The public discussion about health care financing has an enormous relevance for the field of gynaecology and obstetrics. This paper of the financing commission of the DGGG presents essential financial aspects and problems, influencing our specialty. Regarding increasing rates of professional liability, Germany could reach a comparable situation to the USA. A reduction of the actual quality of care can be possible. Moreover, the education and training are essential foci to assure health care on a high level in future. The costs are a general problem due to doubtful representativeness of education and training costs in the DRG system. An appropriate adjustment should be demanded in the inpatient and outpatient setting. The German National Cancer Plan can have several financial influences beside effects on oncological care, e. g. regarding the early detection program of cervical cancer or the strengthening of certified centres (also important for discussions of additional charges) and improvement of tumour documentation and quality assurance. Furthermore, there is an urgent need for action to finance obstetrics and perinatal medicine. Financial resources should be increased and specific procedures of centres should be refunded by the DRG system. Solutions need multiple approaches and the collaboration of all colleagues in the field.

This second part of the paper of the financing commission of the DGGG presents essential financial aspects and problems, influencing our speciality. Among other aspects, the inputs of the DRG system are discussed for our field. The representation of all levels of care and all German states in the InEK calculation system is still a problem. Regarding the inpatient care, the number of cases has decreased by 23% over the last years due to the shift to the outpatient setting. Surgeries in the outpatient setting have increased by 31.5% from the year 2004 to the year 2008 (all specialities). Moreover, a decrease of obstetric cases by 5.7% and cases directly associated with birth by 7.3% has to be mentioned. Altogether, our field has lost 6.8% of all inpatient cases over the last years. The economic pressure is still high and it is difficult to maintain cost recovery. Regarding the personal resources, legal demands are problematic for perinatal centres, e. g. loss of the on-call duty possibility, availability of medical specialist expertise and necessity of permanent standby service of the head of the perinatal centre. Further problems of the inpatient setting concern the field of urogynaecology (lack of young academics, new technical procedures with lack of data, missing representation of complex surgeries and lack of reimbursement in case of surgeries in the outpatient setting) and the benign and malign gynaecological surgery. While standard procedures are almost represented in the DRG system, there is still a high demand for development of appropriate reimbursement for complex and infrequent case constellations, interdisciplinary procedures and simultaneous multiplex surgeries. The collaboration of all colleagues is essential.

Die öffentliche Diskussion über die Finanzierung des Gesundheitswesens in Deutschland besitzt für das Fach Gynäkologie und Geburtshilfe erhebliche Bedeutung. Die vorliegende Publikation der Finanzierungskommission in der DGGG e. V. stellt wesentliche aktuelle Finanzierungsaspekte bzw. Finanzierungsprobleme dar, welche unser Fachgebiet betreffen. In Bezug auf die steigenden Prämien der Berufshaftpflicht ist zu befürchten, dass wir in Deutschland eine Situation erreichen könnten, wie sie z. B. in den USA bereits vorliegt, und zu einer Einschränkung der aktuellen Versorgungsqualität führen kann. Auch die Weiterbildung ist ein wesentlicher Schwerpunkt, um die Versorgung auch zukünftig auf einem hohen Niveau anzubieten. Die Kosten der Weiterbildung sind bei fraglicher Repräsentativität im DRG-System ein grundsätzliches Problem. Hier muss ein adäquater Ausgleich gefordert werden – sowohl im ambulanten als auch stationären Bereich. Der Nationale Krebsplan kann neben den Auswirkungen auf die onkologische Versorgung weitere zahlreiche finanzielle Veränderungen mit sich bringen, z. B. einerseits in Bezug auf die Früherkennung des Zervixkarzinoms, andererseits durch Stärkung der zertifizierten Zentren – auch für die Verhandlung von Zuschlägen – und Optimierung der Tumordokumentation und Qualitätssicherung. Zudem ist im Bereich der Geburtshilfe bzw. Perinatalmedizin ein dringender Handlungsbedarf festzustellen. So sollten die finanziellen Ressourcen in Relation angehoben bzw. die spezifischen Leistungen von Zentren unmittelbar im DRG-System refinanziert werden. Lösungen verlangen multiple Ansätze sowie die Zusammenarbeit aller Kolleginnen und Kollegen.
Since 1996, the German reimbursement system is partly based upon specific fixed rates. Beginning in January 2004, however, the new reimbursement system "DRG" (diagnosis related groups) will group all cases in so called case weights, leading to a fixed compensation per case, predominantly independent from the length of the hospital stay. The increasing impact of accessory diagnoses on the individual case weight represents the major difference between the current and the future system. While under the current system, the number of diagnoses does not influence the monetary reimbursement, the DRG-system will be mainly driven by the number and nature of diagnoses and procedures. Thus, in future, the extent of medical file documentation wil be a major determinant of an institution's financial health. Additional attention has to be paid to conflicts between the current and the DRG-system, which can be generated by discrepant regulatory conditions and will cause an inadequate picture of an institutions case mix. Many institutions in Germany are insufficiently prepared to cope with the new requirements for the intensity and content of medical documentation. This article will provide valuable information about everyday necessities, which have to met in the new DRG era.
Die Mehrheit der geburtshilflichen Fälle wird bereits über das seit 1. 1. 1996 geltende System der Abrechnung nach der Bundespflegesatzverordnung (BPflV) über Pauschalbeträge vergütet. Die Einführung des neuen Fallpauschalensystems der DRG (Diagnosis related groups) zum 1. 1. 2003 führt jedoch zu einer deutlichen Ausweitung der Pauschalisierung der Vergütungssätze und damit zu einem nahezu vollständigen Abschied von den tagesgleichen Pflegesätzen. Die bedeutendste Änderung gegenüber dem bislang geltenden Abrechnungssystem liegt vor allem in der Erlösrelevanz von Nebendiagnosen: Während sich die Dokumentation von Nebendiagnosen bisher nur in den wenigsten Fällen auf den Erlös auswirkte, erfolgt die Höhergruppierung und damit Erlössteigerung im neuen DRG-System in der Geburtshilfe hauptsächlich über DRG-relevante Nebendiagnosen. Der Umfang und die Sorgfalt bei der medizinischen Dokumentation erhalten somit einen auch pekuniär bedeutsamen Aspekt. Erschwert wird die Dokumentation derzeit zusätzlich durch Kodierkonflikte zwischen dem neuen und alten Abrechnungssystem. Viele Kliniken in Deutschland sind bislang nur unzureichend auf die wirtschaftlichen Folgen des neuen Abrechnungssystems vorbereitet. Der folgende Artikel weist auf die neuen Anforderungen einer DRG-gerechten Dokumentation hin und soll so den Einstieg in das neue System erleichtern.
