There are two groups of undergraduate students involved in endoscopic surgery with different degrees of experience: average and more experience. This study proves whether the subjective impression of the laparoscopic trainer is verifiable and which factors influence extreme talent.
The reliability of the Rockwood classification, the gold standard for acute acromioclavicular (AC) joint separations, has not yet been tested. The purpose of this study was to investigate the reliability of visual and measured AC joint lesion grades according to the Rockwood classification.
Penetrating injuries are rare in European populations so their management represents a particular challenge. The aim was to assess early therapeutic aspects that are associated with favourable outcomes in patients with penetrating trauma.
Ziel dieser Studie war es, den aktuellen Stand der Therapie der Schultereckgelenkverletzung in Deutschland zu erfassen und die Entwicklungen der letzten 10 Jahre sowie die Umsetzung neuer Therapieverfahren zu untersuchen.
In diesem Beitrag wird über den Fall eines Patienten mit anterioren Schulterschmerzen und einer periglenoidalen Zyste, assoziiert mit einer GLAD-Läsion („glenolabral articular disruption“), 7 Jahre nach initialem Schultertrauma berichtet. Bei zunächst jahrelang nur geringen und nicht einschränkenden Beschwerden, kam es zuletzt zu einer Symptomzunahme. Die klinische Untersuchung zeigte anteriore Schulterschmerzen bei negativen Instabilitätsstests. Die durchgeführte MRT-Bildgebung zeigte eine periglenoidale Zyste. In der dann folgenden Arthroskopie wurde eine vernarbte anteroinferiore Labrumläsion mit begleitendem drittgradigem Knorpelschaden des anteroinferioren Glenoids festgestellt. Die Knorpelglättung und Refixation des anteroinferioren Labrums führte im Verlauf zur Beschwerdefreiheit des Patienten. Es handelt sich hierbei um den seltenen Fall einer periglenoidalen Zyste im Zusammenhang mit einer GLAD-Läsion.
The present article aims to give an overview of the current surgical techniques for arthroscopic rotator cuff repair. Depending on the affected tendons and the size of the tear several methods of refixation are available. In articular sided partial tears of the supraspinatus tendon a transtendinous repair of the tendon can be performed with good clinical outcomes. In complete tears of the superior and posterosuperior cuff single and double-row techniques are available. Despite the promising biomechanical properties of double-row techniques the respective clinical results are not superior to a single-row technique with a modified suture configuration. Whereas tears of the subscapularis tendon have long been thought to be only repairable in an open surgery the results of arthroscopic refixation especially of the cranial part of the tendon are promising. Biological augmentation of rotator cuff repairs with platelet-rich plasma was not able to increase the healing rate so far.
