Zielsetzung Implant based breast reconstruction gained a high and increasing level of importance during the past years, currently performed with placement of the implant in a pre-pectoral pocket. Although safety and breast aesthetics of this approach are well recognized prepectoral techniques adds a whole new dimension with the development of the next generation of specific for prepectoral implant placement created titanized implant pockets - especially in patients with smooth implants.

Purpose: A cervical screening detects not only cervical dysplasia and cervical cancer. Furthermore it can be in very rare cases a detector of an adenocarcinoma of the ovary. PAP V as a result refurs generally to an invasive cancer. After excluding histologically a cervical and endometrial cancer, ovarian cancer needs to be considered as a differential diagnosis.
UNLABELLED:In gynecologic oncology lymphadenectomy is of prognostic and therapeutic importance because recurrence-free time and survival depend on the metastatic involvement of lymph nodes. Lymphadenectomies are not performed to such an extent as they are indicated. This might be due to a laborious or problematic preparation. The authors therefore report their experience in a seldom taught preparation of the left para-aortic compartment in the form of a learning curve.MATERIALS AND METHODS:To access the left para-aortic area, the descending colon is lifted to open the retroperitoneum along the line of Toldt. The mesentery of the descending colon was separated from the kidney along the fascia of Gerota by blunt preparation. Time was measured from the incision of the peritoneum until the renal vein was clearly visible.RESULTS:The authors collected the data from the first 25 preparations. Mean duration for the left para-aortic preparation was 7.8 minutes compared to 5.9 minutes for the right side. Duration of preparation of the left area dropped from 11.0 minutes within the first patients (#1 to #5) to 3.8 minutes in the last patients (#20 to #25). No complications were observed in the study group linked to the retromesenteric approach described.CONCLUSION:Retromesenteric para-aortic lymphadenectomy is quick to learn. The authors needed 20 preparations to observe a significant drop in the time needed for preparation. Retromesenteric para-aortic lymphadenectomy offers an excellent overview that lightens lymphadenectomy and therefore reduces the risks for patients.
Hyperandrogenism is present in 5%-10% of all women in reproductive age. In the case of rapid onset virilization, newly manifest postmenopausal hirsutism or excessive hyperandrogenemia, an androgen-producing tumor needs to be ruled out. Hormone diagnosis is mandatory. If there is a suspected androgen-producing tumor, stimulation and suppression tests may contribute to a correct diagnosis. Tumor localization can be achieved by transabdominal and vaginal sonography, computer tomography, magnetic resonance imaging and specific nuclear medicine procedures. Androgen producing-tumors are extremely rare and their diagnosis is challenging due to their heterogeneity. Appropriate therapy is based on exact localization and classification of the tumor, which can only be achieved by an interdisciplinary diagnostic approach. The present review summarizes such a diagnostic approach and provides an overview of androgen-producing tumors.
INTRODUCTION:Hysterectomy remains the most common major gynecological operation. This is the first study that describes a new technique of TLH without using any kind of uterine manipulator or vaginal tube (TLHwM) and analyzes the intra- and postoperative surgical outcome of the first 67 cases.PATIENTS AND METHODS:Between October 2008 and December 2009, 67 patients underwent TLH without uterine manipulator or vaginal tube. We analyzed the differences in the outcome by using three different kinds of surgical instruments: in 21 cases the TLHwM was performed using conventional 5 mm bipolar and scissors, in 22 cases using Sonosurgical, and in 24 cases using PKS cutting forceps.RESULTS:There was no intra- or postoperative complications. The overall mean operating time was by TLHwM with salpingo-oophorectomy 98 min and without salpingo-oophorectomy, 80 min. The mean operating time using cutting forceps was significantly lower. The mean uterine weight was 263 g.DISCUSSION:Uterine manipulator seems to be a safe and practical surgical method, especially for patients with vaginal stenosis and in cases of enlarged uterus. With its short operation time and no complication rate, we believe that this method is an enrichment of the laparoscopic hysterectomy techniques.
Ein Hyperandrogenismus findet sich bei 5–10% aller Frauen im reproduktionsfähigen Alter. Ergibt die klinische Untersuchung einen Hinweis auf eine sich rasch entwickelnde ausgeprägte Virilisierung, einen neu aufgetretenen Hirsutismus in der Postmenopause oder zeigt sich eine ausgeprägte Hyperandrogenämie, so besteht bis zum Beweis des Gegenteils Verdacht auf einen Tumor. Dementsprechend muss eine laborchemische Hormondiagnostik erfolgen. Bei fortbestehendem Tumorverdacht ermöglichen Stimulations- und Suppressionstests, transabdominale und vaginale Sonographie, CT, MRT und spezifische nuklearmedizinische Verfahren eine genaue Lokalisationsdiagnostik. Androgenproduzierende Tumoren sind selten und stellen für den Kliniker aufgrund ihrer Heterogenität eine diagnostische Herausforderung dar. Eine adäquate Therapie basiert auf einer exakten Lokalisationsdiagnostik und Tumorklassifizierung. Zudem setzt sie ein interdisziplinäres diagnostisches Vorgehen voraus. Der vorliegende Beitrag erläutert das diagnostische Vorgehen und gibt eine Übersicht über androgenproduzierende Tumoren.
Die in den 1990er-Jahren entwickelte laparoskopische suprazervikale Hysterektomie (LASH) gewinnt zunehmend an Bedeutung; dennoch gibt es bis heute keine prospektiv randomisierte Studie, welche die LASH mit den anderen Hysterektomietechniken vergleicht. Die Vorteile einer laparoskopisch im Vergleich zu einer abdominal durchgeführten Hysterektomie sind hinreichend bekannt. Trotzdem ist die abdominale Methode, auch fast 20 Jahre nach Durchführung der ersten laparoskopischen Hysterektomie, weiterhin weltweit die dominierende. Befürworter der LASH sind der Meinung, dass die Operation leichter durchzuführen ist, weniger invasiv ist und, im Vergleich zur totalen laparoskopischen Hysterektomie (TLH), eine geringere Rate an operationsbedingten Ureterverletzungen und Infektionen aufweist. Kritiker hingegen halten die postoperativ auftretenden Zervixstumpfprobleme, wie persistierende Blutungen und Schmerzen, und die daraus resultierenden Folgeoperationen für zu belastend für die Patientin. Ziel dieser Arbeit ist es, eine Übersicht über die aktuelle Datenlage zum Stellenwert der LASH zu geben.
Aus der minimal-invasiven Diagnostik hat sich die minimal-invasive Chirurgie entwickelt. Kleinere Eingriffe wie Arthroskopien mit ggf. Resektion von Knorpel, Cholezystektomien und Eingriffe an den weiblichen Geschlechtsorganen gehören heute zum operativen Standard. Um das kosmetische Ergebnis weiter zu verbessern und die Invasivität noch mehr zu senken, wurde ein Trokar entwickelt, mit welchem nur noch eine einzige laparoskopische Inzision notwendig ist. Die Technik und erste Erfahrungen sollen in diesem Artikel beschrieben werden.
Maligne Melanome des Genitales sind sehr selten. An der Vulva machen sie etwa 8–10% aller Vulvatumoren aus. Sie haben ihren Altersgipfel in der Postmenopause. Obwohl der kausale Zusammenhang zwischen ultravioletter Strahlung und der Genese von Melanomen sehr gut bekannt ist, scheint die Sonnenexposition bei malignen Melanomen der Vulva keine Rolle zu spielen. Chronisch-entzündliche Reize, virale Infektionen und chemische Noxen werden hauptsächlich für die Entstehung von Melanomen der Vulva verantwortlich gemacht. Die Therapieoptionen sind vielfältig, sie reichen von lokaler Tumorexzision und Sentinel-Node-Biopsie über eine radikale Resektion, mit En-bloc-Vulvektomie und inguinofemoraler Lymphadenektomie, bis hin zur Therapie mit Interferon-α. Einheitliche Empfehlungen finden sich in der aktuellen Literatur nicht. Die Prognose ist durch die hohe Metastasierungsrate und die oft erst sehr spät gestellte Diagnose schlecht. Angaben zur Fünfjahresüberlebensrate liegen zwischen 22 und 54%. Das jeweilige Therapiekonzept sollte in jedem Fall den individuellen Bedürfnissen der einzelnen Patientin angepasst sein. Wir berichten über eine 83-jährige Patientin, die sich mit einem asymptomatischen unklaren Tumor der Klitoris zur therapeutischen Intervention vorstellte.
Zielsetzung: Calcitriol 1,25(OH)2D3 ist aufgrund seiner antiproliferativen und pro-apoptotischen Effekte ein vielversprechendes Target. Die Cyclooxygenase-2 (COX-2) wird durch Onkogene, Wachstumsfaktoren und Zytokine induziert, Faktoren, die in malignen Prozessen eine Rolle spielen. Ziel war die Untersuchung der Expression von COX-2, 15-Prostaglandindehydrogenase (15-PGDH) und Vitamin D-Rezeptor (VDR) sowie des Einflusses von Calcitriol auf den Prostaglandin-Stoffwechsel.
Gibt es möglicherweise eine Korrelation zwischen der Expression von Cyclooxygenase-2 (COX-2), 15-Prostaglandindehydrogenase (15-PGDH) und dem Vitamin D-Rezeptor (VDR)? Gibt es außerdem einen Einfluss von Calcitriol [1,25(OH)2D3] auf den Prostaglandin-Stoffwechsel?
