SummaryRemimazolam, a short‐acting benzodiazepine, may be used for induction and maintenance of total intravenous anaesthesia, but its role in the management of patients with multiple comorbidities remains unclear. In this phase 3 randomised controlled trial, we compared the anaesthetic efficacy and the incidence of postinduction hypotension during total intravenous anaesthesia with remimazolam vs. propofol. A total of 365 patients (ASA physical status 3 or 4) scheduled for elective surgery were assigned randomly to receive total intravenous anaesthesia with remimazolam (n = 270) or propofol (n = 95). Primary outcome was anaesthetic effect, quantified as the percentage of time with Narcotrend® Index values ≤ 60, during surgery (skin incision to last skin suture), with a non‐inferiority margin of ‐10%. Secondary outcome was the incidence of postinduction hypotensive events. Mean (SD) percentage of time with Narcotrend Index values ≤ 60 during surgery across all patients receiving remimazolam (93% (20.7)) was non‐inferior to propofol (99% (4.2)), mean difference (97.5%CI) ‐6.28% (‐8.89–infinite); p = 0.003. Mean (SD) number of postinduction hypotension events was 62 (38.1) and 71 (41.1) for patients allocated to the remimazolam and propofol groups, respectively; p = 0.015. Noradrenaline administration events (requirement for a bolus and/or infusion) were also lower in patients allocated to remimazolam compared with propofol (14 (13.5) vs. 20 (14.6), respectively; p < 0.001). In conclusion, in patients who were ASA physical status 3 or 4, the anaesthetic effect of remimazolam was non‐inferior to propofol.
Zusammenfassung Hintergrund Die umfassende Narkosevorbereitung mittels Anamnese und körperlicher Untersuchung gilt als essenzieller Bestandteil der Qualitätskriterien für eine Narkose. Allerdings ist eine Narkosevorbereitung in der Klinik häufig mit langen Wartezeiten in der Anästhesie-Ambulanz verbunden. Zudem sind regelhaft Wiedervorstellungen aufgrund von fehlenden oder noch ausstehenden Befunden erforderlich. Besonders im Rahmen der COVID-19-Pandemie schien die Implementierung von Telemedizin im präoperativen Setting der Anästhesiologie vielversprechend und sinnvoll. Ziel der Arbeit Diese vergleichende Querschnittstudie soll über eine Patientenbefragung aufzeigen, für welche Patientengruppen eine telemedizinische Narkosevorbereitung geeignet ist, und welche technischen Rahmenbedingungen auf Patientenseite vorhanden sind. Material und Methoden Hierzu wurden anhand eines Fragebogens insgesamt 2080 Patienten (1030 vor, 1050 während der Pandemie) befragt. Für „matched-pairs“-Analysen (Paaranalysen) wurden 630 Paare entsprechend ihres Alters und Geschlechts gebildet. Ergebnisse Während der Pandemie nutzten deutlich mehr Patienten die Möglichkeit der Videotelefonie in ihrem Alltag (30,4 % vs. 41,8 %). Vor der Pandemie bewerteten 31,7 % der Patienten die Videotelefonie als praktische und geeignete Methode für ein Aufklärungsgespräch. Nach der Pandemie stieg diese Zahl der Patienten, die gegenüber einer Videotelefonie aufgeschlossenen sind, auf 46,6 % an. Für die Mehrheit der Patienten war der persönliche Kontakt zu einem Anästhesisten vor Ort wichtig (80,7 % vor vs. 67,4 % nach der Pandemie). Die Zahl der Patienten, die über die notwendige technische Ausstattung für eine Videokommunikation verfügten, stieg ebenfalls infolge der COVID-19-Pandemie (50,4 % vs. 58,2 %). Diskussion Fast die Hälfte der Patienten scheint bereits heute einer telemedizinischen Narkoseaufklärung offen gegenüberzustehen. Es ist davon auszugehen, dass in Zukunft auch ältere Menschen zunehmend die für eine telemedizinische Narkosevorbereitung erforderliche Technik sowie die erforderliche technische Kompetenz besitzen werden. Bei der Implementierung einer telemedizinischen Narkosevorbereitung sollte die Nutzerakzeptanz zentrales Ziel der Konzeptentwicklung sein. Randomisierte kontrollierte Studien können die Potenziale belegen und mögliche Probleme aufzeigen.
Background Comprehensive anesthesia preparation by means of the anamnesis and physical examination is considered an essential part of the quality criteria for anesthesia. Especially due to the shortage of specialists, there are usually long waiting times in anesthesia outpatient departments and patients must frequently return in cases of missing or pending findings. Telemedicine already offers alternatives in the context of video communication. These alternatives are now particularly prominent due to the currently existing COVID-19 pandemic and the resulting recommendations for digitalization. Objectives This comparative cross-sectional study was carried out to show via a patient survey which patient groups are suitable for a telemedical anesthesia preparation and whether the patients are already technically sufficiently equipped. Material and methods For this purpose, a total of 2080 patients (1030 before and 1050 during the pandemic) were interviewed using a questionnaire. For matched paired analyses, 630 pairs were formed according to their age and gender. Results Before and after the pandemic, there was an increase in the percentage of patients already using video communication in their daily lives (30.4% vs. 41.8%). Before the pandemic, 31.7% of patients indicated that they considered this concept of communication to be a practical and appropriate method for an educational conversation and after the pandemic this number increased to 46.6%. For the majority of patients personal contact with a local anesthesiologist was important (80.7% before vs. 67.4% during the pandemic). The number of patients who had the necessary technical equipment for video communication also increased as a result of the COVID-19 pandemic (50.4% vs. 58.2%). Discussion Almost half of the patients already seem to be open to a telemedical preoperative evaluation. As digitalization progresses, older generations are more likely to recognize the benefits and be able to own and use the necessary technology in the near future. User acceptance should be the central goal of concept development. This must be followed by a randomized controlled study to evaluate the potentials but also the problems in the perioperative process.
Phaeochromocytomas and paragangliomas are catecholamine-producing neu roendocrine tumours arising from chromaffin cells of the adrenal medulla or extraadrenal paraganglia. The phaeochromocytoma belongs to the rare tumours and as such occurs in less than 0.1 % of the general population. Atypical presentation or lack of clinical signs can lead to some pheochromocytomas being misdiagnosed as other abdominal tumours or being accidentally discovered. Due to the potential for life-threatening surgical and anaesthesiological complications in patients with phaeochromocytoma not treated preoperatively with alpha-blockers, this tumour type should be included in the differential diagnosis of abdominal tumours of unknown origin. The authors present a rare case of intraoperative hypertension in a 37-year-old male patient during emergency spine stabilisation surgery. Spine stabilisation had to be done because of a sudden onset of paraplegia due to a pathological spinefracture caused by spinal metastases of an unknown tumour. After excluding more common causes of hypertension such as pain, inadequate depth of anaesthesia, a dislocation of the IV lines and even malignant hyperthermia, we came to conclusion that, considering the preoperative CT and MRT-scans, the presumed liver tumour was in fact a gigantic phaeochromocytoma and that the metastases are active. The diagnosis of phaeochromocytoma was confirmed with urinary catecholamine panel and MRI on the first postoperative day.Due to unspecific clinical presentation and large amount of "silent" phaeochromocytomas, the main problem is to anticipate it in the first place. Once the phaeochromocytoma has been anti-cipated, intraoperative management should then follow the same principles that apply to elective cases. A step-by-step approach and eliminating more common conditions, may leave us with a "zebra" - a common presentation of an uncommon disease.
Background: Inhaled nitric oxide (iNO) is a well-established off-label treatment of acute hypoxic respiratory failure and pulmonary hypertension, but high drug cost and lack of consistent proof of clinical benefit have led to restrictive expert recommendations regarding its use. Objectives: To evaluate the current practice of the use of iNO in different hospital settings at an international society level. Design & Setting: Web-based survey distributed via email to ESAIC and EACTAIC members. Methods: Survey responses are depicted as absolute frequencies and percentages that were analysed using Microsoft Excel. Results: One third of the respondents had institutional guidelines for the use of iNO. Pulmonary arterial hypertension, right ventricular failure, persistent pulmonary hypertension of the newborn and adult respiratory distress syndrome were the main indications for treatment with iNO. Prophylactic use of iNO during heart and lung transplantation or VAD implantation surgery was reported by 12-34%. The most frequently reported doses were 10-20 ppm and 20-40 ppm as initial and maximum treatment doses, respectively. Echocardiography was the most universally used form of advanced hemodynamic monitoring during treatment with iNO, followed by pulmonary artery catheterization. Half of the respondents had a fixed strategy to prevent rebound pulmonary hypertension during weaning from iNO, using phosphodiesterase inhibitors, prostacyclins or calcium channel antagonists. Conclusion: In line with the available evidence and expert recommendations, iNO remains a rescue treatment reserved for the most severe cases in highly specialized centres. The observations made in this survey should inspire future research to help better define the role of iNO, also in the setting of an ARDS-pandemic and the emergence of alternative selective pulmonary vasodilators.
Background Resuscitation using a percutaneous mechanical circulatory support device (iCPR) improves survival after cardiac arrest (CA). We hypothesized that the addition of inhaled nitric oxide (iNO) during iCPR might prove synergistic, leading to improved myocardial performance due to lowering of right ventricular (RV) afterload, left ventricular (LV) preload, and myocardial energetics. This study aimed to characterize the changes in LV and RV function and global myocardial work indices (GWI) following iCPR, both with and without iNO, using 2-D transesophageal echocardiography (TEE) and GWI evaluation as a novel non-invasive measurement. Methods In 10 pigs, iCPR was initiated following electrically-induced CA and 10 min of untreated ventricular fibrillation (VF). Pigs were randomized to either 20 ppm ( 20 ppm , n = 5) or 0 ppm ( 0 ppm , n = 5) of iNO in addition to therapeutic hypothermia for 5 h following ROSC. All animals received TEE at five pre-specified time-points and invasive hemodynamic monitoring. Results LV end-diastolic volume (LVEDV) increased significantly in both groups following CA. iCPR alone led to significant LV unloading at 5 h post-ROSC with LVEDV values reaching baseline values in both groups ( 20 ppm: 68.2 ± 2.7 vs. 70.8 ± 6.1 mL, p = 0.486; 0 ppm : 70.8 ± 1.3 vs. 72.3 ± 4.2 mL, p = 0.813, respectively). LV global longitudinal strain (GLS) increased in both groups following CA. LV-GLS recovered significantly better in the 20 ppm group at 5 h post-ROSC ( 20 ppm : − 18 ± 3% vs. 0 ppm : − 13 ± 2%, p = 0.025). LV-GWI decreased in both groups after CA with no difference between the groups. Within 0 ppm group, LV-GWI decreased significantly at 5 h post-ROSC compared to baseline (1,125 ± 214 vs. 1,835 ± 305 mmHg%, p = 0.011). RV-GWI was higher in the 20 ppm group at 3 h and 5 h post-ROSC ( 20 ppm : 189 ± 43 vs. 0 ppm : 108 ± 22 mmHg%, p = 0.049 and 20 ppm : 261 ± 54 vs. 0 ppm : 152 ± 42 mmHg%, p = 0.041). The blood flow calculated by the Impella controller following iCPR initiation correlated well with the pulsed-wave Doppler (PWD) derived pulmonary flow (PWD vs. controller: 1.8 ± 0.2 vs. 1.9 ± 0.2L/min, r = 0.85, p = 0.012). Conclusions iCPR after CA provided sufficient unloading and preservation of the LV systolic function by improving LV-GWI recovery. The addition of iNO to iCPR enabled better preservation of the RV-function as determined by better RV-GWI. Additionally, Impella-derived flow provided an accurate measure of total flow during iCPR.
Zusammenfassung Die Telemedizin ist in einigen Regionen Deutschlands bereits fester Bestandteil des Rettungsdienstes. Dieser Fallbericht handelt von einem Telenotarzteinsatz, bei dem die initiale Telekonsultation durch eine an der Einsatzstelle befindliche Notärztin erfolgte. Der Patient hatte eine lebensbedrohliche ventrikuläre Tachykardie mit zunehmender Kreislaufinstabilität. Dies stellte für die noch unerfahrene Notärztin eine extrem herausfordernde Einsatzsituation dar. Sie entschied sich, telemedizinische Unterstützung in Anspruch zu nehmen. So konnte sie angeleitet werden, bei dem instabilen Patienten einen i.o.-Zugang, eine medikamentöse Therapie und eine elektrische Kardioversion durchzuführen. Die Zusammenarbeit mit dem Telenotarzt ermöglichte der noch unerfahrenen Notärztin, eine leitlinienkonforme Therapie durchzuführen sowie den Patienten zeitnah stabilisiert ins Krankenhaus zu transportieren.
The American Society of Anesthesiologists (ASA) classification is renowned and used in many clinical and scientific contexts as a system for risk classification of surgical patients. The updated version of the ASA classification attempts to facilitate the correct application of the ASA categories for users across all disciplines by providing approved examples of diseases and acute conditions. The addition of speci.c groups of patients to the classi.cation provides the user with further guidance.
Telemedicine has already entered the rescue service in some regions of Germany. This case description is about a telemedical emergency physician case where an emergency doctor was also at the scene of the emergency. The patient had a life-threatening ventricular tachycardia and became hemodynamically unstable. The emergency physician was still inexperienced and overwhelmed by the complex situation. She decided to contact the tele-emergency medical services (tele-ems) and could then be instructed to perform intraosseous access, drug treatment and electrical cardioversion in the unstable patient. The cooperation with the tele-ems physician enabled the still inexperienced emergency physician to perform a guideline-compliant treatment and to transport the stabilized patient to the hospital in a timely manner.
BACKGROUND:In 2016 the German Society of Anesthesiology and Intensive Care Medicine (DGAI) and the Association of German Anesthetists (BDA) published 10 quality indicators (QI) to compare and improve the quality of anesthesia care in Germany. So far, there is no evidence for the feasibility of implementation of these QI in hospitals.OBJECTIVE:This study tested the hypothesis that the implementation of the 10 QI is feasible in German hospitals.MATERIAL AND METHODS:This prospective three-phase national multicenter quality improvement study was conducted in 15 German hospitals and 1 outpatient anesthesia center from March 2017 to February 2018. The trial consisted of an initial evaluation of pre-existing structures and processes by the heads of the participating anesthesia departments, followed by a 6-month implementation phase of the QI as well as a final re-evaluation phase. The implementation procedure was supported by web-based implementation aids ( www.qi-an.org ) and internal quality management programs. The primary endpoint was the difference in the number of implemented QI per center before and after implementation. Secondary endpoints were the number of newly implemented QI per center, the overall number of successful implementations of each QI, the identification of problems during the implementation as well as the kind of impediments preventing the QI implementation.RESULTS:The average number of implemented QI increased from 5.8 to 6.8 (mean of the differences 1.1 ± 1.3; P < 0.01). Most frequently the QI perioperative morbidity and mortality report (5 centers) and the QI temperature management (4 centers) could be implemented. After the implementation phase, the QI incidence management and patient blood management were implemented in all 16 centers. Implementation of other quality indicators failed mainly due to a lack of time and lack of structural resources.CONCLUSION:In this study the implementation of QI was proven to be mostly feasible in the participating German hospitals. Although several QI could be implemented with minor effort, more time, financial and structural resources would be required for some QI, such as the QI postoperative visit.
Sepsis is an acute life-threatening organ dysfunction resulting from an inadequate host response to an infection. The diagnosis of sepsis-associated organ dysfunction should be based on a change of >= 2 points using the Sequential Organ Failure Assessment (SOFA) score. An intravenous crystalloid solution should be administered within the first 3 hours to achieve a haemodynamic stabilisation of the patient. A 0.9% NaCl solution should NOT be used. A schematic minimum infusion amount and a general target value for the mean arterial blood pressure are not recommended. Intravenous anti-infectives should be administered as soon as possible if possible within 1 hour after confirmed diagnosis. The application of an empirical broad-spectrum antibiotic or multiple antibiotics is recommended in order to account for all significant bacteria. Measurements of the procalcitonin value should be done to shorten the duration of antimicrobial therapy.
Der bundeseinheitliche Notarztindikationskatalog bildet die Grundlage der Notarztdisposition, bisher ohne Verbesserungen des Rettungsdienstes zu berücksichtigen. Durch Weiterentwicklungen wie die Einführung des Notfallsanitätergesetzes, die Implementierung von Telemedizin sowie eine deutlich erweiterte Ausstattung der Rettungswagen (RTW) wird dem Rettungsdienst zunehmend eine gestufte Notfallbehandlung ermöglicht. Daraus resultiert die Frage, wie oft ein Notarzt heutzutage erforderlich ist und ob dieser ressourcenschonender eingesetzt werden kann. Ziel dieser Untersuchung ist eine Ersteinschätzung zum Erfordernis der Notärzte an der Einsatzstelle, basierend auf der Selbsteinschätzung der Notärzte. Die Erfordernis eines Notarztes an der Einsatzstelle, Verdachtsdiagnosen und Medikamentengaben wurden anhand einer Befragung von Notärzten im Rettungsdienst der Stadt Aachen sowie Auswertung der Notarztprotokolle im Zeitraum vom 01.12.2017 bis zum 28.02.2018 überprüft. Daneben wurden auch Konsultationen des Telenotarztes durch einen RTW hinsichtlich Verdachtsdiagnosen und Medikamentengaben ausgewertet. Bei insgesamt 6851 erfassten Einsätzen wurden 20,6 % (n = 1410) durch den bodengebundenen Notarzt und 10,5 % (n = 721) durch den Telenotarzt unterstützt. Die Notärzte hielten sich bei 46,7 % (n = 654) aller Notarzteinsätze für nicht erforderlich, waren jedoch im Mittel 25 min vor Ort an der Einsatzstelle gebunden. Der häufigste Grund für das Erfordernis des Notarztes an der Einsatzstelle waren in 29,8 % (n = 225) vital instabile Patienten. In 11 % der rettungsdienstlichen Einsätze war der Notarzt aus seiner Sicht erforderlich. Eine klare Indikationsstellung kann erreichen, dass der Notarzt aufgrund seiner höheren Verfügbarkeit zielgerichteter eingesetzt werden kann. Ein notwendiger Arztkontakt konnte in mehr als 10,5 % der rettungsdienstlichen Einsätze mit Unterstützung durch einen Telenotarzt realisiert werden.
Zusammenfassung Hintergrund Im Jahr 2016 hat die Deutsche Gesellschaft für Anästhesiologie und Intensivmedizin (DGAI) und der Bund Deutscher Anästhesisten (BDA) 10 Qualitätsindikatoren (QI) veröffentlicht, um die Qualität anästhesiologischer Behandlungen vergleichen und verbessern zu können. Bis heute gibt es keine Evidenz darüber, inwieweit sich die Maßnahmen zur Implementierung der QI in den Kliniken umsetzen lassen. Ziel der Arbeit Im Rahmen dieser Studie wurde die Hypothese getestet, inwieweit die vorliegenden 10 QI in deutschen Krankenhäusern implementierbar sind. Material und Methoden Diese prospektive, dreiphasige, nationale, Multizenterstudie wurde in 15 deutschen Krankenhäusern und einem ambulanten Anästhesie-Zentrum von März 2017 bis Februar 2018 durchgeführt. Die Studie bestand aus einer initialen Evaluation präexistierender Strukturen und Prozesse durch die Chefärzte der beteiligten Anästhesieabteilungen, gefolgt von einer sechsmonatigen Implementierungsphase der QI und einer finalen Reevaluation. Der Prozess der Implementierung wurde durch web-basierte Implementierungshilfen ( www.qi-an.org ) und interne Qualitätsmanagementprogramme der Zentren unterstützt. Der primäre Endpunkt war die Differenz der Anzahl implementierter QI pro Zentrum vor und nach der sechsmonatigen Implementierungsphase. Sekundäre Endpunkte waren die Anzahl neu implementierter QI pro Zentrum, die Gesamtzahl erfolgreicher Implementierungen pro einzelnem QI, die Identifizierung von Problemen während der Implementierung sowie die Gründe, die die Implementierung eines QI verhindert haben. Ergebnisse Die durchschnittliche Anzahl implementierter QI pro Zentrum stieg von 5,8 auf 6,8 (Mittelwert der Differenzen 1,1 ± 1,3; p < 0,01). Am häufigsten konnten die QI Perioperativer Morbiditäts- und Mortalitätsbericht (5 Zentren) und QI Temperatur-Management (4 Zentren) implementiert werden. Nach der Implementierungsphase waren die QI Fehlermanagement und Patient-Blood-Management in allen Zentren implementiert. Die Implementierung anderer QI scheiterte in den häufigsten Fällen an Zeitmangel sowie fehlenden personellen und strukturellen Ressourcen. Diskussion Im Rahmen der Studie konnte die Implementierbarkeit der vorliegenden 10 QI in deutschen Krankenhäusern überwiegend belegt werden. Viele der QI konnten mit relativ geringem Aufwand implementiert werden. Für einige QI wären jedoch mehr Personal sowie größere strukturelle und finanzielle Ressourcen notwendig, so z.B. für den QI Postoperative Visite.
