La pneumatose gastrique (PG) est une affection rare. Sa survenue en période néonatale doit faire rechercher une entérocolite ulcéro-nécrosante dans un contexte de prématurité ou un obstacle chirurgical sous-jacent. Nous rapportons un cas de PG chez un nouveau-né à terme, admis au 3e jour de vie pour une occlusion néonatale à ventre plat. La radiographie de l’abdomen sans préparation avait montré une importante distension gastrique avec un aspect de PG et une image en double bulle. L’exploration chirurgicale avait mis en évidence une PG, une atrésie duodénale complète siégeant au niveau de la seconde portion duodénanle (D2) et un pancréas annulaire. L’évolution avait été favorable après duodénostomie.Gastric pneumatosis is a rare pathology. Its occurrence in the neonatal period requires looking for ulcerative-necrotizing enterocolitis in a context of prematurity or an underlying surgical obstacle. We report a case of gastric pneumatosis at a newborn child born at term, admitted on the third day of life for neonatal occlusion with a flat stomach. The abdomen without preparation showed substantial gastric distension with aspects of gastric pneumatosis and embellishes with images showing a double gastric bubble. The surgical exploration showed gastric pneumatosis, complete duodenal atresia located at the level of the second duodenal portion, and an annular pancreas. Progression was favorable after duodenostomy.

Patients affected by type I Gaucher disease (an autosomal recessive inheritance lysosome storage disorder) develop nodular splenomegaly in 20 to 30% of cases where imiglucerase therapy proves ineffective. The lack of response to imiglucerase therapy on spleen nodules could be an indication of the existence or development of a malignant spleen. We report a 47-year-old man with Gaucher disease who presented with a voluminous splenic nodule, in whom therapy was delayed.
