BACKGROUNDGiant thoracic disc herniation (gTDH) is a rare condition. It is defined by a herniation that occupies at least 40% of the thoracic spinal canal and is usually calcified. Several surgical techniques have been described to date but this surgery remains a technically difficult procedure.OBJECTIVETo report the long-term outcome of 53 patients with myelopathy due to gTDH who were operated on by a thoracoscopic approach. The technical details of the preoperative assessment and the surgical procedure are presented.METHODWe present a retrospective study of a database of 53 patients operated for symptomatic gTDH by a thoracoscopic approach. The following clinical parameters were assessed initially and used during follow-up: Frankel grade and JOA score adapted to the thoracic spine (mJOA), pain in the lower limbs and limitation of the walking perimeter to less than 500 meters. The quality of spinal cord decompression was assessed postoperatively by magnet resonance imaging (MRI).RESULTSThe mean follow-up was 78.1 mo (SD 49.4). At the last follow-up visit, clinical examination showed a mean improvement of 0.91 Frankel grade (P < 0.001) and 2.56 mJOA score respectively (P < 0.001). Lower limb pain and walking perimeter were also improved. Postoperative MRI revealed that the resection was complete in 35 cases, subtotal in 13 cases, and incomplete in 5 cases.CONCLUSIONgTDH is a condition that often evolves favorably after surgery. The thoracoscopic approach is a feasible alternative technique.
Les infiltrations peropératoires de produits anesthésiques locaux sont utilisées et considérées comme efficaces dans de nombreuses procédures chirurgicales comme par exemple les arthroplasties totales de genoux. En revanche, l’intérêt des infiltrations de produits anesthésiques locaux, dans la chirurgie des fractures de la charnière thoracolombaire, sont controversées, tant sur leur efficacité que sur la molécule à utiliser. C’est dans ce contexte que nous avons conduit une étude rétrospective, monocentrique, dans le but d’analyser l’efficacité des infiltrations peropératoires de ropivacaïne dans le contrôle de l’analgésie postopératoire des patients ayant bénéficié d’une chirurgie pour fracture de la charnière thoracolombaire. Dans cette étude rétrospective, 76 patients, présentant une fracture de la charnière thoracolombaire nécessitant un geste chirurgical, ont été inclus, scindés en 2 groupes : l’un recevant une infiltration de 20 cc de ropivacaïne 7,5 % (infiltrations sous-aponévrotiques) ; l’autre sans infiltration. Tous les patients présentaient une fracture de T10 à L2, unique, sans autre lésion associée. Tous les patients ont bénéficié d’une ostéosynthèse à ciel ouvert sans geste de décompression (aucun trouble neurologique préopératoire). Tous les patients ont bénéficié du même protocole analgésique per- et postopératoire. Nous avons recueilli le score EVA au réveil du patient ainsi qu’à 2 heures postopératoires et la consommation de morphinique (dose équivalente intraveineuse en milligrammes) dans les 24 premières heures postopératoires. Le score EVA au réveil du patient et à 2 h postopératoires étaient significativement inférieurs dans le groupe avec infiltrations par rapport au groupe sans infiltration (respectivement p = 0,01 et p = 0,002). La consommation de morphinique dans les 24 premières heures postopératoires était réduite d’environ 50 % dans le groupe avec infiltrations par rapport au groupe sans infiltration (p = 0,01). Aucun effet indésirable local ou systémique n’a été retrouvé. Les infiltrations peropératoires de ropivacaïne semblent avoir une efficacité dans le contrôle de la douleur dans les 24 premières heures postopératoires ainsi qu’une réduction de la consommation morphinique.
Study design: Retrospective study.Introduction: Local infiltration analgesia is effective in many surgeries as knee arthroplasty, but the analgesic efficacy of local infiltration analgesia with ropivacaine in trauma spine surgery in T10 to L2 has not been clarified. We conducted a trial to assess the analgesic efficacy of intraoperative local infiltration analgesia (LIA) with ropivacaine.Objective: The aim of the present study was to clarify the effect of intraoperative local infiltration analgesia with ropivacaine on postoperative pain for patients undergoing thoracolumbar junction fracture surgery.Methods: In a retrospective study, in 76 patients undergoing spine surgery for thoracolumbar junction fracture, 20 ml of ropivacaine 7.5% (n R group = 38) was infiltrated using a systematic technique, or no infiltration was realized (n M group = 38). We assessed postoperative pain with Visual Analogue Scale (VAS) and morphine consumption in the 24 first hours.Results: VAS pain score upon awakening and at 2 hours postoperatively were significantly lower in the ropivacaine group (P = 0.01 and P = 0.002). Rescue opioid requirement during the 24 first hours were about 50% lower in the ropivacaine group (P = 0.01). No local or systemic side effects were observed.Conclusion: Intraoperative LIA with ropivacaine in thoracolumbar junction fracture surgery may have an analgesic effect in postoperative pain control (24 hours) with a reduction of VAS and morphine consumption. Porous polyethylene is an excellent restorative material for the reconstruction of large sized cranial defects and can be also used safely in reconstruction of the cranium. The cosmetic results are good, easy to perform, with a low complication rate.
Recombinant human bone morphogenetic protein-2 (rhBMP-2) was recently licensed for local administration during posterior lumbar fusion. In this indication, considerable uncertainty remains about the nature and mechanisms of the many adverse effects of rhBMP-2, such as ectopic bone formation. We report a case of ectopic bone formation with impingement on a facet joint and incapacitating low back pain after minimally invasive transforaminal L5-S1 interbody fusion with local application of rhBMP-2 (InductOs(®)). Revision surgery was eventually performed to alleviate the symptoms by removing the ectopic bone. Caution is in order regarding the use of rhBMP-2 during posterior lumbar fusion. Every effort should be made to minimise the risk of complications.
La Recombinant Human Bone Morphogenetic Protein-2 (rhBMP-2) vient d’obtenir l’autorisation de mise sur le marché pour les arthrodèses par voie postérieure. Mais, ses effets secondaires et leurs mécanismes dans ce cadre d’utilisation restent nombreux et mal compris, notamment en ce qui concerne les hypertrophies osseuses ectopiques. Nous traitons dans ce cas clinique d’une hypertrophie osseuse ectopique entrant en conflit avec une articulaire et responsable de lombalgies invalidantes apparues dans les suites d’une arthrodèse de type MI-TLIF en L5-S1 avec utilisation d’Inductos®. Une reprise chirurgicale a finalement été réalisée afin d’effectuer l’exérèse de cette excroissance et d’améliorer la symptomatologie de la patiente. L’utilisation de rhBMP-2 dans les voies postérieures doit rester prudente et s’accompagner de précautions d’utilisation pour limiter au maximum les risques de complications.
Epidural angiolipomas are uncommon benign tumors of the spine. Their clinical presentation is usually a progressive spinal cord compression. We report the case of a 22-year-old patient who presented with an acute paraparesis and a spontaneous epidural hematoma, which revealed a epidural angiolipoma which extended from C7 to T3. The patient underwent a C7-T3 laminectomy, in emergency, with evacuation of the hematoma and extradural complete resection of a fibrous epidural tumor bleeding. The postoperative course was favorable with regression of neurological symptoms. Epidural angiolipomas can be revealed by spontaneous intratumoral hemorrhage without traumatism. The standard treatment is total removal by surgery.
Background: Metastatic disease of the spine is an increasingly common public health problem. Surgery should be an integral component of the overall cancer treatment plan and, importantly, must neither delay not jeopardize any of the other components. The prognosis governs the choice of the surgical strategy. Tokuhashi et al. developed a prognostic score in 1990, then revised it in 2000 and 2005. Here, our objective was to evaluate the performance of the Tokuhashi score in a cohort of 260 patients and to look for other variables that might improve preoperative outcome prediction. Material and method: We retrospectively established a single-centre cohort of 260 patients who underwent spinal metastasis surgery between 1998 and 2008. For each patient, the following data were collected prospectively: socio-demographic features, history of the malignancy, variables needed to determine the Tokuhashi score, and treatments used. SAS 9.0 software was chosen for the statistical analysis. Variables were described as mean ± SD, overall survival was estimated using the Kaplan–Meier method, and survivals in subgroups were compared by the log-rank test. To assess agreement between survival predicted by the Tokuhashi score and observed survival, we computed Cohen’s kappa and interpreted the results according to Landis and Koch. Results: There were 143 females and 117 males with a mean age of 59 years and overall median survival of 10 months. Median observed survivals in the three Tokuhashi score categories (< 6, 6–12, and > 12 months predicted survival) were 5, 10, and 36 months, respectively. These survival times differed significantly (P < 0.0001). Cohen’s kappa indicated moderate agreement between predicted and observed survivals. Other factors associated with significant survival differences were time from cancer diagnosis to metastasis diagnosis (synchronous, < 2 years, 2–5 years, or > 5 years; P < 0.0001) and age (< 70 years or ≥ 70 years, P = 0.0053). Conclusion: Our cohort study supports the validity and reproducibility of the Tokuhashi score. Our finding that shorter time to metastasis diagnosis and age ≥ 70 years were also significantly associated with survival in our population invites further efforts to improve and update the Tokuhashi score. Resume Introduction Le rachis metastatique est un probleme de sante publique de plus en plus frequent. La chirurgie doit s’integrer dans le schema therapeutique du cancer et surtout ne pas retarder ou compromettre ce traitement. L’element fondamental pour definir la strategie chirurgicale est le pronostic des patients. Tokuhashi et al. ont developpe en 1990 un score pronostique, revise en 2000 et en 2005. L’objectif de cette etude est d’evaluer le score de Tokuhashi dans une cohorte de 260 patients et d’essayer de definir d’autres variables afin d’ameliorer l’evaluation du pronostic de ces patients en pre-operatoire. Materiel et methode Nous avons constitue une cohorte monocentrique retrospective de 260 patients, implementes de facon prospective, operes pour metastase entre 1998 et 2008. Les variables recueillies etaient des donnees socio-demographiques, des donnees relatives a l’histoire de la maladie, les variables necessaires au calcul du score de Tokuhashi et des donnees relatives aux traitements entrepris. L’ensemble des donnees a ete analyse avec le logiciel SAS 9.0. Les variables ont ete decrites a l’aide de leur moyenne et ecart-type. Puis la survie globale a ete estimee a l’aide de la methode de Kaplan-Meier et la comparaison des survies a ete realisee par le test du log-rank. La concordance entre le pronostic predit par le score de Tokuhashi et la survie observee dans la realite a ete testee. Pour cela le coefficient kappa de Cohen a ete utilise, son interpretation est basee sur la table de Landis et Koch. Resultats La cohorte de 260 patients etait composee de 117 hommes et de 143 femmes. La moyenne d’âge etait de 59 ans. La mediane de survie globale etait de 10 mois (IC 95 %). Les medianes de survie observee selon les 3 groupes pronostiques definis par Tokuhashi ( 6 mois, > 1 an) etaient respectivement de 5, 10 et 36 mois (IC 95 %). Leur difference s’est revelee significative ( p 5 ans) s’est revele significatif ( p p = 0,0053). Conclusion Notre cohorte confirme la validite et la reproductibilite du score de Tokuhashi. D’autres parametres tels que l’âge superieur a 70 ans et le delai d’apparition des metastases se sont reveles significatifs dans la population etudiee, ce qui nous encourage a essayer d’ameliorer et de reactualiser ce score. A total of 30 consecutive patients underwent endoscopic transglabellar resection of anterior cranial fossa meningioma between 2010 and 2013. We have modified the transfrontal approach by progressively limiting the extent of the osteotomies that we use for access. After a skin incision of 2 cm in front of the glabella, the anterior wall of the frontal sinus is then osteotomized. Mucosa is drilled. The frontonasal duct is closed and the sinus is cranialized. The posterior table of the frontal sinus is then removed with a round cutting burr. The debulking of the tumor is done under endoscopic view. After the resection of the tumor, the dura mater is closed. The anterior wall of the frontal sinus is replaced and secured with miniplates to the surrounding frontal bone. Of the patients, 70% were women. Tumor locations: 10 giant olfactory grooves, 10 olfactory grooves, 5 tuberculum sellae, and 5 jugum sphenoidale. Common presenting symptom was frontal syndrome (50%), visual change (30%), and anosmia (30%). Gross-total removal was achieved for all patients (Simpsons 2), with no deaths. Three patients had postoperative cerebrospinal fluid (CSF) leak. Around 60% of patients reported visual improvement. At the end of follow-up (22 months) we did not find any evidence of clinical or radiological recurrence. The cosmetic result is excellent for 20 patients and very good for 10 patients. ACF meningioma can be safely removed by an endoscopic transglabellar approach with an excellent cosmetic result, offering significant advantages over the traditional transcranial approach for suitable tumors.
INTRODUCTION:Transoral odontoidectomy is the treatment of choice in cases of anterior bulbo-medullary compression. The development of endoscopic procedures has made it possible to perform odontoidectomy via a minimally invasive endoscopic endonasal approach. We discuss the feasibility, advantages, and limitations of this surgical approach.MATERIALS AND METHODS:We report a two-center retrospective series of patients who underwent endoscopic endonasal odontoidectomy between September 2011 and February 2013. Preoperative characteristics, intraoperative data, clinical course, and postoperative complications were studied. The patients were followed for a minimum of 6 months. Cervico-occipital posterior fusion was performed during the same hospital stay in cases of preoperative instability.RESULTS:Nine patients underwent decompressive odontoidectomy, for rheumatoid pannus in five cases and basilar impression in four cases. All had progressive neurological symptoms. Seven patients also underwent posterior fusion. In six patients, the C1 anterior arch was preserved. Decompression was achieved satisfactorily in all nine cases. The patients were able to resume oral feeding the day after the intervention. No patient required tracheostomy. We observed no dural fistulae or infectious complications. One patient died 2 months after the intervention of a pulmonary embolism. All patients improved in terms of their preoperative neurological status.CONCLUSION:This short series shows the feasibility of the endoscopic endonasal approach for resection of the dens. This approach allows optimal viewing when using angulated instrumentation and seems to result in low morbidity. In some cases, this approach makes it possible to preserve the C1 anterior arch, thus limiting the risk of cranial settling.LEVEL:IV retrospective study.
Frontoethmoidal tumor represents a surgical challenge because of their anatomical location, the difficulty of a total removal, and the cosmetically disfiguring transfacial approaches needed. Recently, expanded endonasal endoscopic approaches have been developed, either alone or in combination with a craniotomy. We have developed a new pure endoscopic approach by an endonasal approach with an endoscopic transsinusal approach in the same procedure. We have performed a transglabellar approach first with the most important resection of the tumor. The opening with the ethmoid is closed by using the posterior wall of the frontal sinus wich is resized and put in the defect. After a modification of the position of the head, we have performed an endonasal endoscopic approach for the resection of the residue of the tumor in the same procedure. Of the patients, four were women. There were two esthesioneuroblastomas (grade 2), two giants frontoethmoidal meningiomas, and two mucoceles. Common presenting symptom was frontal syndrome (four patients), anosmia (four patients), and visual change (three patients). Gross total removal was achieved for all patients, with no deaths. One patient had postoperative cerebrospinal fluid leak. At the end of follow-up (26 months), we did not find evidence of clinical or radiological recurrence. The cosmetic result is excellent for all patients without any problem of cicatrization.
La résection de l’apophyse odontoïde par voie trans-orale est le traitement de référence en cas de compression médullobulbaire antérieure fixée. Le développement des techniques endoscopiques a permis d’envisager la réalisation d’odontoïdectomie par voie endonasale. Nous discutons la faisabilité, les avantages et limites de cette dernière voie. Nous rapportons une série rétrospective bicentrique de patients opérés d’une odontoïdectomie endonasale endoscopique entre septembre 2011 et février 2013. Les caractéristiques préopératoires, les données peropératoires ainsi que l’évolution et les complications postopératoires ont été étudiées. Les patients ont été suivis pendant un minimum de 3 mois. Une stabilisation cervico-occipitale postérieure a été réalisée durant la même hospitalisation en cas d’instabilité préopératoire. Neuf patients ont subi une odontoïdectomie décompressive pour pannus rhumatoïde dans 5 cas et impression basilaire dans 4 cas. Tous présentaient des symptômes neurologiques évolutifs. Cette voie d’abord a pu être privilégiée chez ces patients après étude de leurs caractéristiques radiologiques. Sept patients sur 9 ont également bénéficié d’une stabilisation postérieure. Pour 6 patients, l’arc antérieur de C1 a pu être préservé. La décompression a pu être réalisée de manière satisfaisante dans les 9 cas. Les patients ont pu reprendre une alimentation orale dès le lendemain de l’intervention. Aucun patient n’a nécessité de trachéostomie. Nous n’avons pas observé de fistule durale ou de complication infectieuse. Une patiente est décédée deux mois après son intervention d’une embolie pulmonaire. Tous les patients ont présenté une amélioration de leur statut neurologique préopératoire. Cette courte série montre la faisabilité de la voie endonasale endoscopique pour la résection de l’odontoïde. Cette voie d’abord permet une visualisation optimale par l’utilisation d’optiques angulées et semble compliquée d’une faible morbidité. Cette voie d’abord permet dans certains cas une préservation de l’arc antérieur de C1, limitant ainsi le risque de déstabilisation atlanto-occipitale. IV. Étude rétrospective.

Les douleurs peripheriques sont plus rapidement equilibrees (6 mois), avec une amelioration superieure (70 %) et surtout une stabilite dans le temps qui ameliore nettement la qualite de vie. Concernant les etiologies cerebrales, AIC restent l’indication preferentielle, les lesions tumorales semblent moins satisfaisantes (50 %, n = 2). L’intensite de stimulation est plus faible (2,0 V). Pour les atteintes medullaires, les resultats sont plus decevants (55 % en moyenne). L’equilibre est plus long (2 ans) et la douleur fluctue (en fonction des conditions atmospheriques). L’intensite est plus forte (3,5 V). Le traitement medicamenteux reste inchange. Conclusion.– La stimulation corticale est theoriquement un traitement strictement symptomatique peu importe la cause. Les membres fantomes et algodystrophie ont la reputation d’etre de mauvaises indications. Dans cette etude, les resultats sont satisfaisants et surtout stables dans le temps. Enfin, les atteintes medullaires sont souvent decevantes, mais restent le seul traitement efficace lors des echecs medicamenteux.
L’abord antérieur de la charnière thoracolombaire du rachis permet un traitement des lésions traumatiques infectieuses ou tumorales de T11 à L2, associant décompression médullaire, corporectomie et arthrodèse intersomatique. Cependant, cet abord est également réputé délabrant vis-à-vis des structures anatomiques rencontrées (poumon, péritoine, diaphragme). L’objectif de ce travail était de montrer que la décompression des structures nerveuses comme la reconstruction vertébrale antérieure peuvent être effectuées via un abord moins invasif extrapleural rétropéritonéal (AMIER). Nous décrivons étape par étape la procédure chirurgicale permettant un abord mini-invasif extrapleurale rétropéritonéale de la charnière thoracolombaire. L’AMIER offre une excellente exposition permettant une décompression complète et une reconstruction anatomique antérieure satisfaisante. Les principaux pièges et complications sont avant tout pulmonaire et une approche prudente et rigoureuse permet de limiter les comorbidités par rapport à une thoraco-phrénolombotomie classique.
The minimally invasive transsinusal approach to the subfrontal region provides ready expeditious access to the base of the anterior cranial fossa without the need for brain retraction, craniotomy, or nasoorbital osteotomies. It represents an excellent alternative in the surgical access of midline tumors in this region of the skull base, providing excellent direct, safe exposure.
