RATIONALE AND OBJECTIVES: The iliopsoas bursa, collapsed in its normal state, provides a gliding surface between the hip joint and the iliopsoas musculotendinous complex. Its enlargement is generally non-symptomatic, but can cause pain, an inguinal mass or symptoms related to compression of neighboring structures. However, anatomical studies of this bursa are scarce. Thus, the aim of this study was to describe its morphology and relationship with adjacent structures on CT arthrography, specifically in cases where the bursa communicates with the hip joint. MATERIALS AND METHODS: Consecutive hip CT arthrography examinations in which bursal effusion had been reported were retrospectively analyzed. The iliopsoas bursa was described according to its shape, size and relationship with neighboring structures. CT arthrograms were reviewed for the presence of spontaneous iliopsoas bursa opacification due to communication with the underlying hip joint. RESULTS: 178 hip CT scans were reviewed. An enlarged medial arm in a deep intramuscular location, between the femoral neurovascular bundle medially and the iliopsoas tendon laterally, was always visible. The bursa was unilobed, with a “teardrop” shape in 2 examinations. The lateral arm was distended in 23 cases, with a unilobed or bilobed appearance, respectively presenting a “horseshoe” or “trident” shape. Communication between the hip joint space and the bursa was shown in 25 of the 178 hip CT arthrograms. CONCLUSION: The iliopsoas bursa can be recognized by its close relationship with the iliopsoas tendon, which can divide it, resulting in a “teardrop”, “horseshoe” or “trident” shape in the axial plane.
Introduction. Compared to pain, weakness, and stiffness, snapping phenomena are less frequently reported. The anatomical implication of subacromial bursa on snapping syndrome has not yet been studied despite of the fact that subacromial volume is implicated in this syndrome. The aim of this study is to analyze the anatomical and dynamic implication of the subacromial bursa in snapping syndrome. Methods. We conducted a retrospective of symptomatic case series (n = 9) study including dynamic sonography, video recordings resulting from standardized clinical dynamic examinations, and the results of shoulder magnetic resonance imaging. Nine patients complaining of snapping phenomena of the anterior shoulder (seven males and two females, mean age: 37.1 ± 10.2 years old), in whom dynamic sonography could confirm the diagnosis of snapping subacromial bursa, were included in this study. Results. All the patients included in this study presented non-traumatic painful snapping syndrome without plication before the snap on the dynamic sonography. All complained of a disabling snap of the shoulder associated with pain and without folding before the snapping phenomenon. Four of them had a bursitis of the subacromial bursa diagnosed on their shoulder’s magnetic resonance imagery. No significant statistical correlation (rS = −0.372; p = 0.595) was found between the triggering mechanisms, such as the snap shoulder release position, and the position of the anterior recess of the subacromial bursa relative to the biceps’ tendon. Conclusions. This study highlights the anterior recess of the subacromial bursa as a previously underexplored anatomical contributor to snapping syndrome, particularly in young, physically active individuals, emphasizing the need for dynamic sonography in diagnosing this condition. The anterior recess of the subacromial bursa represents an additional cause of snapping, which especially takes place in young and physically active patients. More than sport practice, professional activities that require repetitive tasks of the shoulder seem to represent a risk factor.
A distal posterior interosseous nerve (DPIN) neuroma related to prior surgery of the wrist can lead to disabling chronic pain. Ultrasound may represent a useful diagnostic tool due to its high resolution and ability to detect iatrogenic neuromas along small nerves of the limbs. However, the utility of ultrasound in the evaluation of both the normal sonographic appearance of the DPIN and traumatic neuromas is limited. We present a series of two patients who suffered from chronic dorsal wrist postoperative pain, where ultrasound detected histologically confirmed DPIN neuromas. After DPIN resection, the pain of our two patients completely disappeared without any functional deficit.
Common benign subcutaneous benign masses and pseudomasses represent a wide spectrum of masses among which lipomas and epidermal cysts account for a vast majority of the lesions encountered in routine practice. Other types of tumors originate from various components of the skin, such as the pilous tract, nerves, veins, arteries, or eccrine glands. In some instances, pseudomasses may be distinguished from tumoral masses using specific signs. When the diagnosis of a subcutaneous lesion lacks specificity for benignity with ultrasound, radiologists should ensure proper management of the patient to rule out the possibility of a rare but possible sarcoma of the subcutaneous layer.
This study sought to evaluate the biomechanical properties of the interface between the rotator cuff and the semicircular humeral ligament or rotator cable (RCa) using histological and biomechanical techniques. Out of 13 eligible cadaver specimens, 5 cadaver shoulders with an intact rotator cuff were included, 8 were excluded due to an injured rotator cuff. The histological study enables us to describe the capsule-tendon interface between the infraspinatus tendon (IST) or supraspinatus tendon (SST) and RCa, and to detect loose connective tissue layers to determine their precise location and measure their length along the interface. The biomechanical study sought to characterize and compare the mechanical strength of the IST-RCa versus SST-RCa interfaces. The average thickness of the RCa was 1.44 ± 0.20 mm. The histological study revealed a loose connective tissue layer at the IST-RCa interface, a finding not observed at the SST-RCa interface. The biomechanical study showed that the rigidity of the SST-RCa interface (72.10–2 N/mm) was 4.5 times higher than for the IST-RCa interface (16.10–2 N/mm) and the average maximum forces reached were 19.0 N and 10.6 N for the SST-RCa and IST- RCa interfaces, respectively. The IST-RCa interface consists of a loose connective tissue layer contrary to the SST-RCa interface. In parallel, two different groups in terms of the mechanical response were identified: the IST-RCa interface group had less rigidity and ruptured more quickly than the SST-RCa interface, therefore emerging as the most vulnerable interface and explaining a potential extension of rotator cuff tears.
Les tumeurs osseuses bénignes ostéoblastiques (ostéome ostéoïde [OO] et l'ostéoblastome [OB]) représentent environ 5 % de la totalité des tumeurs osseuses primitives bénignes. Leur prise en charge se fait, dans la grande majorité des cas, par thermoablation percutanée sous contrôle radiologique pour les OO et chirurgicalement pour les OB. Malheureusement, certaines localisations, notamment juxta-articulaires ou à proximité de structures vasculonerveuses, rendent difficiles l'utilisation des traitements usuels. Dans ces situations, l'utilisation de bisphosphonates (BP) peut être envisagée. Étude rétrospective monocentrique au sein du CHU de Rennes des patients traités par bisphosphonates entre 2019 et 2023. Ces patients ont été identifiés comme à risque de complications en cas de prise en charge chirurgicale ou sous contrôle radiographique après discussion multidisciplinaire. Le traitement utilisé comprenait de l'acide zolédronique 4 milligrammes, en perfusion, tous les mois, réalisée au sein du service de rhumatologie. Le nombre de perfusion était dépendant de l'évolution clinique, avec une réévaluation clinique et d'imagerie (IRM et TDM) systématique réalisée à 2 mois de la troisième perfusion pour discuter d'une éventuelle prolongation de traitement. Étaient recueillies les données démographiques, les traitements instaurés au préalable, la consommation d'antalgiques, l'échelle numérique initiale et après traitement, les données d'imagerie (densité et taille du nidus, évolution du pic artériel, évolution de l'œdème osseux). Les patients étaient informés de la procédure, des bénéfices attendus et des risques éventuels. Quatorze patients atteints d'OO (4 femmes et 10 hommes) et 2 patients (2 hommes) atteints d'ostéoblastome ont été inclus dans cette étude. La moyenne d'âge des patients était de 30 ans (18–49). L'EN moyenne à l'inclusion était de 64/100 (40–90). Avant traitement, la consommation d'antalgiques de paliers 2 et 3 étaient nécessaire chez 44 % des patients associés à des AINS (88 %). Au moment de l'inclusion, 44 % des patients étaient en arrêt de travail. L'indication retenue pour l'utilisation des BP était la nature juxta-articulaire dans 44 % et la proximité de structures vasculonerveuses dans 56 % des cas. La durée moyenne des symptômes avant traitement par BP était de 26 mois (8–72). Le nombre de perfusion moyen nécessaire était de 3 (1–5). Une patiente a présenté un effet secondaire après la première perfusion (uvéite) ayant occasionné l'arrêt du traitement. L'EN moyenne lors de la réévaluation à 2 mois était de 14/100 (0–50), avec une moyenne de variation des EN de −50/100. La durée moyenne de suivi était de 27 mois (6–60). Le résultat a été évalué positif, par auto-questionnaire du patient, complètement ou partiellement chez respectivement 11/16 (68,8 %) et 14/16 (87,5 %). L'usage des BP dans le cadre des tumeurs ostéoblastiques semble être utile en cas de formes juxta-articulaires et à proximité de structures vasculonerveuses où la balance bénéfice/risque de la chirurgie ou de la thermoablation par radiofréquence est défavorable.
Gorham-Stout disease (or “vanishing bone” disease) is a rare mono or polyostotic disease of unknown etiology, characterized by intraosseous angiomatous proliferation leading to bone resorption. We report the case of a 17-year-old woman presenting with symptomatic osteolytic lesions of the frontal vault. Imaging was suggestive of Gorham-Stout disease without argument for other diagnoses. An unusual evolution of the “vanishing bone” lesions was observed on the scan after one year, with a full recovery of the lytic lesions. This report shows for the first time a spontaneous restitutio ad integrum of bone matrix in Gorham-Stout disease.
INTRODUCTION:The distinction between lipoma and well-differentiated liposarcoma (WDLPS), or "atypical lipomatous tumor" (ALT), is crucial as it impacts patient management. A group of European experts led by Benjamin Moulin recently issued a consensus report to define the role of radiology in managing these lesions. It describes an algorithm defining the criteria prompting a diagnostic biopsy of deep lipomatous tumors of the limbs and chest wall. The primary aim of this study was to evaluate the algorithm's diagnostic performance.MATERIALS AND METHODS:Between 2012 and 2019, all biopsies of deep fatty tumors of the limbs or chest wall with a pre-biopsy MRI assessment were recorded at our institution. The MRI scans were reviewed by two radiologists. Each lesion was classified according to biopsy status by applying the algorithm of the European panel. The algorithm's diagnostic performance was assessed by calculating the sensitivity, specificity, positive predictive value and negative predictive value. Inter-rater agreement was also assessed.RESULTS:Of the 156 tumors in our study, 148 (94.9%) required a biopsy, and the algorithm's sensitivity for detecting ALT/WDLPS was 100% with specificity of 6.3% and a PPV of 20.3%. Inter-rater agreement was almost perfect with a kappa value of 0.882.CONCLUSION:The European algorithm demonstrates perfect sensitivity, an important criterion for a screening examination such as MRI in this setting. The algorithm's low specificity, however, emphasizes the need for further studies to redefine the optimum size cut-off value, especially for lesions without atypical criteria or an anatomical location at risk of post-surgical recurrence.
To determine whether high-resolution ultrasound (US) can identify the course and relations of the medial calcaneal nerve (MCN). This investigation was initially undertaken in eight cadaveric specimens and followed by a high-resolution US study in 20 healthy adult volunteers (40 nerves) by two musculoskeletal radiologists in consensus. The location and course of the MCN as well as its relationship to adjacent anatomical structures were evaluated. The MCN was consistently identified by US along its entire course. The mean cross-sectional area of the nerve was 1 mm2 (range 0.5–2). The level at which the MCN branched from the tibial nerve was variable, located a mean of 7 mm (range − 7–60) proximal to the tip of the medial malleolus. At the level of the medial retromalleolar fossa, the MCN was located inside the proximal tarsal tunnel a mean of 8 mm (range 0–16) posterior to the medial malleolus. More distally, the nerve was depicted in the subcutaneous tissue at the surface of the abductor hallucis fascia with a mean direct distance to the fascia of 1.5 mm (range 0.4–2.8). High-resolution US can identify the MCN at the level of the medial retromalleolar fossa, as well as more distally in the subcutaneous tissue at the surface of the abductor hallucis fascia. In the setting of heel pain, precise sonographic mapping of the MCN course may enable the radiologist to make diagnosis of nerve compression or neuroma, and perform selective US-guided treatments. In the setting of heel pain, sonography is an attractive tool for diagnosing compression neuropathy or neuroma of the medial calcaneal nerve, and enables the radiologist to perform selective image-guided treatments such as diagnostic blocks and injections. • The MCN is a small cutaneous nerve which rises from the tibial nerve in the medial retromalleolar fossa to the medial side of the heel. • The MCN can be depicted by high-resolution ultrasound along its entire course. • In the setting of heel pain, precise sonographic mapping of the MCN course may enable the radiologist to make diagnosis of neuroma or nerve entrapment, and perform selective ultrasound-guided treatments such as steroid injection or tarsal tunnel release.
Pseudomyogenic hemangioendothelioma (PMH) is a rare vascular tumor that occurs in young mostly male patients. Seventy percent of PMH cases are multifocal and 25% involve bones. PMH is an indolent tumor with mild local aggressiveness and an unclear pathology. Only two cases of spontaneous regressive bone PMH have been reported. Here, we report the case of a 17-year-old boy with a multifocal bone PMH diagnosed from a chronic pain in his left knee. The PMH affected the right scapula, both humeri, the right olecranon, the second metacarpal bone, the second and fourth right ribs, the thoracic and lumbar spine, the pelvic ring, the left and right femoral neck, and the left patella. Every lesion presented with a lobulated, lytic pattern, sometimes with a peripheral sclerotic rim. MRI showed a tissue lesion with a low intensity on T1-weighted sequences and high intensity on T2-weighted sequences. Enhancement of T1 gadolinium fat-saturated sequences was bright. After discussion, a national specialized board decided to actively monitor the patient and start general chemotherapy in the case of progression. The disease was stable at 3 and 6 months and showed signs of regression at 1 year, which was further confirmed at 2 years. CT scan and MRI highlighted a progressive filling of the tumor with cancellous bone and a regression of the tissue contingent. This case report highlights to a new therapeutic approach for indolent PMH that does not prevent further treatment in the case of progression.
To develop a deep-learning algorithm for anterior cruciate ligament (ACL) tear detection and to compare its accuracy using two external datasets. A database of 19,765 knee MRI scans (17,738 patients) issued from different manufacturers and magnetic fields was used to build a deep learning–based ACL tear detector. Fifteen percent showed partial or complete ACL rupture. Coronal and sagittal fat-suppressed proton density or T2-weighted sequences were used. A Natural Language Processing algorithm was used to automatically label reports associated with each MRI exam. We compared the accuracy of our model on two publicly available external datasets: MRNet, Bien et al, USA (PLoS Med 15:e1002699, 2018); and KneeMRI, Stajduhar et al, Croatia (Comput Methods Prog Biomed 140:151-164, 2017). Receptor operating characteristics (ROC) curves, area under the curve (AUC), sensitivity, specificity, and accuracy were used to evaluate our model. Our neural networks achieved an AUC value of 0.939 for detection of ACL tears, with a sensitivity of 87 • An algorithm for detecting anterior cruciate ligament ruptures was built from a large dataset of nearly 20,000 MRI with AUC values of 0.939, sensitivity of 87 • This algorithm was tested on two external populations from different other countries: a dataset from an American population and a dataset from a Croatian population. Performance remains high on these two external validation populations (AUC of 0.962 and 0.922 respectively).
OBJECTIVES:A recent treatment algorithm suggests that proximal anterior cruciate ligament (ACL) tears with good-to-excellent tissue quality are amenable to primary repair. Our primary objective was to assess the ability of MRI to determine the exact tear location and tissue quality, using arthroscopy as a reference standard.METHODS:In an initial sample of 71 patients with prior ACL surgery (repair or reconstruction), the diagnostic accuracy of MRI was assessed using arthroscopy as a reference standard. Each native ACL tear was graded according to Sherman's arthroscopic classifications during the surgical procedure. MRI scans were retrospectively reviewed for grading, blinded to arthroscopic findings and in consensus by two musculoskeletal radiologists. Tear location and tissue quality were graded using the MRI Sherman tear location (MSTL), MRI Sherman tissue quality (MSTQ) and simplified MRI Sherman tissue quality (S-MSTQ) classifications. Intra- and inter-observer agreement was assessed on a second sample of 77 patients. MRI classification accuracy was compared by McNemar's tests. Intra- and inter-observer agreement was assessed using Cohen's kappa coefficient.RESULTS:Regarding tear location, diagnostic accuracy was 70% (50/71) based on the MSTL classification. Diagnostic accuracy for tissue quality was 52% (15/29) based on the MSTQ classification and 90% (26/29) for the S-MSTQ classification (p = 0.003). Inter-observer agreement was good for MSTL (κ = 0.78) and moderate-to-good for the MSTQ and S-MSTQ classifications (κ = 0.44 and 0.63 respectively).CONCLUSIONS:MRI seems to be accurate in assessing tear location and tissue quality and may help clinicians to predict the reparability of ACL tears.KEY POINTS:• MRI seems to be accurate in assessing tear location and tissue quality and may help clinicians to predict the reparability of ACL tears. • High intra-observer agreement was demonstrated when grading the tear location into one of five types. • The diagnostic accuracy of the simplified MRI tissue quality classification, involving deletion of the ligament stump signal criterion, was better than that observed with the MRI Sherman tissue quality classification, but was moderate to good in terms of inter- and intra-observer agreement.
L’infiltration graisseuse de la coiffe des rotateurs (CR) est un des principaux facteurs influençant le devenir après réparation chirurgicale. Après avoir présenté la physiopathologie et l’histoire naturelle de l’infiltration graisseuse, nous rappellerons les principes et limites des mesures semi-quantitatives de l’infiltration graisseuse des muscles de la CR. Le concept de mesure quantitative par l’imagerie de déplacement chimique sera développé dans un second temps. Bien que les performances des méthodes semi-quantitatives telles que la classification de Goutallier (CG) aient dans le passé fait l’objet de controverses en termes de reproductibilité, elles restent largement utilisées pour juger du degré d’infiltration graisseuse des muscles de la CR. La méthode quantitative de type « Dixon » repose sur la différence de rotation existant entre les protons de l’eau et de la graisse. Appliquée sur une coupe sagittale oblique (coupe en « Y »), la quantification de la fraction graisseuse présente une excellente reproductibilité intra et interobservateur. Les applications cliniques se développent autour de la prédiction de la rupture itérative et du devenir fonctionnel post-chirurgical. Cependant, l’absence de correspondance claire entre les grades semi-quantitatifs de la CG et les mesures de fraction graisseuse d’une part et l’absence d’étude de haut niveau de preuve sur la relation entre la fraction graisseuse et le devenir fonctionnel après réparation chirurgicale d’autre part, limitent encore aujourd’hui la généralisation de cette méthode. La mesure de la fraction graisseuse permet une estimation reproductible de l’infiltration graisseuse des muscles de la CR. Bien que les fractions graisseuses préopératoires puissent être utilisées pour conseiller les patients sur les résultats attendus, des études cliniques complémentaires restent nécessaires pour préciser la place de cette technique récente. Fatty degeneration of rotator cuff is one of the main factors influencing the outcome after surgical. After presenting the physiopathology and natural history of fatty degeneration, we will recall the principles and limits of semi-quantitative fatty degeneration measurements of the RC muscles. The concept of quantitative measurement by chemical shift imaging will be developed in a second step. Although the performance of semi-quantitative methods, particularly the Goutallier classification (GC), has in the past been subject to controversy in terms of reproducibility, it is still widely used to assess the fatty degeneration of RC muscles.The Dixon method is a chemical shift imaging method using the in-phase/out-of-phase cycling of fat and water. Applied on an oblique coronal section (scapula Y-view), fat fraction shows excellent intra- and inter-observer reproducibility. Clinical applications include prediction of iterative rupture and post-surgical functional outcome. However, the lack of a significant correspondence between the GC and fatty degeneration, as well as the absence of a high level of evidence on the relationship between fatty degeneration and functional outcome after surgical repair, still limit a wider us of this technique today. The fat fraction allows a reproducible estimate of the fatty degeneration of the RC muscles. Although preoperative fat fraction can be used to advise patients on the expected results, further clinical studies are still needed to clarify the relevance of this technique.
L’anticorps anti-RANKL (denosumab) est utilisé depuis plusieurs années dans l’ostéoporose et en oncologie. Il est aussi maintenant fréquemment utilisé dans les pathologies tumorales osseuses, bénignes ou malignes, à des doses et à une fréquence plus importante que dans l’ostéoporose. Les protocoles de suivi post-traitement et les modalités d’arrêt dans ce sont moins définis que dans le cadre de l’ostéoporose. Nous avons suivi des patients ayant été traités par denosumab, soit en adjuvant de la chirurgie, soit comme seul traitement lorsque la chirurgie était trop risquée du fait de la localisation. Les indications étaient posées par le chirurgien et la RCP des tumeurs osseuses bénignes (TOB). Ils ont bénéficié d’un suivi rhumatologique, biologique (bilan phosphocalcique [calcium, phosphore, 25OH Vitamine D, PTH], du dosage de marqueurs de résorption [crosslaps]) et d’une mesure de densité minérale osseuse (DMO) à l’arrêt du traitement par denosumab et lors du suivi. Quatre patients (2 femmes et 2 hommes) ont été inclus entre 2020 et 2022. La moyenne d’âge à l’inclusion était de 21 ans (15–33). Les pathologies sous-jacentes étaient une tumeur à cellules géantes (TCG) et trois kyste osseux anévrysmal (KOA). Le protocole thérapeutique était d’une injection mensuelle de 120 mg en injection sous-cutanée après des doses rapprochées au cours du premier mois. La durée moyenne de traitement était de 21 mois (12–24), pour une moyenne de doses cumulées de 2015 mg (1320–2540). Trois des quatre patients avaient bénéficié d’une prise en charge chirurgicale préalable. Les bilans phosphocalciques à l’inclusion étaient normaux à part le dosage de 25OH vitamine D qui était diminué chez 3 patients sur 4 (moyenne de 26 ng/mL [22–30]). Les crosslaps sériques ont été dosés en moyenne 5 mois (2–7) après l’arrêt du traitement et étaient bas (moyenne = 0,16 [0,1–0,19]). Les DMO à l’inclusion étaient normales pour tous les patients. L’évolution des marqueurs de résorption a montré une augmentation en moyenne de 0,1 mg/L par mois à partir du 4e mois à la suite de l’arrêt du traitement par denosumab, avec une augmentation plus tardive, à partir du 7e mois chez le patient le plus âgé (33 ans). La moyenne des marqueurs de résorption avant bisphosphonates (BP) était de 1,072 mg/L (0,823–1,41). Augmentation de la calcémie jusqu’à 3,2 mmol/L chez la patiente la plus jeune (15 ans), ayant rendu nécessaire une hospitalisation en urgence. Les quatre patients ont eu un traitement par BP (acide zolédronique 5 mg IV) dans un délai moyen de 10 mois après la fin du traitement (6–15). Après une seule perfusion d’acide zolédronique 5 mg, aucun autre traitement n’a été nécessaire au cours d’un suivi moyen de 20 mois. Concernant l’évolution de la mesure de DMO après l’arrêt du denosumab, la perte osseuse entre l’inclusion et la mise en place du traitement par BP était très significative de −0,073 g/cm2 (0,053–0,108) au niveau lombaire, en revanche il n’y avait pas de perte au niveau du col fémoral ou à la hanche totale. Dans notre courte série, des patients ayant bénéficié d’un traitement par denosumab dans le cadre du traitement d’une TCG ou d’un KOA, ont présenté une augmentation du remodelage osseux avec une perte osseuse trabéculaire ou une hypercalcémie sévère dans un cas, après l’arrêt du traitement. Il paraît important de confirmer ces données sur une plus large cohorte de malades de manière multicentrique.
La sarcoïdose est une pathologie granulomateuse d’origine indéterminée touchant principalement les poumons. Le traitement de première ligne est la corticothérapie, qui peut être responsable d’une ostéoporose cortico-induite. L’objectif de cette étude était d’évaluer de façon rétrospective chez des patients atteints de sarcoïdose l’existence d’une ostéoporose (densitométrique ou fracturaire), notamment vertébrale. Parmi les 217 patients de l’étude SISTER (évaluation TDM des sacro-iliaques dans la sarcoïdose) suivis entre janvier 2011 et décembre 2017 pour une sarcoïdose au CHU de Rennes [1], 199 avaient eu un scanner thoraco-abdomino-pelvien (TAP) permettant l’évaluation du rachis. En plus des données déjà recueillies dans SISTER (manifestations cliniques de la sarcoïdose, explorations fonctionnelles respiratoires (EFR), biologie), les données sur les fractures ostéoporotiques (survenues avant ou après le scanner TAP), les densitométries osseuses, les facteurs de risque d’ostéoporose (toxiques, traitements, ostéoporose secondaire) ainsi que les traitements pharmacologiques anti-ostéoporotiques ont été recueillis de façon rétrospective. La recherche de fractures vertébrales sur les scanners TAP a été réalisée par double lecture et gradée selon la classification de Genant (0 à 3). Un consensus avec un troisième lecteur a été réalisé en cas de discordance. Parmi les 199 patients suivis en moyenne 64,5 ± 61,9 mois, une ostéoporose fracturaire était signalée dans les comptes-rendus chez respectivement 17 (8,5 %) et 9 (4,5 %) patients. En tout, 59 DMO ont été réalisées. Il s’agissait surtout de fractures vertébrales pour 11 patients avec en moyenne 3,3 fractures par patient. Six patients avaient d’autres fractures sévères. La relecture des scanners TAP a permis l’identification de 27 fractures (14 lombaires et 13 thoraciques) chez 15 patients, classées selon Genant de grade 1, 2 et 3 dans 41 %, 37 % et 22 % des cas et étaient méconnues chez 9 patients. L’ostéoporose fracturaire et/ou densitométrique représentait au total 12,6 % des patients. La présence d’une ostéoporose fracturaire et/ou densitométrique était associée de façon significative à un âge plus élevé au diagnostic de la sarcoïdose et au moment de l’instauration de la corticothérapie, à une clairance de la créatinine et une capacité pulmonaire totale et une diffusion pulmonaire du CO (% théorique) plus basses, à une lymphopénie ainsi qu’au nombre de facteurs de risques d’ostéoporose. Pour les 101 patients avec une corticothérapie active suivis à partir d’octobre 2014, date des recommandations françaises sur l’ostéoporose cortico-induite [2], un traitement pharmacologique anti-ostéoporotique était indiqué chez 53,1 % patients mais prescrit chez seulement 32,7 % d’entre eux. Pour les 46,9 % des patients restants ayant reçu une corticothérapie après 2014, une DMO a été réalisée dans 53,1 % des cas et traitement anti-ostéoporotique prescrit dans 22,4 % des cas. Au total, un traitement anti-ostéoporotique a été instauré chez 33 patients pour traiter une ostéoporose fracturaire (33,3 %), une ostéoporose densitométrique non fracturaire (12,4 %) ou pour prévenir l’ostéoporose cortico-induite (54,5 %). Les traitements débutés étaient un bisphosphonate per os (72,7 %) ou IV (28 %) et parfois du tériparatide (16,7 %). L’ostéoporose dans la sarcoïdose n’est pas rare (12,6 %). Sa prise en charge nécessite une évaluation clinique et/ou densitométrique. La prévention de l’ostéoporose cortico-induite, l’évaluation densitométrique et l’instauration d’un traitement pharmacologique sont insuffisantes.
Sacroiliitis and spondyloarthritis (SpA) have been associated to sarcoidosis. Sarcoidosis bone involvement of the sacral or iliac bones has been reported to mimic SpA. We aimed to evaluate the prevalence of structural sacroiliitis and structural changes of the sacroiliac joints (SIJ) in patients with sarcoidosis by abdominal-pelvic computed tomography (AP-CT). In this monocentric retrospective study, three blinded readers evaluated AP-CT that had already been performed on patients with sarcoidosis and classified them as normal, degenerative, or inflammatory. A consensus was reached for the divergent cases. Erosion, ankylosis, and sclerosis, classically associated with sacroiliitis, were noted. SpA was defined according to the ASAS 2009 classification criteria. We identified 217 patients with proven sarcoidosis who underwent AP-CT. Only three patients had sacroiliitis by CT and four had SpA, representing 1.38% and 1.85% of the patients, respectively. Degenerative SIJs represented 28.1% of patients and were significantly associated with age, at least one pregnancy, rural lifestyle, ankylosis, diffuse idiopathic skeletal hyperostosis, sclerosis, and the presence of osteophytes. Four patients had axial bone sarcoidosis. Sacroiliitis, SpA, and degenerative changes of the SIJ have been highlighted by AP-CT in patients with sarcoidosis. Osteoarthritis of the SIJ in sarcoidosis was associated with age, pregnancy, and rural lifestyle. Further studies are needed to assess the link between SpA and sarcoidosis.
•La spondyloarthrite et la sacro-iliite sont régulièrement associées à la sarcoïdose, en particulier en présence de lombalgie inflammatoire.•Une atteinte granulomateuse des os pelviens mimant une sacro-iliite peut survenir dans la sarcoïdose ; la présence de changements unilatéraux sur la radiographie suggère une atteinte granulomateuse.•L’existence d’une arthropathie de l’articulation sacro-iliaque associée à la sarcoïdose reste sujette à discussion.•Les modalités d’imagerie utilisées pour évaluer l’articulation sacro-iliaque dans la sarcoïdose sont encore hétérogènes.
The present work is aimed studying the visibility and position of the vasto-adductor membrane with ultrasonography and demonstrating that injection performed under this membrane allows to infiltrate the saphenous nerve. It was analyzed with ultrasonography in four cadaveric subjects and in 13 volunteers. This membrane was clearly visible and methylene blue was located underneath it after injection in all cadaveric subjects. This study demonstrates that it can be used as a reliable anatomic landmark when performing an injection for both regional anesthesia and in the treatment of saphenous nerve tunnel syndrome.
