Registered nurses are the largest single professional group working in the field of cancer care and support one of the most vulnerable patient cohorts in the healthcare system. Cancer nurses are known to experience high rates of burnout, but there are significant limitations to current research on the unique stressors experienced by this group of nurses, particularly since the emergence of COVID-19. This study employs the Job Demands Resource Model (JD-R model) to better understand the experiences of Victorian cancer nurses’ work and describe factors which ameliorate burnout and work-related stress. A multiple case study research design was used in this study, with two groups of cancer nurses making up a total of 30 participants, allocated to separate cases bounded by geographical location. A two-phase study of Victorian cancer nurses in metropolitan and regional healthcare services was conducted from 2019 to 2021. Data included field notes and in-depth interviews. Data analysis used a process of elaborative coding, with a pre-conceived coding framework based on the JD-R model. A combination of thematic analysis and storyline analysis was employed to analyse the data. A cross-case analysis of similarities and differences identified the job demands affecting cancer nurses, and conversely, any positive job resources which may buffer these demands. Job demands identified in both cases appeared to have similar causes but were more explicitly linked to poor resourcing in the regional case. Job resources identified in both cases were similar, but it was noted how few job resources were available to buffer the many demands inherent in cancer nurses’ work. This multiple case study found that the work of cancer nurses is high in demands and low in resources. Despite challenging work conditions, findings identified a highly engaged workforce. The job resources identified in this study suggest there are modifiable strategies to cultivate a supportive work environment for cancer nurses.
Background: Cancer nurses are known to experience high rates of burnout. Despite cancer nurses being critical to providing care for people in rural and remote areas, there is limited research exploring the unique challenges and job demands experienced by regional cancer nurses, and the job demands that may act to buffer these stressors. Aim: This study aims to identify antecedents of burnout among Victorian regional cancer nurses over a two-year period, by examining the job demands and resources prevalent in their work. We will build on existing literature by identifying specific job resources that can ameliorate job demands experienced by regional cancer nurses. Methods: A qualitative case study with participants located in regional Victoria, Australia, was employed. Semistructured interviews were conducted both before and during the COVID-19 pandemic. Findings: Patient aggression, emotional demands, unsupportive peers and management, and resourceconstrained work environments were reported as distinct job demands; while job significance, supportive peers and leadership, and connection to community were reported as job resources. Conclusion: In regional settings, patient aggression, workforce shortages, hazardous work environments, and inconsistent digitisation of patient management systems are common characteristics of under-resourced workplaces. Despite these challenges, participants reported a strong desire to remain in the profession, mostly due to their commitment to their communities. Health professionals impacted by patient safety incidents are referred to as 'second victims'. The aim of this review was to map available information on second victim recovery programs for nurses following a patient safety incident which employs education and training via simulation methodology. A scoping review was undertaken with a literature search conducted between 2021 and 2023 using the CINAHL (EBSCO), MEDLINE, ProQuest (Nursing), EMBASE (Ovid), Scopus, ERIC, Web of Science, PubMed and PsycINFO (OVID) databases and included reference lists. A total of six papers were included. Simulation methods were used in the training of peer responders, with no papers discussing the use of formal educative approaches or simulation in support programs for second victims. Some evidence identified denoted recovery elements in second victims. More evidence is required to classify the recovery status of second victims and to understand the appropriateness of educative approaches in second victim support programs, specifically the role of simulation and it's potential to positively influence learners’ skills, emotions, and ability to reflect constructively on past patient safety incidents.
BACKGROUND:Existing studies that seek to understand nurses' experiences of burnout are dominated by cross-sectional, quantitative survey designs employing predetermined measures, often overlooking important job-related stressors that can be highly dependent on industry and professional contexts. Cancer nurses are a group of professionals who warrant special attention, as burnout in this profession is often attributed to high job demands and the challenge of caring for a vulnerable cohort of patients. A deeper understanding of the job demands associated with cancer nursing is required to provide insights about the work experiences of cancer nurses and identify aspects that mitigate burnout and stress.PURPOSE:This study describes the antecedents of burnout among Australian cancer nurses by focusing on the demands and resources inherent in their work. We aim to build on the existing literature by identifying job resources that may serve to mitigate the antecedents of burnout.METHODOLOGY/APPROACH:An in-depth interview study of cancer nurses across a spectrum of age and experience in Australian metropolitan public health care services was conducted over a 2-year period that coincided with the COVID-19 pandemic. The job demands and resources model framed this study of job-related factors associated with burnout and conversely job resources that may foster work engagement.RESULTS:Patient aggression, workload, emotional demands, and abusive peers and managers were reported as distinct job demands, whereas job significance and supportive peers who demonstrated leadership, along with task variety, were identified as job resources.CONCLUSION:Australian cancer nurses work in an environment where job demands are increasingly disproportionate to job resources, leading to significant risk of burnout.PRACTICE IMPLICATIONS:Our study identifies modifiable strategies for improving work conditions for this group who play a critical role in the health care system.
Introduction: The clinical placement learning environment is a critical component of nursing education where Australian nursing students spend a minimum of 800 h. Identifying components of successful clinical placements for undergraduate nursing students is therefore paramount.Purpose: To assess nursing students' views of the learning environment during clinical placement with an emphasis on the pedagogical atmosphere, leadership style of the ward manager, and premises of nursing on the unit or ward.Material and Methods: The study used Clinical Learning Environment, Supervision and nurse teacher (CLES + T) questionnaire to examine 150 final year undergraduate students' perceptions of the clinical placement learning environment. The questionnaire was anonymous and completed by the students at the end of their clinical placement. The statistical program SPSS v22 was used. Principal components analysis (PCA) for data reduction was run on the 42-question section of the first dimension ('pedagogical atmosphere on the ward') of the questionnaire that measured the perceptions of the learning environment of the clinical placement of the 150 final-year undergraduate nursing students. The comments sections of the factors were subjected to interpretive content analysis to create the themes for the two components.Results: Principle Component Analysis revealed two components that had eigenvalues greater than one: 'Happy to Help' Component 1 and 'Happy to be Here' Component 2. These components were statistically significant (p < 0.0005), using Bartlett's Test of Sphericity indicating that the data was likely factorizable. These components scored higher than any other related factors.Conclusions: Student nurses value a welcoming workplace where staff and educators are happy to help and have a positive attitude to student presence on the wards. More than any other factors these ward-based factors appear to have the strongest influence on student satisfaction. (C) 2016 Elsevier Ltd. All rights reserved.
