Three children from the same family exhibiting the same anteroposterior and vertical skeletal discrepancies (hypodivergent Class II, division 1) were treated by 3 different therapeutic solutions with strictly identical final objectives.The eldest was treated by extractions of 14-24-35-45; the second underwent no extractions and the youngest had extractions of 16-26.The aim of this paper is to provide a clinical reflection regarding a number of treatment options in cases of severe, hypodivergent, skeletal Class II.
Le sacrifice de dents saines est une décision grave qui peut être interprétée comme une mutilation puisqu’elle diminue le patrimoine dentaire du patient. Elle doit donc être prise le plus rationnellement possible, sans découpage du problème en tranches spatiales et temporelles : la décision doit être globale, c’est-à-dire prendre en compte l’ensemble de la denture, en ne considérant pas seulement son aspect immédiat, mais surtout en envisageant aussi précisément que possible son devenir. Les obligations éthiques imposent en outre une information complète du patient sur les choix, même différés, des extractions. Merrifield détermine l’espace disponible de la zone postérieure à partir du calcul de la distance comprise entre le bord antérieur du ramus et la face distale de la première molaire inférieure ; cet auteur évalue la croissance de cette zone. Il en déduit la mesure de la dysharmonie dento-maxillaire postérieure, élément de l’analyse de l’espace total. L’étude d’un échantillon de 33 patients, où la politique «extractionniste» a comporté des extractions de prémolaires et la conservation des troisièmes molaires, permet l’analyse des facteurs qui ont autorisé l’évolution des dents de sagesse, malgré un pronostic initial parfois réservé, selon Merrifield. Les facteurs retenus sont : une croissance postérieure plus importante et plus durable que la prévision selon Merrifield, ainsi qu’une croissance alvéolaire verticale postérieure qui augmente l’espace rétro-molaire, l’ensemble s’accompagnant de la difficulté de la lecture de la limite postérieure de l’arcade mandibulaire par rapport au niveau du bord antérieur du ramus.
The aim of the present study was to assess the bond strength of root fillings in oval-shaped canals prepared with the self-adjusting file (SAF) system.A careful specimen selection resulted in 2 equal groups, each consisting of 12 extracted mandibular canines with oval canals that had vital pulps before extraction. One group was subjected to the SAF protocol, and the other group underwent conventional protocol, which was the ProTaper system with syringe-needle irrigation. Full-strength sodium hypochlorite was used as an irrigant for both groups. The teeth were obturated in a standardized way, filled with a lentulo spiral as the root filling, and then prepared for micropush-out assessment by using root slices of 1-mm thickness. Loading was performed on a universal testing machine at a speed of 0.5 mm/min−1. The Student's t test for pairwise comparisons was applied to assess the effect of each preparation technique on the push-out bond strength.All specimens showed measurable adhesive properties to root dentin. In addition, no premature failure occurred. The group-by-location interaction was significant (P = .0071); thus, the group comparisons were dependent on the canal third. Overall, the push-out bond strength was the highest in the coronal third and the lowest in the apical third. SAF-prepared specimens displayed significantly higher bond strengths (P = .0012, 0.51–5.9 MPa).The present study showed that SAF preparation markedly influenced root-filling push-out bond strength in oval-shaped canals. Further investigations are needed to provide a better understanding of the physicochemical modifications of the root dentin prepared with the SAF cleaning-shaping-irrigation system.
