Résumé Les tumeurs neuroectodermiques primitives se voient rarement au niveau de la région cervicofaciale. Elles sont localisées habituellement au niveau de l’orbite et de la glande parotidienne. La localisation nasopharyngée est exceptionnelle. Nous rapportons le cas d’un homme de 48 ans, présentant une obstruction nasale droite ainsi qu’une tuméfaction cervicale évoluant depuis six mois. L’endoscopie nasale trouve une formation bourgeonnante du cavum. L’examen histologique a montré une prolifération tumorale maligne à petites cellules rondes, avec une forte positivité à l’anti-synaptophysine, à l’anti-NSE ainsi qu’à l’anti-CD99. Le patient a été traité par radiochimiothérapie. On n’a pas noté de récidive avec un recul de cinq ans.

Le teratome de la parotide est une malformation tumorale vestigiale rare contenant des derives plus ou moins differencies, des trois feuillets embryonnaires. A la lumiere d’une nouvelle observation et des donnees de la litterature, on se propose de mettre l’accent sur les aspects cliniques, histologiques et therapeutiques de cette affection exceptionnelle dans cette localisation. The TSH receptor (TSHR) is the major regulator of thyroid hormone biosynthesis in human thyrocytes by regulating the transcription of a number of genes including thyroglobulin (TG) and thyroperoxidase (TPO). Until recently, it was thought that TSHR initiated signal transduction pathways only at the cell-surface and that internalization was primarily involved in TSHR desensitization and downregulation. Studies primarily in mouse cells showed that TSHR internalization regulates gene transcription at an intracellular site also. However, this has not been shown for genes involved in thyroid hormone biosynthesis in human thyrocytes. We used human thyrocytes in primary culture. In these cells, the dose-response to TSH for gene expression is biphasic with low doses upregulating gene expression and higher doses decreasing gene expression. We used two approaches to inhibit internalization. In the first, we used inhibitors of dynamins, dynasore and dyngo-4a. Pretreatment with dynasore or dyngo-4a markedly inhibited TSH upregulation of TG and TPO mRNAs, as well as TG secretion. In the second, we used knockdown of dynamin 2, which is the most abundant dynamin in human thyrocytes. We showed that dynamin 2 knockdown inhibited TSHR internalization and decreased the TSH-stimulated levels of TG and TPO mRNAs and proteins. Lastly, we showed that the level of the activatory transcription factor phosphorylated cAMP response element binding protein (pCREB) in the cell nuclei was reduced by 68% when internalization was inhibited. We conclude that upregulation of genes involved in thyroid hormone synthesis in human thyrocytes is, in part, dependent on internalization leading to nuclear localization of an activated transcription factor(s).
But de l'étude : Cette étude rétrospective avait pour but de rapporter les résultats du traitement chirurgical dans une série de 210 patients opérés pour un goitre cervicothoracique.Patients et méthode : De 1982 à 1996, 210 patients porteurs d'un goitre cervicothoracique, dont 80 % de femmes, ont été opérés par voie cervicale, sauf deux par sternotomie. Deux patients qui avaient des contre-indications opératoires n'ont pas été opérés. Vingt-cinq patients ont été opérés pour un goitre récidivé plongeant. L'intervention a consisté en une thyroïdectomie totale dans 27 % des cas, une thyroïdectomie subtotale dans 33 % des cas et une lobectomie dans 40 % des cas. Dans 160 cas l'extraction du prolongement rétrosternal a été facile. Dans 48 cas, l'exérèse du goitre plongeant a été facilitée par la découverte du nerf récurrent à son point d'entrée dans le larynx et la dissection vers le bas des attaches trachéales du lobe. La libération complète de la portion cervicale a permis l'ascension de la partie thoracique même volumineuse.Résultats : Trois carcinomes papillaires ont été découverts. Une paralysie récurrentielle transitoire est survenue chez 7,2 % des patients et une hypoparathyroïdie transitoire chez 13,4 % des patients. Une paralysie récurrentielle définitive a été observée dans 1,2 % des cas, une hypoparathyroïdie persistante dans 2,1 % des cas. Parmi les 25 patients traités pour une récidive de goitre, trois ont eu une parésie récurrentielle transitoire, un une paralysie récurrentielle définitive, quatre (20 %) une hypocalcémie transitoire et un (4 %) une hypocalcémie persistante.Conclusion : Le scanner cervico-médiastinal et l'IRM permettent d'évaluer les prolongements thoraciques du goitre. L'exérèse thyroïdienne a été réalisée 208 fois sur 210 par voie cervicale. La dissection complète de la partie cervicale du goitre facilite l'ascension du prolongement thoracique.Substernal goiters. Report of 212 cases.Aim of the study: The aim of this retrospective study was to report the results of the surgical treatment in a series of 210 patients operated on for substernal goiters.Patients and method: From 1982 to 1996, 210 patients with substernal goiters, including 80 % of women, were operated on via a cervical approach in 208 cases, via a sternotomy in two cases. Two patients with operative contra-indications were not operated on. Twenty-five were operated on for a substernal recurrence of a goiter. In 160 cases, extraction of the substernal portion was easy. In 48 cases, removal of the substernal portion was facilitated by the discovery of the recurrent nerve at its entering into the larynx and a downward dissection of the tracheal attachments of the lobe. The complete dissection of the cervical portion made easier the ascension of the substernal portion even in very large substernal components.Results: Three papillary carcinomas were diagnosed. A transient laryngeal nerve palsy occurred in 7,2% of the patients and a transient hypoparathyroidism in 13,4%, A definitive laryngeal nerve palsy occurred in 1,2% of the patients, and a persistant hypoparathyroidism in 2,1%. Of the 25 patients who underwent surgery for recurrence of a goiter, three (12%) developped a transient laryngeal nerve palsy, one (4%) a permanent nerve palsy, four (20%) a transient hypoparathyroidism and one (4%) a persistent hypocalcemia.Conclusion: CT scan and MRI are the best explorations to evaluate intrathoracic extension of substernal goiters. Thyroidectomy was performed via a cervical incision in 208 patients and via a sternotomy in two patients only. The complete dissection of the cervical portion with discovery of the recurrent nerve at its entering into the larynx, facilitates the ascension of the substernal portion even in very large substernal goiters.
To identify the chemical differences which lead to the different therapeutic effects of dried rehmannia root (DRR) and prepared rehmannia root (PRR), we compared the chemical composition of decoctions of randomly purchased DRR and PRR using ultra performance liquid chromatography (UPLC) coupled with time-of-flight mass spectrometry and high performance liquid chromatography (HPLC) coupled with evaporative light scattering detection (ELSD) with the aid of multivariate statistical analysis. Both approaches clearly revealed compositional and quantitative differences between DRR and PRR. UPLC-MS data indicated stachyose, rehmaionoside A (or rehmaionoside B), acteoside (or forsythiaside, or isoacteoside), 6-O-coumaroylajugol (or 6-O-E-feruloylajugol, or 6-O-Z-feruloylajugol) as important discriminators between DRR and PRR decoctions. HPLC-ELSD analysis showed that the content of fructose in the decoctions of PRR was about four times greater than that of DRR (P<10−5), while sucrose content in the decoctions of PRR was only about one seventh of that in DRR (P<0.01). Our results suggest that some compounds, such as fructose, stachyose and rehmaionoside, may be responsible for the differing therapeutic effects of DRR and PRR. Furthermore, improvements in quality control for PRR, which is currently lacking in the Chinese Pharmacopoeia, are recommended.
Introduction: Nasal meningocele, which is usually congenital, is a rare anomaly resulting from meningeal herniation into the nasal cavities through a bone defect in the skull base.Case report: An 8-day-old boy was referred with respiratory disturbance and nasal obstruction. Examination showed a cyst-like grayish swelling filling the right nasal cavity. CT scan showed opacity, of fluid-like density, filling the right nasal fossa, in contact with a small bony defect in the right cribriform plate. MRI ruled out herniated brain parenchyma and enabled diagnosis of meningocele. The patient was operated on at the age of 2 months through a transnasal endoscopic approach. Immediate postoperative course was favorable. MRI control at 8 months was normal.Discussion: Modern imaging (CT scan and MRI) is of paramount importance in the preoperative evaluation of nasal meningocele. Endoscopic endonasal (EE) surgery is currently the treatment of choice.Conclusion: Steady progress in instrumentation, technique and skills will increase the feasibility of skull-base surgery using an endonasal approach in the pediatric population. Introduction : Le carcinome adenoide kystique anciennement appele cylindrome est une tumeur epitheliale maligne rare representant 10% des tumeurs malignes nasosinuiennes. Objectif : Nous rapportons un cas de carcinome adenoide kystique du sinus maxillaire et nous discutons particularites cliniques, histologiques, les modalites therapeutiques ainsi que les facteurs pronostiques de cette entite. Observation : Il s’agit d’une patiente âgee de 40 ans, ayant consulte pour une obstruction nasale chronique gauche evoluant depuis plus d’une annee associee a une anosmie et une epistaxis unilaterale recidivante. L’endoscopie nasale avait objective une formation polyploide blanchâtre comblant la fosse nasale gauche. Le scanner du massif facial avait montre un comblement de la fosse nasale et du sinus maxillaire gauches avec erosion osseuse. Une biopsie de la lesion etait en faveur d’un adenome pleomorphe, la patiente a ete operee par voie para latero-nasale gauche. L’histologie definitive a conclu a un carcinome adenoide kystique. Le bilan d’extension a distance etait negatif. La patiente a eu une radiotherapie post operatoire. On n’a pas note de recidive apres un recul de 4 ans. Conclusion : Le carcinome adenoide kystique nasosinusien est une tumeur rare souvent diagnostiquee a un stade avance du fait d’une croissance lente. Son potentiel evolutif est marque par une incidence elevee de recidives locales et des metastases a distance pouvant apparaitre plusieurs annees apres le traitement de la tumeur primitive. Introduction : Adenoid cystic carcinoma initially called cylindroma is a rare malignant epithelial tumor, that accounts for 10% of all malignancies of the sinonasal tract. Purpose : we report a case of adenoid cystic carcinoma of the maxillary sinus, and discuss through a review of literature its histoclinical features, therapeutic modalities and prognosis factors. Case report : A 40-year-old woman presented with a left chronic nasal obstruction associated with anosmia and recurrrent homolateral epistaxis. Nasal endoscopy revealed white polypoid mass of the left nasal cavity. A computed tomography performed, showed a filling of left sinonasal cavities with bone invasion. A biopsy made the diagnosis of plemorphic adenoma. A paralatero-nasal external surgery was done. Final histological examination concluded to the diagnosis of cystic adenoid carcinoma. Post operative radiotherapy was performed. There were no local recurrence or distant metastases during 4 years of follow-up. Conclusion : Sinonasal adenoid cystic carcinoma is a rare tumor characterized by a slow growth rate. The best treatment is complete surgical resection. This tumor is characterized by the occurrence of local recurrence and distant metastases often several years after treatment of the primary tumor. Les méningocèles nasales sont des anomalies rares, le plus souvent congénitales, qui résultent d’une hernie des méninges dans la cavité nasale à travers un défect osseux de la base du crâne (BDC). Un nouveau-né âgé de huit jours nous a été adressé pour une gêne respiratoire et une obstruction nasale. L’examen a objectivé un comblement de la fosse nasale droite par une formation grisâtre d’allure kystique. Le scanner a montré un comblement de la fosse nasale droite par une formation de densité liquidienne au contact d’un petit défect osseux au niveau de la lame criblée. L’IRM a écarté la présence de parenchyme cérébral en intralésionnel et a permis de poser le diagnostic de méningocèle. Le patient a été opéré à l’âge de deux mois par voie endonasale endoscopique. Les suites postopératoires immédiates étaient simples. L’IRM de contrôle pratiquée après huit mois était normale. L’imagerie (TDM et IRM) occupe une place fondamentale dans le bilan préopératoire des méningocèles nasales. La chirurgie endonasale sous contrôle endoscopique est actuellement le traitement de choix. Des progrès constants de l’instrumentation, de la technique et des compétences permettront d’augmenter la faisabilité de la chirurgie de la BDC par voie endonasale dans la population pédiatrique.
Évaluer l’efficacité du dosage en post-mortem de la troponine I cardiaque dans l’identification des lésions myocardiques quelle qu’en soit la cause.C’est une étude prospective intéressant 72 corps ayant fait l’objet d’une autopsie médico-légale au service de médecine légale de l’hôpital universitaire de Fattouma Bourguiba de Monastir. Sont exclus de l’étude les cas où le délai post-mortem dépassait 48 heures et ceux ayant fait l’objet d’une réanimation. Dans tous les cas nous avons procédé à un prélèvement du sang cardiaque, du sang périphérique et du liquide péricardique lors de l’autopsie avec dosage de la troponine I cardiaque respectivement dans ces trois milieux. Nous avons étudié la corrélation entre les taux de troponine I cardiaque dans les différents sites de prélèvement avec l’existence ou non de lésions myocardiques à l’examen histologique du cœur. Les courbes receiver-operator characteristic (ROC) ont été dressées et les aires sous la courbe ont été calculées. Les logiciels statistiques que nous avons utilisés sont SPSS (version 12.0) et MedCalc Statistical Software (version 11.0). Les résultats sont considérés significatifs si p < 0,05.Les taux de troponines I cardiaques dans le liquide péricardique, le sang cardiaque et le sang périphérique sont corrélés significativement avec la présence de lésions myocardiques avec une valeur de p respectivement de 0,0007, 0,0009 et 0,004. Pour une valeur seuil de 108 ng/ml, le liquide péricardique a montré la meilleure sensibilité et la meilleure spécificité dans la détection des lésions myocardiques avec une aire sous la courbe ROC de 0,925.Le dosage post-mortem de TnIc a un intérêt primordial ; d’une part, il peut servir d’outil adjonctif valable pour soutenir le diagnostic d’une lésion myocardique à l’origine du décès dans les cas ou les résultats morphologiques post-mortem sont peu concluants. D’autre part, le dosage de TnIc peut être utile dans le triage des corps en vue d’indiquer ou non la pratique d’une autopsie complète.The aim of this study is to analyse the diagnostic efficacy of post-mortem dosage of cardiac troponine I in cadaver fluids in detection of myocardial damage.Our study is prospective, interesting 72 corps autopsied at the Department of Forensic Medicine of the University Hospital Fattouma Bourguiba of Monastir-Tunisia. Were excluded from the study, resuscitated cases and those examined more than 48 h after death. Levels of cardiac troponine I were measured in pericardial fluid, cardiac blood and peripheral blood. Statically significant correlations between different variables levels of cardiac troponine I and cardiac damage were studied. Receiver-operator characteristic (ROC) curves were generated and areas under the curves were determined. SPSS (version 12.0) et MedCalc statistical software (version 11.0) were used for statistical analysis. Results were considered to be statistically significant when P < 0.05.Cardiac troponin I levels in pericardial fluid, cardiac and peripheral blood are correlated significantly between subject with and without observable signs of myocardial damage with a P value respectively at 0.0007, 0.0009 and 0.004. ROC curves analysis showed that the pericardial fluid have the best sensibility and specificity with a cut-off level at 108 ng/ml and an area under the curve at 0.925.Our data indicate that cardiac troponin I may be a powerful aid in the diagnosis of myocardial damages. This biological test can be used in triaging sudden deaths before to external examination versus complete autopsy.
Nous rapportons une observation d'un papillome inversé originale par sa double localisation à la muqueuse nasale droite et à l'oreille moyenne homolatérale, ses multiples récidives et son extension aux structures intracrâniennes ayant réalisé un syndrome de masse et mis en jeu le pronostic vital de la patiente. Les papillomes inversés sont exceptionnellement multifocaux et la localisation à la muqueuse tympanique a rarement été décrite.We report a rare case of an inverted papilloma with an unusual clinical course: development in the middle ear, multiples reccurences, and invasion of the temporal bone.
