Objective To analyze the clinical features of cognitive impairment in patients with Parkinson's disease(PD),and explore its mechanism for early diagnosis of Parkinson's cognitive impairment.Methods The clinical data of 50 patients with PD and 50 non-PD patients were compared,which included data of brain CT and MRI scan results,MSSE score,UPDRS score,and the Wechsler Adult Intelligence Scale subtests results,main identification parameters screening for early diagnosis of cognitive dysfunction in PD,such as imaging parameters and cognitive function scores.Results The scores of MSSE,VIQ,PIQ,MQ and UPDRS of PD group were significantly lower than those in the control group(P<0.01).The mean widths of the cerebral sulci of the frontal lobe,parietal lobe,and lateral body measured by head CT or MRI were lower in PD group than in the non-PD group(P<0.01).Conclusion PD patients have some cognitive impairment,the more severe the symptoms of Parkinson's disease,the more obvious the cognitive dysfunction;and the intellectual impairment is severe than memory impairment is.The major pathological changes of PD cognitive impairment may be appeared as the width changes of cerebral sulci of the frontal lobe,parietal lobe and lateral body.
Objective To compare the differences in curative effect between general treatment model and stroke unit model in the treatment of stroke in the elderly inpatients, to explore the standardized management model, for improving efficacy and reducing the degree of invalidity and mortality. Methods A total of 1200 cases with acute cerebrovascular disease were admitted into our department from April 2002 to April 2007. All patients were randomly assigned either into stroke unit or general ward to receive treatment, and the data of two groups were compared. The key indexes included mortality, hospitalization time, Barthel Index (BI), National Institutes of Health Stroke Scale (NIHSS) and Oxford Handicap Scale (OHS). All the patients or their family members were asked to have the Visual Analogue Scales (VAS). The data of cost and effect were analyzed with SPSS software. Results The complication rate and mortality rate were significantly lower in stroke unit group than in general ward group (P< 0.01). The scales of BI, NIHSS, OHS and VAS were significantly better in stroke unit group than in general ward group when discharged (P<0.05). The hospitalization time was shorter in stroke unit group than in general ward group (P<0.05). There was no significant difference in the costs of hospitalized patients between the two groups (P>0.05).Conclusions Stroke unit model can improve the physical activity and life quality of patients and reduce disease mortality without increasing the cost of patients.
【Objective】To research on the relationship between cerebral infarction and carotid atherosclerosis in senile patients.【Methods】Color Doppler ultrasound was used to measure the formation of lipid spots in both carotid arteries developed in the100 cases of senile cerebral infarction patients.【Results】All the cases had different degree of carotid atherosclerosis,among which plaque formation accounted for 73%.The frequent locations of plaques were carotid artery bifurcations,common carotid arteries and internal carotid arteries.【Conclusion】Carotid atherosclerosis is a significant risk factor for cerebral infarction in senile patients.Carotid artery examination should be a routine clinical item for the aged.
Objective:To observe the therapeutic effect and safety of edaravone on acute cerebral infarction.Methods:Fifty cases of patients suffering acute cerebral infarction were randomly divided into 2 groups:the treatment group (24 cases) received edaravone plus routine medication for 14days,while the control group (26 cases) received routine medication only.The NIHSS and Barthel indexes were quantified at baseline and after treatment to evaluate the neurologic impairment and therapeutic effect.Results:The NIHSS score and the Barthel index of the treatment group were superior to those of the control group (P0.05).Conclusions:Edaravone has good effectiveness on acute cerebral infarction with appropriate neuroprotective effect.
目的观察银杏叶制剂舒血宁注射液治疗缺血性脑血管病的疗效。方法将100例缺血性脑血管病患者分为试验组和对照组,对照组给予常规治疗,试验组加用舒血宁注射液,对两组患者的疗效进行比较分析。结果试验组显效8例,有效38例,无效4例,总有效率为92%;对照组显效5例,有效36例,无效9例,总有效率为82%,两组总有效率间差异有统计学意义(P<0.05)。结论银杏叶制剂对缺血性脑血管病有较好的疗效,安全性强,值得临床推广使用。
近十年来急性心肌梗死(AMI)的溶栓疗效得到肯定,急性脑梗死的溶栓治疗也已处于临床Ⅱ期或Ⅲ期试用阶段.急性缺血性脑卒中溶栓治疗的主要目的是溶开血栓,快速恢复梗死区微循环,从而获得脑血流的早期重灌注,减轻缺血程度,限制神经细胞及其功能的损害.
Objective To explore the characteristics of the elderly emotional disorders and countermeasures.Methods Two hundred cases of elderly anxiety and depression of the data were analyzed,relationships between elderly anxiety,depression and physical disease,gender,age,education,occupation,sleep and so on were discussed.Results The elderly anxiety and depression in the level of education at all levels of people occured,and low education were symptoms of lighter,higher education were symptoms of heavier.The patients of stroke,hypertension,coronary heart disease,COPD,cancer,sleep disorders and in patients with peptic ulcers were usually had emotional disorders.Male was more than female,elderly patients were more than younger patients.Conclusion The elderly anxiety,depression are related to physical disease,age,gender,education,occupation,sleeping disorder.Doctors should pay attention to identify the elderly emotional disorders,timely processing.
AIM: To investigate the effect of transient ischemia attack(TIA) on phospho-lipid level and the effect of traditional Chinese herb - Yangxueqingnao par-ticles. METHODS: To establish the animal model of TIA in mice by injectingPeroxide through caudal vein, and then feed the mice with Yangxueqingnaoparticles in different dose. After evoking 4 times of TIA and cut off the headthe other day, then measure the level of lysophosphatidic acid(LPA), generalphospholipidin blood plasma and brain tissue. RESULTS: Score of the groupwithout treatment was 1.0±0.8, while that of group administered withYangxueqingnao particles(2 g/kg) was 0.5±0.4(P<0.05) LPA level ofTIA mice were 6. 1:1±0.4and 2.1±1.3 and increased significantly com-pared with that of the control.This changes can be reversed by treatment as canbe seen the result 4.9±1.6. general phospholipid level of TIA mice in braintissue decreased significantly compared with that of the control as the result:(2.3±1.1) and (6.6±2.8) μmol/g, respectively. After treatment, generalphopholipid level can reach (4.3±2.0) μmol/g.CONCLUSION: LPA andgeneral phospholipid levehin blood plasma and brain tissue are abnorma] inexperimental TIA mice, but can be reversed by Yangxueqingnao particles andit can be used in treatment.
Objective:To study the effect of TIA on LPA and the influence of Yangxueqingnao granule on it. Methods: Mice TIA model was established by injection of peroxide. Yangxueqingnao granule was given by gastric infusion once daily for 4d and the brain and plasma level of LPA were determined. Results: The stroke score was (0. 500 ± 0. 408) for Yangxueqingnao granule group (2g · kg -1) and (0. 969 ± 0. 790) for model group, and there was significant difference between the two groups. The plasma LPA was (6.12 ± 0. 35)μmol·L -1 for model group, (2. 11 ± 1. 27)μmol· L-1 for normal group and (4.48 ± 1. 55)μmol·L-1 for Yangxueqingnao granule group (4g·kg-1), there were significant difference between groups ( P 0. 05 ) . The brain LPA level of the model group was significantly lower than that of the normal group[(2.27 ± 1.13) vs (6.58 ± 2. 77)μmol·g-1 ,P0.05] ,and that of Yangxueqingnao granule group (4g · kg ) markedly increased in comparison with that of the model group [(4.26± 1.99) vs (2.27 ± 1. 13)μmol·g-1, P0.05]. Conclusion: The LPA changes induced by TIA can be reversed by Yangxueqingnao granule.
