L’objectif de ce travail est d’évaluer le bénéfice d’un programme court de rééducation de 2 semaines sur la présence au travail et le recours aux arrêts de travail à un an chez des patients lombalgiques chroniques. Il s’agit d’une étude rétrospective. Tous les patients ayant bénéficié d’une rééducation de deux semaines en hospitalisation de médecine physique et de réadaptation entre janvier 2018 et mars 2019, pour le motif lombalgie chronique ont été inclus. Sur les 311 patients éligibles, 134 répondaient aux critères d’inclusions. Sur ces derniers, 96 patients ont répondu au questionnaire à un an d’évaluation. Concernant les patients qui ont pu être évaluée à un an, l’âge moyen était de 46 ans, l’IMC 25,5, l’ancienneté de la lombalgie était de 5,5 ans [2 ; 15], 27 % avaient une radiculalgie associée. Cinquante-cinq pour cent des patients avaient une activité professionnelle à l’entrée, 31 % étaient en arrêt de travail, 14 % sans emploi. La médiane de la durée des arrêts cumulés d’arrêts de travail était de 210 j [83–338]. À la sortie les patients étaient améliorés en termes de douleur (EVA à 3 versus 4,6 à l’entrée, p < 0,001). À un an 64 % avaient une activité professionnelle, 24 % étaient en arrêt de travail, 12 % sans emploi. On observe une diminution de 15 % des arrêts de plus de 6 mois, une diminution de 12 % des arrêts de plus de 3 mois, une diminution de 7 % des arrêts de plus de 1 mois par contre une augmentation de 10 % des arrêts de moins d’1 mois. Les deux secteurs les plus représentés dans le groupe non présent au travail à un an étaient le bâtiment (23 %) et l’aide et soin à la personne (20 %). À un an 73 % pratiquait des auto exercices en moyenne 4 fois par semaine. Quarante-six pour cent pratiquaient une activité physique en moyenne deux fois par semaine. Quatre-vingt-cinq pour cent des patients à un an étaient satisfaits ou très satisfaits du programme. Cette étude rétrospective montre qu’un programme de 2 semaines de rééducation augmente le nombre de patients lombalgiques ayant une activité professionnelle à un an (64 % versus 55 % mais sans différence statistiquement significative p à 0,24), diminue la durée des arrêts de travail (p à 0,023).
OBJECTIVE:An expert working group was set up at the initiative of the French Ministry of Sports with the objective of harmonising the management of sport related concussion (SRC) in France, starting with its definition and diagnosis criteria.RESULTS:Definition: A clinical definition in 4 points have been established as follows: Concussion is a brain injury: 1) caused by a direct or indirect transmission of kinetic energy to the head; 2) resulting in an immediate and transient dysfunction of the brain characterised by at least one of the following disorders: a) Loss of consciousness, b) loss of memory, c) altered mental status, d) neurological signs; 3) possibly followed by one or more functional complaints (concussion syndrome); 4) the signs and symptoms are not explained by another cause. Diagnosis criteria: In the context of the direct or indirect transmission of kinetic energy to the head, the diagnosis of concussion may be asserted if at least one of the following signs or symptoms, observed or reported, is present within the first 24hours and not explained by another cause: 1) loss of consciousness; 2) convulsions, tonic posturing; 3) ataxia; 4) visual trouble; 5) neurological deficit; 6) confusion; 7) disorientation; 8) unusual behaviour; 9) amnesia; 10) headaches; 11) dizziness; 12) fatigue, low energy; 13) feeling slowed down, drowsiness; 14) nausea; 15) sensitivity to light/noise; 16) not feeling right, in a fog; 17) difficulty concentrating.CONCLUSION:Sharing the same definition and the same clinical diagnostic criteria for concussion is the prerequisite for common rules of management for all sports and should allow the pooling of results to improve our knowledge of this pathology.
La tendinopathie patellaire est une pathologie fréquente et invalidante chez les sportifs, notamment dans les sports à impulsion comme le volley-ball. L’objectif principal de cette étude était d’identifier des facteurs de risque intrinsèques de tendinopathie patellaire chez le jeune volleyeur. Cette étude prospective a été réalisée d’août 2012 à avril 2013. Elle comportait une évaluation clinique, échographique, musculaire par isocinétisme et des tests de sauts en début de saison puis une réévaluation, uniquement clinique, en fin de saison. Les sujets ayant développé une tendinopathie patellaire étaient comparés aux sujets sains. Les sujets du groupe tendinopathie patellaire (TP) (6) étaient plus âgés (17,2 ± 0,4 ans vs 16,2 ± 0,9 ans ; p = 0,02) et présentaient une raideur des ischiojambiers plus importante (angle poplité de 24 ± 12° vs 14 ± 9° ; p = 0,04) que les sujets sains (16). Ils avaient un moment de force maximale du quadriceps en excentrique à vitesse lente (30°/s) plus faible que les sujets sains (2,7 ± 0,2 Nm/kg vs 3,2 ±0,5 Nm/kg ; p = 0,05). L’âge, la raideur musculotendineuse des ischiojambiers et un déficit de force du quadriceps en excentrique à vitesse lente (30°/s) sembleraient être des facteurs de risque intrinsèques de TP chez le jeune volleyeur. Patellar tendinopathy is a common and disabling injury among athletes, especially in sports with jumps such as volleyball. The aim of this study was to determine intrinsic risk factors of patellar tendinopathy among young volleyball players. This prospective study was conducted from August 2012 to April 2013. It included a clinical examination, ultrasound, muscle isokinetic assessment and tests of jumps at the beginning of the season, and then only a clinical examination at the end of the season. Athletes who developed patellar tendinopathy were compared to healthy subjects. Patellar tendinopathy (PT) group athletes (6) were older (17.2 ± 0.4 years vs 16.2 ± 0.9 years, P = 0.02) and had a stiffness of hamstrings higher (popliteal angle of 24 ± 12° vs 14 ± 9°, P = 0.04) than healthy athletes (16). They had an eccentric quadriceps peak torque at slow speed (30°/s) lower than healthy athletes (2.7 ± 0.2 Nm/kg vs 3.2 ± 0.5 Nm/kg, P = 0.05). Age, stiffness of hamstrings and an eccentric strength deficit of quadriceps at slow speed (30°/s) would be intrinsic risk factors of PT among young volleyball players.
BACKGROUND:Physical exercise is widely prescribed in rehabilitation programmes for low back pain (LBP). The LBP patient often asks whether this physical activity should be maintained and, in some cases, whether he/she should resume or take up a sport.PURPOSE:To answer these two questions by performing a review of literature on the efficacy and safety of post-rehabilitation physical activities and sport in LBP.METHOD:A systematic search of computerized databases from 1990 to 2011 was performed using grade 1 to 4 studies articles in English or French.RESULTS:Of the 2583 initially identified articles, 121 articles were analysed. Globally, physical activities like swimming, walking and cycling, practiced at moderate-intensity help to maintain fitness and control pain. Inconsistent results were found for avoiding recommendations according to the nature of PA. Sport activities, except ballgames, can be easily resume or take up as tennis, horse riding, martial arts, gymnastics, golf and running which can be performed at a lower intensity or lower competitive level.DISCUSSION AND CONCLUSION:Moderate but regular physical activity helps to improve fitness and does not increase the risk of acute pain in chronic LBP patients. The resumption of a sport may require a number of adaptations; dialogue between the therapist and the sports trainer is therefore recommended.
La prise en charge des lésions associées de la styloïde ulnaire fait encore débat. Bien que la relation entre fracture de la styloïde ulnaire et lésion du complexe fibrocartilagineux triangulaire du carpe (CFCT) ait été décrite, les travaux récents montrent que les ostéosynthèses par plaques verrouillées (OPV) permettent de ne pas la synthèser. Le but de notre étude est de comparer la répercussion de ces lésions sur la force de prono-supination à l’aide de tests isocinétiques, outil de référence. Deux groupes de patients opérés avec OPV ont été réalisés, l’un avec fracture de la base de la styloïde que l’on associe à une lésion du CFCT, l’autre sans fracture styloïdienne. Un troisième groupe de patients témoins sans antécédents de traumatisme a été intégré. Les patients inclus avaient entre 18 et 50 ans et un recul minimum de 10 mois. Les critères d’exclusion regroupaient la présence d’antécédents traumatiques sur les membres supérieurs, une fracture articulaire complexe et la survenue de complications postopératoires. L’évaluation comprenait l’examen des mobilités, une évaluation fonctionnelle puis des mesures bilatérales de la force de serrage et de prono-supination en isométrique et isocinétique. L’ensemble des résultats a été analysé en comparant les rapports individuels entre les deux côtés à l’aide d’un test de Kruskall-Wallis avec un seuil p fixé < 0,05. Trente-six sujets ont été inclus - 12 pour chaque groupe. La moyenne d’âge était de 31,1 ans et le recul moyen de 17,9 mois. Aucune différence significative n’a été retrouvée concernant la perte de mobilité, les scores fonctionnels et la force de préhension. En revanche, une différence significative de force a été identifiée lors des tests isocinétiques à 45° s demandant l’effort maximum que ce soit en pronation (p = 0,0068) ou en supination (p = 0,022). Cette différence n’a pas été retrouvée lors des tests isocinétiques à 120° s ou lors des tests isométriques. Cette étude permet de mettre en évidence à l’aide d’un outil dynamique une diminution de la force de prono-supination chez les sujets avec fracture de la styloïde ulnaire lors des efforts soutenus. Elle soulève la question d’une prise en charge complémentaire du compartiment ulnaire à la phase initiale chez des sujets jeunes à forte demande fonctionnelle.
styloid; – group 2: patients without fracture of the ulnar styloid or just a tip fracture. A control group (Group 3) was composed of six healthy patients without history of trauma of the wrist. All the patients were tested bilaterally for isokinetic and isometric pronation and supination strength. Clinical outcomes such as grip strength, range of motion, Mayo Clinic Wrist score (MWS), DASH score and Lidstrom classification were evaluated by an independent observer and correlated with isokinetic results. Statistical analysis was performed with Kruskall-Wallis test and Wilcoxon test (P < 0.05). Results.– There were no significant differences in range of motion; grip strength; MWS and DASH scores and isometric strength of pronation and supination. Isokinetic evaluation showed a significant decrease of the supination strength between group 1 compared to group 2 and 3 at 458/s (P < 0.01). Discussion.– This study suggests that fracture of the base of the ulnar styloid has no effect on upper extremity specific questionnaires. However, a significant decrease in isokinetic supination force is expected. More specific health questionnaires may confirm the clinical effect of this loss of strength. Consequently, more aggressive treatment for the ulnar styloid may be proposed in case of fracture of the radius associated with ulna in young and active patients.
OBJECTIVE:Isokinetic strengthening is a rehabilitation technique rarely used in stroke patients. However, the potential benefits of force and endurance training in this population are strongly suspected.METHOD:This literature review synthesizes the results of clinical trials on this topic. The research was conducted on PubMed, using "Stroke", "rehabilitation", "isokinetic", "upper limb" and "training" as keywords.RESULTS:Seventeen studies focusing on the use of isokinetics in assessment or rehabilitation (six studies) following stroke were reviewed. For the lower limb, muscle strength and walking ability improved after isokinetic rehabilitation programs. For the upper limb, the only two studies found in the literature suggest improvement in the strength of the trained muscles, of grip force, of the Fugl-Meyer motor score and of global functional capacities. This review does not reveal any consensus on the protocols to be implemented: type of muscle contraction, velocities….CONCLUSION:While isokinetic strengthening has not proven its efficiency in rehabilitation of the upper limb following stroke, its interest with regard to rehabilitation of the lower limbs has been recognized. Randomized controlled trials in this field are necessary to confirm its efficiency, especially concerning upper arm rehabilitation.
OBJECTIVE:We estimate that there are about 50,000 persons who survived poliomyelitis in their childhood in France (mean age estimated between 50 and 65 years). After a few decades of stability, 30 to 65% of individuals who had been infected and recovered from polio begin to experience new signs and symptoms.METHOD:Review of the literature on Pubmed with the following keywords "Poliomyelitis" and "Post-Polio Syndrome (PPS)".RESULTS:These new signs and symptoms are characterized by muscular atrophy (decreased muscle mass), muscle weakness and fatigue, muscle and/or joint pain. All these symptoms lead to significant changes in mobility with falls and inability to carry on with daily life activities. There are several intricate causes. The normal aging process and weight gain are regularly blamed. Respiratory disorders and sleep disorders must be looked for: respiratory insufficiency, sleep-related breathing disorders such as sleep apnea, restless legs syndrome. Orthopedics complications are quite common: soft-tissue pathologies of the upper limbs, degenerative pathologies of the large joints or spinal cord, fall-related fractures. Finally, the onset of an authentic PPS is possible.CONCLUSION:The therapeutic care of this late functional deterioration requires regular monitoring check-ups in order to implement preventive measures and appropriate treatment. This therapeutic care must be multidisciplinary as physical rehabilitation; orthotics and technical aids are all essential.
