Fragestellung: Auch fast 10 Jahre nach der Zulassung der Thrombolyse wird diese noch zu selten angewendet. Ein häufiger Grund weshalb Patienten, die prinzipiell für eine Thrombolyse in Frage kommen nicht behandelt werden, ist, dass die Symptomatik initial als zu leicht einschätzt wird. Es wird eine Übersicht über die bisherige Datenlage zur Thrombolyseindikation bei Patienten mit „milder“ Schlaganfallsymptomatik gegeben.
Hyperthermie führt zu einer Beeinflussung verschiedener Körper-Regelgrößen, wobei insbesondere Interaktionen mit dem kardiovaskulären bzw. dem zentralen Nervensystem zu lebensbedrohlichen Folgen führen können. Der vorliegende Beitrag beschreibt die Symptome, die Pathogenese und die Therapieansätze der häufigsten 3 Hyperthermie-Syndrome (maligne Hyperthermie, Serotonin-Syndrom und malignes neuroleptisches Syndrom). Dem Leser die differenzialdiagnostische Abgrenzung dieser 3 Entitäten zu erleichtern, ist das Ziel dieser Übersicht. Weiterhin wird die mögliche Assoziation zwischen Myopathien und Hyperthermie-Syndromen diskutiert.
Hyperthermia affects almost all endogenous regulatory systems, where especially cardiovascular and central nervous system interactions can result in life threatening complications. This review illustrates signs and symptoms, pathophysiology and therapeutic options of the three most common hyperthermic syndromes in neurology: malignant hyperthermia, serotonine-syndrom and malignant neuroleptic syndrome. The aim of this contribution is to enable the reader to make the differential diagnosis of these three disease entities. Furthermore the association of other specific myopathies and hyperthermia syndromes is discussed.
Background: Thrombolytic therapy is frequently withheld in patients with minor stroke symptoms. However, recent studies demonstrate that a substantial proportion of these patients dies or remains permanently disabled because of underestimation of symptom severity at baseline or secondary deterioration. We aimed to assess the safety and outcome of thrombolysis therapy in patients with minor but disabling stroke symptoms. Methods: 32 patients presenting with mild symptoms were treated with intravenous recombinant tissue-type plasminogen activator between April 2006 and April 2008. Data were extracted from a prospectively collected database. Baseline demographic data, and clinical, laboratory and imaging findings were analyzed. Outcome was assessed using the modified Rankin Scale (mRS) score at 3 months and was dichotomized into favorable (mRS 0–1) versus unfavorable (mRS 2–6). Results: In the majority of patients, the left hemisphere was affected, with aphasia representing the most common symptom leading to treatment decision. The frequency of perfusion lesion (46%) and vessel occlusion (35%) at baseline was high but had no effect on the outcome at 3 months in our series of treated patients. Outcome was favorable in 94% of patients, and 47% recovered without any persisting symptom. Only one asymptomatic and no symptomatic hemorrhage was observed. Conclusion: Our data support current guidelines and international licenses which give no lower National Institutes of Health Stroke Scale (NIHSS) limit for intravenous thrombolysis (IVT). Considering the accumulating evidence that the natural course in patients with mild symptoms is not as favorable as often assumed and taking the low risk of bleeding in those patients into account, patients with mild but disabling symptoms should be treated with IVT regardless of their baseline NIHSS score.
Diabetes mellitus und Schlaganfall sind epidemiologisch und gesundheitsökonomisch bedeutende Krankheitsbilder und bedürfen immer einer fachübergreifenden Diagnostik und Therapie. Der Diabetes erhöht das Schlaganfallrisiko auf das 2- bis 4fache. Schlaganfallpatienten mit Diabetes erfordern eine längere Akutbehandlung, weisen eine erhöhte Sterblichkeit auf und erholen sich weniger gut im Rehabilitationsprozess. Bei etwa 20–30% der Patienten mit einem ersten Schlaganfallereignis wird eine bisher nicht bekannte gestörte Glukosetoleranz oder sogar ein manifester Diabetes festgestellt. Bereits in der Prädiabetesphase kommt es zu Gefäßschäden in Form der endothelialen Dysfunktion und beginnenden Arteriosklerose. Zur Abschätzung des Gefäßbefundes dienen die laborchemische Untersuchung auf Mikroalbuminurie und die sonographische Bestimmung der Intima-Media-Dicke der A. carotis communis. In der Akutbehandlung des Schlaganfalls ist die Hyperglykämie besonders kritisch, da sie die Wirksamkeit der Thrombolyse reduziert und mit einer stärkeren Gewebeschädigung einhergeht. Spezifische klinisch-neurologische Befunde komplizieren die Therapie in der Akutphase, die unbedingt auf einer „Stroke Unit“ erfolgen soll. In der Sekundärprävention haben neben einer optimalen Blutzuckerkontrolle die risikoadaptierte Thrombozytenfunktionshemmung und Statine einen festen Stellenwert.
Diabetes and stroke are important clinical conditions from epidemiological and economical points of view. An interdisciplinary approach is always indicated to diagnose and treat these diseases optimally. Diabetes elevates the risk of stroke between two- and four-fold. Diabetic stroke patients need longer in-patient treatment, have a higher mortality rate and recover less well during the rehabilitation process. Approximately 20%-30% of first-ever stroke patients reveal an impaired glucose tolerance or even manifest diabetes. Already in the prediabetic phase, vessel damage can be seen in the form of endothelial dysfunction and early arteriosclerosis. Measurements of microalbuminuria and intima-media thickness of the common carotid artery enable an assessment of the vascular system. Hyperglycemia represents a serious risk during the acute treatment phase, as it diminishes the effect of thrombolysis and worsens the ischemic parenchymal lesion, Specific neurological signs and symptoms complicate the acute phase of stroke treatment, which should always be carried out in a stroke unit. For secondary prevention purposes, risk-adapted platelet inhibition and statins are recommended.
