La prise en charge des récidives des cancers de l'ovaire repose sur la chimiothérapie intraveineuse associée ou non à une chirurgie de cytoréduction. La chimiohyperthermie intrapéritonéale (CHIP) est parfois proposée en complément d'une chirurgie complète. Le but de cette étude est d'évaluer la faisabilité, la morbidité et la survie de la CHIP associée à une cytoréduction chirurgicale complète dans le cadre de la prise en charge de patientes présentant une première récidive d'un cancer de l'ovaire.Entre 2005 et 2010, 27 patientes ont été opérées pour une récidive d'un cancer de l'ovaire. Parmi elles, 17 patientes (63 %) ont bénéficié d'une CHIP.Seize patientes (94 %) étaient en résection complète en fin d'intervention. Aucune patiente n'est décédée en postopératoire. Deux patientes ont eu des complications peropératoires : une plaie vésicale et une section de l'uretère. Huit patientes ont présenté des complications postopératoire dont 3 de grade 3 ou supérieur (deux défaillances d'organe et une reprise chirurgicale). Quinze patientes ont présenté une récidive avec une médiane de survie sans progression de 11,9 mois (IC 95 % [5,4–32,9]) depuis la CHIP. La médiane de survie globale depuis le diagnostic était de 107,8 mois.Ces résultats montrent que l'association de la CHIP, à une cytoréduction complète pour les récidives de cancer de l'ovaire, présente une morbidité et une survie acceptables. Les résultats de l'étude française multicentrique en cours (CHIPOR) sont attendus afin de généraliser cette prise en charge.The management of recurrent ovarian cancer is based on intravenous chemotherapy with or without debulking surgery. Fifteen patients had a recurrence with a median DFS of 11.9 months (95% CI [5.4–32.9]) from the HIPEC. The median overall survival from diagnosis was 107.8 months.These results showed that the association of HIPEC with a complete cytoreduction for recurrent ovarian cancer presents acceptable morbidity and survival. The results of the ongoing French multicenter study (CHIPOR) are expected to generalize this support.
Les maladies de Horton (artérite gigantocellulaire), Takayasu et Behçet sont responsables des principales aortites non infectieuses. Une aortite doit être suspectée en cas d’épaississement de la paroi et en cas d’anévrisme ou d’occlusion de l’aorte ou de ses branches en l’absence de facteurs de risques cardiovasculaires habituels. En cas d’atteinte sévère, ces lésions peuvent nécessiter une réparation chirurgicale. Mais la quiescence de la maladie doit être obtenue avant traitement chirurgical ou endovasculaire pour éviter que se développent des complications telles qu’un faux anévrisme anastomotique ou une thrombose de stent, particulièrement fréquents dans ces pathologies. La fréquence des anévrismes de l’aorte (en particulier ascendante) dans la maladie de Horton incite à son dépistage systématique, ainsi qu’à la surveillance régulière de toute l’aorte au cours du suivi. Les maladies de Behçet et Takayasu exigent le plus grand contrôle de l’inflammation ainsi que la connaissance de certains artifices techniques et stratégies opératoires sans quoi le risque de récidive est très important. La revue de la littérature nous montre que le traitement endovasculaire pourrait diminuer la morbidité péri-opératoire mais n’a pas permis de diminuer les complications à moyen ou long terme dans ces indications.Non-infectious aortitis are usually due to giant cell arteritis (temporal arteritis), Takayasu and Behçet's diseases. Aortitis should be suspected in the presence of aortic wall thickening or of aneurysm or occlusion of the aorta and its branches in the absence of characteristic cardiovascular risk factors. Surgery is required in case of severe damage. But the quiescence of the inflammatory disease must be obtained before endovascular or surgical treatment to prevent complications such as anastomotic false aneurysm or stent thrombosis, especially common in this disease. The frequency of aortic aneurysms (in particular of ascending aorta) in giant cell arteritis encourages its systematic screening, as well as regular monitoring of the entire aorta during the follow-up. Behçet's and Takayasu diseases require the greatest control of inflammation and the knowledge of some surgical tricks to avoid the risk of recurrence. The literature review shows that endovascular treatment could reduce perioperative morbidity but did not allow reducing long-term complications.
Introduction. - Whipple's disease is a systemic infection that may mimic sarcoidosis in its initial presentation. The heart involvement is not uncommon and consists generally in an endocarditis. Myocarditis is less common and is usually accompanied by impairment of heart conduction.Case report. - We report a 56-year-old man with Whipple's disease associated with a myocarditis, initially diagnosed as having a sarcoidosis with cardiac injury. The contribution of the histology and molecular biology on intestinal sampling made it possible to rectify the diagnosis.Conclusion. - The diagnosis of Whipple's disease should be considered in the presence of a systemic granulomatosis with or without heart involvement. Introduction. - Bilateral hilar lymphadenopathy, with or without lung parenchymal infiltrates, is the most common radiographic finding in patients with sarcoidosis. Atypical Pulmonary findings have been uncommonly reported and include multiple large lung nodules, cavitation, lobar collapse, pleural effusions or pneumothorax.Observation. - We report a 21-year-old non caucasian patient who presented with pulmonary nodular infiltration and sinonasal involvement revealing sarcoidosis. Thoracic and sinus computed tomographic scan showed both multiple excavated large lung nodules and micronodules, hilar lymphadenopathy and sinus thickening. Laboratory studies disclosed elevated angiotensin converting enzyme serum level (120 UI/L). Outcome was favorable after institution of corticosteroids (at an initial dose of prednisone of 1 mg/kg/day); at eight-month-follow-up, the patient was asymptomatic, while receiving prednisone 22.5 mg/day.Conclusion. - In patients exhibiting unusual pulmonary manifestations, diagnosis of sarcoidosis relies on compatible clinical signs, evidence of non-caseating granulomas, and exclusion of underlying conditions including infections, malignancy and other granulomatous diseases (Wegener disease, pneumoconiosis). Introduction.– Décrite pour la première fois par Hamrin en 1965, l’atteinte de l’artère mésentérique est rare au cours de la maladie de Horton ; en effet, l’analyse de la littérature ne nous a permis d’en recenser que 30 cas. Cette complication vasculaire de la maladie de Horton ne doit pas être méconnue, car elle est grevée d’une mortalité importante (37 %). Patients et méthodes.– Cette étude rétrospective a porté sur les cas de d’artérite inflammatoire mésentérique en rapport avec une maladie de Horton, colligés dans notre service entre janvier 1998 et janvier 2008. Résultats.– Il s’agissait de deux hommes et d’une femme d’âge moyen 75 ans. Les patients avaient des signes généraux (n = 3), des douleurs abdominales (n = 1), des symptômes orientant vers une maladie de Horton à type de céphalées (n = 1). La biopsie d’artère temporale était contributive chez deux patients. Le diagnostic de l’atteinte artérielle mésentérique a été porté par angioscanner chez tous les patients ; l’angioscanner objectivait un épaississement circonférentiel et régulier de la paroi de l’artère mésentérique supérieure. De plus, il mettait en évidence l’existence d’une aortite de l’aorte thoracoabdominale (n = 2) ou abdominale (n = 1). Lors du suivi, l’angioscanner a montré une amélioration (n = 2) ou une stabilisation des lésions (n = 1). Conclusion.– Notre travail indique que l’artérite inflammatoire mésentérique peut constituer le mode de révélation de la maladie de Horton. De plus, il suggère que lorsqu’une exploration par angioscanner de l’aorte est réalisée chez les patients porteurs de maladie de Horton, son examen doit être complet et concerner l’aorte mais également ses branches, et particulièrement les artères mésentériques.
A 24-year-old man attempted suicide by intravenous injection of elemental mercury. The exact quantity of injected elemental mercury was unknown. Five days later, the patient presented with painful erythema involving the anterior part of both forearms. On admission, laboratory findings disclosed the following: haemoglobin: 14.5 mmol/l, white blood cell count: 8.34 × 109/l, platelet count: 321 × 109/l, urea: 5.4 mmol/l, creatinine: 68 µmol/l; other tests, including liver tests, blood protein electrophoresis, proteinuria and urinalysis, were within normal limits. Lors du suivi, l’angioscanner montre une amélioration des lésions rtérielles mésentériques chez les deux patients. Conclusion. – Notre travail montre que l’angioscanner est un examen utile pour dépister les atteintes artérielles mésentériques de la maladie e Horton. Introduction. - Mesenteric involvement has been rarely reported in giant cell arteritis (GCA). Our literature search using the Medline database I (1966-2008). reviewing all articles in English and French languages identified only 3 1 cases of mesenteric ischemia related to GCA. We report two additional cases.Case reports. - We report two men with GCA. At diagnosis of GCA-associated mesenteric involvement, patients exhibited: fever/fatigue (n = 2), abdominal complaints (it = 1). clinical sips of GCA (it = 1). In both patient,,, abdominal CT-scan showed Circumferential halo around the Superior mesenteric artery. At systematic follow-up. CT-scan revealed improvement of mesenteric damage in both patients.Conclusion. - Our study indicates that CT-scan is it useful technique in diagnosis of GCA-associated mesenteric involvement. Clinical outcome was favorable after institution of intralesional antimony therapy.DISCUSSION:Cutaneous leishmaniasis is still recognized to be an opportunistic infection. Its frequency is indeed higher in immunocompromized subjects, mainly HIV-infected patients. In our patient, both polymyositis and steroid therapy were predisposing factors of cutaneous leishmaniasis onset; prednisone therapy has been postulated to be associated with immune dysfunction leading to: reduced blood T cells' levels (CD4 et CD8) as well as decreased cytokine synthesis (e.g. interferon gamma).
