The primary objective of this study is to determine the prevalence of contrast-induced nephropathy (CIN) in patients rapidly evaluated in the emergency department (ED) for acute cerebral infarction who undergo contrast-enhanced computed tomography (CECT) of the head. We conducted an observational, retrospective cohort study of patients who presented to the ED with signs and symptoms of acute ischemic cerebral infarction that were potentially eligible for intravenous (IV) thrombolysis. All patients being considered for IV thrombolysis seen within a single academic tertiary care hospital system that is a Joint Commission Primary Stroke Center with an annual ED visit >160,000 from May 2008 thru February 2011 were evaluated for inclusion in this study. In this institution, patients eligible for IV thrombolysis undergo a rapid imaging protocol (non-contrast head CT, CT angiogram of the head, and CT perfusion study) prior to laboratory results of baseline creatinine. Patients were excluded if they did not receive CECT, they did not have creatinine measurements at 48 to 72 hours, or if they were already receiving dialysis. Data collected included baseline comorbidities, sex, age, serum creatinine level on presentation, and serum creatinine levels daily for the next 5 days after presentation. CIN was defined as either an absolute increase in serum creatinine of 0.5 mg/dl or a relative increase of 25% within 48-72 hours of contrast administration. Prevalence and 95% confidence intervals (CI) were computed using the Agresti-Coull Modified Wald Binomial Method. 403 patients were evaluated utilizing the rapid assessment protocol for acute ischemic stroke. Only 168 of these patients were able to be included for analysis of CIN; 164 patients did not have adequate follow-up creatinine measurements, 69 patients did not receive contrast dye, and 2 patients were receiving dialysis. 3% (5/168, 95% CI 1.1-7%) of patients developed CIN within 48 hours of receiving contrast, and this increased to a total of 5.4% (9/168, 95% CI 2.7-10%) within 72 hours. However, only 1.2% (95% CI 0-4.5%) and 2.4% (95% CI 0.7-6.2%) of the patients with normal baseline creatinine levels developed CIN at 48 and 72 hours, respectively. The prevalence of CIN in our cohort of patients was 5.4% within 72 hours of contrast administration. This low prevalence should encourage physicians to not delay CECT to measure serum creatinine levels when evaluating patients in the ED for acute stroke as “time is brain.”

Nuclear borehole logging is practically the only technique that has the capacity for providing quantitative in-situ coal quality information in real time. The SIROLOG gamma–gamma and neutron–gamma (PGNAA) borehole logging procedures have been providing vital characteristics of the coal seams at Callide coalfields since 1993. Together with an accurate delineation of coal seams, the SIROLOG procedures estimate ash content, density and some ash constituents accurately enough for relatively inexpensive rotary chip holes to be accepted as points of observation in calculations of reserves.
The PGNAA technique was tested for the in-situ determination of ash, Fe, Si, Al and density of coal seams in water-filled boreholes. The technique is also able to determine in situ the deformation temperature of coals for which a correlation exists between the deformation temperature and the percentage of Al, Si and Fe present in coal. The logging tool employed a 2.5 μg 252Cf neutron source.
Let R be a commutative unitary ring. Recently, several authors have been interested in the preservation of seminormality in passing from R to the polynomial ring R [X, ,..., X,,J. In this paper we give a proof in the case of power series rings and the proof is both short and applicable in the polynomial setting. The chief difficulty lies in proving that “relative” stability is preserved in passage to the power series ring. We briefly recall the pertinent terminology. If R is a ring, then following Swan [3] we say that R is seminormal if whenever b, c E R satisfy b3 = c2 there is an element a E R such that a2 = b and a3 = c. If R c T are rings, then R is said to be seminormal in T if and only if whenever u E T with u*, a3 E R, it follows that a E R. It can be seen that this is equivalent to saying that whenever (x E T with an, cP+ l,..., E R for some positive integer n, it follows that (r E R. Indeed, if R is seminormal in T and if n is taken to be minimal so that on, on+‘,..., E R, then (u”~‘)“, (c?‘)~ E R unless n = 1, in which case u E R, anyway. Control SV40- transformed human fibroblasts can be readily adapted to growth on medium containing galactose as sole hexose source (galactose-MEM). However, most cells from a line of SV40- transformed skin fibroblasts from a patient with galactosemia (galactose- 1- phosphate Uridylyltransferase (GALT) deficiency) died in galactose-MEM. Surviving cells of this line either grew in completely sugar-free media or had acquired significant amounts of GALT activity. Two presumptive revenant cell lines with GALT activity were characterized in detail. The expression of GALT in these two lines was stable in nonselective conditions. Each had different reaction maximum velocities with respect to uridine diphosphoglucose (UDPG) concentration as compared to residual activity in the parental cell strain or control cells. Both appeared to demonstrate heat-inactivation profiles for GALT that differed from the parental cells or controls. UDPG concentration was found to significantly alter the thermostability of GALT. A competitive radioimmunnoassay for GALT showed that these two lines had amounts of the GALT protein comparable to that of the parental cell strain or control cells. The electrophoretic mobility of GALT from the two presumptive revertants was found to differ from control cells. It was concluded that structural gene changes were probably responsible for the apparent reversion in these lines.
A new human diploid fibroblast-like cell line has been established from lung tissue of a female fetus. This has been frozen away in large quantity and characterized for use in research and related purposes. This is the first of a planned series of human cell lines to be established, characterized, and banked in large quantity in support of the National Institute on Aging research and general cell biology.
A specific interaction Lagrangian for an isotopic doublet boson is proposed which, to the first approximation in a perturbation expansion, can account for the lifetimes and branching ratios in K-π decays. Since parity is rigorously conserved in all K-π interactions, the K-meson is described as a parity doublet. The total Lagrangian is invariant under charge conjugation and time reflection. The strong interaction, of the form gSϕK†σ†χKχπ, is a scalar in isotopic space and is invariant under parity conjugation (in order to have scalar and pseudo-scalar K masses equal). The weak interaction, of the form gW(σ + α σ0†χKχπ, allows a change in total isotopic spin of 12 or 32. The final-state pions are considered to be in plane-wave states. The following dimensionless constants are obtained: gS = 1.6; gW = 5 × 10−8; α = −0.16. No account is taken of any of the K-decay modes involving neutrinos, nor of the production or scattering properties of the K-particles. The eight theoretically distinct K-mesons are reduced in number to the four actually distinguishable in K-decays.
A general, first-order, linear, relativistic wave equation for a particle of arbitrary half-integral maximum spin is presented. The structure of the general theory is described briefly, while its specialization to a particle of maximum spin one is treated at greater length. The special theory describes not only the simple scalar (S), pseudoscalar (P), vector (V), and axial-vector (A) mesons, but also composite “particles” possessing S, P, V, and A components, all components differing in rest mass. The restriction to unique rest mass allows the wave function to satisfy the Klein-Gordon equation. The restriction to a positive-definite energy density eliminates all composite particles except the S-P, the S-A, and the P-V. The theory is invariant under the extended Lorentz group, charge conjugation, and Hermitian conjugation.
