Acute traumatic central cord syndrome (ATCCS) is the most common type of incomplete spinal cord injury, characterized by predominant upper extremity weakness, and less severe sensory and bladder dysfunction. ATCCS is thought to result from post-traumatic centro-medullary hemorrhage and edema, or, as more recently proposed, from a Wallerian degeneration, as a consequence of spinal cord pinching in a narrowed canal. Magnetic Resonance Imaging is the method of choice for diagnosis, showing a typical intramedullary hypersignal on T2 sequences. Non-surgical treatment relies on external cervical immobilization, maintenance of a sufficient systolic blood pressure, and early rehabilitation, and should be reserved for patients suffering from mild ATCCS. Surgical management of ATCCS consists of posterior, anterior or combined approaches, in order to achieve spinal cord decompression, with or without stabilization. The benefits of early surgical decompression in the setting of ATCCS remain controversial due to the lack of clinical randomized trials; recent studies suggest that early surgery (less than 72 hours after trauma) appears to be safe and effective, especially for patients with evidence of focal anatomical cord compression.Le syndrome centromédullaire aigu post-traumatique (SCAPT) est la forme la plus fréquente de lésion médullaire incomplète, caractérisée par la prédominance d’une faiblesse motrice aux membres supérieurs et accompagnée d’une dysfonction sensitive et vésicale moindre. Le SCAPT pourrait résulter d’une hémorragie et d’un œdème intramédullaire ou plutôt, comme récemment démontré, d’une dégénérescence Wallérienne. Dans les deux cas l’origine serait un pincement médullaire aigu chez un patient avec canal cervical étroit. L’IRM reste la technique de référence pour le diagnostic car permet de mettre en évidence le typique hypersignal intramédullaire en séquence T2. Le traitement non-chirurgical comprend l’immobilisation cervicale, le maintien d’une bonne pression sanguine systolique et la réhabilitation précoce, et il devrait être réservé aux patients présentant les formes modérées de SCAPT. Le traitement chirurgical du SCAPT peut se faire par abord antérieur, postérieur ou combiné consistant principalement à faire une décompression de la moelle, sans ou avec une stabilisation vertébrale. Les bénéfices portés par une décompression chirurgicale précoce dans un contexte de SCAPT sont controversés dans la littérature, ceci s’explique par le manque d’essais cliniques randomisés à ce sujet. Les études récentes suggèrent qu’une décompression précoce (moins de 72 heures après le trauma) semble être sans trop de danger et efficace particulièrement pour les patients avec mise en évidence d’une compression médullaire focale.
We report on two cases of spontaneous resorption of a calcified thoracic hernia. This phenomenom is widely recognised in lumbar and cervical hernia, but is exceptional at the thoracic level. The potential mechanisms underlying this resorption are discussed trough a review of the literature. We think this could be another argument for a “wait and watch” period before a surgical decision in the patients who have few symptoms. Nous rapportons le cas de deux patients présentant une résorption spontanée de hernie discale thoracique calcifiée. L’involution spontanée de hernies discales est un phénomène connu pour les localisations cervicales et lombaires. Les cas concernant les hernies thoraciques sont exceptionnels, notamment concernant les hernies calcifiées. Nous discutons les autres cas décrits et les phénomènes pouvant être impliqués dans ces résorptions. Ces disparitions spontanées nous confortent à proposer une période de surveillance clinique et radiologique chez les patients peu ou pas symptomatiques avant de discuter une éventuelle décision chirurgicale.
Tungsten trioxide (WO2.9) nanoparticles were prepared, to our knowledge for the first time, by pulsed laser ablation in distilled water. The experiments were performed irradiating a tungsten target with a second harmonic (532 nm) Nd:YAG laser beam varying the operative fluence between 1 and 7 J cm−2 and the ablation time up to 120 min. As evidenced by means of transmission electron microscopy (TEM), at all investigated fluences, small nanostructures of 2–6 nm were accompanied by larger particles with a diameter of about 10–20 nm and aggregates of about 80–100 nm. A plasma shielding effect was evidenced upon increasing the laser fluence, while if the ablation time is increased the amount of particles increases as well, supporting the scalability of the production technique. The deposited nanoparticles stoichiometry has been verified by X-ray photoelectron spectroscopy (XPS), while the optical bandgap values were determined by UV–vis optical absorption measurements.

Pineal tumors are rare and include a large variety of entities. Germ cell tumors are relatively frequent and often secreting lesions. Pineal parenchymal tumors include pineocytomas, pineal parenchymal tumor of intermediate differentiation, pineoblastomas and papillary tumors of the pineal region. Other lesions including astrocytomas and meningiomas as well as congenital malformations i.e. benign cysts, lipomas, epidermoid and dermoid cysts, which can also arise from the pineal region. Imagery is often non-specific but detailed analysis of the images compared with the hormone profile can narrow the spectrum of possible diagnosis.Les tumeurs de la région pinéale sont rares avec une grande variété de types cellulaires. Les tumeurs germinales représentent la majorité de ces tumeurs, qui sont souvent actives du point de vue hormonal. Les tumeurs du parenchyme pinéal incluent les pinéocytomes, les tumeurs pinéales de différentiation intermédiaire, les pinéoblastomes et les tumeurs papillaires de la région pinéale. D’autres lésions et malformations congénitales peuvent se développer au sein de la glande pinéale ou à son voisinage, elles sont représentés par les kystes bénins, les astrocytomes, les lipomes, les kystes dermoïdes et épidermoïdes. Le bilan radiologique est souvent aspécifique mais une analyse détaillée des images comparées avec le profil hormonal peut aider à cibler les hypothèses diagnostiques.
Subperiotal hematoma of the orbit associated with extradural hematoma has been rarely reported in the literature. We report a case of an 8-year-old girl who presented after traumatism, a subperiotal hematoma of the orbit with two extradural hematomas. During the first clinical examination realized while the patient was unconscious, left exophthalmia was found, but after conscious improvement, we discovered ophtalmoplegia and a grade I papilla edema. The CT scan showed two extradural hematomas and a left intraorbital hematoma. The intraorbital hematoma was drained by surgical way but the extradural hematomas were not surgical. The outcome has been marked by the neurological improvement and the oculomotricity recovering.
L’essor du VTT de descente est important depuis le début des années 1990. Sa pratique en montagne et les vitesses élevées sont à l’origine de nombreux accidents. Le but de cette étude était de faire un inventaire de la traumatologie du VTT de descente et de décrire les causes et les mécanismes lésionnels.Au total, 129 patients ont été inclus de manière prospective sur la période de juillet 2014 à novembre 2016. Cela représentait 138 cas vus en consultation pour 178 lésions. Il y avait 92 % d’hommes et 8 % de femmes d’âge moyen 27,9 ans (11–69 ans).En ce qui concerne le bilan lésionnel, l’atteinte des membres supérieurs (MS) prédominait avec 118 lésions (66 %), suivi des membres inférieurs (MI) avec 37 lésions (21 %) et des lésions de l’axe du corps (23 lésions soit 13 %). Les lésions de l’épaule et de la ceinture scapulaire étaient de loin les plus fréquentes (66, soit 56 % des lésions des MS) avec, entre autres, 25 fractures de la clavicule, 21 disjonctions acromioclaviculaires et 10 luxations antéro-internes de l’épaule. Au niveau des MI, les entorses du genou prédominaient avec 7 entorses, soit 19 % des lésions des MI. Enfin, les traumatismes crâniens représentaient 43 % des lésions de l’axe du corps.Les accidents de VTT de descente sont dominés par les traumatismes des membres supérieurs et tout particulièrement l’épaule et la ceinture scapulaire. Malgré des protections de plus en plus sophistiquées, celles-ci sont insuffisantes pour prévenir ces lésions ou, tout au moins, en diminuer l’incidence.The rise of downhill mountain biking has been significant since the early 1990s. Mountain biking and many accidents are related to high speeds. The aim of this study was to make an inventory of downhill mountain biking traumatology and to describe the causes and mechanisms of injuries.In total, 129 patients were included prospectively over the period from july 2014 to november 2016. This represented 138 admissions for 178 lesions. There were 92% men, and 8% women mean aged 27.9 years (11–69 years old).Regarding the lesion assessment, lesions of the upper limbs (UL) predominated with 118 lesions (66%), followed by the lower limbs (LL) with 37 lesions (21%) and lesions of the body axis (23 lesions i.e 13%). Lesions of the shoulder and the shoulder girdle were by far the most frequent lesions (66 or 56% of lesions of the UL) with, among others, 25 fractures of the clavicle, 21 acromio-clavicular dislocations and 10 anterior shoulder dislocations. At the LL level, sprains of the knee were the most frequent with 7 sprains or 19% of lesions of LL. Finally, head trauma represented 43% of injuries to the body axis.Downhill mountain biking accidents are dominated by trauma to the upper limbs, particularly trauma to the shoulder and shoulder girdle. Despite increasingly sophisticated protections, they are insufficient to prevent these lesions or at least reduce their incidence.
Knowledge of the encephalon anatomy is crucial for neurosurgical practice, especially the main cortical functional structures and their connections. General organisation of the encephalon is presented with frontal, parietal, occipital, temporal, limbic and insular lobes and their Brodmann correspondence. Secondly, subcortical anatomy will be presented with main white matter fasciculi in three separated categories: association, commissural and projection fibers. Main association fibers are inferior occipitofrontal fasciculus, superior longitudinal fasciculus, arcuate fasciculus, inferior longitudinal fasciculus, uncinate fasciculus, and cingulum. Commissural fibers include anterior commissure, corpus callosum and fornix. Projection fibers are internal capsule and optic radiations.
