: Elder abuse and violence in home care is an important issue. The phenomenon is clearly widespread and has the potential to cause harm in many different ways. The main focus here is on mistreatment, abuse or neglect of older people - for example by relatives, outpatient care services or other participants in the health and social care system. However, caregivers can also be affected by aggression or violent behavior from those in need of care. Overall, there is little evidence of effective preventive interventions in the home environment. Family doctors in particular can play an important role in prevention.
Zusammenfassung Das Thema Vereinbarkeit von Beruf und Pflege ist in Deutschland gesellschaftlich relevant und nimmt zukünftig weiter an Bedeutung zu. Schon heute sind zwei von drei pflegenden Angehörigen unter 65 Jahren erwerbstätig. Arbeitnehmende, die privat Pflegeaufgaben übernehmen, stehen oft vor erheblichen Herausforderungen und drohen überdies dem Arbeitsmarkt teilweise oder vollständig entzogen zu werden. Gleichzeitig sind pflegende Angehörige die Basis der pflegerischen Versorgung im Sinne des SGB XI in Deutschland. Vereinbarkeitskonflikte werden in der Forschung als bedeutsame Stressoren für erwerbstätige pflegende Angehörige gesehen, positive Arbeitssituationen dagegen als mindestens etwas entlastend eingeschätzt. Erschwerend für die Vereinbarkeitsbemühungen wirkt sich u. a. aus, wenn die Pflegekonstellation mit einer Demenzerkrankung verbunden ist. Die Unterstützungsbemühungen von Unternehmen und Politik für erwerbstätige pflegende Angehörige haben sich in den vergangenen Jahren intensiviert, erscheinen aber nach wie vor verbesserungsbedürftig. In der aktuellen Coronavirus-Pandemie hat sich die Situation für viele pflegende Angehörige insgesamt verschärft. So zeigen die Ergebnisse der vorliegenden ZQP-Befragung, dass erwerbstätige Angehörige signifikant häufiger von einer Verschlechterung der Pflegesituation in der ersten Infektionswelle im Vergleich mit nicht-erwerbstätigen berichten. Ein Großteil der Befragten gibt an, dass die Vereinbarkeit von Pflege und Beruf nun noch schwieriger geworden sei.
Background While the relation between care involvement of informal caregivers and caregiver burden is well-known, the additional psychosocial burden related to care involvement during the COVID-19 pandemic has not yet been investigated. Methods A total of 1000 informal caregivers, recruited offline, participated in a cross-sectional online survey from April 21 to May 2, 2020. Questionnaires were used to assess COVID-19-specific changes in the care situation, negative feelings in the care situation, problems with implementation of COVID-19 measures, concerns/excessive demands, loss of support, change in informal caregivers’ own involvement in care and problems with provision, comprehension & practicability of COVID-19 information, and to relate these issues to five indicators of care involvement (i.e., being the main caregiver, high expenditure of time, high level of care, dementia, no professional help). Binomial and multiple regression analyses were applied. Results Across indicators of care involvement, 25.5–39.7% reported that the care situation rather or greatly worsened during the COVID-19 pandemic, especially for those caring for someone with dementia or those usually relying on professional help. In a multiple regression model, the mean number of involvement indicators met was associated with age (β = .18; CI .10–.25), excessive demands (β = .10, CI .00–.19), problems with implementation of COVID-19 measures (β = .11, CI .04–.19), an increase in caregiving by the informal caregivers themselves (β = .14, CI .03–.24) as well as with no change in the amount of caregiving (β = .18, CI .07–.29) and loss of support (β = −.08, CI −.16–.00). No significant associations with the mean number of involvement indicators met were found for gender, educational level, change in the care situation, negative feelings, and provision, comprehension & practicability of COVID-19 information. Conclusion Those caregivers who perceived extensive care burden were those who suffered most during the pandemic, calling for structural support by the healthcare system now and in the future. Trial registration This article does not report the results of a health care intervention on human participants.
Introduction: Death and dying are very personal and sensitive topics. Many people are concerned that for example pain or loneliness in connection with serious illness could burden the dying process and threaten their dignity. Fear or anxiety in relation to death and dying has been examined in different patient groups and types of health care providers. However, since data about the subject in home care patients and their informal caregivers is rare, the aim of the study is to analyze fears related to death and dying in this vulnerable population. Material and Methods: A cross-sectional study in home care services throughout Germany was conducted. A total of 656 questionnaires could be used for analysis from clients and 499 from informal caregivers. 5 items were used to generate an Index of Death-related Fear (Cronbachs-α=0.8). The influence of age, gender and level of care dependency on the Index of Death-Related Fear was analyzed in a linear multiple regression. Results: Clients “very much” (“rather yes”) feared the “loss of personal dignity” with 25.5% (18.6%) and “physical suffering” 26.7% (21.5%). The standardized β for level of care dependency was 0.129 (95 CI 0.0140.164) and 0.138 (95 CI 0.069 0.583) for gender (female) on the Index of Death-Related Fear. Conclusion: Fear regarding death and dying among individuals receiving home care is high, especially concerning the loss of personal dignity and physical suffering. Half of the clients do not speak about the subject. Health care professionals working in community care should offer clients to talk about death and dying.
Die Anzahl Hundertjähriger hat sich in den letzten Jahren stark erhöht und wird weiterhin zunehmen. Viele von ihnen sind auf Hilfe im Alltag angewiesen, und Kinder sind eine Hauptunterstützungsquelle. Bisherige Studien schenkten der Pflegesituation unter Berücksichtigung langer gemeinsamer Vergangenheiten zwischen Hundertjährigen und ihren Kindern kaum Aufmerksamkeit. Ziel der Untersuchung war eine vertiefende Analyse der Pflegesituation Hundertjähriger aus der Perspektive sich sorgender Töchter und Söhne vor dem Hintergrund langer gemeinsamer Vergangenheiten und familialer Beziehungsstrukturen. Mit 13 Töchtern und Söhnen Hundertjähriger wurden leitfadengestützte Interviews geführt. Diese wurden in Anlehnung an die Dokumentarische Methode ausgewertet. Die Ergebnisse zeigen, dass Pflegesituationen und daraus erwachsende Belastungen auch Resultat einer bis ins sehr hohe Alter wirksamen familialen Rollen- und Beziehungsdynamik sind. Aus diesem Grund sollten sie auch im Kontext der bisherigen gemeinsamen Vergangenheiten betrachtet werden. Hier bedarf es weiterer Forschung, um differenzierte Angebote für sorgende Angehörige weiterentwickeln zu können.
BACKGROUND:The number of centenarians in Europe is increasing; many face health impairments. Adult children often play a key role in their care, but there is a lack of research into what it means for these caregiving relatives to be confronted for many years with their parents' end of life (EOL), dying and death as well as their own advancing age.AIM:This study aims to analyse the challenges of caregiving adult children regarding their parents' end of life and the related burdens and barriers they report.MATERIAL AND METHODS:Semi-structured interviews were conducted with 13 caregivers following a theory-based and tested guideline. The computer-aided coding and evaluation followed the structured content analysis approach.RESULTS:The analysis showed three main themes: 'Confronting EOL', 'Communicating about death and dying' and 'Assisting in the terminal phase'. The respondents commented on burdensome demands and concerns about the future. Further, a strong underlying presence of intra- and interpersonal conflicts relating to EOL became apparent.DISCUSSION:The results indicate several potential burdens for centenarians' caregiving offspring. They are confronted with a double challenge resulting from the combination of their own advanced age and experiencing the burdens of their parents' very old age. Further, some participants struggled with their own unclear perspective on the future because of the relative but unclear proximity of the parent's death. Multiple conflicts and overlapping conflict dimensions emphasise the potential of the EOL topic to influence the well-being of family caregivers and care recipients.LIMITATIONS:The convenience sample used for the study may cause limitations, for example, the fact that persons with a formally lower educational status are not represented.CONCLUSION:The findings suggest that interventions designed for family-related care situations should include topics like 'Finiteness and grief', 'Communicating about dying and death' and 'Decisions and dispositions at EOL'.
Die steigende Zahl langlebiger Menschen wirft Fragen zu psychosozialen Bedingungen eines außergewöhnlich langen Lebens auf. In diesem Zusammenhang wird aus psychologischer Perspektive das Konzept der Resilienz genutzt, um besondere Widerstandsfähigkeit als individuelle Kompetenz zum Erhalt oder zur Wiederherstellung des Funktionsniveaus zu beschreiben. Resilienz wird aus dieser Sicht als relativ stabiles Persönlichkeitsmerkmal verstanden, das allerdings im höheren Alter an Bedeutung verlieren kann. Im vorliegenden Beitrag wird – im Gegensatz zur üblichen Lebensphasenbetrachtung – eine soziologische Perspektive über den gesamten Lebensverlauf eingenommen. Dies erfolgt unter Einbeziehung des „Rational-choice“-Ansatzes, um Bedingungen, Grundlagen, Strategien und Ergebnisse von Handlungsentscheidungen aus qualitativen biografischen Schilderungen (Leitfadeninterviews) 15 hundertjähriger Frauen und Männer zu beschreiben und zu diskutieren. Resilienz wird in diesem Sinne als Handlungsfähigkeit begriffen. Es zeigt sich, dass über die gesamte Lebensspanne hinweg bis ins höchste Alter Potenziale für resilientes Handeln bestehen, Handlungsfähigkeit dabei jedoch in hohem Maße kontextabhängig ist.

Persons in need of care and their family members should be able to realistically assess and compare the quality of care services. Recent legislation therefore states that the current procedure to assess and present care quality has to be developed further. While quality measurements can be informed by the literature, knowledge on suitable formats for presenting quality information that is suitable for care-receiving persons and their families is highly fragmentary.The Development of quality reporting in long-term care project, a cooperation between the Charite, the federal association of the AOK (AOK-Bundesverband), and the Center for Quality in Care (ZQP), provides the current findings on the information requirements of persons in need of care and their family members when choosing acare facility. Aspects related to nursing staff have aparticularly high relevance, e.g. respectful behavior of the personnel, well-trained caregivers, sufficient staff density, seldom changes in the personnel of the institution, and thoroughly conducted care that is adjusted to individual needs. These criteria have been given the highest relevance by persons in need of care as well as their family members.The findings hint at the necessity to provide comprehensive and detailed information about the staffing of care facilities in the future: for example, on the staff ratio and qualifications. Apart from that, comprehensive knowledge on formats to present information is urgently needed - taking into account the needs of care-dependent persons and their families and how information can be optimally processed.
