The aim of this study was to describe the preliminary results after reconstruction of segmental bone defects (SBDs) in the upper limb of six patients. This retrospective study included three men and three women with an average age of 35 years (range 18-62 years), who had four primary and two secondary SBDs. The average length of the SBD was 4.5 cm (2-10 cm). According to the SOFCOT classification, type I (1 case), type II (4 cases) and type III (1 case) defects were identified. The reconstruction involved the humerus in two cases and the forearm bones in four cases. A posterior plaster cast was applied in all patients during the first stage of treatment. Internal fixation was used during the second phrase (five plates, one K-wire). The mean time elapsed between the first and the second stages of treatment was 3 months (2-4 months). At the final follow-up, bone union was obtained in five patients after an average of 4.6 months (4-6 months). The progression was favorable after the first stage of the induced membrane technique. However, two cases of sepsis were observed after the second stage of treatment, one evolving to osteitis that caused graft resorption. The induced membrane technique is a sequential technique used for treating SBDs. It is an alternative method of bone reconstruction in the upper limb. (C) 2016 SFCM. Published by Elsevier Masson SAS. Ewing sarcoma is a rare disease, characterized by a severe evolution. Its diagnosis is based on radiology, scanner and IRM, and is confirmed by histology. The chemotherapy associates Vincristin, Actinomycin, Adriamycin and Cyclophosphamid (VACA). Local treatment initially based on radiotherapy, is now more and more surgical. In this article, the authors report a case of Ewing’s sarcoma of the foot (the three cuneiforms bones) in a 34-year-old man. This observation presents a particular aspect because of the age of the patient and its localization. The evolution was fatal and the patient died before treatment.

The antibacterial activity and corrosion inhibition effect of 4,5-dichloro-2-n-octyl-4-isothiazolin-3-one is studied for carbon steel immersed in artificial seawater. Results showed that this compound produced a good antibacterial activity against the metal-oxidizing bacteria. Also the electrochemical studies indicated that it totally owned a very good inhibition effect in artificial seawater with and without bacteria. Equivalent circuits have two time constants in the presence of biocide, which suggests that an adsorption layer is produced on the metal surface and thickened by varying the exposure time, as two maxima could be observed in the Bode phase plots after 12 h.
Background: The use of autologous bone graft in orthopaedic traumatology is not uncommon. But little work, from West African subregion, has been devoted to sites used as sources of autologous bone grafts. Objective: The purpose of this study was to evaluate the evolution of these different sampling sites of autologous graft. Materials and methods: The authors report the results of a retrospective study of 84 patients operated on between 2000 and 2005 for various bone diseases requiring autologous bone graft at CHU Treichville (in Abidjan). Excluded from this study were bone grafts sourced from femoral heads harvested during arthroplasty of the hip. The series consist of 52 men and 32 women. Sites of bone autografts were iliac in 55 patients, 18 ends of the upper tibia, 7 olecranons, 3 fibulas and 1 greater trochanter of the femur. The evaluation of patients was clinical and radiographic and included the existence or not of pain, stiffness, assessment of the scar and the existence of a local deformation. Results: Outcome was satisfactory in 72 patients. 6 patients reported pain in the iliac crest. In 11 patients, the scar to the iliac crest was unsightly. Conclusion: The authors conclude that the commonly used donor sites for autologous bone grafts are safe. Objective: To assess the surgical treatment of pseudoarthrosis of the patella in the orthopaedic and traumatology unit of the Teaching Hospital of Treichville (Abidjan- Côte d\'Ivoire). Methods: Six patients (3 males and 3 females) of 43 years average age (from 36 to 48 years old) with pseudoarthrosis of the patella were admitted in the unit. The lesions were caused by road traffic accident in one patient and domestic accidents in 5 patients. The average time before reporting to the hospital for consultation was 130 days [93- 181 days]. A pseudoarthrosis was due to failure of an initial osteosynthesis and 5 were due to late treatment subsequent to traditional bone setting. In every case, surgery was done with tension band wiring, without graft (4 cases) and associated with cancellous bone graft in two cases. All the patients were reviewed at an average time of 44 months later [range from 28 to72 months] and examined according to Bostman score. Result: No immediate and secondary post operative complication was observed. We noted 2 cases of osteopenia of the patella, a patellofemoral arthrosis in 4 cases, and a lower post operative patella in every patient with an average index of Caton- Deschamps at 0.34 cm (From 0.15 to 0.51 cm) were observed. Two malunion of fractures and an amyotrophy compared with healthy thigh were observed. The average Bostman functional score was excellent (28.83 points/30). Conclusion: Pseudoarthrosis of the patella in our centre is the consequence of traditional bone setting or late treatment. Osteosynthesis with or without graft of these lesions allow bone union despite a functional insufficiency which does not disturb the patient\'s physical activity. Les auteurs rapportent un cas de gangrène sèche du pied gauche observé dans le service de traumatologie du CHU de Treichville en décembre 2005. Cette complication est survenue chez une patiente immunodéficitaire aux virus 1 et 2 du VIH et sous traitement antituberculeux (RHZ). Aucun antécédent pathologique particulier n’a été retrouvé. Cette ischémie sévère a été traitée par amputation au tiers supérieur de la jambe. Les suites opératoires ont été simples. Après un recul de 15 mois, la patiente est en bon état général. Cette observation pose le problème du diagnostic étiologique d’une gangrène ischémique chez une patiente doublement immunodéprimée.

La reduction chirurgicale a necessite des interventions sur les os et sur les tissus mous. L’evolution a long-terme se deteriore sur le plan radiographique tandis que les resultats fonctionnels demeurent satisfaisants. En pratique courante, les luxations de la rotule sont des lesions traumatiques rares auxquelles sont attribuees une origine congenitale responsable d’une anomalie morphologique du genou (genu valgum, genu flexum, genu recurvatum, dysplasie, etc.) ou de l’appareil extenseur (hyperlaxite, instabilite, etc.). Les luxations traumatiques sont habituellement recentes et accessibles au traitement orthopedique. De ce fait la decouverte d’une luxation traumatique negligee de rotule constitue une observation clinique exceptionnelle.
The authors report a case of dry gangrene of the left foot observed in the trauma service at Treichville university hospital in December 2005. This complication arose in a patient with both HIV-1 and HIV-2, who was also being treated for tuberculosis (RHZ). No particular pathological antecedent was found. This hard ischemia was treated by amputation of 1/3 of the upper leg. The follow-up to the operation was straightforward. Fifteen months after the operation, the patient is in good general health. This observation reveals the problem of etiologic diagnosis of an ischemic gangrene in a doubly immunodeficient patient.
OBJECTIVES:Determine the frequency of early chirurgical recaptures, their causes and therapeutic indications in order to prevent them.INTRODUCTION:The early chirurgical recaptures have been proposed to prevent early post operative infections. This repeat intervention must stop post operative sepsis and the evolutive process. The therapeutic indications have been opened to different complications as mechanic and technical complications. This study permitted to determine the causes of this treatment and its interest in our daily hospital practice.TOOLS AND METHODS:This retrospective study covered a period of 5 years between January 2000 and December 2005. During this period, 2562 operations have been realised in our service, and 68 cases (2.65%) concerned early chirurgical recaptures. Only 46 patients were concerned by the study according to our criteria which was that the early intervention would be realised before 4 weeks after the first operation.RESULTS:The average age of patients was 37.6 does not equal 13.3 years with a frequency of ages from 20 to 39 years about 47.8%. The initial injuries were varied and dominated by open fractures (12 cases) and foot diabetic gangrenes (08 cases). The deadline of the help and technical short comings has equally formed etiologic factors.CONCLUSION:The indications of early recaptures in orthopaedic traumatology are various. The knowledge of these indications is necessary for the prevention of certain post-operation difficulties.
Le kyste osseux anévrismal est une tumeur rare. Les auteurs en rapportent un cas dans une localisation claviculaire. Ils rappellent les circonstances du diagnostic et les modalités thérapeutiques propres à cette lésion. La survenue du kyste dont l'observation est rapportée, est liée à des perturbations hémodynamiques vasculaires locales. Sa découverte a été faite dans un contexte post-traumatique, sa nature exacte a été confirmée par l'examen anatomopathologique. L'exérèse radicale, à l'opposé des traitements conservateurs, semble éviter la récidive. En définitive, la résection segmentaire de la clavicule dont l'exérèse a peu de conséquence fonctionnelle, peut être utilisée lors de la découverte d'un kyste osseux anévrismal évolué dans cette localisation.
Aneurismal bone cyst is a rare tumour. The authors report an exceptional case in its clavicle localization. They review the diagnosis circumstances and treatment modalities of this pathology. The occurrence of the cyst is linked to vascular homodynamic disturbance and its discovery in a post-traumatic context has been assessed by the histology exam. Although numerous therapeutic trials, radical removal versus conservatory methods avoid relapse. Finally, resection of the clavicle leading to few functional consequences should be recommended for the cure of evoluted aneurismal bone cyst.
