The challenge in treating traumatic hemipelvectomy is the dynamics of the complex and life-threatening consequences of the injury. These include skin and soft tissue defects, osseous, neural and vascular injuries as well as the subsequent hemostatic derangement and organ dysfunction as part of the shock process. The treatment requires rapid and targeted decisions to save the patient's life. In this particular case a 34-year-old farmer was trapped between a wheeled loader and a stationary trailer. Upon arrival at the hospital the patient was in a state of hemorrhagic shock with accompanying acute traumatic coagulopathy and a grade III open pelvic trauma with complete ischemia of the left leg and a bladder injury. After performing emergency surgery and a two-stage approach for pelvic stabilization the patient's condition deteriorated up to multiorgan failure, necessitating left-sided hemipelvectomy as an immediate life-saving salvage procedure. In the further course multiple revision surgeries and plastic reconstructions due to wound infections and the presence of skin and soft tissue damage were required. Due to the rare confrontation with this type of injury in everyday practice and the absence of a universal treatment algorithm, the following case report is intended to contribute to a better understanding of the treatment and to illustrate the coherent interactions of the individual organ systems affected.
Zusammenfassung Die Herausforderung in der Behandlung der traumatischen Hemipelvektomie ist die Dynamik der komplexen und lebensbedrohlichen Verletzungsfolgen. Dies umfasst zum einen den Haut- und Weichteildefekt, die ossären, nervalen und vasalen Verletzungen, zum anderen die konsekutiv hämostaseologische Entgleisung und Organdysfunktion im Rahmen des Schockgeschehens. Die Behandlung an sich fordert schnelle und zielgerichtete Entscheidungen, um das Leben des Patienten zu erhalten. Im vorliegenden Fall wurde ein 34-jähriger Landwirt zwischen einem Radlader und einem stehenden Anhänger eingeklemmt. Bei seiner Ankunft im Krankenhaus befand sich der Patient in einem hämorrhagischen Schock mit begleitender akuter traumatischer Koagulopathie und einem III-gradig offenen Beckentrauma mit einer vollständigen Ischämie des linken Beins sowie auch einer Blasenverletzung. Nach notfallmäßiger operativer Versorgung und zweizeitiger Stabilisierung des Beckens kam es im weiteren Verlauf zu einer Verschlechterung des Zustandes hin zum Multiorganversagen, wodurch die linksseitige Hemipelvektomie als lebensrettende Maßnahme notwendig wurde. Anschließend waren bei Wundinfektion und bestehenden Haut- und Weichteilschaden mehrere Revisionseingriffe und plastische Rekonstruktionen notwendig. Aufgrund der im Alltag seltenen Konfrontation mit dieser Art von Verletzung und eines nichtallgemeingültigen Therapiealgorithmus soll folgender Fallbericht zum besseren Verständnis der Behandlung sowie auch zur Darstellung der in sich zusammenhängenden Wechselwirkungen der einzelnen betroffenen Organsystemen dienen.
Über Verkehrsunfälle, die lebensbedrohliche Verletzungen oder dauerhafte Behinderungen nach sich ziehen, ist in Deutschland wenig bekannt. Im Rahmen der vorliegenden Arbeit wurden in 6 Landkreisen und 2 kreisfreien Städten mit zusammen mehr als 1,3 Millionen Einwohnern über 12 Monate alle Straßenverkehrsunfälle erfasst, bei denen Verkehrsteilnehmer schwerstverletzt wurden. Hierzu wurden sowohl lebensbedrohlich Verletzte mit einem ISS ≥ 16 gezählt, die in Krankenhäusern behandelt wurden, als auch initial Verstorbene. Für die Totalerhebung wurden sowohl retrospektive als auch prospektive Daten verwendet. Neben Verletzungsmustern wurden auch wesentliche Merkmale des Unfallhergangs, Fahrzeugbeschädigungen und die Benutzung bzw. Auslösung von Insassenschutzsystemen dokumentiert. Insgesamt 131 schwerstverletzte Verkehrsteilnehmer (ISS ≥ 16) wurden im Studienzeitraum in Krankenhäuser eingeliefert, von denen 19 in der Klinik verstarben. Weitere 66 Patienten erlagen noch an der Unfallstelle ihren Verletzungen. Der Anteil an Schwerstverletzten im medizinisch fachlichen Sinn (ISS ≥ 16) an der Zahl der amtlichen Schwerverletzten (n = 1125) beträgt lediglich 10 %. Sowohl unter den klinisch behandelten als auch den unmittelbar verstorbenen Verkehrsteilnehmern bildeten Pkw-Benutzer den größten Anteil. Schwerstverletzte waren am häufigsten die Folge eines Frontalaufpralls. Der Seitenaufprall stellte einen weiteren Schwerpunkt des Unfallgeschehens von Pkw dar, wobei fast die Hälfte dieser Kollisionen Alleinunfälle gegen Bäume etc. waren. Fahrzeugüberschläge hatten besonders für nicht angegurtete Insassen schwerste Folgen. Verunglückte Fußgänger und Radfahrer wiesen ein hohes durchschnittliches Lebensalter auf. Bei diesen Beteiligten war besonders bei sofortigem Tod das Überrollen ein häufiger Unfallmechanismus. Benutzer motorisierter Zweiräder waren fast ausschließlich männlich und wiesen im Durchschnitt die höchste Gesamtverletzungsschwere auf. Alleinunfälle waren in dieser Gruppe selten die Ursache von lebensbedrohlichen Verletzungen. Unter allen Verkehrsteilnehmern zählten Lungenkontusionen zu den häufigsten Verletzungsarten. Relevante Extremitätentraumata traten dagegen besonders bei Motorrad- und Pkw-Fahrern im Frontalaufprall in Erscheinung. Zu relevanten Schädel-Hirn-Traumata kam es vorrangig bei Pkw-Insassen im Seitenaufprall und bei Fußgängern und Radfahrern. Bei Radfahrern ohne Helm entstanden diese Verletzungen auch bei Alleinstürzen. Die Ergebnisse unterstreichen die Bedeutung des Unfallhergangs für das Verletzungsmuster. Gleichzeitig wird die Notwendigkeit deutlich, mit passiven Sicherheitsmaßnahmen wie Vorhang- und Fußgängerairbags sowie durch verstärkte Nutzung von Fahrradhelmen das Risiko für relevante Kopfverletzungen herabzusetzen.
Limited information exists in Germany about road traffic accidents that result in life-threatening injuries or long-term disabilities. In the course of the presented study, all accidents with most severely injured road users occurring in six counties and two larger cities with a population of more than 1.3 million were collected over a 12-month period. The cases include both patients with life-threatening injuries (ISS = 16) who were treated in hospitals and victims who deceased on scene already. For this total survey of accidents prospective and retrospective date were used. Beside injury patterns, important characteristics of the collision, vehicle damage and the use of helmets or seat belts and the activation of airbags were documented. Altogether, 131 victims of road traffic accidents with life-threatening injuries were admitted to trauma centres during the study period. 19 of them died in-hospital. Further 66 victims died on scene. Only a proportion of 10% among the so called "seriously injured", as defined in national statistics (n = 1125), had an ISS = 16 after road traffic accidents. Passenger car occupants represented the major portion among both the clinically treated and the immediately deceased. Severe injuries resulted most frequently from frontal impacts. Side impacts constituted another major collision scenario. Vehicle roll-overs produced severe outcomes especially for unbelted occupants. Almost all motorcyclists were male and presented the highest average of injury severity overall. Single accidents were rarely a cause for most severe trauma in this group. Of all injury types, lung contusions counted among the most frequent injuries. Serious fractures of extremities occurred particularly in motorcyclists and car drivers after frontal impacts. Severe trauma to the brain was mostly the result of car occupants sustaining lateral impacts or when pedestrians or bicyclists struck. Firstly, the results underline the importance of the mechanism of injury of severely injured patients due to road traffic accidents. Secondly, the necessity becomes clear to reduce the risk for severe head injury by means of secondary safety measures like curtain air-bags and pedestrian airbags as well as by an increased use of bicycle helmets.
A total survey of road traffic accidents involving most severely injured, defined as sustaining a polytrauma or severe monotrauma (ISS more than 15) or being killed, was conducted over 14 months in a large study region in Germany. Data on injuries, pre-clinical and clinical care, crash circumstances and vehicle damage were obtained both prospectively and retrospectively from trauma centers, dispatch centers, police and fire departments. 149 patients with a polytrauma and eight with a severe monotrauma were recorded altogether. 22 patients died in hospital. Another 76 victims had deceased at the accident scene. In 2008, 49 percent of patients treated with life-threatening injuries were car or van occupants, 21 percent motorcyclists, 18 percent cyclists and 10 percent pedestrians. Among fatalities at the scene, vehicle occupants constituted an even larger portion. The number of road users with life-threatening trauma in the region was extrapolated to the German situation. It suggests that 10 percent among the seriously injured as defined in national accident statistics are surviving accident victims with polytrauma or severe monotrauma. Zusammenfassung Die vorliegende prospektiv konsekutive Studie beurteilt die Wirksamkeit von niedrig-intensivem, gepulstem Ultraschall zur Behandlung von Frakturheilungsstörungen. 86 von 100 Behandlungen konnten mit Erfolg abgeschlossen werden. Obwohl es sich bei den 100 Fällen um insgesamt 64 “delayed unions” und 36 “non-unions” handelte, wurde die Ultraschallbehandlung in allen Fällen als Alternative zu der eigentlich indizierten Operation durchgeführt. Mit Ausnahme von 10 Skaphoidheilungsstörungen erfolgte in keinem Fall eine zusätzliche Behandlung der Fraktur. Die Stratefizierung des Datenmaterials zeigte keine signifikanten Unterschiede in Heilungsrate und Behandlungszeit zwischen “delayed unions” und “non-unions” oder zwischen atrophen und hypertrophen Heilungsstörungen. Bei der Beurteilung der Heilungsrate von 86% muss berücksichtigt werden, dass es sich aufgrund der Ein- und Ausschlusskriterien um ein präselektioniertes Krankengut handelte. Trotzdem erscheint unseres Erachtens die Wirksamkeit der niedrig intensiven Ultraschalltherapie zur Behandlung von Frakturheilungsstörungen evident.
This prospective consecutive study judges the effectiveness of pulsed low-intensity ultrasound for treatment of boney healing disorders. 86 out of 100 treatments were successful. Although in these 100 cases 64 delayed unions and 36 nonunions were enrolled ultrasound therapy was performed as an alternative to the indicated operation in every case. Excluding ten cases of ununited fractures of the scaphoid no additional therapy was performed in any case. Stratifying the data no significant differences in healing rate and treatment time were observed between delayed unions and non-unions and between atrophic and hypertrophic healing disorders. Judging the healing rate of 86% one has to take into consideration that according to our in- and excluding criteria we had a preselected sample of patients. Nevertheless the effectiveness of pulsed low-intensity ultrasound for treatment of disorders of the fracture repair process is evident.

Low intensity pulsed ultrasound (frequency: 1,5 MHz, pulsed by 1 KHz, signal burst width: 200 μsec, intensity: 30 mW/cm2) offers the opportunity of an alternative treatment. In contrast to magnetic field therapy, electric stimulation or use of extracorporal shock waves this therapy is judged positively all over the literature. Within an own series of 76 cases at present time we can show a 86 % healing rate under defined conditions. Within this paper we present three case reports, which clearly show the efficiency of low intensity pulsed ultrasound.
At our hospital, closed dislocations of the elbow without neurovascular or osseous lesion are treated conservatively after successful reduction in the absence of a tendency to redislocation. Indications for surgery are irreducible dislocations and those with a tendency to redislocation as well as open soft tissue injuries, neurovascular and osseous concomitant injuries. During a period of 10 years 113 patients were treated according to this strategy. Of these patients, 46 had simple dislocations of the elbow and 67 suffered from dislocations with osseous lesions. In 95 patients follow-ups could be performed after a mean period of 55 months. In more than 75% of cases good or excellent functional results were documented; unfavourable results were seen only in patients with osseous lesions. Nearly all of the patients had a stable elbow joint at the end of therapy.

Geschlossene Ellenbogenluxationen ohne neurovasculären oder knöchernen Begleitschaden werden in unserer Klinik nach erfolgter Reposition bei fehlender Reluxationstendenz konservativ behandelt. Irreponible oder nicht retinierbare Ellenbogenverrenkungen sowie solche mit begleitendem offenen Weichteilschaden oder knöchernen bzw. neurovasculären Begleitverletzungen werden operativ angegangen. Innerhalb von 10 Jahren wurden 113 Patienten entsprechend diesem Therapieregime versorgt. Dabei handelte es sich in 46 Fällen um Luxationen ohne knöcherne Begleitverletzung, in 67 Fällen um Luxationsfrakturen. 95 Patienten konnten nach durchschnittlich 55 Monaten nachuntersucht werden. Wir fanden in über 3/4 der Fälle funktionell exzellente und gute Ergebnisse; die schlechten Ergebnisse traten nur bei Patienten mit Luxationsfrakturen auf. Bis auf eine Ausnahme lag am Ende der Behandlung ein stabiles Ellenbogengelenk vor.
