Background: Early readmissions after liver transplantation (LT) are a source of significant resource expenditure. Our goal was to reduce readmissions without adversely affecting patient outcomes. Methods: We devised a protocol to address readmissions that set readmission guidelines, expanded outpatient services, and refined peritransplant teaching. Data collected included patient demographics, length of stay (LOS), readmission events and reason, inpatient charges, and patient survival. Data were compared for the time periods before and after protocol implementation. Results: From Jan 2012 to Mar 2017, 304 adult primary LTs were performed. The protocol was initiated Oct 2013. Before the protocol (n = 121), 40% and 53.7% of patients were readmitted at 30 and 90 days, respectively. After the protocol (n = 183), 15.8% and 25.6% of patients were readmitted at 30 and 90 days, respectively (p < 0.001). The reduction in readmissions at 90 days resulted in a 33.5% inpatient hospital cost reduction. Median LOS for the transplant admission (8 days) and 1-year patient survival (91%) was unchanged over the study period. Conclusion: Our protocol significantly reduced early hospital readmissions after LT, resulting in substantial cost savings, without negatively affecting LOS or patient survival.
Kidneys from nondirected donors (NDDs) have historically been allocated directly to the deceased donor wait list (DDWL). Recently, however, NDDs have participated in kidney exchange (KE) procedures, including KE 'chains', which have received considerable media attention. This increasing application of KE chains with NDD participation has occurred with limited ethical analysis and without ethical guidelines. This article aims to provide a rigorous ethical evaluation of NDDs and chain KEs. NDDs and bridge donors (BDs) (i.e. living donors who link KE procedures within KE chains) raise several ethical concerns including coercion, privacy, confidentiality, exploitation and commercialization. In addition, although NDD participation in KE procedures may increase transplant numbers, it may also reduce NDD kidney allocation to the DDWL, and disadvantage vulnerable populations, particularly O blood group candidates. Open KE chains (also termed 'never-ending' chains) result in a permanent diversion of NDD kidneys from the DDWL. The concept of limited KE chains is discussed as an ethically preferable means for protecting NDDs and BDs from coercion and minimizing 'backing out', whereas 'honor systems' are rejected because they are coercive and override autonomy. Recent occurrences of BDs backing out argue for adoption of ethically based protective measures for NDD participation in KE.
Background. The risk of recurrence of glomerulonephritis in kidney transplant recipients on a steroid-free maintenance immunosuppression protocol is unknown. Methods. We studied the 4-year graft and patient survival in 105 adult kidney transplant recipients who received their transplant for glomerulonephritis (GN) and were treated with a protocol incorporating rapid discontinuation of prednisone for 5 days (group 1). We compared these outcomes to two control groups; 439 concurrent recipients who received a transplant for causes other than GN (group 2) and to 260 kidney transplant recipients who received an allograft for GN between 1994 and 1999 and were maintained on a steroid-based immunosuppressive protocol (group 3). Results. The 4-year graft and patient survival were similar in the three groups. Acute rejection-free survival was also similar. Serial annual serum creatinine and estimated GFR were also comparable amongst the three groups. Two grafts were lost in group 1 from biopsy-proven recurrent GN and eight other subjects had evidence of histological recurrence at 11.2±11.9 months. Seven grafts were lost for recurrent disease in group 3 and 15 others had evidence of histological recurrence at 29.1±32.6 months. The mean time to graft loss from recurrence was 52±22 months. Conclusion. A regimen that utilizes rapid discontinuation of steroids conveys no added risk of graft loss from recurrent GN in the short term but longer follow-up is needed. A consideration should be made to discontinue corticosteroids in the potential recipients who are on them at the time of transplantation.
The waiting list for a kidney or simultaneous pancreas-kidney transplant is growing, and waiting times are getting longer. As a consequence, transplant candidates are dying while waiting for a transplant. In a retrospective analysis, we studied whether patients on our list who died while waiting were good candidates. From January 1, 2002, through September 30, 2004, 85 candidates on our list died. Of these, 71% were waiting for a first transplant; 62% had a current panel-reactive antibody (PRA) level of 0%. Of the 85 candidates who died, the mean (+/-SD) age was 53 +/- 11 years; mean waiting time from listing to death, 979 +/- 749 days. The most common cause of death was cardiovascular disease. Many of those candidates who died while waiting were young, first-transplant candidates with a low PRA level. But only limited information was available; prospective studies are necessary to determine whether or not they were, in fact, good candidates.
We compared three maintenance immunosuppressive regimens in a rapid discontinuation of prednisone protocol. From March 1, 2001, through December 31, 2003, 239 first and second kidney transplant recipients (166 LD; 73 DD) were randomized. All recipients were treated with Thymoglobulin; all received steroids intraoperatively and for 5 days postoperatively. Randomization was to cyclosporine-mycophenolate mofetil (n = 85); high-level tacrolimus (TAC) (8-12 ng/mL)-low-level sirolimus (SRL) (3-7 ng/mL) (n = 72); or low-level TAC (3-7 ng/mL)-high-level SRL (8-12 ng/mL) (n = 82). We found no difference at 24 months between groups in patient, graft, death-censored graft, or acute rejection-free graft survival, or in kidney function. Wound complications were more common in SRL-treated recipients (p = 0.02); we found no other differences between groups in complication rates. Our data suggest that excellent patient and graft survival and low rejection rates can be obtained using a variety of maintenance protocols without prednisone.
