On dispose de peu de données françaises comparatives sur les caractéristiques, la prise en charge et le devenir des patients hospitalisés pour un infarctus avec sus-décalage de ST (STEMI). À partir du registre FAST-MI 2010, les données des patients admis pour STEMI ont été analysées en fonction de la région d’hospitalisation ; 6 grandes régions ont ainsi été constituées en les regroupant à partir des régions administratives en vigueur en 2010. On constate des variations régionales de la typologie des patients, reflétant pour une part les variations démographiques des différentes régions. La prise en charge aiguë varie, avec notamment, pour des raisons géographiques et d’organisation, une plus grande utilisation de l’angioplastie primaire en région parisienne. Dans l’ensemble, toutefois, et en dépit des variations statistiques, la prise en charge dans les différentes régions montre plus de similitudes que de différences importantes. Les complications, notamment la mortalité hospitalière, ne sont pas statistiquement différentes dans les différentes régions.Data on regional variations in the characteristics, management and early outcome of patients admitted with ST-elevation myocardial infarction (STEMI) in France are limited. We used data from the FAST-MI 2010 registry to determine whether regional specificities existed, dividing the French territory into 6 larger geographical regions. Variations in the patients’ characteristics were found, partly related to regional variations in demography. Acute reperfusion strategy showed more use of primary percutaneous coronary intervention in the greater Paris area, compared to other regions, which would be expected owing to geography and local availability of catheterization laboratories. Overall, however, inhospital management showed more similarities than differences across regions. Complications, and in particular inhospital mortality, did not differ significantly among regions.
Le stress et le burnout sont des facteurs majeurs auxquels peuvent être exposés les praticiens de la santé, médicaux ou paramédicaux durant leur exercice professionnel. Le burnout émousse la volonté et altère la motivation, et conduit à une altération croissante des capacités à s’engager et à rebondir. Nous analysons dans cet article les facteurs stressants, les possibilités de faire face aux situations difficiles chez les praticiens de la santé. La plupart des facteurs qui sont susceptibles de provoquer un burnout sont liés aux conditions du soin auprès du malade, notamment pour les malades chroniques, gériatriques ou en risque vital. Cependant, la plupart des facteurs précipitant le burnout résult du management inadéquat des unités de soins et en particulier des insuffisances du soutien humain du personnel soignant.Stress and burnout are major factors that medical doctors and nurses often have to deal with while in their work environment. Burnout is a loss of will in which motivation is damaged, resulting in an increasing inability to mobilize interest and capabilities. This paper was aimed at identifying stressors, effective coping skills, and the impact that stress has on formal caregivers. Most of the stressors triggering the burnout risk were the result of patient care, particularly with chronic, geriatric and/or unstable patients. Most stressors were the result of management of the unit, notably inadequate staffing.
L’objectif de cet article est d’analyser à la lumière de la littérature l’intérêt d’une approche cognitivo-comportementale dans le traitement de la maladie asthmatique. Les thérapies cognitivo-comportementales (TCC) ont fait leurs preuves dans la prise en charge des émotions négatives, des comportements inadéquats et de la non-compliance aux traitements médicamenteux, suscités par les maladies chroniques. Les bases conceptuelles et les techniques des TCC sont adaptées à la maladie asthmatique. L’élaboration d’un diagnostic centré sur le patient et prenant en compte les dimensions biomédicales, socioprofessionnelles, psychoaffectives et cognitives, conduit à la mise en œuvre d’une stratégie thérapeutique comportant, outre le traitement médicamenteux, une série de mesures éducatives et de compétences à acquérir par le patient (auto-observation des symptômes, autogestion des crises, contrôle et adaptation à l’environnement et aux activités, stratégies d’ajustement, etc.) ainsi que l’évaluation des compétences du patient et des bénéfices en termes de morbidité, de qualité de vie et de conséquences sociales. L’approche cognitivo-comportementale de l’asthme est utile, notamment dans l’établissement d’une relation médecin-malade empathique et collaborative, dans la distinction entre composantes subjectives et objectives de la dyspnée, dans l’élaboration et la mise en œuvre du programme d’éducation, dans le diagnostic et la prise en charge des émotions négatives et des stratégies de coping inadaptées souvent associées à la maladie asthmatique, et dans l’évaluation des compétences du patient et des résultats en termes de morbidité et de qualité de vie du patient.
Social phobia is one of the most frequent mental disorders and is accessible to two forms of scientifically validated treatments: anti-depressant drugs and cognitive behavior therapies (CBT). In this last case, graded exposure to feared social situations is one of the fundamental therapeutic ingredients. Virtual reality technologies are an interesting alternative to the standard exposure in social phobia, especially since studies have shown its usefulness for the fear of public speaking. This paper reports a preliminary study in which a virtual reality therapy (VRT), based on exposure to virtual environments, was used to treat social phobia. The sample consisted of 36 participants diagnosed with social phobia assigned to either VRT or a group-CBT (control condition). The virtual environments used in the treatment recreate four situations dealing with social anxiety: performance, intimacy, scrutiny, and assertiveness. With the help of the therapist, the patient learns adapted cognitions and behaviors in order to reduce anxiety in the corresponding real situations. Both treatments lasted 12 weeks, and sessions were delivered according to a treatment manual. Results showed statistically and clinically significant improvement in both conditions. The effect-sizes comparing the efficacy of VRT to the control traditional group-CBT revealed that the differences between the two treatments are trivial.
Social phobia is one of the most frequent psychiatric disorders and is accessible to two forms of scientifically validated treatments: anti-depressant drugs and cognitive-behavioral therapies. Graded exposure to feared social situations (either in vivo or by imagining the situations) is fundamental to obtain an improvement of the anxious symptoms. Virtual reality (VR) may be an alternative to these standard exposure techniques and seems to bring significant advantages by allowing exposures to numerous and varied situations. Moreover studies have shown that human subjects are appropriately sensitive to virtual environments. This chapter reports the definition of a VR-based clinical protocol and a study to treat social phobia using virtual reality techniques. The virtual environments used in the treatment reproduce four situations that social phobics feel the most threatening: performance, intimacy, scrutiny and assertiveness. With the help of the therapist, the patient learns adapted cognitions and behaviors when coping with social situations, with the aim of reducing her or his anxiety in the corresponding real life situations. Some studies have been carried out using virtual reality in the treatment of fear of public speaking, which is only a small part of the symptomatology of most of social phobic patients. The novelty of our work is to address a larger group of situations that the phobic patients experience with high anxiety. In our protocol, the efficacy of the virtual reality treatment is compared to well established and well validated group cognitive-behavioral treatment.
Alors que les dispositifs d’accueil des urgences pédopsychiatriques semblent de plus en plus fréquentés par les adolescents, nous avons cherché à déterminer les spécificités cliniques et sociodémographiques de ces patients, encore mal connus.Nous avons réalisé une analyse de la revue de littérature, répertoriée à partir des principaux moteurs de recherche.La fréquentation des urgences pédopsychiatriques est en augmentation depuis plus de 20 ans, avec une nette prédominance des adolescents et une surreprésentation des accompagnements équivalents à l’aide sociale à l’enfance (ASE) : 22,7 à 43 % des patients. Les premiers motifs de consultation aux urgences sont l’agitation et la violence. Les principaux diagnostics qui y sont posés sont les troubles de l’humeur et les troubles des conduites, dont le trouble oppositionnel avec provocation. On relève une part importante de retours aux urgences dans l’année : près d’un tiers des adolescents, avec notamment comme facteur de risque la présence d’un suivi ASE.Sur le plan clinique, on retrouve en proportion équivalente les troubles internalisés et externalisés. Les spécificités les plus marquantes concernent davantage l’environnement de ces adolescents, avec notamment une proportion importante de suivis par l’ASE. Pour ces adolescents qui consultent dans l’urgence, un des enjeux thérapeutiques est de pouvoir les réintégrer dans des soins s’inscrivant sur la durée, grâce à un indispensable travail en amont entre les différents professionnels qui les accompagnent.Au vu des données de la littérature, les connaissances cliniques et sociodémographiques concernant les adolescents adressés pour « urgence psychiatrique » doivent encore être développées, de manière à mieux cerner une problématique importante et à proposer des projets de soins au plus près des besoins de cette population à haut risque.While the reception arrangements for child psychiatric emergencies seem increasingly frequented by adolescents, we sought to identify clinical and sociodemographic characteristics of these patients, yet poorly explored and understood.We conducted an analysis of the literature review listed from the major search engines.Attendance at child psychiatric emergencies is increasing for over 20 years, with a predominance of adolescents and accompaniment by an overrepresentation of social assistance to children (child welfare): 22,7% to 43% of patients. The first emergency department visit patterns are agitation and violence. The main diagnoses that are registered are mood disorders and conduct disorders, including oppositional defiant disorder. A significant proportion of these adolescents come back to emergencies procedures within the year: nearly one third of adolescents, including a history of child welfare monitoring as a risk factor.In the different studies, the same proportion of internalizing problems and externalizing problems are registered by clinician investigators. The most striking characteristics relate more to the environment of these teenagers, including a significant proportion who are followed by child welfare. For those teenagers consulting in an emergency service, a therapeutic issue is to reintegrate them into a sufficient long-term care, thanks to disponibility, accessibility and an essential preliminary work and partnership coordination between the various professionals who accompany them.Based on literature data, knowledge about clinical and sociodemographic adolescents referred for “psychiatric emergency services” need to be developed in order to better understand an important issue and to propose care projects closer to the needs of this high-risk population.
