each receptor.By the investigation for anti-angiogenesis function of JZB01 on HUVEC cells and anti-tumor function on MDA-MB-231 cells and K562 cells,the MrTT assay confirmed that the fusion protein had anti-angiogenic effect and enhanced anti-tumor activities.Thus,by establishing a platform to obtain fusion protein with high molecular weight in CHO cells system along with moderate costs,this study provides a robust methodology for producing therapeutic antibodies at laboratory level.And more trials will be taken to enlarge the application to industrial scale which could be applied in vitro and vivo preclinical tests.

OBJECTIVE To explore the preventive measures for nosocomial infections in postanesthesia recovery room(PACU).METHODS The understanding of hospital infection for medical personnel was improved through the regular disinfection and monitoring for environment air,the rational distribution of the PACU and settings,the prevention and control measures of PACU staff,the sterilization management measures of reusable items on PACU and the processing measures of pollution item.RESULTS The results of air cultured monthly were satisfactory.Since the establish of PACU,happened infection case associated with the PACU of nosocomial infections on our hospital since Dec.2009.CONCLUSION It can supervise the staff at all levels to strictly implement the rules and regulations of the disinfection and isolation,effectively reducing the indirect infection way.Through the practice,the explored measures to prevent hospital infection are effective,it is worthy to be promoted.
