To report in vivo experience of a delivery assist catheter as ascending aid for a large-bore catheter for intracranial thromboaspiration.
BACKGROUND:Familial hemiplegic migraine (FHM) is a rare monogenic form of migraine with aura with three distinct genetic subtypes (FHM1-3). Imaging studies during acute FHM attacks are scarce in the literature. This is particularly true for the FHM2 subtype.PATIENTS AND METHODS:In this monocentric study, we retrospectively evaluated imaging data of four different patients with genetically confirmed FHM2. Analysis comprised a total of eight cMRI and two CT perfusion studies, which were obtained during a total of six different attacks.RESULTS:cMRI investigations at all available time-points were without evidence of cytotoxic edema. The most prominent finding (four attacks in three patients) was swelling and/or cortical hyperintensity of the affected cerebral hemisphere on T2/FLAIR-weighted MRI. Further changes, encountered only in a few patients, included increased perfusion of the affected hemisphere (as assessed by perfusion CT) as well as dilatation of the middle cerebral artery.CONCLUSION:Our data from a sizeable cohort of FHM2 patients highlight that swelling / cortical hyperintensity of the clinically affected cerebral hemisphere - which has been previously reported mainly in FHM1 - can be observed also in FHM2. Further, they suggest that these changes, which tend to be present not in the very beginning, but rather later on during attacks, may be a possible correlate of the prolonged attack duration in our patients.
Kernaussagen Prinzipiell differenziert man beim Schlaganfall zwischen ischämischen Schlaganfällen (85 %) und Hirnblutungen (ca. 15 %). Eine Unterscheidung zwischen den beiden Entitäten ist anhand der klinischen Symptome nicht möglich. Der Verdacht auf einen Schlaganfall erfordert eine sofortige Bildgebung des Kopfes zur Differenzierung, da sich die Behandlungskonzepte konträr unterscheiden und die Therapie möglichst früh eingeleitet werden soll: „Time is Brain“. Bei der Primärversorgung von Patienten mit V. a. ICB oder ischämischen Schlaganfall steht die Sicherung und Stabilisierung der Vitalfunktionen im Vordergrund. Intrazerebrale Blutungen (ICB) sind trotz moderner Intensivmedizin weiterhin mit einer hohen Mortalitäts- und Komplikationsrate assoziiert. Bei der ICB besteht die weitere Behandlung in Blutdruckeinstellung, antikonvulsiver, hämostatischer und Hirndrucktherapie, u. U. operativer Therapie.
Zusammenfassung: Die internukleär Ophthalmoplegie ist die häufigste supranukleäre Störung der Blickmotorik. Pathologisch-anatomisches Korrelat ist eine Läsion des medialen Längsbündels, der Verbindung zwischen den Kerngebieten des III. und VI. Hirnnervs. Ein isoliertes, akutes Auftreten einer internukleären Ophthalmoplegie ist meist entzündlich bedingt, zum Beispiel bei einer multiplen Sklerose. Wir berichten über einen Patienten mit akut aufgetretener, isolierter internukleärer Ophthalmoplegie, bei dem mittels diffusionsgewichteter MRT eine kleine ischämische Läsion des medialen Längsbündels im Mittelhirn nachgewiesen werden konnte. Abstract: Internuclear ophthalmoplegia is the most frequent supranuclear form of gaze paresis. It is caused by a lesion of the median longitudinal fascicle, which connects the nuclei of the IIIrd and VIth cranial nerves. An acute, isolated onset of this syndrome is highly suggestive of an inflammation of the central nervous system, mostly of multiple sclerosis. We report clinical and imaging findings of a patient with acute onset of internuclear ophthalmoplegia, in whom an isolated infarction of the median longitudinal fascicle in the midbrain could be demonstrated by diffusion-weighted MRI.
