La méthadone est utilisée dans de nombreux pays comme traitement antalgique des douleurs liées au cancer. Nous rapportons le cas d'un patient ayant bien répondu à la méthode d'analgésie autocontrôlée préconisée comme méthode d'équilibration par le protocole de l'Afssaps 2010.Methadone is currently used in many countries for the relief of cancer-related pain. We report the case of a patient in palliative care who was successfully treated with patient-controlled methadone analgesia protocol, the French National Drug Agency.
La migraine est une maladie cérébrale complexe qui implique dans sa physiopathologie de nombreux circuits neuronaux et de nombreux neurotransmetteurs. Le point de départ de la crise migraineuse reste l’objet d’un grand débat, mais l’hypothalamus, avec ses multiples connexions avec d’autres parties du système nerveux central et son contrôle souverain sur la glande hypophysaire et le système nerveux autonome, est un candidat très sérieux. Beaucoup des symptômes prémonitoires de la crise trouvent leur origine dans l’hypothalamus. Le lien entre les fluctuations hormonales et la migraine chez la femme trouve son origine dans l’hypothalamus. La mise en jeu de l’hypothalamus a un effet anti-nociceptif largement démontré, mais cela n’exclut qu’il puisse aussi induire la douleur migraineuse par un mécanisme de désinhibition des voies centrales de la douleur. L’hypothalamus contrôle l’équilibre des systèmes sympathique et parasympathique, largement impliqués dans la migraine. Les orexines, hormones d’origine hypothalamique, sont impliquées dans l’équilibre des cycles du sommeil, de l’alimentation et de la thermorégulation et dans les fonctions neuroendocriniennes, autonomiques et nociceptives. Elles pourraient jouer un rôle déterminant dans l’origine de la crise migraineuse et expliquer l’influence du sommeil, du comportement alimentaire et du surpoids dans la survenue des crises. Une activation de l’hypothalamus a été mise en évidence en TEP dans les premières heures de la crise migraineuse, mais en l’absence d’enregistrements faits pendant la phase prémonitoire, il est impossible de dire si cette activation est la cause ou la conséquence de la douleur migraineuse.Migraine is a complex brain disease. Les céphalées dites circonstancielles « à la toux, à l'effort et à l'activité sexuelle » sont incluses par l'International Headache Society dans le groupe 4 des « autres céphalées primaires ». Notre travail a pour but de réaliser une étude clinique et neuroradiologique chez les patients présentant ce type de céphalées circonstancielles afin d'essayer d'avancer dans la compréhension des mécanismes physiopathologiques de ces céphalées. Il s'agit d'une étude nationale multicentrique menée au cours de laquelle les patients ont été évalués cliniquement par un même questionnaire et ont été explorés avec une IRM cérébrale et une véno IRM. Cette dernière a été réalisée selon des séquences préétablies et interprétées de manière consensuelle par deux neuroradiologues séniors, aveugle vis-à-vis du diagnostic clinique des sujets. Une VRM a été également réalisée au sein d'un groupe de patients « témoins » non céphalalgiques venant pour un dépistage ou un bilan d'extension. Quarante et un patients présentant une céphalée circonstancielle ont été recrutés au sein de six centres français : 20 patients avec une céphalée liée à l'activité sexuelle, 11 avec une céphalée à l'effort et 10 patients avec une céphalée à la toux. Les patients présentant une céphalée à la toux ont une moyenne d'âge plus élevée, une comorbidité avec la migraine moins fréquente que dans les céphalées liées à l'effort ou à l'activité sexuelle. Les céphalées d'effort et liées à l'activité sexuelle ont une présentation clinique semblable caractérisée par une céphalée de durée plus longue que celle observée lors de la céphalée à la toux. L'association avec la maladie migraineuse est plus fréquente. Sur le plan neuroradiologique, la VRM révèle des modifications du système veineux situées soit sur les sinus transverses, soit sur les jugulaires. Dans la population céphalée à la toux, un seul patient a un système veineux normal ou avec des modifications de calibre < 50%. Dans le groupe des céphalées à l'effort les patients ne semblent pas avoir de lésions jugulaires significatives, mais ce segment n'a pas été exploré de façon constante en fonction des centres. Enfin, les patients présentant des céphalées liées à l'activité sexuelle ont des sténoses veineuses à la fois sur les sinus transverses et les jugulaires. Les patients présentant une céphalée circonstancielle semblent présenter un nombre plus important de modifications du système veineux cérébral. Cependant ce type de céphalée reste relativement rare et cette hypothèse nécessite d'être confirmée sur un échantillonnage plus important afin de voir la proportion précise de patients présentant des modifications du système veineux cérébral.
A migraine attack is due to the development of a vasomotor storm within the brain and overlying meninges. The crisis is initiated in the hypothalamus and in the adrenergic and serotonergic nuclei situated in the midbrain and pons. Activation of these nuclei results in arteriolar vasoconstriction in the cerebral cortex and also disturbs the inhibitory mechanisms involved in pain regulation. In order for the crisis to develop, there has to be a genetic predisposition (migraine susceptibility). In a rare migraine variant, familial hemiplegic migraine (FHM), various genetic mutations have been described. These affect the functioning of ionic channels located within the neuronal and astrocyte membranes, thus producing abnormal excitability of the cerebral cortex and increasing the susceptibility to cortical spreading depression (CSD). The neurological symptoms of the migrainous aura result from the passage of one or more CSDs over the corresponding zone of the cortex. The CSD spreads through the cortex slowly (3-4 mm/min), which accords with the progressive development of the symptoms of the aura (aura progression). The headache derives from activation of the trigemino-vascular system (TVS). These sensory fibres travel in the 1(st)branch of the trigeminal nerve (V(1)) and innervate the meningeal veins and arteries on the cortical surface. Their main mediator is CGRP (calcitonin gene-related peptide). Release of this causes sterile inflammation around the blood vessels with vasodilation, extravasation of plasma proteins, degranulation of mast cells and activation of platelets. Thus, a substantial quantity of serotonin is released into cerebral venous blood during migraine attack. Activation of the TVS might either arise from "peripheral" initiation due to the passage of a CSD in the cortex (migraine with aura) or result from a "central" phenomenon of disturbance of mechanisms of pain regulation in the brain stem (migraine without aura).
Methods: SMILE was an observational study carried out in France among office-based general practitioners (GPs) and neurologists from November 2005 to July 2006 to assess the determinants of prescription of migraine preventive therapy in primary care medicine. A total of 1467 GPs and 83 neurologists were included, treating 5417 and 248 migraine sufferers, respectively.Results: The main factors leading physicians to deem a patient eligible for preventive treatment were perceived medication overuse and frequency of headaches, and secondarily, severity of headaches and functional impact. On the other hand, patient satisfaction with the acute treatment of attacks and triptan use, and secondarily, a long migraine history were found to influence patient eligibility negatively.Discussion/conclusion: Noticeably, psychiatric disorders (anxiety, stress) did not appear, aside from somatic factors, among the determinants that significantly influence physicians' judgment about the option of establishing a preventive treatment. However, they are important features of migraine condition and should be listed among the factors guiding choices about migraine preventive therapy.
Background: Photophobia is an abnormal sensitivity to light experienced by migraineurs during attacks. The pathophysiology of photophobia is poorly understood. Nevertheless, 2 facts appear to have a link with photophobia: visual cortex hyperexcitability on the one hand and interactions between visual pathway and trigeminal nociception on the other. Methods: We used H215O PET to study photophobia induced by continuous luminous stimulation covering the whole visual field in 8 migraineurs during spontaneous migraine attacks, after headache relief by sumatriptan and during attack-free interval. The intensity of the luminous stimulation provoking photophobia with subsequent headache enhancement was specifically determined for each patient. Results: We found that low luminous stimulation (median of 240 Cd/m2) activated the visual cortex during migraine attacks and after headache relief but not during the attack-free interval. The visual cortex activation was statistically stronger during migraine headache than after pain relief. Conclusion: These findings suggest that ictal photophobia is linked with a visual cortex hyperexcitability. The mechanism of this cortical hyperexcitability could not be explained only by trigeminal nociception because it persisted after headache relief. We hypothesize that modulation of cortical excitability during migraine attack could be under brainstem nuclei control.
This paper proposes a computationally efficient procedure for skew detection and text line position determination in digitized documents, which is based on the cross-correlation between the pixels of vertical lines in a document. The determination of the skew angle in documents is essential in optical character recognition systems. Due to the text skew, each horizontal text line intersects a predefined set of vertical lines at non-horizontal positions. Using only the pixels on these vertical lines we construct a correlation matrix and evaluate the skew angle of the document with high accuracy. In addition, using the same matrix, we compute the positions of text lines in the document. The proposed method is tested on a variety of mixed-type documents and it provides good and accurate results while it requires only a short computational time. We illustrate the effectiveness of the algorithm by presenting four characteristic examples.
La crise de migraine correspond à un orage vasomoteur qui se développe au sein du cerveau et de ses enveloppes méningées. Les générateurs de la crise sont situés dans l’hypothalamus et des noyaux adrénergiques et sérotoninergiques, situés dans le mésencéphale et la protubérance. L’activation de ces noyaux provoque une vasoconstriction artériolaire dans le cortex cérébral et aussi une dérégulation des systèmes inhibiteurs de contrôle de la douleur. Il faut pour que la crise se développe une prédisposition génétique, désignée depuis longtemps sous le terme de « terrain migraineux ». Dans une forme rare de migraine, la migraine hémiplégique familiale (MHF), on a démontré l’existence de diversesmutations génétiques qui dérèglent le fonctionnement de canaux ioniques situés sur la membrane du neurone et de l’astrocyte, provoquant ainsi une excitabilité anormale du cortex cérébral et une susceptibilité accrue à générer une dépression corticale envahissante (DCE). Les symptômes neurologiques de l’aura migraineuse sont provoqués par le passage d’une ou plusieurs DCE sur la zone correspondante du cortex. La DCE se propage sur le cortex à vitesse lente (3–4 mm/min), ce qui correspond bien au développement progressif des symptômes de l’aura (« marche migraineuse »). La céphalée migraineuse provient de l’activation du système trigéminovasculaire (STV). Ces fibres sensitives cheminent dans la première branche du nerf trijumeau (V1) et innervent les artères et les veines méningées situées sur la surface du cortex. Leur principal médiateur est le CGRP (calcitonin gene-related peptide). Sa libération provoque autour des vaisseaux une inflammation stérile avec vasodilatation, extravasation des protéines plasmatiques, dégranulation des mastocytes et activation des plaquettes, ce qui libère dans le sang veineux cérébral du migraineux en crise une quantité importante de sérotonine. L’activation du STV pourrait être soit d’origine « périphérique » du fait du passage d’une DCE sur le cortex (migraine avec aura), soit d’origine « centrale » par démodulation des systèmes de contrôle de la douleur dans le tronc cérébral (migraine sans aura).
