Zusammenfassung Ziele der Studie: Der Diagnostik beruflicher Belastungen und des Umgangs mit ihnen kommt in der psychosomatischen Rehabilitation eine besondere Bedeutung zu. Ein etabliertes Instrument zur Erfassung arbeitsbezogener Verhaltens- und Erlebensmuster ist der AVEM, er ist jedoch für den Einsatz in der klinischen Routine sehr lang und zeitintensiv in der Auswertung. Es soll daher überprüft werden, ob sich eine Kurzversion generieren lässt, welche die 3 in der Vergangenheit beschriebenen Sekundärfaktoren Arbeitsengagement, Widerstandskraft und Emotionen des AVEM ausreichend reliabel und valide für individualdiagnostische Zwecke erfassen kann, und sich ggf. auch für ein Screening von Patienten mit auffälligen Erlebens- und Verhaltensmustern eignet. Methodik: Es wurde ein Datensatz von jeweils konsekutiven Stichproben aus 3 Kliniken der psychosomatischen Rehabilitation mit N = 10 635 Patienten verwendet, die zum Aufnahmezeitpunkt den AVEM beantwortet haben. Die Stichprobe wurde per Zufall auf 2 gleichgroße Konstruktions- und Validierungsstichproben aufgeteilt. Mittels explorativer Hauptkomponentenanalysen sollen die jeweils ladungsstärksten Items für die 3 neuen Skalen in der Konstruktionsstichprobe ausgewählt und anhand der Validierungsstichprobe psychometrisch überprüft werden. Für den Einsatz als Screeninginstrument sollen mögliche Cut-Off-Werte aus den Verteilungsmustern der Skalenwerte abgeleitet werden. Ergänzend werden Zusammenhänge der 3 neuen Skalen mit soziodemografischen, berufs- und diagnosebezogenen Merkmalen sowie Mustern arbeitsbezogenen Erlebens und Verhaltens untersucht. Ergebnisse: Die 3 durchgeführten Hauptkomponentenanalysen erklären in der Konstruktionsstichprobe auf dem jeweils ersten Faktor zwischen 31 % und 34 % der Varianz. Für die ausgewählten 20 Items findet sich in der Validierungsstichprobe in einer 3-Faktoren-Struktur die erwartete Zuordnung der Items. Die Reliabilität der so gebildeten 3 neuen Skalen liegt im guten Bereich mit Werten von Cronbach’s α zwischen 0,84 und 0,88. Die Benennung der 3 neuen Skalen erfolgt in Anlehnung an die Bezeichnung der Sekundärfaktoren. Cut-off-Werte zur Identifikation auffälliger Patientenangaben werden vorgeschlagen. Schlussfolgerung: Als Vorteile dieses Vorschlags für eine verkürzte und modifizierte Auswertung mittels AVEM-3D kann angeführt werden, dass mit einer deutlich geringeren Anzahl an Items 3 relevante Dimensionen arbeitsbezogener Verhaltens- und Erlebensmuster reliabel zu erfassen sind. Die Auswertung und Interpretation ist einfach und ökonomisch. Basierend auf der vorliegenden Stichprobe liegen zudem Mittelwerte und Standardabweichungen als Vergleichswerte zu Beginn der psychosomatischen Rehabilitation vor. Weitere unabhängige psychometrische Überprüfungen der Reliabilität, Validität sowie auch Änderungssensitivität sind notwendig, in denen nur die Items der verkürzten Version verwendet werden. Praktikabilität und Validität der vorgeschlagenen Cut-off-Werte können noch nicht ausreichend beurteilt werden. Schließlich bleibt zu prüfen, inwieweit der AVEM-3D auch in anderen Indikationsgruppen über die Psychosomatik hinaus und bei gesunden Probanden sinnvoll eingesetzt werden kann.
Objectives: The assessment of work pressures is of particular importance in psychosomatic rehabilitation. An established questionnaire is the Occupational Stress and Coping Inventory (German abbr. AVEM), but it is quite long and with regard to scoring time-consuming in routine clinical care. It should therefore be tested, whether a shortened version of the AVEM can be developed, which is able to assess the formerly described three second-order factors of the AVEM, namely Working Commitment, Resilience, and Emotions, sufficiently reliable and valid, and which also may be used for screening of patients with prominent work-related behavior and experience patterns. Methods: Data were collected at admission from consecutive samples of three hospitals of psychosomatic rehabilitation (N = 10,635 patients). The sample was randomly divided in two subsamples (design and validation sample). Using exploratory principal component analyses in the design sample, items with the highest factor loadings for the three new scales were selected and evaluated psychometrically using the validation sample. Possible Cut-off values ought to be derived from distribution patterns of scores in the scales. Relationships with sociodemographic, occupational and diagnosis-related characteristics, as well as with patterns of work-related experiences and behaviors are examined. Results: The three performed principal component analyses explained in the design sample on the respective first factor between 31 % and 34 % of the variance. The selected 20 items were assigned to the 3-factor structure in the validation sample as expected. The three new scales are sufficiently reliable with values of Cronbach's α between 0,84 and 0,88. The naming of the three new scales is based on the names of the secondary factors. Cut-off values for the identification of distinctive patient-reported data are proposed. Conclusion: Main advantages of the proposed shortened version AVEM-3D are that with a considerable smaller number of items the three main dimensions of relevant work-related behavior and experience patterns can be reliably measured. The proposed measure is simple and economic to use and interpret. Based on the present sample we provide means and standard deviations as reference at admission of psychosomatic rehabilitation. As a limitation it should be mentioned that further evaluation of reliability, validity and sensitivity to change restricted to the items of the shortened version is necessary. The practicability and validity of the proposed cut-off values cannot yet be conclusively assessed. Finally, the validity of the AVEM-3D in groups of indications other than psychosomatic patients and in healthy persons remains to be examined.
Objectives: Children of cancer patients have an increased risk for developing emotional problems. While psychosocial cancer counselling services are available all over the country, it is unclear if parents seek for help and if specific approaches for families are offered.Methods: A survey was made of outpatient cancer counselling services in Germany (n=228). The response rate was 56%. Descriptive and content analyses of the data have been used.Results: Providers estimate that 55% of their patients are between 18 and 55 years with 18% of them having minor children. However, only 53% do regularly ask their patients if they have minor children. Family- or child-settings are not provided regularly. Over 60% of providers would welcome special courses on this subject.Conclusion: Although psychosocial care of minor children is mentioned in outpatient psychosocial cancer counselling guidelines, children are not included regularly. A stronger emphasis on this topic in further education has to be made.
Kinder krebskranker Eltern gelten als belastete Risikogruppe. Krebsberatungsstellen sind nahezu flächendeckend angesiedelt. Inwiefern jedoch Eltern Unterstützung in Anspruch nehmen oder spezifische Hilfen angeboten werden, ist unklar. Postalische Befragung von Krebsberatungsstellen in Deutschland (n=228). Der Rücklauf betrug 56% (n=128). Die Analyse der Daten erfolgte mittels deskriptiver Statistik und qualitativer Inhaltsanalyse. Die Mitarbeiter der psychosozialen Krebsberatungsstellen schätzen, dass 55% ihrer Patienten zwischen 18 und 55 Jahren alt sind und 18% minderjährige Kinder haben. Trotzdem wird nur in 53% aller befragten Einrichtungen zu Beginn der Beratung regelhaft nach dem Vorhandensein von minderjährigen Kindern gefragt. Familien- oder Kindgespräche gehören nicht zur Routineversorgung. Über 60% der Befragten bekundet großes Interesse an Fortbildungsmöglichkeiten zu dem Thema. Obwohl die Versorgung minderjähriger Kinder zu den Vorgaben der S1-Leitlinien zur ambulanten psychosozialen Krebsberatung gehört, wird ein Einbezug der Kinder nicht immer umgesetzt. Eine stärkere Gewichtung des Themas im psychoonkologischen Weiterbildungscurriculum ist notwendig.
Purpose: Prostate cancer patients often suffer from treatment-associated morbidities which lead to severe physical and mental impairments. Nevertheless, only a relatively small percentage of this patient population uses medical rehabilitation services; there is still a lack of evidence concerning possible factors causing use and non-use of services. Therefore, this study exploratively aims at the identification of predictors of the use of rehabilitation services in a cohort of prostate cancer patients.Method: In a prospective multicentre study to evaluate outpatient oncological rehabilitation services, n=242 prostate cancer patients who used outpatient or inpatient rehabilitation services ("users") were compared with n=253 prostate cancer patients who did not use rehabilitation ("non-users") at measure point 1. At the beginning of the rehabilitation programme and at the end of primary treatment, respectively, patients completed a self-report questionnaire consisting of standardized instruments designed to assess the following independent variables: autonomy striving (TPF), self-efficacy expectancy (GSE), distress (distress thermometer), anxiety and depression (HADS-D), quality of life (SF-8), social support (SSUK), rehabilitation motivation: readiness to change, knowledge, scepticism regarding rehabilitation services (PAREMO-20). Illness- and treatment-related variables (tumour state, comorbidity, primary therapies) were documented by the attending oncologists.Results: Only few significant and effective differences between rehabilitation users and non-users are observed: Non-users are suffering more often from gastrointestinal comorbidities (18% vs. 2%, w=0.25). Both patient groups report severe quality-of-life impairments, particularly with respect to their physical functioning. Non-users achieve significantly higher scores on the scale "scepticism" (PAREMO-20) than users (eta(2)=0.19). The scale "scepticism" is also identified as the dominant predictor of utilization of rehabilitation services (R-2=0.23). Suffering from comorbid diseases of the digestive system, self-efficacy expectation and employment status emerge as further significant predictors (R-2=0.12, and R-2=0.02 each). With increasing scepticism and in case of comorbidity the probability of rehabilitation utilization was decreasing.Conclusion: Besides illness-related variables prostate cancer patients' expectancies regarding the effectiveness of rehabilitation services to alleviate their medical conditions seem to be more relevant for rehabilitation utilization than sociodemographic variables or psychosocial distress. Further studies should investigate the replicability of these results and should focus on the doctor-patient-communication and the amount of information about goals and concepts of rehabilitation services prostate cancer patients are told. For clinical practice of information-giving about rehabilitation services it can be recommended not only to assess patients' physical and mental conditions but also to explore their expectations and concerns about rehabilitative treatment options in order to remove any doubts and to be able to optimize oncological care.
