BACKGROUND & AIMS:Protein-energy wasting (PEW) is increasingly becoming a clinical problem in maintenance hemodialysis patients and guidelines call for nutritional interventions. Serum prealbumin (transthyretin) represents a critical nutritional marker positively correlated with patient survival and negatively correlated with morbidity. Nutritional counseling, oral supplementation as well as intradialytic parenteral nutrition (IDPN) are recommended to fight PEW, however clinical trials on their use are scarce. METHODS:We conducted a prospective, multicenter, randomized, open-label, controlled, parallel-group Phase IV clinical trial in 107 maintenance hemodialysis patients suffering from PEW to assess the impact of IDPN on prealbumin and other biochemical and clinical parameters reflecting nutritional status. Patients randomized to the intervention group received standardized nutritional counseling plus IDPN three times weekly over 16 weeks followed by a treatment-free period of 12 weeks. The control group received standardized nutritional counseling only. Main trial inclusion criteria included moderate to severe malnutrition (SGA score B or C), maintenance hemodialysis therapy (3 times per week) for more than six months, and presence of two out of the following three criteria: albumin <35 g/L, prealbumin <250 mg/L, phase angle alpha <4.5° assessed by bioelectrical impedance analysis (BIA). Changes in serum prealbumin, albumin, transferrin, phase angle alpha, subjective global assessment (SGA) score and health-related quality of life using the 12-item short form health survey (SF-12) were investigated. RESULTS:IDPN significantly increased prealbumin (p < 0.05), showing rapid rise within 16 weeks of treatment and sustained response thereafter. In the full analysis set (n = 83), 41.0% of 39 patients receiving IDPN achieved a relevant (i.e., at least ≥15%) increase in prealbumin over baseline at week 4 compared to 20.5% of 44 patients in the control group. Considerably more patients with IDPN therapy achieved an increment of prealbumin >30 mg/L at week 16 (48.7% vs. 31.8%). Prealbumin response to IDPN therapy was more prominent in patients suffering from moderate malnutrition (SGA score B) compared to patients with severe malnutrition (SGA score C). CONCLUSIONS:The results of this trial demonstrate for the first time that IDPN therapy, given three times weekly in a 16-week short-term intervention, results in a statistically significant and clinically relevant increase in mean serum prealbumin, a surrogate marker for outcome and survival in hemodialysis patients suffering from PEW, and is superior to nutritional counseling. Ein guter Ernährungszustand ist für Patienten unter permanenter Nierenersatztherapie ein Zeichen guter Behandlung und guter Prognose. Je nach Art des Behandlungsverfahrens und der bisherigen Ernährungsgewohnheiten muss sich der Patient in der Regel hinsichtlich der Zufuhr von Wasser und bestimmten Nahrungsbestandteilen wie z. B. Phosphor, Natrium und Kalium einschränken. Zudem wird Dialysepatienten eine Vielzahl von Medikamenten verschrieben, die regelmäßig eingenommen werden sollen.
Der Gehalt der Nahrungsmittelbestandteile wird als molekulare Masse (z. B. Phosphor und Natrium) pro Gewichts- oder Volumeneinheit angegeben. Besonders bei quantitativen Angaben ist es nicht zulässig, diese Angaben mit den jeweiligen Salzen dieser Substanzen (Phosphat, Kochsalz) zu verwechseln. Es ist unkorrekt und irreführend, wie es oft geschieht, diese Bezeichnungen (Phosphor und Phosphat) und (Natrium und Salz) synonym zu verwenden. Die "physiologische" Kochsalzlösung und die Angabe von Phosphat als Nahrungsmittelbedarf sind Beispiele falscher Mengenangaben von erheblicher klinischer Bedeutung.
Prurigo nodularis is a skin disease with unknown origin which is frequent in patients on maintenance renal replacement therapy. Here, we report a patient who was dialyzed for more than 10 years with different dialyzers using different dialyzer membranes. Each time when the patient was switched to a polymethylmethacrylate membrane (Filtryzer BK-1.6P), itching and skin lesions disappeared completely. This effect could be verified three times.
Es gehört zum Standard einer Diätberatung, einem Hämodialysepatienten zu empfehlen, täglich nur 500 ml mehr zu trinken als der möglicherweise noch vorhandenen Restdiurese entspricht. Außerdem soll er so wenig wie möglich Salz mit der Nahrung aufzunehmen, da Salz angeblich Wasser im Körper des Patienten bindet. Wenn der Patient diese Vorgaben nicht einhalten kann, wird ihm meist fehlende Kooperationsbereitschaft vorgeworfen. Hierbei wird übersehen, dass innerhalb der kurzen Dauer einer Dialysebehandlung massiv in den Salz-Wasser-Haushalt eingegriffen wird. In vielen Fällen wird der Durst erst durch das Salz ausgelöst, das dem Patienten im Verlauf der Dialyse zugeführt wird. Auch erhalten Patienten heute zahlreiche Medikamente, die zu Mundtrockenheit und Durst beitragen. Aus diesen Gründen ist eine Diätberatung notwendig, die auch die aktuellen Gegebenheiten der Dialysetherapie und der aktuellen Medikation berücksichtigt.
Der Erfolg einer Dialysetherapie bei einer terminalen Niereninsuffizienz basiert zu großen Teilen auf der begleitenden ernährungsmedizinischen Beratung des Patienten. Zum einen können die Patienten einige Substanzen, insbesondere Phosphat, in der zur Verfügung stehenden Zeit nicht hinreichend ausscheiden. Auf der anderen Seite ist zur Vermeidung einer Malnutrition eine ausreichende Aufnahme von Energie und Eiweiß mit der Nahrung notwendig. Darüber hinaus greift der Stoffaustausch im Dialysator bei jeder Behandlung massiv in den Wasser- und Elektrolythaushalt des Patienten ein, was einer besonderen diätetischen Beachtung bedarf.
Eine Proteinmalnutrition ist bei ca. 20–35 % aller Dialysepatienten erkennbar und Teilursache einer erhöhten Mortalität. Abgesehen von organischen Störungen sind deren Ursachen eine inadäquate Dialysetherapie, eine inadäquate medikamentöse Therapie sowie psychische Störungen. Die Therapie besteht neben einer sorgfältigen Ernährungsberatung unter Berücksichtigung der spezifischen Bedingungen eines Dialysepatienten, in der Optimierung der Dialysebehandlung sowie in enteraler oder parenteraler Ernährung mit speziellen Nährlösungen, die auf die Dialysebehandlung ausgerichtet sind.
La malnutrition protéique est un facteur important pour la survie des patients hémodialysés. Pour cela les professionnels de la nutrition recommandent un apport protéique journalier standard de 1,1 à 1,2 g/ (kg × jour) [9]. Mais ceci ne tient pas compte de l’âge ni du sexe des patients. Les analyses du taux du catabolisme protéique (protein catabolic rate PCR) à l’aide de la cinétique de l’urée présentées ici montrent cependant une corrélation avec l’âge et avec le sexe des patients, dès que l’on se réfère au PCR par rapport au poids absolu. Si le PCR se réfère au poids maigre les différences liées au sexe disparaissent. Nous préconisons donc de calculer les besoins protéiques en tenant compte de la masse maigre et non pas par rapport au poids corporel brut et de tenir également compte de l’âge des patients dans les recommandations alimentaires.
Eine Proteinmalnutrition tritt bei 20–35 % aller Dialysepatienten auf und trägt zu einer erhöhten Mortalität bei. Die Ursachen können organische und psychische Störungen, eine inadäquate Dialysetherapie sowie eine unpassende medikamentöse Behandlung sein. Die Therapie der Proteinmangelernährung besteht in einer sorgfältigen Diätberatung unter Berücksichtigung der spezifischen Erfordernisse eines Dialysepatienten, in der Optimierung der Dialysebehandlung sowie in der enteralen oder parenteralen Ernährung mit speziellen Nährlösungen, die auf die Dialysetherapie ausgerichtet sind.
Protein malnutrition is regarded as a risk factor of survival of hemodialysis patients. That's why good practice guidelines recommend a daily protein intake of 1.1 - 1.2 g/(kg x day) irrespective of the gender and the age of patients [9]. The investigations presented here using urea kinetic modelling exhibit significant differences in protein catabolic rate (PCR) in gender and age of the patients as far as PCR is related to (kg body weight x day). The difference in gender disappears when PCR is related to kg lean body mass x day. From these findings it is recommended to relate PCR to lean body weight instead of body weight. Objective To evaluate the protein permeability of different high-flux dialysis membranes. Methods The filtrate was sampled from the dialysate compartment of different dialysers during the routine dialysis therapy. SDS-PAGE analysis was performed with silver staining method according to the modification of Melzer and consecutive laser densitometry. Before sampling all the dialysis mode was changed to pure ultrafiltration. Results The protein pattern of filtrate from dialysis membranes was similar to that of the glomerular membrane containing IgG, transferrin, albumin, retinol binding protein and beta-2-microglobulin. Comparing different membranes there were considerable differences depending on cut-off, charge and adsorption capacity of the particular membrane. In all membranes, permeability of proteins decreased during one treatment session. Conclusion Protein permeability of high-flux dialysis membranes is similar to the glomerular membrane but modified according to poresize, surface charge, adsorption and time on dialysis. In contrast to the glomerular membrane, in each of the investigated membranes protein permeability decreases during the session.
BACKGROUND Several effects of hemodialysis, including hemoconcentration, alterations of hemostasis or hemorheology and endothelial activation, could potentially interfere with cerebral blood flow (CBF) regulation. These treatment-specific changes may also be crucial for the enhanced incidence of stroke in uremic patients. Nevertheless, the influence of hemodialysis on CBF has not been yet adequately studied. PATIENTS AND METHODS We registered mean blood flow velocity (MFV) in the middle cerebral artery (MCA) during hemodialysis treatment in order to evaluate its contribution on CBF changes. Transcranial Doppler ultrasonography (TCD) of the MCA was performed continuously during hemodialysis treatment in 18 stable patients (10 males and 8 females, mean age 62 +/- 11 years) with end-stage renal disease of various origin. Blood pressure (mmHg), heart rate (/min), ultrafiltration volume (ml), BV changes (deltaBV by hemoglobinometry, %), arterial blood gases (pO2, blood oxygen content, pCO2), hemostasis activation (thrombin-antithrombin III complex, ELISA) and fibrinogen (Clauss) were measured simultaneously at the beginning of treatment and every hour thereafter. RESULTS Before the hemodialysis session the MFV in the MCA was within normal range (57.5 +/- 13.0 cm/s, ref. 60 +/- 12) and was mainly dependent on the patients' age (r = -0.697, p < 0.01). The blood flow velocity in the MCA decreased significantly from 57.5 +/- 13.0 cm/s before the beginning to 48.3 +/- 11.1 cm/s after four hours (n = 18, p < 0.05) and to 43.9 +/- 8.9 cm/s after five hours (n = 9, p < 0.05) of hemodialysis treatment. During hemodialysis treatment, the percentual changes of MFV in the MCA (delta%MFV) were interrelated to the ultrafiltration volume (r = -0.486, p < 0.01), the blood volume (BV%, r = 0.369, p < 0.01) and the percentual changes of the hematocrit (r = -0.358, p < 0.01), of the arterial blood oxygen content (delta%acO2, r = -0.420, p < 0.01) and of the plasma fibrinogen levels (delta%fibrinogen, r = 0.244, p < 0.05). CONCLUSION A significant continuous decrease of the MFV in the MCA was observed during hemodialysis treatment, which inversely correlated both with ultrafiltration volume, BV changes and changes of plasma fibrinogen. The ultrafiltration-induced hemoconcentration with concomitant rise of hematocrit and oxygen transport capacity, may partly explain the alterations in the cerebral MFV observed during hemodialysis.
