This study exploratively investigated a possible relationship between suicidal/self-injurious behavior and the change in self-reported eating disorder symptoms as well as emotional dysregulation and interpersonal difficulties during eating disorder-specific treatment. For this purpose, the symptom index of the Eating Disorder Inventory-2 (EDI-SI) and the scale for the assessment of impulsivity and emotional dysregulation of borderline personality disorder (IES-27) with its subscales suicidal and self-injurious behavior (SVV/SEV), emotional dysregulation (EmotDys) and interpersonal difficulties (BezSchw) were collected from 167 female patients at a psychosomatic clinic. The participants were divided into four groups depending on the occurrence of SVV/SEV before the start and/or end of treatment. A one-way multivariate analysis of variance (MANOVA) with repeated measures showed an interaction of time and SVV/SEV group with regard to EDI-SI, EmotDys and BezSchw. The effect sizes of the pre-post comparison of the EDI-SI were very different in the groups divided according to the occurrence of SVV/SEV (0.27<g*< 1.32). No significant improvement in the EDI-SI was found on average for people with new occurrence of SVV/SEV during the course of treatment. People who were able to cease SVV/SEV in the course of treatment also benefited in terms of the EDI-SI. The decisive factor for the consideration of SVV/SEV in patients with eating disorders appears to be less the SVV/SEV reported before the start of treatment, but rather newly occurring or persistent SVV/SEV during the course of treatment.
The "Questionnaire of Thoughts and Feelings" (QTF) is being used as screening instrument as well as tool for treatment planning and treatment evaluation for patients with borderline personality disorder (BPD). The primary goal of this study was the validation of the dimensional structure of the short version with 14 items, QTF-14. Additionally, item characteristics, reliability and evidence of convergent validity were examined. A diagnostically homogenous sample of patients with BPD (N=3035) of a psychosomatic clinic was presented with several self-assessment inventories, including the QTF-14. The expected single-factor model of the structure of the FGG showed unacceptable model fit indices (CFI=0.751; TLI=0.706; RMSEA=0.115; SRMR=0.073). Exploratory factor analysis showed evidence of two or three specific factors. In a subsequent confirmatory factor analysis, a bifactor model with two specific factors proved to be preferable (CFI=0.956; TLI=0.936; RMSEA=0.054; SRMR=0.034). The internal consistency of the total scale as well as the suggested subscales "Relationship Difficulties and Emotional Dysregulation" and "Autoaggression" was acceptable to good (omega=.81-.84; alpha=.79-.85). Associations with similar scales were as expected. Good psychometric properties of the QTF-14 can be confirmed in this study. Using the suggested subscales could support treatment planning.

Zusammenfassung Der „Fragebogen zu Gedanken und Gefühlen“ (FGG) wird sowohl als Screeninginstrument als auch zur Therapieplanung und -evaluation bei PatientInnen mit Borderline-Persönlichkeitsstörung (BPS) eingesetzt. Primäres Ziel der vorliegenden Studie war die Validierung der zugrundeliegenden faktoriellen Struktur der 14 Items umfassenden Kurzversion FGG-14. Zusätzlich sollten Itemcharakteristika, Reliabilität und Hinweise auf konvergente Validität ermittelt werden. Eine diagnostisch homogene Stichprobe von PatientInnen mit BPS (N=3035) einer psychosomatischen Klinik erhielt verschiedene Selbstbeurteilungsinstrumente, darunter den FGG-14. Das erwartete einfaktorielle Modell der Struktur des FGG hatte keine ausreichende Güte der Modellfitindizes (CFI=0,751; TLI=0,706; RMSEA=0,115; SRMR=0,073). Explorative Faktorenanalysen zeigten Hinweise auf zwei oder drei spezifische Faktoren. In einer erneuten konfirmatorischen Faktorenanalyse erwies sich ein Bi-Faktor-Modell mit zwei spezifischen Faktoren als überlegen (CFI=0,956; TLI=0,936; RMSEA=0,054; SRMR=0,034). Die interne Konsistenz der Gesamtskala sowie der vorgeschlagenen Subskalen „Beziehungsschwierigkeiten und emotionale Dysregulation“ sowie „Autoaggression“ nahm akzeptable bis gute Werte (ω=0,81–0,84; α=0,79–0,85) an. Die Zusammenhänge zu konstruktnahen Skalen waren erwartungsgemäß. Die psychometrischen Gütekriterien des FGG-14 können damit bestätigt werden. Die Betrachtung der Subskalen kann die Behandlungsplanung unterstützen.
Zusammenfassung Trotz der hohen Relevanz dissoziativer Phänomene bei der Borderline-Persönlichkeitsstörung (BPS) ist das Konzept der pathologischen Dissoziation bisher kaum in diesem Kontext untersucht worden. Zur Operationalisierung von pathologischer Dissoziation wurden diverse Indizes vorgeschlagen, deren klinische Nützlichkeit in der vorliegenden Studie ebenso untersucht wurde wie ihre Häufigkeit, klinischen Korrelate und prädiktive Bedeutung. Hierzu bearbeiteten 487 stationäre BPS-Patienten bei Aufnahme Selbstbeurteilungsverfahren zu Dissoziation (Dissociative Experiences Scale, DES-28) sowie allgemeiner und Borderline-typischer Pathologie (Gesundheitsfragebogen für Patienten [PHQ-D], Borderline Symptom Liste [BSL-95]). Diejenigen Patienten, die mindestens 6 Wochen an der stationären Dialektisch-Behavioralen Therapie (DBT) teilgenommen hatten, füllten am Ende erneut den PHQ‑D und die BSL-95 aus ( n zwischen 342 und 361). Je nach Operationalisierung der Dissoziation litten zwischen knapp 20 % und 37 % der BPS-Patienten unter pathologischer Dissoziation. Diese war eng mit allgemeiner und Borderline-typischer Symptomatik bei Aufnahme assoziiert. Ein hohes Ausmaß an pathologischer Dissoziation zu Behandlungsbeginn prädizierte ein schlechteres symptombezogenes Therapieergebnis. Konsistent mit einer Vielzahl anderer Studien unterstreichen diese Befunde die Relevanz von Dissoziation bei der BPS. Die aus nur 8 Items der DES-28 bestehende DES-Taxon (DES-T) kann pathologische Dissoziation anwendungs- und auswertungsfreundlich erheben, erweist sich als mindestens ebenso relevant wie die DES-28 und hat für das symptombezogene Behandlungsergebnis sogar eine höhere prädiktive Relevanz. Daher empfiehlt sich ein Screening aller BPS-Patienten auf pathologische Dissoziation mit der DES‑T.
Despite the high relevance of dissociative phenomena in borderline personality disorder (BPS) the concept of pathological dissociation has so far been barely investigated in this context. To operationalize pathological dissociation, several indices have been proposed. Their clinical utility as well as the frequency, clinical correlates and predictive significance of pathological dissociation were investigated in this study. For this purpose, 487 inpatients with BPD completed self-assessment procedures for dissociation (dissociative experiences scale, DES-28) as well as general and borderline-typical pathology (patient health questionnaire, PHQ-D, borderline symptom list, BSL-95) on admission. Those patients who had participated in inpatient dialectical behavior therapy (DBT) for at least 6 weeks completed the PHQ-D and BSL-95 again before discharge (n =342-361). Depending on the operationalization of dissociation, between nearly 20% and 37% of BPD patients suffered from pathological dissociation. This was closely associated with general and borderline-typical symptomatology on admission. A large extent of pathological dissociation on admission predicted poorer symptom-related treatment outcome. Consistent with a multitude of other studies, these findings underline the relevance of dissociation in BPD. The DES-Taxon (DES-T), consisting of only 8 items from the DES-28, which can detect pathological dissociation in an application-friendly and evaluation-friendly manner, proves to be at least as relevant as the DES-28 and even has a higher predictive relevance for symptom-related treatment outcome. Therefore, screening of all BPD patients for pathological dissociation with the DES-T is recommended.
Zusammenfassung Zwar besteht Übereinstimmung darüber, dass die gestörte Emotionsregulation ein zentrales Merkmal der Borderline-Persönlichkeitsstörung (BPS) ist, doch gibt es anhaltend weiterhin konkurrierende Ansichten zur dimensionalen Struktur der Störung. Die „Skala zur Erfassung der Impulsivität und emotionalen Dysregulation“ (IES-27) wurde bisher erfolgreich bei PatientInnen mit BPS eingesetzt. Das Ziel der vorliegenden Studie war Hinweise auf die zugrundeliegende dimensionale Struktur mithilfe konfirmatorischer Faktorenanalysen zu gewinnen. Eine heterogene Stichprobe von PatientInnen mit BPS (n=484), Aufmerksamkeits-Defizits-(Hyperaktivitäts-)Störung (n=191) und Essstörungen (n=197) einer psychosomatischen Klinik erhielt mittels computergestützter Testung verschiedene Selbstbeurteilungsinstrumente. Es zeigte sich wie vermutet ein Bi-Faktor-Modell mit einem generellen Faktor und den 3 spezifischen Faktoren „Emotionale Dysregulation“, „Beziehungsschwierigkeiten“ und „Suizidales und selbstverletzendes Verhalten“ überlegen (ΔCFI=0,032). Die interne Konsistenz der Gesamtstichprobe nahm Werte zwischen ω=0,83−0,93 an. Es zeigten sich erwartungsgemäße Zusammenhänge zu konstruktnahen Subskalen. Die psychometrischen Gütekriterien der IES-27 konnten damit bestätigt werden. Die Bildung von Subskalen erlaubt eine differenzierte Darstellung und damit eine spezifischere Behandlungsplanung.
There is a growing consensus that emotion dysregulation is the defining feature of borderline personality disorder, but there are also persisting competing views on the pathology and dimensional structure of the disorder. The "Impulsivity and Emotion Dysregulation Scale" (IED-27) is already being used for assessing patients with borderline personality disorder. The particular aim of this study was to gain evidence of the dimensional structure of the disorder by confirmatory factorial analysis. A heterogeneous sample of patients with borderline personality disorder (n = 484), attention deficit (hyperactivity) disorder (n = 191) and eating disorders (n = 197) of a psychosomatic clinic were given general and specific self-assessment inventories via computer-assisted testing system, including the IED-27. Results showed a bifactor model with a general factor and three specific factors: "emotional dysregulation", "relationship difficulties" and "self-injuries and suicidal behaviours", to be the model with the best fit (Delta(CFI) = 0,032). The internal consistency of the sample seems to be good (omega = 0,83-0,93) and expected correlations to similar subscales could be proved. Good psychometric criteria of the IED-27 were replicated in this study. Prospectively, the subscales allow a more detailed representation and specific treatment planning.
Borderline personality disorder (BPD) is a severe mental disorder which is accompanied by impaired functioning, complex psychosocial problems and incurs large costs. However, it rarely has been examined whether BPD symptoms form a hierarchical (Mokken) scale, which would imply that symptoms are ordered along a latent dimension of borderline severity. In this study the data from 1,198 inpatients of a psychosomatic clinic were examined. A non-parametric Mokken analysis was employed to investigate if the BPD symptoms measured by the impulsivity and emotion dysregulation scale (IES-27) formed a hierarchical scale. 16 of the 27 items, mainly regarding emotion dysregulation, formed a monotonous Mokken scale with a very strong hierarchy of BPD symptoms. These results imply that single BPD symptoms provide information about BPD severity, suggesting several important consequences regarding escalation of BPD, diagnosis and treatment, e.g. how to optimize treatment planning.
Die Dialektisch-Behaviorale Therapie (DBT) ist die am besten empirisch abgesicherte störungsspezifische Psychotherapie für Patienten mit einer Borderline-Persönlichkeitsstörung (BPS). Auch ihre Anwendung im stationären Rahmen hat sich in kontrollierten und naturalistischen Studien als wirksam erwiesen. Ein Großteil der Studien inkludiert jedoch ausschließlich Frauen, sodass unklar bleibt, ob die Wirksamkeit der DBT auch für Männer mit einer BPS gilt. In der vorliegenden naturalistischen Psychotherapiestudie wurden daher 79 Männer und 366 Frauen, die eine stationäre DBT beendeten, mithilfe verschiedener Selbstbeurteilungsverfahren zur Borderline-spezifischen (Borderline Symptom Liste; BSL) und allgemeinen Psychopathologie (Brief Symptom Inventory; BSI) untersucht und hinsichtlich der Veränderungen der Aufnahme- und Entlassungswerte verglichen. Die Wirksamkeit der DBT unterschied sich nicht zwischen Männern und Frauen und die korrespondierenden Effektstärken lagen durchgängig im niedrigen bis mittleren Bereich. Gemessen an der BPS-spezifischen Pathologie sprachen 56,4 % aller Patienten auf die Behandlung an, und 12,8 % remittierten in dem Sinne, dass ihre Werte am Ende der Therapie im funktionalen Bereich lagen. Weder für die allgemeine noch für die Borderline-spezifische Psychopathologie fanden sich Geschlechtsunterschiede bei den Response- und Remissionsraten. Die Studie liefert erste empirische Evidenz dafür, dass Männer mit einer BPS von einer stationären DBT in ähnlicher Weise profitieren wie Frauen.
