In life-threatening diseases such as cancer, frequent side effects, changes in the disease process and major surgical interventions during the diagnosis and treatment process may cause patients to experience various difficulties in the adaptation process and this may trigger the emergence of mental problems. In this context, it is important to determine the personality traits of individuals in order to better understand their level of adaptation to the disease and coping strategies in studies conducted to prevent mental problems. In this study, it was aimed to determine the personality traits of patients with breast cancer by using descriptive research method, since it is thought to have an important contribution to the diagnosis and treatment of patients. The sample of the study consisted of 86 breast cancer patients by non-probability random sampling method. A Personal Information Form and Multiphasic Personality Inventory (Minnesota Multiphasic Personality Inventory - MMPI) were used as data collection tools. L (Lie), F (frequency-infrequency), Pd (psychopathic deviation), and Ma (hypomania) subscale mean scores of they significantly differed by the age of the participants, perceived family income status, and the presence of a prior breast cancer diagnosis (p<0.05). Patients without a history of breast cancer responded to scale items more randomly than those with a history did. Patients without prior breast cancer had more difficulty complying with society rules and values than patients with prior breast cancer. Determining the "personality structure" of patients with breast cancer will significantly contribute to planning interventions that provide benefits for the adaptation and spiritual strengthening process of patients for all health professionals.
Background Living liver donors are known to experience many physical symptoms such as pain in the postoperative period. Aim This qualitative study was conducted to examine pain experiences and coping methods of living liver donors experiencing postoperative chronic pain. Methods The study sample consisted of 16 living liver donors. Data were collected using a personal information form and a semi-structured questionnaire. Quantitative data were analyzed as number, mean, standard deviation, and percentage. Qualitative data were analyzed within the framework of thematic and content analysis methods. Results The mean postoperative chronic pain rate of living liver donors was 4.5. Eight of these people reported experiencing sudden, pinprick-like pain after liver transplantation. Living liver donors stated that the greatest factor that increased their pain was exertion/spontaneous movement. Half of them stated that the pain they experienced after transplantation had no impact on their daily life activities. Most of the living liver donors stated that the pain they experienced had no impact on their social lives; however, it caused sadness and psychological deterioration. The most preferred method to cope with pain was the use of medication. Conclusions Living liver donors experienced moderate postoperative chronic pain, and a significant proportion of them reported psychological symptoms related to the pain they experienced. A multidisciplinary pain team can help with pain management by providing training conducted in transplant centers.
Kanser gibi yaşamı tehdit eden hastalıklarda, tanı aşamasında, tedavi sürecinde sık karşılaşılan yan etkiler, hastalık sürecindeki değişimler ve gerçekleştirilen majör cerrahi müdahaleler, hastaların uyum sürecinde çeşitli zorluklar yaşamalarına neden olabilmekte ve bu durum ruhsal sorunların ortaya çıkışını tetikleyebilmektedir. Bu bağlamda, ruhsal sorunların önlenmesi amacıyla yapılan çalışmalarda, bireylerin hastalığa uyum düzeylerini ve baş etme stratejilerini daha iyi anlamak için sahip oldukları kişilik özelliklerinin belirlenmesi önem taşımaktadır. Bu araştırmada hastaların tanı ve tedavi aşamasında önemli katkısı olacağı düşünüldüğünden, betimsel araştırma yöntemi kullanılarak meme kanserli hastaların kişilik özelliklerinin belirlenmesi amaçlanmıştır. Araştırmanın örneklemini olasılıksız rastlantısal örnekleme yöntemi ile 86 meme kanserli hasta oluşturmaktadır. Veri toplama araçları olarak Kişisel Bilgi Formu ve MMPI Çok Yönlü Kişilik Envanteri (Minnesota Multiphasic Personality Inventory - MMPI) kullanılmıştır. Araştırmaya katılan hastalarda L (yalan), F (sıklık-nadirlik), Pd (Psikopatik Sapma) ve Ma (Hipomani) alt ölçek ortalama puanlarının, katılımcıların yaşlarına, algılanan aile gelir durumuna ve eski meme probleminin varlığına göre anlamlı düzeyde farklılaştığı görülmektedir (p<0.05). Daha önce meme problemi yaşamayan hastaların yaşayan hastalara göre ölçek maddelerini daha fazla gelişigüzel şekilde yanıtladığı görülmüştür. Daha önce meme problemi yaşamayan hastaların yaşayan hastalara göre daha fazla toplumsal kurallara ve değerlere uymada güçlük yaşadığı bulunmuştur. Meme kanseri tanısı alan hastalarda, bireye özgü bir olgu olan “kişilik yapısının” belirlenmesinin, başta hemşireler olmak üzere, bütüncül bakım sunan tüm sağlık profesyonellerine hastaların uyum süreçlerini destekleyici ve ruhsal açıdan güçlenmelerine katkı sağlayacak müdahalelerin planlanmasında önemli bir rehberlik sağlayacağı düşünülmektedir.
Background: Currently, palliative care plays an important role in increasingly aging populations and their long-term care needs. Surgical palliative care is important for improving quality of life. This study aimed to determine the knowledge level of nurses working in surgical clinics about palliative surgery. Method: This research was conducted as a descriptive study. We included 182 nurses working in surgical clinics who agreed to participate and met the inclusion criteria for participation in the study. The Personal Information and Palliative Surgery Information Forms were used to collect data. Data were collected between November 30, and December 15, 2022. The analysis of the data obtained was performed using the Statistical Program in Social Sciences (SPSS), Version 25. The level of significance was set at P < .05. Results: Nurses referred to palliative care as end-of-life care, 73.1% did not care for palliative care patients, and 84.6% did not receive training in palliative care. Surgical clinic nurses were found to have "moderate knowledge about palliative surgery. A statistically significant difference was found between the average palliative surgery knowledge of surgical clinic nurses and those trained in education and palliative care (P < .05). Conclusions: The results of the study showed that surgical clinic nurses had moderate knowledge about palliative surgery. Moreover, the nurses who have received training in palliative care and had high level of education exhibited a higher level of knowledge.
Aim: Postoperative ileus is a common complication that increases the length of hospital stay and causes morbidity and mortality. In this study, palliative care needs of a patient, who had ileus in the late period after abdominal surgery and therefore had a colostomy, are discussed. Case report: In this 90 years old/male patient, N.Ö., colostomy was opened due to ileus formation in the late period after abdominal surgery. The patient, who was hospitalized in the intensive care unit for 27 days, was followed up at home after discharge. The pressure ulcer that developed in the intensive care unit has become a pressure ulcer that cannot be staged at home. The patient, whose general condition was impaired, was admitted to the palliative care service on 27.07.2022. The patient was admitted to the unit as confused with contractures in his arms and legs, pain, malabsorption, and fluid-electrolyte imbalance. It was determined that he had acidosis and secretion-related respiratory problems. As a result of the applied nursing interventions, oral R 1-2 was started, the pressure ulcer regressed to stage 2, his appetite and oxygen saturation increased, and his pain and secretions decreased. However, no reduction in contractures has been observed and the risks associated with providing/maintaining a safe environment remain. Inferences: As a result of the care and follow-up given to the patient, it was determined that there was a significant decrease in the symptoms observed when the patient was admitted to the clinic. In the content of the discharge training prepared in line with the nursing care plans to inform the patients and their relatives, it is important to include information about the causes, signs, and symptoms of ileus.
Outbreaks caused by environmental effects such as COVID-19, MERS, SARS, or Ebola cause problems such as fear, anxiety, loneliness, depression, and hopelessness for people with or without a high risk of illness. This study aims to determine the levels of hopelessness and anxiety experienced by nursing students during the COVID-19 outbreak. This descriptive and cross-sectional study was conducted with students of a state university. In the collection of the data, Descriptive Characteristics Form prepared by the researchers, the Generalized Anxiety Disorder Test (GAD-7), and the Beck Hopelessness Scale (BHS) were used. The mean GAD-7 score of the students participating in the study was 6.65 ± 5.03 and the mean BHS score was 6.11 ± 4.91. It was determined that the score of GAD-7 and BHS were a significant positive high relationship between them (p < 0.001). Anxiety and hopelessness levels were found to be statistically significantly higher for those whose sleep and eating patterns affected negatively due to the pandemic, those who feared the pandemic and its economic effects, and those who did not want to work in the hospital during the pandemic period. There was a relationship between anxiety and hopelessness levels of nursing students participating in the study.
Hopelessness and fatigue are common symptoms experienced by breast cancer patients in the post-operative period. Supportive interventions, such as art making or art therapy, are reported to have a positive effect on the symptoms experienced by cancer patients. This semi experimental study aims to evaluate the effect of painting on post-operative hopelessness and fatigue in breast cancer patients. The population of the study consisted of all adult patients who had surgery for breast cancer in a university hospital in Turkey and lived in the city center. The sample of the study consisted of 110 patients, 55 in the experimental group and 55 in the control group. The sample included patients who were selected through the improbable random sampling method. The research was completed between April 2016 and January 2018. Patient Information Form, Piper Fatigue Scale and Beck Hopelessness Scale were used to obtain data. Percentage, mean, standard deviation, chi-square, independent group’s t test, paired t test and correlation were used in the analysis of the data. It was observed that the hopelessness and fatigue scores of the patients in the experimental group decreased statistically significantly after the painting study (p=0.001). No statistically significant change was observed in the hopelessness and fatigue scores of the patients in the control group. In this context, it is thought that nurses should know the effect of painting on hopelessness and fatigue and include it in their practices. It may be recommended to conduct new studies in order to evaluate the effects of painting on different patient groups.
Background: Although the port catheters have many beneficial effects on the patient's quality of life, it was reported in the literature that patients experience pain in the incision area after the implantation or anxiety before, during or, after the implantation.Distraction is a simple and effective method in the management of pain and anxiety. Aims: To determine the effect of virtual reality (VR) distraction intervention on pain, anxiety, and vital signs of oncology patients undergoing port catheter implantation. Design: Randomized controlled study Methods: The study was carried out with 139 participants (69 intervention, 70 control) between September 2019 and January 2020. Data were collected before, during, and after the implantation by using a patient identification form, State Anxiety Inventory (SAI), a table for vital signs, and a visual analog scale for pain severity. In the intervention group, a virtual reality device, movies, and relaxing music were provided and patients were instructed to use it during the implantation and when they felt pain after the implantation. There wasn't any blinding for patients or researchers throughout the study. Results: There was an increase in pain scores of both groups after the implantation; however, pain scores in the intervention group were lower and there was a statistically significant difference between the groups after the implantation. In the intervention group, there was a decrease in anxiety, systolic blood pressure, diastolic blood pressure, heart rate, and respiratory rate, and an increase in SpO2. VR use has a great effect on pain scores (Cohen's d = 3.023) and a great effect on SAI scores (Cohen's d = 8.770). Conclusions: VR distraction intervention was found an effective way to reduce pain, anxiety, systolic blood pressure, diastolic blood pressure, heart rate, and respiratory rate and increase the SpO2 of the patients undergoing port catheter implantation. Bu çalışmanın amacı; kanser hastalarının taburculuk öncesi öğrenim gereksinimlerini belirlemektir. Tanımlayıcı ve kesitsel tipteki bu çalışma, onkoloji kliniğinde tedavi gören 106 hasta ile yürütülmüştür. Veriler, “Hasta Tanıtıcı Bilgi Formu” ve “Hasta Gereksinimleri Ölçeği” ile elde edildi. Veri analizi, tanımlayıcı istatistikler, Mann Whitney U Testi, Kruskal Wallis ve Linear Regresyon ile yapıldı. Hastaların yaş ortalaması 40.94±4.25’dir. Araştırmada; hastaların aldıkları toplam ölçek puan ortalamasının 188.41 ± 20.51 olduğu belirlendi. Ölçeğe ait tüm alt boyut puan ortalamalarının yüksek (ilaçlar; 35.78 ± 4.05, yaşam aktiviteleri; 29.63 ± 2.88, toplum ve izlem; 19.40 ± 2.36, duruma ilişkin duygular; 18.17 ± 3.55, tedavi ve komplikasyonlar; 36.90 ± 3.00, yaşam kalitesi; 34.15 ± 3.76, cilt bakımı; 16.38 ± 3.24) olduğu saptandı. Cinsiyet, yaş, eğitim düzeyi, gelir durumu, hastalık tanısı ve hastalık süresi ile toplam puan ortalaması arasında istatistiksel olarak anlamlı fark olduğu bulundu (p<0.05). Hastaların taburculuk öncesi öğrenim gereksinimlerinin yüksek düzeyde olduğu saptandı. Hemşirelerin taburculuk öncesi hasta öğrenim gereksinimlerini belirlenmesi ve bireye özgü eğitimler planlanması ve uygulanması önerilmektedir.
Objective: To examine the knowledge, behavior, and clinical decision-making abilities of surgical nurses.Methods: The research was conducted in 95 surgical nurses working in a state and a university hospital. Data were collected using A Personal Information Form, the Pain Questionnaire, and the Clinical Decision Making Questionnaire in February 2018-July 2020. Data analysis was performed with SPSS 25 and legal permissions were obtained.Results: The statement “In cases where the necessary information about the surgery to be performed is not given, the severity of postoperative pain increases with increasing anxiety” received the highest number of correct answers (88.4%), while the statement “Opioids should not be given to patients with a substance abuse history because these patients are at high risk for recurrent addiction” received the least number of correct answers (7.4%). It was determined that 57.9% of the nurses observed the patient's behaviors to describe pain severity, and 76.8% of them used a pain scale in pain assessment. Of the nurses, 93.7% stated that the most commonly used pain relievers in their clinics were non-opioid drugs.Conclusion: It is noteworthy that the surgical nurses in this study had inaccurate/incomplete information about the use of pain scales and placebo, drug/opioid side effects, and non-pharmacological interventions. Furthermore, it was found that nurses experienced ethical dilemmas due to the risk of respiratory depression. Therefore, it is recommended to review the knowledge/practices related to pain management during nursing education/in-service training in order to increase the quality of pain management in surgical nurses.
BACKGROUND:This study was descriptive, prospective, and correlational conducted to evaluate the investigate the relationship between pain beliefs and postoperative pain severity in liver transplant patients. METHODS:The study included 118 liver transplant patients. Pain beliefs in the preoperative period were assessed by the Pain Beliefs Scale. Pain intensity was evaluated using the Numerical Rating Scale in postoperative 24-72 h. Multiple normal distribution and homogeneity of variance in the analyses were checked using the SPSS 25 package program, and Structural Equation Modeling was established. Whether the severity of pain and pain beliefs were affected by sociodemographic, disease, and pain characteristics was assessed. RESULTS:It was found that the mean pain severity of the patients was low. Mean psychological belief value was higher than the organic belief value. It was observed that there was a moderate positive correlation between the patients' mean pain severity score and pain belief total and sub-dimension score means (p < 0.05). In the Structural Equation Modeling established, it was found that psychological beliefs were more effective than organic beliefs on pain severity. CONCLUSION:The results of our study show that pain beliefs and pain severity are related. This study suggests a new perspective on the role of clinical and demographic factors on postoperative pain and shows that determining pain beliefs is important in pain control.
Amaç: Araştırma cerrahi hastalarının COVID-19 anksiyete düzeyleri ile mizah tarzları arasındaki ilişkinin incelenmesi amacıyla tanımlayıcı ilişkisel tipte
Giriş/Amaç: Kanıt uygulamanın bilimsel olarak değerlendirilmesine dayanan bilgidir. Hemşirelik uygulamalarının kanıta dayalı olması bakım kalitesini ve bakım sonuçlarını iyileştirmek, bakımı standardize etmek ve hemşire memnuniyetini artırmak gibi sonuçlar açısından önemlidir. Bu araştırmanın amacı karaciğer nakli hemşirelerin kanıta dayalı hemşireliğe yönelik tutumlarının belirlenmesidir. Gereç ve Yöntem: Tanımlayıcı tipteki araştırma Şubat 2021 ile Nisan 2021 tarihleri arasında bir üniversite hastanesinin karaciğer nakli enstitüsünde yapıldı. Araştırmanın yapılabilmesi için gerekli kurum izni ve etik izin alındı. Araştırma örneklemini 109 hemşire oluşturdu. Verilerin toplanmasında Kişisel Bilgi Formu ve Kanıta Dayalı Hemşireliğe Yönelik Tutum Ölçeği kullanıldı. Bulgular: Araştırmaya katılan hemşirelerin büyük çoğunluğunun kanıta dayalı hemşirelik hakkında bilgi sahibi olmadığı (%56.0) ve mesleki yayınları takip etmediği (%59.6) saptandı. Hemşirelerin kanıta dayalı hemşireliğe yönelik tutum puan ortalaması 38.06 ± 2.99 olarak belirlendi. Yaşı 25 ve altında olan, eğitim durumu lisansüstü düzeyde olan, 19 yıldır ve üzeri çalışan, kanıta dayalı uygulamalar hakkında bilgi sahibi olan, toplantılara katılan ve bilimsel dergileri takip eden hemşirelerin toplam tutum puanlarının istatistiksel olarak anlamlı derecede daha yüksek olduğu belirlendi(p<0.05). Sonuç: Karaciğer nakli hemşirelerinin kanıta dayalı hemşireliğe yönelik tutumları düşük düzeydedir ve hizmet içi eğitim vb. eğitim programlarıyla geliştirilmelidir.

Bu araştırma böbrek nakli olmuş hastalarda depresyon, anksiyete, stres seviyeleri ile immünsupresif tedaviye uyum arasındaki ilişkiyi belirlemek amacı ile tanımlayıcı ve ilişki arayıcı türde yapıldı. Araştırma verileri 20 Kasım ile 10 Aralık 2021 tarihleri arasında toplandı. Araştırma sonunda 95 hastaya ulaşıldı. Verilerin analizi SPSS 25 programı kullanılarak yapıldı ve gerekli tüm yasal ve etik izinler alındı. Araştırma sonunda hastaların depresyon, anksiyete, stres, DASS-21 ve İTUÖ toplam puanları sırasıyla; 5.15±3.61, 5.16±3.78, 9.62±4.93, 19.94±10.37 ve 11.42±3.29 olarak belirlendi. Kadın hastaların depresyon puan ortalamalarının erkek hastalara göre daha yüksek olduğu tespit edildi. Serbest mesleği olan hastaların memur olarak çalışan hastalara göre daha yüksek bir depresyon puan ortalamasına sahip olduğu sonucuna ulaşıldı. Geniş aile yapısına sahip hastaların İTUÖ sıra ortalamalarının çekirdek aile yapısına sahip hastalara göre daha yüksek olduğu saptandı. Bu araştırmada hastaların İTUÖ puanları ile depresyon, anksiyete ve stres puanları arasında istatistiksel açıdan önemli bir ilişki tespit edilemedi. Böbrek nakli olmuş hastaların depresyon, anksiyete, stres seviyeleri ile immünsupresif tedaviye uyumları arasında ilişki bulunmadı. Daha büyük örneklem gruplarıyla benzer nitelikte araştırmalar yapılması önerilebilir.
This study was conducted experimentally to evaluate the effect of neuro-linguistic programming (NLP) on fear of COVID-19 in kidney transplant patients. The study was carried out between June 2021 and October 2021. The Personal Information Form and COVID-19 Fear Scale (FCV-19S) were used to collect data. The obtained data obtained were evaluated using the SPSS 25 software. NLP was found to reduce the fear of COVID-19 in kidney transplant patients. Clinical nurses can use NLP techniques to support patients with fear in similar patient groups. Patients can be provided with access to training programs where they can learn NLP techniques. Background: This study was descriptively conducted to evaluate the fear of COVID 19 and its social effects on patients who had liver transplant. Methods: The study was conducted between September 2020 and April 2021 in a liver transplant institute affiliated with a university hospital. The sample of the study was 135 patients. Personal Information Form, Fear of Covid 19 Scale, and Questionnaire of Social Impact of COVID 19 Pandemic were used to collect data. Results: It was determined that the Fear of Covid 19 Scale mean score of the patients was 21.25 +/- 6.99. As the fear of COVID 19 increases in patients who had liver transplant, it was determined that their desire to be in crowded environments, to prefer public transportation, to go to the doctor for examination and their focusing on various objectives were decreasing. Also, fear of COVID 19 increased the difficulty in sleep, storage of food and cleaning materials, washing hands frequently, using masks and gloves when going out, health concerns, doubts about disease symptoms, orientation towards healthy eating, worries about the future and questioning the meaning of life. Conclusion: The results show that it is important for transplant centers to be able to provide guidance and psychological counseling services to liver transplant patients, who are significantly affected by COVID 19, through telemedicine or various technological opportunities.
Objective: This descriptive and relational study was conducted to examine the relationship between palliative care patients' humor styles and death anxiety. Materials and methods: The study sample consisted of 282 palliative care patients treated in the palliative care clinics of a training and research hospital between January 2021 and August 2021. Personal Information Form, Humor Styles Questionnaire (HSQ), Thorson-Powell Death Anxiety Scale (TPDAS) and Palliative Performance Scale (PPS) were used as data collection instruments. Data analysis was performed in SPSS 25.0 software using mean, standard deviation and percentile, Kolmogorov-Smirnov Goodness-of-Fit Test, Significance test of difference between two means, ANOVA, Post-hoc test, Pearson's Correlation test and regression analysis. The principles of the Declaration of Helsinki were followed in the study, ethics committee approval and institutional permission were obtained. Results: It was found that the mean age of the patients was 49.58±9.56 and 52.1% were hospitalized in the palliative care clinic for 5-10 days. It was determined that the most frequently used humor style by the patients was “Affiliative Humor” (31.7%), and the least used humor style was “Aggressive Humor” (19.5%). The Humor Styles Questionnaire (HSQ) subscale scores of the patients were determined as 31.05±7.11, self-enhancing humor 28.34 ± 6.94, aggressive humor 26.85±7.37, self-defeating humor 23.50± 6.21. Aim: This study was conducted using a prospective randomized single-blind clinical trial model to examine the effect of neuro-linguistic programming (NLP) on depression, anxiety, and stress in liver transplant patients.Materials and Methods: This study was conducted with liver transplant patients at a university’s Liver Transplant Institute between June 2021 and December 2021. The sample size of the study was determined as 84 (Experimental group: 42, control group: 42). Personal information form and Depression, Anxiety, and Stress Scale (DASS-21) were used to collect data. The data obtained from the research were evaluated with SPSS 25.Results: In the experimental group, the mean score of DASS-21 was found to be 38.95 ± 11.96 before the NLP application and 25.21 ± 5.43 after the NLP application. It was found statistically significant that the value obtained in the second measurement was lower than the value obtained in the first measurement (p<0.05). It was determined that NLP application significantly decreased all parameters of depression, anxiety, and stress (p<0.05). In the control group, the mean score of DASS-21 was determined as 51.05 ± 7.53 in the first measurement and 52.57 ± 7.92 in the second measurement. It was found statistically significant that the value obtained in the second measurement was higher than the value obtained in the first measurement (p<0.05).Conclusion: This study found that NLP reduced depression, anxiety, and stress in liver transplant patients. In line with this result, it can be suggested that nurse’s benefit from NLP practice and receive training on NLP in managing the depression, anxiety, and stress experienced by liver transplant patients.
