Osteoporosis is considered the most frequent skeletal manifestation of systemic mastocytosis (SM). We performed a retrospective analysis of sixty patients (37 males and 23 females) who underwent a bone biopsy in the assessment of SM or in the assessment of unexplained bone fragility. Thirty-three had simultaneously a bone marrow biopsy with a Jamshidi's needle; this sample was used for immunohistochemical analysis (tryptase, c-KIT. CD20, VCAM-1). Bone biopsy was realized in 42 cases in the assessment of SM to provide histologic proof of the disease and in 18 cases in the assessment of unexplained bone fragility and surprisingly revealed a SM. An increased bone turnover was observed in patients with SM with elevated eroded surfaces, osteoclast number and bone formation rate. In addition to nodules of mast cells (MC), a high number of MC was directly apposed on the trabeculae, affixed on the osteoblasts or the lining cells. The VCAM-1 adhesion protein recognizing α4β7 and α4β1 integrins may be a candidate to explain this particular adherence. One third of the bone marrow biopsies did not exhibit MC nodules or MC infiltration and led to a false negative diagnosis for SM. SM can be discovered in the assessment of fracture or osteoporosis. Transiliac bone biopsy allows for the diagnosis of the disease more accurately than bone marrow biopsy; it also provides a histomorphometric analysis of bone remodeling.
La prescription d’un traitement de fond de l’ostéoporose est associée avec une réduction de 30 à 70 % du risque de fracture. La prolongation du traitement par les anti résorbeurs est associée avec une majoration du risque de fracture atypique du fémur et d’ostéonécrose mandibulaire. En raison de l’efficacité rémanente des BP, il est souvent proposé aux patients de suspendre le traitement, après une séquence thérapeutique de 3 à 5 ans. L’incidence et la localisation des fractures survenant au cours de cette période de “vacances thérapeutiques” sont encore mal connues. L’objectif de cette étude était de décrire la survenue des fractures chez plus de 200 patients, d’une part au cours de la période thérapeutique et d’autre part dans les 3 années suivant la suspension du traitement de fond. La cohorte comporte 203 patients traités dans le cadre d’une consultation spécialisée d’un CHU. Chez tous ces patients, le traitement a été suspendu par le rhumatologue avec ensuite une surveillance organisée tous les 2 à 3 ans. Nous avons évalué la gravité initiale de l’ostéoporose (âge, facteurs de risque, DMO, fractures), l’évolution de la DMO et la survenue des fractures : – au cours de la séquence thérapeutique puis ; – au cours de la période de suspension thérapeutique. Tous les patients ont eu une VFA ou des radiographies du rachis à intervalles réguliers. Au début de la séquence thérapeutique, les patients sont âgés en moyenne de 63,1 ans avec un IMC de 23,8 kg/m2. L’ostéoporose est fracturaire chez 112 patients (55,2 %) avec une majorité de fractures vertébrales (91 patients, 44,8 %). Le traitement de fond comporte un bisphosphonate chez 202 patients (raloxifène seul dans un cas) toujours associé à de la vitamine D. Pendant la séquence thérapeutique, d’une durée moyenne de 72,3 mois, nous avons observé la survenue de fractures chez 27 patients (13,3 %) dont 23 patients avec une fracture majeure. La localisation concerne en particulier les vertèbres (n = 16), le bassin et l’ESF (n = 3), l’humérus et le tibia (n = 4), les cotes (n = 1) et le poignet (n = 3). Pendant la période de suspension thérapeutique, d’une durée moyenne de 46 mois, nous avons observé la survenue de fractures chez 38 patients (18,7 %) dont 31 patients avec une fracture majeure, avec en particulier les vertèbres (n = 27), le bassin et l’ESF (n = 4), l’humérus (n = 2) et le poignet (n = 7). Si l’on prend en compte les 36 premiers mois suivant la suspension du traitement, la densité osseuse reste inchangée sur le rachis et le col fémoral et l’incidence des fractures est faible : 18 patients (8,8 %) dont 13 fractures majeures et 5 fractures du poignet. L’analyse en régression logistique multivariée (intégrant âge, DMO, facteurs de risque clinique, statut fracturaire, durée du traitement) montre que pendant la période thérapeutique, l’âge élevé (RR 1,08, p = 0,008) et la durée du traitement (RR 1,23 p = 0,007) sont associés avec la survenue des fractures. Pendant la période de suspension thérapeutique, le caractère fracturaire initial de l’ostéoporose (RR 3,6, p = 0,012), la DMO fémorale de fin de séquence (RR 0,48, p = 0,014) et la durée de traitement (RR 1,18, p = 0,017) sont associés à la survenue des fractures. Cette étude montre que 13 % des patients traités pour une ostéoporose fracturent sous traitement et que lors des 3 années suivant l’arrêt des BP, la DMO reste inchangée et 9 % des patients présentent une nouvelle fracture. Cette étude suggère que la suspension d’un traitement par BP pendant 3 ans après une séquence thérapeutique bien conduite n’est pas délétère pour les patients, l’incidence des fractures étant plus faible que sous traitement.
OBJECTIVE OF THE STUDY:Alcohol-induced secondary osteoporosis in men has been characterized by higher fracture prevalence and a modification of bone microarchitecture. Chronic alcohol consumption impairs bone cell activity and results in an increased fragility. A few studies highlighted effects of heavy alcohol consumption on some microarchitectural parameters of trabecular bone. But to date and to our knowledge, micro- and macro-mechanical properties of bone of alcoholic subjects have not been investigated.PATIENTS:In the present study, mechanical properties and microarchitecture of trabecular bone samples from the iliac crest of alcoholic male patients (n=15) were analyzed and compared to a control group (n=8).MATERIALS AND METHODS:Nanoindentation tests were performed to determine the tissue's micromechanical properties, micro-computed tomography was used to measure microarchitectural parameters, and numerical simulations provided the apparent mechanical properties of the samples.RESULTS:Compared to controls, bone tissue from alcoholic patients exhibited an increase of micromechanical properties at tissue scale, a significant decrease of apparent mechanical properties at sample scale, and significant changes in several microarchitectural parameters. In particular, a crucial role of structure model index (SMI) on mechanical properties was identified.CONCLUSIONS:3D microarchitectural parameters are at least as important as bone volume fraction to predict bone fracture risk in the case of alcoholic patients.
The Stickler syndrome (SS) has been described as a “hereditary progressive arthro-ophtalmopathy” by Stickler in 1965, due to mutations on the collagen genes. Currently about 40 different genes have been identified which encode for at least 27 different collagens. The majority of mutations occur in the COL2A1 gene on chromosome 12q13 (SS type I). Mutations in COL11A1 are less frequent (SS type II). More recently, mutations in COL11A2 and in the COL9A1 gene have been reported with particular phenotypes. The main features of this autosomal inherited disease are ocular, auditory with orofacial abnormalities and early-onset osteoarthritis. We report the clinical presentation of an adult and his son, with a particular focus on the bone status of the father, radiography, bone densitometry and transiliac bone biopsy showing that he was suffering from osteoporosis. The lumbar bone mineral density was low with a Z-score at −2.9. Transiliac bone biopsy showed a dramatic decrease of trabecular bone volume (8.6%; Nl: 19.5±4.9%), thin trabeculae and a disorganized trabecular network. A slight increase of osteoid parameters was observed. Bone resorption was markedly increased with an excessive number of active (TRAcP+) osteoclasts. The cortical width was normal, but a slight increase of cortical porosity was found. Osteoporosis has been rarely described in the SS. It might be useful to systematically perform a bone densitometry in all patients with SS and to discuss the indication of a transiliac bone biopsy in severe cases.
Fibrodysplasia ossificans progressiva is a very rare heritable disease characterized by a progressive heterotopic endochondal ossification, occurring in the first decade of life, and leading thereafter to a severe ankylosis of the spine, limbs and jaw, with a progressive and severe functional disability. To date the cause of the disease remains unknown and no medical treatment has been proved efficient. It has recently been shown that a recurrent mutation in activation domain of the activin-receptor IA (ACVR1), a BMP receptor, could lead to an abnormal signalling pathway of BMP-4 and contribute to the occurrence of the devastating lesions characteristic of the disease.
La enfermedad de Paget es una osteopatía focalizada que afecta a una o a varias piezas óseas. Se caracteriza por una remodelación anárquica y excesiva del tejido óseo, con aumento de la actividad de resorción y de formación, que conduce a una desorganización de la estructura del tejido óseo y de la morfología de los huesos afectados. La etiología exacta sigue siendo desconocida; en la actualidad, además de una hipotética etiología viral se discute el papel de diferentes mutaciones, como la del secuestosoma, en la aparición de la enfermedad. Este enfoque genético hace suponer que una «selección» de los pacientes más expuestos a las complicaciones de la enfermedad permitiría adoptar medidas preventivas precoces. Aunque es casi siempre asintomática, la enfermedad de Paget se manifiesta en un tercio de los casos, aproximadamente, por dolores y deformaciones óseas que alteran la calidad de vida, así como por complicaciones osteoarticulares (artropatías, fisuras) o neurosensoriales (compresión medular). Por fortuna, la degeneración sarcomatosa, de pronóstico sombrío, es poco frecuente. Para el estudio inicial es necesario realizar una exploración física, determinar la concentración de las fosfatasas alcalinas totales, efectuar una serie de radiografías estándar y una gammagrafía ósea de cuerpo entero; en la mayoría de los casos, estas pruebas permiten establecer el diagnóstico, evaluar las lesiones óseas que pueden presentar complicaciones y definir las indicaciones terapéuticas. Hoy en día, además de los aminobisfosfonatos (orales o en perfusión intravenosa), se dispone de potentes tratamientos antirresortivos de acción prolongada. Así, el tratamiento permite normalizar la concentración de fosfatasas alcalinas, y al controlar la remodelación celular ósea, es posible reducir o incluso evitar la aparición de complicaciones, a veces graves, a las que expone esta afección ósea.
The number of serum 25-hydroxyvitamin D (25OHD) assays has increased tenfold in France in less than 10 years, sometimes for invalidated reasons. In 2013, the French National Authority for Health (Haute autorité de santé, or HAS) limited the indications for serum 25OHD measurements to rickets/osteomalacia, older adults with recurrent falls, monitoring of kidney transplant in adults, and surgical treatment of obesity in adults. Our aim here was to note that other indications for serum 25OHD measurements are supported by previous literature and by a number of national and international recommendations, in particular the following: any situation of bone fragility, any chronic renal failure <45 mL/min/1.73m2, any situation of malabsorption, clinical signs consistent with vitamin D deficiency or vitamin D overload, and calcium phosphorus evaluation. We suggest that the measurement of serum 25OHD concentration should remain reimbursed as part of these extended indications.
Multiple myeloma is due to the proliferation of malignant plasma cells which increase the number of osteoclasts leading to trabecular and cortical bone osteolysis. The 5T2MM murine model reproduces the human disease and microcomputed tomography is a precise tool to investigate bone loss. Bisphosphonates (zoledronic acid or pamidronate) are used in preventing osteolysis. However, loss of cortical bone in not possible to quantify by histomorphometry on histological sections or microCT images. Osteolysis was studied in mice grafted with the 5THL subline to see if one drug was more active after 10 weeks. Mice were distributed into 4 groups: control, untreated, treated with pamidronate or with zoledronic acid. The left femurs were embedded undecalcified and sectioned at 7 μm. The right tibias and femurs were analyzed by microCT and trabecular morphometric parameters were obtained. Cortical bone osteolysis was analyzed by developing a new algorithm to unwrap microCT sections of the cortices, allowing measurement of the number of perforations, porosity and mean perforation area. The bisphosphonates had no significant effect on the tumor growth as evidence by the absence of effect on the M-protein level. Cortical perforations were evidenced on histological sections and their number seemed to be reduced by both bisphosphonates. MicroCT was used to quantify the trabecular bone: a bone loss was evidenced in the untreated myeloma group and both bisphosphonates appeared equal to preserve trabecular mass. However, the number and size of cortical perforations cannot be determined on 3D models. Unwrapping microCT images provided flat images allowing a precise determination of cortical perforations. Pamidronate did not reduce the number and size of cortical perforations but significantly reduced porosity. Zoledronic acid appeared significantly superior and considerably reduced all parameters. Unwrapping microCT image is a new method allowing the measurement of cortical perforations in bone malignancies, a parameter that cannot be measured correctly on 2D histological sections.
Monoclonal gammopathies of undetermined significance (MGUS) have been shown to be associated with an increased risk of fractures. This study describes prospectively the bone status of MGUS patients and determines the factors associated with vertebral fracture. We included prospectively 201 patients with MGUS, incidentally discovered, and with no known history of osteoporosis: mean age 66.6±12.5 years, 48.3% women, 51.7% immunoglobulin G (IgG), 33.3% IgM and 10.4% IgA. Light chain was kappa in 64.2% patients. All patients had spinal radiographs and bone mineral density measurement in addition to gammopathy assessment. At least one prevalent non-traumatic vertebral fracture was discovered in 18.4% patients and equally distributed between men and women. Fractured patients were older, had a lower bone density and had also more frequently a lambda light chain isotype. Compared with patients with κ light chain, the odds ratio of being fractured for patients with λ light chain was 4.32 (95% confidence interval 1.80–11.16; P =0.002). These results suggest a high prevalence of non-traumatic vertebral fractures in MGUS associated with lambda light chain isotype and not only explained by low bone density.
OBJECTIVES:We recently introduced a new methodology called quantitative X-ray imaging (qXRI) to investigate bone mineral density in isolated rodent bones. The aims of the present study were to compare DXA and microCT with qXRI in a rat model of disuse osteoporosis.METHODS:Fourteen Copenhagen rats were injected with a single dose of botulinum toxin (BTX - 2 UI) in the right Mus quadriceps femoris. The left hindlimb serves as control. Areal BMD and vBMD were determined with a Hologic Discovery-W device and a Skyscan 1172 microcomputed tomograph (microCT). Absorbing material density (AMD) was determined on digitized X-ray images obtained with a Faxitron M020 device.RESULTS:All three methods highlighted significant lower values for aBMD, vBMD and AMD in trabecular and cortical bone in the BTX-injected side. In trabecular bone, aBMD, vBMD and AMD were significantly correlated with BV/TV. In cortical bone, only aBMD and vBMD were significantly correlated with cortical bone mass On the other hand, only AMD was significantly correlated with the mechanical parameters bending strength and bending modulus.CONCLUSIONS:qXRI is a rapid and cheap method to assess trabecular bone mass in isolated rodent bones and can be used as a surrogate for the densitometry of small animals.
Pour le diagnostic de fracture vertébrale (FV) chez l’homme, la concordance entre les médecins est faible. Évaluer la concordance entre experts pour le diagnostic de FV, chez l’homme, sur radiographie standard. Évaluer cette concordance après une réunion de consensus et établissement d’un algorithme. La concordance de 13 rhumatologues, expérimentés, a été calculée chez 30 hommes ostéoporotiques. Une réunion de consensus a été mise en place ; ont été analysés 28 autres dossiers d’hommes ostéoporotiques avec suivi radiologique et FV incidente confirmée. Les experts ont défini puis hiérarchisé 18 critères sémiologiques radiologiques et établi un algorithme de diagnostic de FV. Onze experts ont relu le premier set. La comparaison des deux lectures, sans et avec algorithme, a été étudiée. Après consensus et usage de l’algorithme les résultats sont : nombre de patients fracturés (présentant au moins une FV) de 13 à 26 sur 30 (13 à 28 à la 1ère lecture), moyenne de la concordance à l’échelle du patient : 75 % (70 % à la 1ère lecture) ; nombre de FV détectées sur 390 vertèbres évaluées : 18 à 59 (18 à 98 à la 1ère lecture) ; moyenne du pourcentage de concordance entre les experts à l’échelle de la vertèbre 92 % (89 % à la 1ère lecture). L’algorithme a permis une bonne amélioration de la concordance, surtout pour 8 des 11 lecteurs. Il existe des discordances entre les experts pour le diagnostic de FV. L’algorithme a permis d’améliorer la concordance.
La supplémentation en calcium d’origine pharmacologique est largement utilisée pour la prévention de la carence en apports alimentaires de calcium ou comme traitement adjuvant de l’ostéoporose. Quelques études, analyses secondaires d’essais thérapeutiques, méta-analyses font état d’un risque augmenté d’accidents cardiovasculaires, essentiellement des infarctus du myocarde. Cet effet indésirable pourrait être causé par la constitution de calcifications vasculaires. Il n’existe pas pour l’instant d’essai clinique destiné à démontrer spécifiquement la réalité de ce risque. Dans le doute, il est recommandé d’ajuster les apports de calcium par la supplémentation aux besoins de l’individu en fonction de son âge, de son sexe et de sa situation physiologique tout en privilégiant les apports de calcium d’origine alimentaire.
Le syndrome de fragilité est secondaire à différents processus physiopathologiques d’intensités très variables d’un sujet à l’autre qui conduit, du fait d’une baisse des réserves d’une personne, à une décompensation fonctionnelle rapide en cas de stress telle qu’une pathologie fracturaire ou douloureuse. La diminution des capacités de réserve va freiner les capacités à réaliser des efforts physiques chez la personne âgée. La fatigabilité et la sédentarité vont contribuer à l’ostéoporose et la sarcopénie. Laisser la fragilité s’installer contribue à ce cercle vicieux, à l’origine de conséquences comme le risque de chutes et d’événements fracturaires. La personne âgée n’est pas toujours consciente de cet état de fragilité, de son instabilité ou parfois le banalise : cet état de santé peut être vécu passivement par le sujet âgé, ses aidants ou les soignants. Or, l’enjeu est justement l’inverse : savoir repérer activement cet état Fragile dont le caractère encore réversible doit inciter à s’y intéresser. Un événement marquant sur la santé d’un sujet âgé, comme une chute au cours de l’année passée, peut être une complication de la fragilité : elle a valeur d’alarme et témoigne d’une désadaptation de l’individu à son environnement. L’expertise rhumatologique a toute sa place chez la majorité des sujets âgés fragiles. Le repérage de facteurs de risque de fragilité a pour but de stabiliser la santé de l’individu et de contribuer à l’établissement d’une stratégie thérapeutique rhumatologique afin d’obtenir l’effet optimum que l’on attendrait chez un adulte plus jeune. Les recommandations de cette prise en charge de la fragilité participent ainsi à limiter le déconditionnement, la sarcopénie et finalement la perte d’autonomie.Multiple physiopathological processes are supported the concept of frailty syndrome. Clinical frailty presentation is heterogeneous from one subject to other mainly related to individual decrease of reserves. Frailty is associated sometimes to early functional decline in the event of stress such as fracture or painful pathology. The decrease in reserve capacity will slow down the level of physical exercise of the elderly. Thus sedentary lifestyle will contribute to osteoporosis and sarcopenia. Let the frailty settle contribute to a vicious circle that develops is accompanied by exhaustion and insidiously increase sarcopenia, the risk of falls and fracture events. Old persons living in the community are not always aware of their frailty status and its progression, that however can be perceived by the elderly, family and relatives or caregivers. Much more, at the opposite, the challenge being to identify frailty this state whose character yet reversible should encourage interest. A milestone event that modified the health status of an old person, such as a fall during the past year, can be a indicator of frailty. In this situation, fall must be considered as an alarm sign of a inadapted response of the old person to his surroundings. Rheumatologic skill is fully part of care for frail old people. Identifying risk factors for frailty in order to improve the health of the individual and contribute to the positive effect of treatment strategy that would be expected to be similar that in a younger adult. These recommendations to prevent Frailty may help to limit functional decline, sarcopenia and disabilities.

Ceci ne signifie pas qu’il faille doser ce parametre biologique chez tous les patients. Bone remodeling is under complex regulation from nervous, hormonal and local signals, including gut hormones. Among the gut hormones, a role for the glucose-dependent insulinotropic polypeptide (GIP) has been suggested. However, the rapid degradation of GIP in the bloodstream by the ubiquitous enzyme dipeptidyl peptidase-4 (DPP-4) precludes therapeutic use. To circumvent this problem, a series of N-terminally modified GIP agonists have been developed, with N-AcGIP being the most promising. The aims of the present study were to investigate the effects of N-AcGIP on bone at the micro-level using trabecular and cortical microstructural morphology, and at the tissue-level in rats. Copenhagen rats were randomly assigned into control or N-AcGIP-treated groups and received daily injection for 4weeks. Bone microstructural morphology was assessed by microCT and dynamic histomorphometry and tissue-level properties by nanoindentation, qBEI and infra-red microscopy. Four week treatment with N-AcGIP did not alter trabecular or cortical microstructural morphology. In addition, no significant modifications of mechanical response and properties at the tissue-level were observed in trabecular bone. However, significant augmentations in maximum load (12%), hardness (14%), indentation modulus (13%) and dissipated energy (16%) were demonstrated in cortical bone. These beneficial modifications of mechanical properties at the tissue-level were associated with increased mineralization (22%) and collagen maturity (13%) of the bone matrix. Taken together, the results support a beneficial role of GIP, and particularly stable analogs such as N-AcGIP, on tissue material properties of bone.
La présente revue de la littérature a pour objectif d’argumenter les nouvelles recommandations françaises publiées en 2012 retenant le risque de chute, évalué par l’antécédent de chute dans la dernière année, comme indication de la mesure de la densité minérale osseuse (DMO) par l’absorptiométrie biphotonique par rayons X (DXA). Cette recommandation repose sur le fait que premièrement, l’ostéoporose et le risque de chute représentent les deux premiers facteurs de risque de fracture non vertébrale après la ménopause. Deuxièmement, la mesure de la DMO par DXA apporte une information significative sur le risque de fracture, indépendamment du risque de chute. Ainsi, un sujet chuteur aura d’autant plus de risque de se fracturer qu’il aura une DMO basse. Troisièmement, les traitements anti-ostéoporotiques n’ont montré une efficacité anti-fracturaire qu’en cas d’ostéoporose ostéodensitométrique. Quatrièmement, la prévalence de l’ostéoporose est élevée chez les sujets chuteurs d’autant que ceux-ci sont fragiles [chutes répétées, sarcopénie (faible masse et force musculaire), réduction de mobilité, perte de poids, en particulier], ces facteurs étant des facteurs de risque communs d’ostéoporose et de chute. Cependant, l’indication de la DXA doit être nuancée chez les sujets chuteurs dont l’espérance de vie est limitée, les traitements n’ayant montré une efficacité anti-fracturaire qu’après 12 mois d’administration et en cas de faible accessibilité à la DXA (éloignement, patients dépendants, ayant une altération cognitive sévère par exemple). Des travaux sont souhaitables pour mieux définir comment intégrer le risque de chute et la fragilité dans l’évaluation du risque de fracture par la DMO et le score FRAX®.
The agreement for vertebral fracture (VF) diagnosis in men, between doctors is poor.Objectives: To assess the agreement for VF diagnosis, in men, on standard radiographs, between experts, before and after consensual workshop and establishing an algorithm.Methods: The agreement between thirteen experimented rheumatologists has been calculated in thirty osteoporotic men. Then, the group discussed in a workshop and 28 other radiograph sets of osteoporotic men with follow-up radiographs and incident confirmed VF, have been reviewed. The experts identified and hierarchised 18 pathological features of vertebral deformation and established an algorithm of VF diagnosis. Eleven experts have realized a second reading of the first set of radiographs. We compared the agreement between the 2 readings without and with the algorithm.Results: After consensus and the use of the algorithm the results are: number of fractured patients (with at least 1 VF) according to the experts varies from 13 to 26 patients out of 30 (13 to 28 during the first reading). The agreement between the experts at the patient level is 75% (70% at the first reading). Among the 390 vertebrae analyzed by the experts, the number of VF detected varies from 18 to 59 (18 to 98 at the first reading). The agreement between the experts at the vertebral level is 92% (89% at the first reading). The algorithm allows a good improvement of the agreement, especially for 8 of the 11 experts.
