Background: Medial meniscal ramp lesion (MMRL), lateral meniscus root tear (LMRT), and anterolateral ligament (ALL) tear are individual injuries that have been described in patients who have an anterior cruciate ligament (ACL) tear. However, the prevalence of these lesions and their combination has not been defined. Purposes: To define the individual and combined prevalence of MMRL, LMRT, and ALL tears in a case series of patients undergoing ACL reconstruction and to identify the risk factors for combined injuries. Study Design: Cross-sectional study; Level of evidence, 3. Methods: Patients aged >15 years undergoing primary ACL reconstruction between January 2019 and June 2021 were enrolled in the study. A preoperative ultrasound scan was performed to look for an ALL tear. The presence of MMRL and LMRT was determined during a standardized arthroscopy exploration. A multivariate logistic regression model was used to determine the individual effect of patient variables on the risk of associated single, dual, triad, or tetrad injuries (MMRL, LMRT, ALL, ACL), represented by an adjusted odds ratio. Results: The case series consisted of 602 patients who underwent primary ACL reconstruction. An isolated ACL injury was present in 147 patients (24%). A dual injury was detected in 34 patients (6%) who had ACL-MMRL, 16 (2.65%) who had ACL-LMRT, and 265 (44%) who had ACL-ALL. A triad injury was detected in 80 patients (13.28%) who had ACL-ALL-MMRL, 36 (6%) who had ACL-ALL-LMRT, and 3 (0.5%) who had ACL-MMRL-LMRT. A tetrad injury pattern was detected in 21 patients (3.5%). Multivariate analysis showed that the occurrence of tetrad injury was significantly lower in older patients (adjusted odds ratio by year, 0.93 [95% CI, 0.88-0.99]; P = .028). Identifying LMRT increased the likelihood of finding MMRL-ALL injuries by 2.11 times (95% CI, 1.09-3.12; P = .031). Conclusion: Isolated ACL tear is less common than combined injuries, which are quite frequent. Younger age is a risk factor for combined injuries. The search for damaged secondary stabilizers of the knee must be meticulous and systematic, especially when 1 injured structure has already been diagnosed.
OBJECTIVES:The aim of this literature review was to establish consensus with respect to the anatomic features of the articular branches innervating the hip joint and the distribution of sensory receptors within its capsule.METHODS:Five electronic databases were queried, with the search encompassing articles published between January 1945 and June 2019. Twenty-one original articles providing a detailed description of sensory receptors around the hip joint capsule (n=13) and its articular branches (n=8) were reviewed.RESULTS:The superior portions of the anterior capsule and the labrum were found to be the area of densest nociceptive innervation. Similar to the distribution of nociceptors, mechanoreceptor density was found to be higher anteriorly than posteriorly. Hip joint capsular innervation was found to consistently involve the femoral and obturator nerves, which supply the anterior capsule, and the nerve to the quadratus femoris, which supplies the posterior capsule. The femoral, obturator, and superior gluteal nerves supply articular branches to the most nociceptor-rich region of the hip capsule.CONCLUSIONS:The femoral and obturator nerves and the nerve to the quadratus femoris were found to consistently supply articular branches to both the anterior and posterior capsule of the hip joint. The anterior capsule, primarily supplied by the femoral and obturator nerves, and the superior labrum appear to be the primary pain generators of the hip joint, given their higher density of nociceptors and mechanoreceptors.LEVEL OF EVIDENCE:Anatomy study, literature review.
Background: Leg-length discrepancy (LLD) is a frequent and recurring issue after total hip arthroplasty (THA). It can lead to discomfort and litigation due to the patient’s perception of this inequality. Purpose: The aim of this study was to evaluate the functional impact of postoperative LLD and length correction in patients after primary THA. We hypothesised that clinical outcomes are inversely correlated to length correction and residual discrepancy. Methods: In a prospective cohort, we included 121 patients undergoing unilateral primary THA. Patients were categorised into 3 groups based on their residual LLD using routine EOS imaging; Shorter (<-5 mm); Equal (-5 mm to +5 mm); and Longer (>+5 mm). Delta of Correction (DC) was defined as the difference between pre- and postoperative discrepancy measured on EOS imaging. Functional scores (HHS, Postel Merle d’Aubigné, and WOMAC) were measured preoperatively and at last follow-up. The average follow-up was 24 months. We evaluated the clinical and radiographic outcomes regarding the residual LLD and DC. Results: All patients had improvement in their functional scores at last follow-up. There was no difference in clinical outcomes, regardless of preoperative or residual LLD. The highest scores were found in the population with the smallest variations (DC -5 to +5 mm, HHS 96). Conversely, poorer results were found in shortened patients (DC <-5 mm), HHS 86, and lengthened patients, corrected by > 15 mm, with HHS of 83, p < 0.005. Patient perception of postoperative LLD correlated with the DC (p = 0.004). Conclusion: The DC measurement is a more effective assessment than arthroplasty.
We describe an ultrasound-guided anterolateral ligament (ALL) reconstruction technique that uses the gracilis tendon and can be added to any anterior cruciate ligament reconstruction technique. Preoperative ultrasound imaging is used to view the ruptured ALL and confirm the location of bony landmarks. Two minimally invasive incisions are made: one posterior to the lateral epicondyle and one posterior to the Gerdy tubercle. After anterior cruciate ligament graft fixation, the 2-strand gracilis tendon is introduced from the tibial incision, under the fascia lata, toward the femoral incision. The ALL graft is secured to the femur with a 5.5-mm anchor, positioned posteriorly and proximally to the lateral epicondyle. The distal end of the graft is tightened in full extension and fixed to the tibia with a ligament staple posterior to the Gerdy tubercle. This ALL technique requires no graft preparation.
Background: Outcome prediction at the early sclerotic stage of Legg-Calve-Perthes disease (LCPD) is valuable to select patients likely to benefit from early surgery. The metaphyseal apparent diffusion coefficient (ADC) ratio correlated significantly with Herring's classification of LCPD in a preliminary study of 49 MRIs, in which values greater than 1.63 were associated with poor outcomes. The objective of this study was to determine whether the femoral neck ADC ratio, with the 1.63 cut-off, determined at the initial stage of LCPD correlated with medium-term radiographic outcomes. Hypothesis: The metaphyseal ADC ratio correlates significantly with medium-term radiographic outcomes of LCPD. Materials and methods: A prospective study was performed in 27 children (mean age, 13 years; range, 9.5-16 years) who underwent 49 MRIs at the sclerosis or fragmentation stage of unilateral LCPD. ADCs measured bilaterally at the femoral head and neck were used to compute the corresponding ADC ratios between the affected and unaffected sides. The patients received regular follow-up for at least 5 years. The correlation between the ADC ratios and Stulberg grade at last follow-up was assessed. Results: After a mean follow-up of 6.8 years (range, 5.2-8.4 years) from the date of the first MRI, 13 hips were Stulberg 1 or 2, 13 were Stulberg 3 or 4, and 1 was Stulberg 5. The metaphyseal ADC ratio increased significantly with the Stulberg grade (p < 0.01). When only MRIs obtained at the early stage of sclerosis were considered, the correlation remained significant (p = 0.03). It was also significant in the subgroup of surgically treated patients (p < 0.0001) but was not significant in the subgroup without surgery (p = 0.51). A metaphyseal ADC ratio greater than 1.63 was associated with a worse Stulberg grade (p = 0.02). Discussion/conclusion: Diffusion-weighted MRI is a non-irradiating and non-invasive investigation that contributes to the management of LCPD when used in combination with morphological MRI sequences. Elevation of the femoral neck ADC is a finding of adverse prognostic significance that correlates with Herring's grade at the fragmentation stage and with Stulberg's grade at the healed stage. Early ADC elevation in the affected femoral neck can serve to select those patients most likely to benefit from early surgery before the fragmentation stage, i.e., before Herring's classification can be applied. L’incidence et la précocité des ruptures du ligament croisé antérieur (LCA) dans la population pédiatrique sont en augmentation. L’algorithme thérapeutique reste controversé chez les patients avant maturité osseuse concernant l’indication, le moment de la chirurgie, le choix de la technique et le type de greffon. Des troubles de croissance ont été rapportés et demeurent une préoccupation dans la décision thérapeutique. L’objectif primaire était d’observer les tendances actuelles sur la prise en charge des ruptures du LCA avant maturité osseuse auprès des membres de la Société francophone d’arthroscopie (SFA). L’objectif secondaire était de déterminer l’incidence, le type et la sévérité des troubles de croissance après reconstruction du LCA. Les dernières données de la littérature tendent au développement des techniques transphysaires et à la reconstruction chirurgicale précoce y compris chez l’enfant prépubère. Une invitation par messagerie électronique à remplir un questionnaire de 52 questions a été envoyée à tous les membres de la SFA. La participation était volontaire et les réponses confidentielles. Les données étaient automatiquement collectées via le site SurveyMonkey. Des statistiques descriptives étaient appliquées. Cent quarante-deux chirurgiens membres de la SFA ont répondu au questionnaire, soit un taux de réponse de 11 %. À la question « quel traitement recommandez-vous pour une rupture du LCA chez un enfant pré-pubère », 14 % ont répondu « reconstruction du LCA dans les 3 mois ». À la même question concernant un enfant pubère, 35 % recommandaient la reconstruction du LCA dans les 3 mois. Le tunnel tibial préféré était transphysaire chez un enfant pré-pubère (44,4 %) et transphysaire chez un enfant pubère (97,7 %). Le tunnel fémoral préféré était épiphysaire chez un enfant pré-pubère (62,2 %) et transphysaire pour un enfant pubère (55,5 %). Sept pour cent des participants ont rapporté des troubles de croissance après reconstruction du LCA. L’état actuel des pratiques demeure à ce jour non consensuel. La tendance est à la réalisation de tunnels transphysaires y compris chez l’enfant prépubère. Les cas rapportés de troubles de croissance significatifs sont relativement rares mais doivent faire l’objet de précautions techniques. Étude épidémiologique descriptive.
IV, descriptive epidemiological survey.
La détermination du pronostic articulaire dès la phase de condensation dans la maladie de Legg-Perthes-Calvé (MLPC) est précieuse afin de sélectionner les patients pouvant bénéficier d’une chirurgie précoce. Une corrélation positive significative avait été établie entre le ratio du coefficient apparent de diffusion métaphysaire et la classification de Herring dans une série préliminaire portant sur 49 IRM. La valeur seuil de ratio du coefficient apparent de diffusion (ADC) supérieure à 1,63 était associée à un mauvais pronostic. Cette corrélation pronostique se vérifie à moyen terme sur l’évolution radiographique. Étude prospective chez 27 enfants (âge moyen 13 ans ; 9,5 à 16) présentant une MLPC unilatérale. Quarante-neuf IRM ont été réalisées en phase de condensation ou de fragmentation. La quantification bilatérale de l’ADC de la tête et du col fémoral a permis de mesurer le ratio d’ADC entre côté pathologique et côté sain. Les patients étaient revus régulièrement jusqu’à un recul minimal de 5 ans. Le ratio d’ADC était comparé au grade de Stulberg au dernier recul. Au recul moyen de 6,8 ans (5,2–8,4) à partir de l’IRM initiale, 13 hanches étaient classées Stulberg 1 et 2, 13 Stulberg 3 et 4, et une Stulberg 5. Le ratio d’ADC métaphysaire augmentait significativement avec le grade de Stulberg (p < 0,01) et cette corrélation était aussi significative pour les IRM réalisées au stade précoce de condensation (p = 0,03). Cette corrélation se vérifiait dans le groupe de patients opérés (p < 0,0001) mais n’était pas significative dans le groupe de patients non opérés (p = 0,51). Les patients présentant un ratio d’ADC supérieur à 1,63 avaient un grade de Stulberg de plus mauvais pronostic (p = 0,02). Non irradiante et non invasive, l’IRM de diffusion en complément des séquences morphologiques est utile pour la prise en charge de la MLPC. L’élévation de l’ADC dans le col apparaît comme un élément pronostique péjoratif statistiquement corrélé au grade de Herring au stade de fragmentation et au grade de Stulberg au stade de séquelle. L’élévation précoce de la diffusion dans le col pathologique permettrait de sélectionner les enfants susceptibles de bénéficier plus précocement d’un traitement chirurgical avant le stade de fragmentation nécessaire à l’établissement du grade de Herring. L’objectif secondaire était de déterminer l’incidence, le type et la sévérité des troubles de la croissance associés à la reconstruction chirurgicale du LCA. Une invitation par courrier électronique à remplir un questionnaire de 52 questions a été envoyée à tous les membres de l’EPOS, POSNA et SFA. La participation était volontaire et les réponses confidentielles. Les données étaient automatiquement collectées via SurveymonkeyR. Des statistiques descriptives étaient appliquées. Quatre cent quarante chirurgiens membres des différentes sociétés ont répondu au questionnaire. À la question « quel traitement recommandez vous pour une rupture du LCA chez un enfant pré-pubère », 14 % de la SFA, 17 % de l’EPOS et 70 % du POSNA ont répondu « reconstruction du LCA dans les 3 mois ». À la même question concernant un enfant pubère, 35 % de la SFA, 61 % de l’EPOS et 83 % de la POSNA, recommandaient la reconstruction du LCA dans les 3 mois. Le tunnel tibial préféré était épiphysaire chez un enfant prépubère (40 %) et transphysaire chez un enfant pubère (78 %). Le tunnel fémoral préféré était épiphysaire chez un enfant pré-pubère (53 %) et transphysaire pour un enfant pubère (61 %). 21 % des participants ont rapporté des troubles de croissance après reconstruction du LCA, en particulier au fémur. Il y avait un total de 5 cas d’inégalité de longueur >2 cm, 1 cas de varus > 10°, 3 cas de valgus > 10° et 1 cas de recurvatum > 10°. L’étude a mis en évidence des disparités dans la prise en charge entre les membres des différentes sociétés. La tendance est à la réalisation de tunnel transphysaire, en particulier chez l’enfant pubère. Les cas reportés de troubles de croissance cliniques sont relativement rares. Cette étude permet d’appréhender les tendances actuelles en soulignant les différences géographiques sur la prise en charge des ruptures du LCA chez l’enfant.
La corrélation entre l’inégalité de longueur (ILMI) postopératoire après une arthroplastie totale de hanche et les scores fonctionnels est très controversée dans la littérature. Il n’existe pas dans la littérature de relation démontrée entre ILMI clinique, radiologique et la perception de cette inégalité. La mesure de l’ILMI postopératoire peut être optimisée par le système EOS qui permet d’obtenir un cliché corps entier de face et de profil digitalisé, avec une irradiation diminuée, et une évaluation plus précise des paramètres pelviens. Le but de cette étude était d’évaluer le retentissement fonctionnel du Delta de correction entre l’inégalité pré et postopératoire, à l’aide de mesures EOS 2D de bassin chez des patients ayant bénéficié d’une PTH de première intention. Nous avons évalué prospectivement 121 patients présentant une atteinte unilatérale dégénérative de l’articulation coxo-fémorale candidats à une arthroplastie totale de hanche. Le delta de correction, correspondant à la différence entre ILMI préopératoire et postopératoire, était mesuré selon la méthode de Woolson (U radiologiques–apex des petits trochanters) sur clichés EOS 2D de bassin de face en charge pour chaque patient. Les scores fonctionnels standardisés (HHS, PMA et WOMAC), en préopératoire et avec un recul postopératoire moyen de 8 mois étaient analysés en fonction du Delta. Tous les patients ont été améliorés sur les scores fonctionnels à 8 mois de recul. Il n’existait pas de différence sur les résultats fonctionnels postopératoires chez ces patients quel que soit l’inégalité résiduelle. Cependant, le harris hip score diminue lorsque le Delta augmente significativement (p = 0,005)- groupe (< –5 mm) HHS = 82,67 ; groupe (–5 à 1 mm) HHS = 95 ; groupe (0 à 5 mm) HHS = 95,06 ; groupe (6 à 10 mm) HHS = 91,47 ; groupe (11 à 15 mm) HHS = 91,71 ; groupe (> 15 mm) HHS = 83,67. Plus les patients étaient inégaux initialement, plus la correction était importante mais avec des résultats fonctionnels inférieurs (p = 0,005). La perception d’inégalité postopératoire était corrélée au Delta de correction (p = 0,004) La correction d’une inégalité de longueur préopératoire doit tenir compte de l’inégalité préopératoire. Une correction trop importante peut exposer le patient à des résultats cliniques moins bons. Le delta de correction est donc une variable plus intéressante que l’inégalité de longueur postopératoire, pour prédire les résultats fonctionnels après une arthroplastie totale de hanche.
