Einleitung: Neurologische Erkrankungen wie der Morbus Parkinson, die Multiple Sklerose oder Schlaganfälle sind Risikofaktoren für eine Osteoporose. Wenig ist bislang zur ALS bekannt. Wir berichten den Fall einer 70-jährigen Patientin mit bekannter ALS und multiplen Wirbelkörperfrakturen, die wir mit Teriparatid behandelten.
Indolent systemic mastocytosis (ISM) is a group of heterogenous diseases characterized by abnormal accumulation of mast cells in at least one organ. ISM can be a cause of osteoporosis. The aim of this study is to determine the prevalence, and the prognosis of ISM in a cohort of patients with osteoporosis. In this monocentric and retrospective study, patients with osteoporosis who did not receive a bone biopsy (cohort 1) and patients that subsequently received a diagnostic bone biopsy for differential diagnosis (cohort 2) are compared with patients who are diagnosed with ISM (cohort 3). A total of 8392 patients are diagnosed with osteoporosis. Out of these patients 1374 underwent a diagnostic bone biopsy resulting in 43 patients with ISM. These figures indicate that ISM is diagnosed in 0.5% of patients with osteoporosis and in 3.1% (men 5.8%) of patients who underwent bone biopsies. Patients with ISM sustained significantly more vertebral fractures in comparison to patients in cohort 2 (4.4 ± 3.6 versus 2.4 ± 2.5 vertebral fractures, p < 0.001) and women were significantly younger compared to cohort 2 (57.3 ± 12 versus 63.6 ± 12 years, p < 0.05). Only 33% showed an involvement of the skin (urticaria pigmentosa). ISM is a rare cause of osteoporosis (0.5%). However, in a subgroup of rather young male patients with osteoporosis the prevalence is more than 5%. Thus, ISM should be considered in premenopausal women and men presenting with vertebral fractures even if urticaria pigmentosa is not present.
Our randomized cross-over study compared exercise against a postural garment (Posture Plus Force) in the management of cervical pain in female health professionals. The garment showed a similar effect to exercise in pain reduction, a decrease in the use of pain relievers and fostered compliance. Study Design. Randomized cross-over study. Objective. The aim of this study was to compare exercise, the criterion standard, to the postural garment PosturePlusForce in the management of nonspecific cervical pain in women. We also analyzed both interventions with regards to baseline posture, use of pharmacological pain relievers, compliance, and comfort. Summary of Background Data. The prevalence of neck pain has increased during the last decade, preferentially affecting women. Those suffering from this condition may manifest a decrease in quality of life and inability to work. Consistent recommendations highlight the importance of exercise and posture for neck pain improvement. Methods. A total of 32 female health care professionals with cervical pain (≥3 on the visual analogue scale) entered the trial. Participants were allocated to either performing exercises or wearing the postural garment. The cross-over between interventions was separated by a 3-month washout period. Primary outcomes included pain intensity and posture. Secondary outcomes comprised cervical pain-related disability, psychological factors, physical activity, global perceived effect of treatment, and garment comfort. Treatment compliance, medication use, and adverse events were also recorded. Results. Both interventions showed a significant improvement in pain in subjects with an adherence >60%. However, in participants with dorsal hyperkyphosis (>45°), the garment demonstrated a greater reduction in pain than exercise (P = 0.019). Additionally, those wearing the garment needed fewer pain relievers than those performing exercises (P = 0.007). Compliance was >50% for both interventions and comfort was contingent on season. Conclusion. In our study, PosturePlusForce showed, at least, a similar effect on pain to exercise, although those with dorsal hyperkyphosis exhibited a greater reduction in pain and related variables with the garment. Pain relievers were less required by those wearing PosturePlusForce than by those performing the exercises. Level of Evidence: 1
Anfang Juli 2020 sind weltweit etwa 10,5 Mio. Menschen von COVID-19 betroffen. Bei einer geschätzten Mortalität von 6,5 % (5%–9 %) sind etwa 680000 bereits daran verstorben [1]. Die Mortalität wird hervorgerufen durch ein schweres akutes Lungenversagen, verursacht durch das Coronavirus 2 (SARS-CoV-2), welches zuerst Ende 2019 in Wuhan, China, beobachtet worden war [2]. Glücklicherweise konnte die Ausbreitung des Virus und die dadurch hervorgerufene Mortalität durch rechtzeitig eingeleitete Maßnahmen zur Prävention (strikte Isolierung und Quarantäne) bei paralleler Priorisierung stationärer Behandlung und konsequenter Testung von Risikopatienten in Deutschland vergleichsweise stark eingeschränkt werden. So beziffert das Robert Koch-Institut (RKI) zum 06.07.2020 die Zahl der Infektionen mit 197607, die Zahl der Genesenen mit 182029 bei insgesamt 9024 Todesfällen [3]. Damit ist zum jetzigen Zeitpunkt noch von 6554 Infektionen auszugehen. Allerdings droht jederzeit das Auftreten neuer lokaler Herde, die ohne rechtzeitige Bekämpfung sehr schnell eine zweite nationale Infektionswelle zur Folge haben könnten. Zum Verlauf der Infektion bei den Genesenen ist bis heute nur wenig bekannt. Insbesondere können keine Angaben zum Ausmaß an Immobilisation und zum Anteil an Hospitalisierungen gemacht werden. Einigkeit besteht hier lediglich darüber, dass der Anteil der bettlägerigen Patienten mit zunehmendem Lebensalter exponentiell ansteigt [4], [5].
Pregnancy and lactation-associated osteoporosis (PLO) is a very rare form of osteoporosis. Vertebral fractures either occur in the last trimester of pregnancy or after childbirth. Pathogenesis is still unclear. Until recently, almost no data existed in regards to the quality of life and long-term clinical outcome of patients with PLO. To determine the long-term clinical outcome of patients with pregnancy and lactation-associated osteoporosis (PLO) with respect to the following factors: pain, quality of life, mental condition, vertebral fractures, and capacity to work. In this single-center retrospective study, patients were reviewed for at least 2 years, more than 50% of them were followed up until menopause. Bone density (DXA) and vertebral fractures were assessed. Standardized questionnaires were used to analyze factors such as quality of life (Qualeffo-41), anxiety and depression (PHQ-4), and pain (the visual analog scale [VAS]). Additionally, a questionnaire was designed in order to evaluate and discuss some of the reasons behind the occurrence of mental distress at the onset of symptoms. Our report shows the clinical course of 20 patients with PLO, 11 of them until menopause (on average 16.3 years after onset of symptoms). When diagnosis was made, 5.4 vertebral fractures were noticed on average. Three of the 20 patients developed subsequent fractures in the following years. The diagnosis of PLO was made on average after 3.3 months after the onset of symptoms. At the beginning of the investigation, physical and mental health of all patients were poor, but improved within the first 2 years and continued doing so until menopause. The average time it took to return to employment was 3.3 years. PLO has a significant impact on pain, mental state, quality of life, and capacity to work. However, the long-term prognosis is promising. The severe mental distress is presumably related to several contributing factors in life such as physical integrity and independence, family life, employment, and financial security.
With increasing longevity, hip fractures become more and more a serious burden not only for societies in developed civilization, but also for emerging countries. According to worldwide projections 1.5 million people are affected each year. Although a lot of research has been performed over the last decade, there is still a lack of standardized and evidence-based approaches for prevention, treatment, and rehabilitation of this worst complication of osteoporosis.Therefore, the evidence base for this article was synthesized in accordance with SIGN methodology. Databases searched include Medline, Embase, Cinahl and the Cochrane Library between March 1999 and March 2019. The following terms are used: osteoporosis, hip fracture, rehabilitation, falls, muscle strength, nutrition, exercise, balance, sway, and hip protectors. Moreover, reference lists from included studies were checked and author's names were searched for additional studies.Possibly, the best approach to rehabilitation after hip fracture is a multi-disciplinary team co-ordinating medical, social, educational and vocational measure for training or retraining the individual to the highest possible level of function. In order to prevent thromboembolism low-dose anti-coagulation therapy (e.g. fondaparinux, rivaroxaban) may be used for approximately two weeks after surgery. This should be accompanied by a daily nutritional intake of at least 20 g protein, 1200 mg of elemental calcium and 800 I. U. of vitamin D, whereas in severe vitamin D insufficiencies recommendations may be certainly higher.After surgical repair of the hip fracture, an anti-resorptive medication may be started. While balance training and performing of Tai Chi has been shown to reduce fall risk and thereby also decrease hip fracture risk, the use of hip protectors is still under evaluation and cannot be generally advocated.
ZusammenfassungDas Ziel der Medizinischen Trainingstherapie bei Osteoporose und Wirbelkörperfrakturen besteht darin, die Rückenmuskulatur zur Verbesserung einer aufrechten Körperhaltung zu kräftigen. Deshalb sollte ein spezifisches Trainingsprogramm entsprechend den individuellen Fähigkeiten der Patienten zuerst im Sitzen begonnen werden, um dann mit zunehmendem Kraftzuwachs auf das Stehen überzugehen. Mit abnehmender Brüchigkeit und zunehmender Stabilität kann dann auch ein isometrisches Krafttraining für die Rückenmuskulatur begonnen werden. Durch ein derartiges Training konnte im Rahmen einer prospektiven randomisierten Studie bei peri- oder unmittelbar postmenopausalen Frauen bei zunehmender Kraft der Rückenmuskulatur das spätere Auftreten von Wirbelkörperfrakturen um etwa zwei Drittel gesenkt werden. Zusätzlich können durch das „SPEED-Programm“ signifikante Verbesserungen von Rückenschmerzen, Kyphose, Sturzrisiko und Leistungsfähigkeit im Alltag erreicht werden. Dabei handelt es sich um ein dynamisches, propriozeptives Training zur Kräftigung der Rückenextensoren. Hierzu wird das „Posture Training Support“ (PTS) eingesetzt. Diese Unterstützung besteht aus einem, je nach Trainingszustand, unterschiedlich schweren Rucksack, der zu einer Abnahme von Rundrücken und Schmerzen führen kann, wie sie durch Wirbelkörperkompressionsfrakturen entstanden sind. Durch die verbesserte Aufrichtung wird dann auch ein Reiben von Rippen auf den Beckenkämmen, was in extremen Fällen durchaus auftreten kann, vermieden.Insgesamt sollte die medizinische Trainingstherapie zu einer Zunahme an Muskelkraft führen, ohne die Wirbelsäule durch zusätzliche Kräfte zu sehr zu belasten. Dadurch gelingt es dann auch, Stürzen und Frakturen vorzubeugen. Ähnlich wie bei der medikamentösen Behandlung sollte auch die Bewegungstherapie individuell dosiert werden.Im Rahmen der Rehabilitation können flexible, semi-rigide Rückenorthesen an die individuelle Form der Wirbelsäule angepasst werden. Über einen sogenannten „Biofeedback-Mechanismus“ kann ebenfalls eine Kraftzunahme im Bereich der Rumpfmuskulatur bei vorhandenen Wirbelkörperfrakturen erreicht werden. Vermutlich über eine Schmerzreduktion ist eine erhöhte Leistungsfähigkeit im Alltag erreichbar. Klinische Studien zeigen, dass im Verlauf einer Tragezeit von 6 bis 12 Monaten durch eine Kräftigung der Rückenstrecker eine aufrechtere Haltung mit verbessertem Standgleichgewicht erzielbar ist. Möglicherweise kann hierdurch auch eine Verringerung der Sturz- und Frakturrate erreicht werden.
This article gives the clinically practising osteologist an overview of the socio-medical assessment of patients with osteoporosis. The aims behind the assessments depend on different areas of law, with different fundamental questions. Reasons for assessing patients with osteoporosis include questions about work capability, employability, degree of disability, need for care, and the need for rehabilitation.The main focus of this article is the assessment of employability (pension law) and the capacity to work (health insurance law). The presented criteria are designed to assess the qualitative capacities (workload) and the quantitative capacities (working time) in pension legislation. Risk groups are defined and assigned a maximum lifting load. The justification behind the socio-medical evaluation is based on the International Classification of Functional Disorders (ICF). Patients with vertebral body fractures should be treated as part of an inpatient osteological rehabilitation. A comprehensive socio-medial assessment is carried out there.
The objective of exercise in the treatment of osteoporosis is to improve axial stability through strengthening of back extensor muscles. Therefore, a back extension exercise program specific to one's musculoskeletal competence and pain can be performed in a sitting position and later advanced to the prone position. When fragility is resolved, back extension is performed against resistance applied to the upper back. This kind of back muscle training has been shown in a prospective randomized, controlled clinical trial performed in peri-or immediate postmenopausal women a significant reduction of vertebral compression fractures later in life approximately by two thirds. Another significant reduction in back pain, kyphosis, and risk of falls and an improvement in the level of physical activity have been achieved through the SPEED (Spinal Proprioceptive Extension Exercise Dynamic) program. In addition, the application of a "Posture Training Support" (PTS) using a backpack may decrease kyphosis and pain related not only to compression fractures but also reduce iliocostal friction which may occur in severe cases. Therapeutic exercise should address osteoporosis-related deformities of axial posture which can increase risk of fall and fracture. Thus, the role of a therapeutic exercise program is to increase muscle strength safely, decrease immobility-related complications, and prevent fall and fracture. As with pharmacotherapy, therapeutic exercises are individualized. Concerning rehabilitation flexible, semi-rigid back orthoses which are fitted to the individual form of the spine by an experienced orthopaedic technician may increase the strength of trunk muscles via a so-called bio-feed-back mechanism. Several prospective, randomized, controlled, clinical trials have been demonstrated that this increase in trunk muscle strength is followed by an increase in the activities of daily living and a reduction of pain. Within a training course of six to twelve months a significant strengthening of the back extensor muscles may be achieved. This in turn is associated with a better posture and a reduction in postural instability. Thus a further reduction in falls and non-vertebral fractures may be observed. Thoracolumbar, semi-rigid orthoses, however, need to find a balance between the often conflicting requirements of function, cosmetics, and acceptability. This is a prerequisite for a better quality of life with pain reduction, decreased limitations of daily living, and augmented well-being.
ZusammenfassungDer Gesetzgeber stellt Menschen mit Behinderung unter einen besonderen Schutz. Die Schwere der Behinderung wird anhand des „Grads der Behinderung/GdB“ beschrieben. Einer Funktionsstörung im Rahmen einer Erkrankung wird ein GdB zugeordnet. Die Versorgungs-Medizin-Verordnung regelt die Höhe des jeweiligen GdB in insgesamt 18 Kapiteln. Im Rahmen dieser Arbeit wurden den abstrakten Angaben der Versorgungs-Medizin-Verordnung konkrete Kriterien für die Beurteilung der Funktionseinschränkungen bei Osteoporose zugeordnet. Es ist das Ziel dieser Arbeit, für die Ermittlung des GdB bei Patienten mit Osteoporose konkrete, allgemein anerkannte und nachvollziehbare Kriterien zu etablieren um bundesweit einheitliche Bewertungsmaßstäbe sicherzustellen.
Jeder dritte Mensch über 65 Jahren erleidet mindestens einmal im Jahr einen Sturz, der Knochenbrüche, lange Klinikaufenthalte und im Fall von Komplikationen auch den Tod zur Folge haben kann. Senioren, die sich regelmäßig bewegen, sind laut einer Metaanalyse in den Cochrane Database of Systematic Reviews [1] weniger gefährdet. Dabei kommt es weniger auf kräftige Muskeln an als auf die Beweglichkeit und das Training des Gleichgewichtssinns.
Legislation places people with disabilities under special protection. The severity of a disability is described by the degree of disability/GdB. The "Versorgungs-Medizin-Verordnung" (Ordinance on Medical Care) regulates the severity of the different GdB in a total of 18 chapters. As part of this publication, definite criteria for the assessment of functional restrictions in osteoporosis were added to the abstract information from the "Versorgungs-Medizin-Verordnung" (The Ordinance on Medical Care). It is the aim of this paper to establish specific, generally accepted, and comprehensible criteria for the determination of the GdB in patients with osteoporosis, so that uniform evaluation standards throughout Germany can be established.
ZusammenfassungDieser Artikel gibt dem klinisch tätigen Osteologen einen Überblick über die sozialmedizinische Begutachtung von Patienten mit Osteoporose. Abhängig vom Begutachtungsanlass liegen unterschiedliche Rechtsgebiete mit unterschiedlichen Fragestellungen zugrunde. Die häufigsten Begutachtungsanlässe bei Patienten mit Osteoporose sind die Frage nach Arbeitsfähigkeit, Erwerbsfähigkeit, Grad der Behinderung, Pflegebedürftigkeit und Rehabilitationsbedürftigkeit.Der Schwerpunkt dieses Artikels liegt auf der Begutachtung der Erwerbsfähigkeit (Rentenrecht) und der Arbeitsfähigkeit (Krankenkassenrecht). Es werden Kriterien dargestellt, durch die das qualitative (Arbeitsschwere) und quantitative (Arbeitszeit) Leistungsvermögen im Rentenrecht begutachtet werden können. Es werden Risikogruppen definiert, denen eine maximale Hebebelastung zugeordnet wird. Die Begründung der sozialmedizinischen Einschätzung erfolgt anhand der Internationalen Klassifikation der Funktionsstörungen (ICF). Patienten mit Wirbelkörperfrakturen sollten im Rahmen einer stationären osteologischen Rehabilitation behandelt werden. Dort erfolgt eine umfassende sozialmedizinische Begutachtung.
Die „U.S. Preventive Services Task Force” (USPSTF), die das Gesundheitsministerium zu Fragen der Krankheitsprävention berät, spricht sich für ein Osteoporose-Screening von Frauen ab dem 65. Lebensjahr aus. Jüngere postmenopausale Frauen sollten sich untersuchen lassen, wenn es Hinweise auf ein erhöhtes Risiko gibt. Für Männer fand das Gremium keine gesicherten Anhaltspunkte auf einen Nutzen. Die Empfehlungen und der Evidenz-Report wurden im U.S. amerikanischen Ärzteblatt „JAMA” (2018; doi:10.1001/jama.2018.7498 und 10.1001/jama.2018.2018/6537) veröffentlicht (1).
