OBJECTIVE To analyze the effect of CD56 expression on the prognosis of newly diagnosed multiple myeloma (MM) patients and explore the relationship between CD56 with clinical characteristics. METHODS In this retrospective study, the clinical data and laboratory parameters of 175 newly diagnosed MM patients from February 2015 to December 2020 in the Second Hospital of Anhui Medical University were collected. The patients were divided into CD56+ and CD56- groups based on the expression of CD56, and the general data and laboratory parameters of the two groups were compared. The patients were followed up to June 30, 2021, and progression-free survival (PFS) and overall survival (OS) were recorded. PFS and OS curves of the two groups were plotted respectively, and the survival differences were compared. Univariate and multivariate Cox regression analyses were performed to analyze the effect of CD56 on the prognosis of newly diagnosed MM patients. RESULTS In 175 newly diagnosed MM patients, 57(32.6%) cases were in the CD56-group and 118 (67.4%) cases in the CD56+ group. There was significant correlation between CD56 expression and ISS stage, ECOG score, platelets, β2-microglobulin, creatinine, and extramedullary disease (all P <0.05). The incidence of extramedullary disease in the CD56- group was significantly higher than that in the CD56+ group (29.8% vs 12.7%, P =0.006). The median follow-up time of the whole cohort was 23.6 (1.0-78.6) months. The median PFS of patients in CD56+ group and CD56- group were 18.6 (1.2-77.6) and 12.2 (1.0-49.0) months, respectively, and the median OS of the two groups were 27.6 (1.4-77.7) and 19.7 (1.0-78.6) months, respectively. The 2-year PFS rate in the CD56+ group was significantly higher than that in the CD56- group (57.6% vs 36.8%, P =0.010), and the 2-year OS rate in the CD56+ group was higher than that in the CD56- group, but it didn't reach statistical significance (74.6% vs 64.9%, P =0.158). The results of univariate Cox regression analysis showed that the PFS was significantly shorter in newly diagnosed MM patients with advanced age, type IgG, high ECOG score, decreased platelet count, increased lactate dehydrogenase level, extramedullary disease, and CD56- (all P <0.05), the OS was significantly shorter in patients with high ECOG score, decreased platelet count, increased lactate dehydrogenase level, extramedullary disease, and CD56- (all P <0.05). The results of multivariate Cox regression analysis showed that advanced age, type IgG, elevated lactate dehydrogenase level, extramedullary disease, and CD56- were independent prognostic factors for poor PFS (all P <0.05); and decreased platelet count, elevated lactate dehydrogenase level, and extramedullary disease were independent adverse prognostic factors for OS (all P <0.05), while there was no significant independent correlation between CD56 and OS (P >0.05). CONCLUSION Most of the newly diagnosed MM patients have positive expression of CD56. Loss of CD56 expression was associated with unfavorable biological and clinical parameters and poor prognosis, suggesting that CD56 has important clinical value in the prognosis of newly diagnosed MM patients.
Objective:To investigate the relationship between metabolic parameters of 18F-FDG PET-CT and clinical characteristics in newly diagnosed patients with multiple myeloma (MM). Methods:The clinical data of 47 MM patients who underwent 18F-FDG PET-CT at initial diagnosis from June 2018 to December 2020 in the Second Affiliated Hospital of Anhui Medical University were retrospectively analyzed. The association of metabolic parameters of 18F-FDG PET-CT with the following clinical parameters including age, gender, disease classification, DS stage, international staging system (ISS) stage, hemoglobin, albumin, serum calcium, serum crearinine, lactate dehydrogenase, β 2-microglobulin, high-sensitivity C-reactive protein (hs-CRP), bone marrow plasma cell ratio, the number of lesions and extramedullary lesions. Results:Among 47 MM patients, in terms of tumor metabolic volume (MTV), patients at DS stage Ⅲ was higher than those at stage Ⅰ-Ⅱ [172.88 (42.69, 391.55) vs. 19.63 (4.24, 42.61), P < 0.001], patients with serum calcium ≥ 2.65 mmol/L was higher than those with serum calcium <2.65 mmol/L [310.71 (99.68, 549.62) vs. 58.00 (19.63, 248.34), P = 0.038], patients with hs-CRP ≥ 3.3 mg/L was higher than those hs-CRP < 3.3 mg/L [211.39 (57.79, 397.70) vs. 35.34 (7.91, 153.02), P = 0.002], and patients with the number of lesions >3 was higher than those with the number of lesions ≤ 3 [211.39 (57.79, 393.02) vs. 16.43 (5.12, 38.23), P < 0.001]. In terms of total lesion glycolysis (TLG) value, patients at DS stage Ⅲ was higher than those at stage Ⅰ-Ⅱ [460.44 (92.62, 1 113.85) vs. 46.68 (9.32, 89.89), P = 0.001], patients with bone marrow plasma cell ratio <10% was higher than those with bone marrow plasma cell ratio ≥10% [1 039.36 (615.28, 1 167.80) vs. 150.80 (45.36, 821.01), P = 0.031], patients with serum calcium ≥ 2.65 mmol/L was higher than those with serum calcium < 2.65 mmol/L [1 031.55 (251.87, 2 115.98) vs. 177.59 (45.82, 761.66), P = 0.033], patients with hs-CRP ≥ 3.3 mg/L was higher than those hs-CRP < 3.3 mg/L [487.40 (141.38, 1 107.02) vs. 63.44 (19.40, 634.31), P = 0.003], patients with lesion number >3 was higher than those with lesion number ≤3 [625.32 (150.80, 1 134.35) vs. 40.04 (10.96, 70.88), P < 0.001]. There were no statistically significant differences in the maximum standardized uptake value (SUV max) and mean standardized uptake value (SUV mean) among different clinical parameter groups (all P > 0.05). Conclusion:MTV and TLG in 18F-FDG PET-CT metabolic parameters can better reflect the characteristics of MM patients compared with SUV max and SUV mean.
目的:调查中老年患者脑出血后急性期深静脉血栓形成(DVT)的发病率,并探讨DVT的影响因素.方法:选择2019年9月至2021年5月在温州医科大学附属第一医院就诊且发病24 h内住院的脑出血患者245例,在患者脑出血后10~14 d内进行双下肢静脉超声检查,得出脑出血后急性期DVT发病率.通过单因素分析和二元Logistic回归分析中老年患者脑出血后并发DVT的影响因素.结果:245例中老年脑出血患者中79例患者发生DVT,发病率为32.24%,其中39例(49.37%)无DVT的临床症状.二元Logistic回归分析显示,年龄≥70岁(OR=1.73,95%CI=1.12~2.98)、下肢美国国立卫生研究院卒中量表评分≥3分(OR=4.13,95%CI=2.15~8.97)、连续卧床时间≥3 d(OR=4.73,95%CI=2.25~9.78)、Wells评分≥2分(OR=3.54,95%CI=1.62~6.67)、入院时D-二聚体高(OR=3.06,95%CI=1.01~3.49)是脑出血后急性期DVT发生的独立危险因素,早期康复治疗是DVT发生的保护因素(OR=0.55,95%CI=0.28~0.96).结论:中老年人脑出血后DVT发生率较高,接近一半的患者无DVT临床症状.中老年急性期卒中患者发生DVT的独立危险因素较多,建议及时对存在危险因素的患者进行DVT监测和干预,并结合脑出血早期康复治疗以减少DVT的发生.
目的 分析外周血血小板计数与淋巴细胞比值(PLR)在多发性骨髓瘤患者预后中的价值.方法 收集2015年1月-2019年9月安徽医科大学第二附属医院收治的138例初诊多发性骨髓瘤患者的临床资料,依据受试者工作特征(ROC)曲线确定PLR的最佳截断值并将患者分为高PLR组和低PLR组,比较两组一般资料及实验室指标,并分析PLR在预后中的价值.结果ROC曲线分析显示,RLR预测多发性骨髓瘤生存最佳截断值为133.88,其中AUC为0.600,灵敏度46.60%,特异度76.60%.高PLR组和低PLR组分型、有无髓外疾病比较,差异有统计学意义(P<0.05).138例多发性骨髓瘤患者共58例死亡,死亡率为42.03%;高PLR组中位OS为29个月(1~71个月),低PLR组中位OS为27个月(1~79个月),组间比较,差异有统计学意义(P<0.05);单因素Cox分析显示,EOCG评分、血小板计数、PLR水平、乳酸脱氢酶、髓外病变与多发性骨髓瘤患者OS有关;多因素Cox分析显示,血小板计数<100×109/L、PLR≥133.88、乳酸脱氢酶≥250 U/L、髓外病变是影响多发性骨髓瘤患者OS的独立不良预后因素.结论 初诊时外周血PLR与多发性骨髓瘤髓外病变相关,对预后具有一定的预测价值,是多发性骨髓瘤患者OS的独立预后预测指标.
Abstract Background Multiple myeloma is a hematologic disorder of abnormal plasma cell proliferation. Although there are some agents with different mechanisms in the clinic, the treatment of multiple myeloma is still challenging for the reason that its recurrence and progression are common. Therefore, it is critical to determine novel biomarkers to improve the prognosis of patients. Methods Firstly, raw data of GSE82307 was collected from the Gene Expression Omnibus database. Secondly, the top 50% of most variant genes were employed to construct a gene co-expression network in the R.WGCNA algorithm, and module significance and module membership were utilized to identify hub modules and hub genes respectively. The gene ontology enrichment and Kyoto encyclopedia of genes and genomes pathway analysis were carried out to assess biological characteristics. Then a protein-protein interaction network was conducted based on the STRING website and Cytoscape software. Next, differentially expressed genes were analyzed using the limma R package. Finally, survival analysis was performed by Kaplan–Meier plotter to evaluate prognosis. Results 10826 genes were used to construct a co-expression network. In this network, the blue module was identified as a hub module in which 68 genes were identified as hub genes. Furthermore, 46 differentially expressed genes were screened in samples of GSE82307. Integrating hub genes and differentially expressed genes, we determined 14 key genes. Finally, survival analysis revealed that ten genes CDCA5, CEP55, HJURP, CDC20, FOXM1, RRM2, TTK, CENPE, SKA1, NUF2 were related to the relapse and prognosis of multiple myeloma. Conclusion Our study suggested that CDCA5, CEP55, HJURP, CDC20, FOXM1, RRM2, TTK, CENPE, SKA1, NUF2 may be potential biomarkers for predicting the relapse and prognosis of multiple myeloma.
A commercial viable solid-state direct current circuit breaker with fast switching performance for the use in the 1-100kV range requires a low loss on-resistance close to commercial available mechanical breakers [1]. Diamond provides excellent semiconductor properties for those breaker applications with a material characteristics superior in high electric field breakdown strength, higher thermal conductivity, and high charge carrier mobility as compared to silicon, silicon carbide, and gallium nitride [2-4]. These properties will enable diamond electronic devices that will be more energy efficient while keeping the active area of devices to lower values, which offers a potential cost advantage in case of a better availability of large-area substrates [5,6]. Diamond diodes [7-9], diamond field effect transistors [10], and gate turn-off thyristor devices [11-13] are proposed to be the core unit in a medium voltage direct current (MVDC) solid-state circuit breaker [14,15]. A reduced energy loss in solid-state breakers by using diamond will enable a competitive placement of diamond breakers on the MVDC market with a decisive advantage of a much faster response to electrical circuit faults and more robust DC electrical energy delivery systems with >1MW capacity. The diamond-based diode and FET development focuses on devices in the 1-5kV range and the diamond thyristors cover a 15-20kV range. Higher voltages would ultimately be achieved with series circuit arrangements of the diamond devices. Diamond offers also excellent properties for applications in the biomedical sector [16]. The manufacturing of tailored biopharmaceuticals, stem cells, or human tissue is being attempted through the utilization of single-use perfusion bioreactors due to its minimal spacial and financial costs, and its ability to execute critical parallel processing methods [17,18]. However, continuous monitoring of analytes in multifaceted protein mixtures and achieving full automation of production are challenges inhibiting commercialization of tailored products to a reasonable price. Constant and instantaneous monitoring of a bioreactor both ensures flexibility with the opportunity for quick adjustments and eliminates contamination risks from manual sampling. Also, it is essential that the bioactive layer of an integrated biosensor monitoring system sustains the lifetime of the processing cycle, as well as display a shelf life compatible with standard inventory consumption. Achieving and maintaining the specific conditions necessary to produce complex bio-products requires a robust real-time monitoring with redundancy and automated control of a variety of nutrients, intermediate metabolites, and cell products within the bioreactor. Diamond biosensors exhibit low biofouling rates and show promising results in the in-situ monitoring of the bioprocess conditions for the full operation time of single-use bioreactors [19]. Operational human IL-8 antibodies have been successfully adhered to the diamond sensor surface [20]. A high antibody concentration and activity as a measure of biosensor sensitivity is obtained using a modified ELISA procedure. Modular Multilevel Converter Based High Voltage Direct Current has been paid wide attention and engineering application due to its technological advantages and shares great development prosperity. The analysis and solution to solve the suppression of the capacitance voltage fluctuation is the key part to determine operating properties of MMC. This paper firstly analyzes the structure of MMC based on its working principle, and the principle of capacitance voltage fluctuation of MMC. Then the paper comes up with the derived model of the capacitance voltage fluctuation based on modulation ratio in order to realize the control of capacitance voltage fluctuation control by modulation ratio control. Finally, after conducting research on the relationship between the capacitance voltage fluctuation, modulation ratio and the optimum percentage of second-order circulating current injection, this paper comes up with the optimum application condition when the second-order circulating current should be injected or isolated to best suppress the capacitance voltage fluctuation and bridge arm current respectively under sine wave modulation and three-order harmonic wave modulation.
Conventional automatic clothes hangers in real practice all fail to free from manpower that they still need to be controlled by human and they have no integrated functions. In this paper, a weather auto-inductive and multifunctional clothes hanger is proposed and designed, and its prototype is finally designed and produced in software and hardware to prove the adaptability of this paper, which has met the goal to realize the full automatic self-control based on the changing weather and brings a lot conveniences to people in high-speed developing society nowadays.
The system in the paper based on Matlab platform. With the aid of image processing toolbox of soil image analysis and processing, the soil grain size distribution and its inclination angle can be got. It overcomes the insufficiency of the existing image edge extraction method, and proposes a new type of detection method, in order that the region of interest can be more accurately extract. The system is helpful to predict the possibility of the regional landslides.
Traditional matrix algorithm endures advantages of low requirements in FTU, high accuracy in Single-supply and radiation distribution network and so on. However, it will meet with erroneous judgment when it is used traditional matrix algorithm as long as it inserts with distributed generator. This article mainly focuses on analyzing the limitation of uses of implementation rational, which deals with the implementation of the matrix algorithm in the fault location algorithms in distribution network, applied in the distribution network with distributed generation. Then realizing a modified T-matrix algorithm based on these problems remained to be solved.
The interference on the geo-electrical resistivity observation from DC power lines is a key issue puzzling seismic monitoring. According to the working principle of geoelectrical resistivity observation instrument,the reason of interference on geoelectrical resistivity observation is analyzed.The calculation formulas of potential in soil, which is represented as horizontal two-layer electrical model, are derived by equivalent complex image method which responding to the ground electrodes of DC power line with various models: one vertical ground electrode, horizontal double-ring ground electrode and four vertical ground electrodes.In this way, the values of interference on geoelectrical resistivity observation from DC power lines under various situations are calculated and researched.The results show that the value of interference could be reduced 50% for horizontal double-ring ground electrode, and four vertical ground electrodes could be reduced 75%.
