Objectives Onychomycosis is a fungal infection of the nail bed, nail matrix, and nail plate. Lasers have recently been studied as future clinical options for onychomycosis. We sought to evaluate the efficacy of the long-pulsed Nd:YAG 1064-nm laser on onychomycosis using a two-stage treatment. Methods A total of 100 affected nails (88 toenails and 12 fingernails from 22 patients) were treated using a long-pulsed Nd:YAG 1064-nm laser. The self-controlled treatment schedule included the first stage (1 session per week for 8 weeks) and the second stage (1 session every 4 weeks for 16 weeks). Patients were followed up with for 12 weeks after the last laser treatment. Direct microscopy, cultures, and clinical assessments were performed at various time points. Results Of the nails studied, 84% were infected by Trichophyton rubrum, while others were coinfected with T. rubrum and other fugal strains. The mycological clearance rate and the clinical efficacy rate of the nails were 29% and 21% after the first stage, 69% and 35% after the second stage, and 67% and 39% during follow-up, respectively. The second stage of laser treatment significantly improved the mycological clearance and clinical efficacy. Conclusion Long-pulsed Nd:YAG 1064-nm laser two-stage treatment was effective for onychomycosis, with significantly improved mycological clearance and clinical efficacy. 目的:应用不同设备不同方案评估激光治疗甲真菌病的疗效差异.方法:通过回顾性研究,观察2011-2014年北京友谊医院皮肤科门诊应用不同的激光设备对182例甲真菌病患者进行不同次数不同方案治疗后的疗效差异.PinpointeTM FootlaserTM 长脉宽Nd∶YAG1 064 nm激光(Laser M)治疗102例,分为4次治疗组、8次治疗组及12次治疗组;欧洲之星TM靓甲TM Nd∶YAG1 064 nm长脉宽激光(Laser S)治疗59例,分为追加治疗组及8次治疗组;奇致TM ML-3420型Nd∶YAG1 064 nm脉冲激光(laser N)治疗21例,均为4次治疗.结果:激光M:4次治疗组24周有效率为37.4%,8次治疗组24周有效率为44.0%,12次治疗组24周有效率为55.4%.激光S:追加治疗方案24周有效率为39%;8次治疗方案24周有效率62.5%.激光N:4次治疗方案24周有效率为52.6%.结论:3种激光设备治疗甲真菌病均有显著效果,且无不良反应;同一激光设备增加治疗次数可以提高有效率;相同的治疗方案,不同的激光治疗有效率有一定的差异,可能与医师操作手法及参数选择有关;治疗间隔不宜过长,应该在真菌恢复生长能力之前或增长至治病数量之前予以巩固治疗;轻中度甲真菌病患者或者病甲数目较少或患者有基础疾病伴有肝肾功能不良者可单纯选择激光治疗,重度甲真菌病或是病甲数目较多的患者应考虑药物和激光联合治疗.
Objective To evaluate the effect of 800nm diode laser for hair removal in 136 cases. Methods Hair removal was performed on 157 areas of 136 patients with Lightsheer 800 nm pulsed diode laser(pulse duration: 30 or 100ms or in auto-mode;frequency≤2 Hz;spot diameter: 9 mm×9 mm;energy density: 15-40 J/cm2).The laser parameter was adjusted by skin and hair types.Retrospective data of 136 patients and 157 areas treated was analyzed,covering the curative effects on the facial and non-facial areas treated as well as the effects after the treatment of 1 to 4 times and above 4 times respectively.Data was also gathered on patient satisfaction. Results The total effective rates for the facial and non-facial area were 65.1% and 72.3% respectively.The effective rates for the facial and non-facial area were 44.2% and 67.2% respectively after the treatment of 1-4 times,and 88.8% and 99.0% respectively after the treatment of more than 4 times(P0.05).While 3% of the patients received facial treatment being extremely satisfied with the curative effects,59% of them felt the effects were satisfactory.For the patients received non-facial treatment,the rates were 16% and 65% respectively.Only 4 cases of complication occurred,and no hyperpigmentation was found. Conclusions The 800 nm diode laser system is effective and safe for hair removal.The hair-removing effect of 800 nm diode laser is better on facial areas than on non-facial areas.
