PURPOSE: To report an inter-eye difference of the ocular pulse amplitude (OPA) in a case of innominate steal syndrome, as measured by recording applanation tonom etry.METHODS: OPA was measured in a 49-year,old male before and after percutaneous transluminal angioplasty of the innominate artery.RESULTS: Before dilation of the stenotic innominate artery, OPA was 2.00 (+/- 0.49) mm Hg in the right and 3.46 (+/- 0.53) mm Hg in the left eye. After dilation, OPA was 3.26 (+/- 0.5 1) mm Hg in the right eye and 3.25 (+/- 0.99) mm Hg in the left eye.CONCLUSION: Recording applanation tonometry may be useful for identifying occlusive disease of extracranial vessels in an office setting.
Zusammenfassung Hintergrund. Endoskope werden zunehmend in der Diagnostik und Therapie von Tränenwegpathologien eingesetzt. Die vorliegende Untersuchung soll Ergebnisse der Tränenwegendoskopie mit einem Gradientenindex-(GRIN-)Endoskop darstellen. Patienten und Methode. 44 Patienten im Alter zwischen 18 und 87 Jahren (Duchschnittsalter 59 Jahre +/−18,9) mit chronischer Epiphora oder Symptomen von Tränenwegaffektionen wurden mit einem starren GRIN-Endoskop untersucht. Die Untersuchung erfolgte ambulant in Tropfanästhesie. Ergebnisse. Alle 44 Patienten wiesen eine Pathologie im Bereich der abführenden Tränenwege auf. 2 Patienten wiesen chronische Veränderungen ohne Stenosen auf, 25 Patienten eine relative und 17 Patienten eine totale Stenose. Präsakkal waren 16 relative und 12 totale Stenosen. Postkanalikulär konnten 9 relative und 5 totale Stenosen nachgewiesen werden. Einseitige Pathologien fanden sich bei 33 Patienten und beidseitige Befunde bei 11 Patienten. Das verwendete GRIN-Endoskop konnte bei allen Patienten eingesetzt werden und ermöglichte eine gute Bildqualität der entsprechenden Pathologien. Schlussfolgerung. Untersuchungen mit einem starren GRIN-Endoskop konnen das Spektrum der Diagnostik und Therapie von Pathologien im Bereich der ableitenden Tränenwege erweitern. In dem vorliegenden Patientengut ( n =44) sind die präsakkalen Pathologien ( n =28) doppelt so häufig wie die postkanalikulären ( n =14). Weitere Entwicklungen des Gerätes werden die Indikation erweitern.
PURPOSE To establish a standard clinical procedure for measuring intraocular pressure (Po) with the contact lens tonometer (CLT), to demonstrate possibilities for analyzing ocular pulsation and performing ophthalmodynamometry. METHODS A reliable histogram-based analyzing system for determining Po is used. First, the ocular pressure was registered with the CLT method and compared with the Goldmann applanation pressure, measured immediately beforehand. Second, ocular pulsation was studied by recording Po for 30 to 70 seconds, and the pulse amplitude was then analyzed. Third, an ophthalmodynamometry method during slit-lamp examination was tested. The central retinal artery was observed through the contact lens while the appositional force was elevated. A mark was set at the systolic and diastolic pressure while observing arterial pulsation (similar to the Korotkoff sounds). RESULTS Compared with the Goldmann method measurements, Po obtained with the CLT method yielded a linear regression of r = 0.7 (right eye) and r = 0.68 (left eye), and was therefore highly significant (P < 0.0001, two-tailed). Analysis of the pulse amplitude showed great variability (range, 1 to 9 mm Hg; means, 2.9 mm Hg [right eye] and 3.0 mm Hg [left eye]). The dependence of the pulse amplitude on Po and age was shown with a correlation coefficient of r = 0.55 and r = 0.59, respectively. The difference between the right and left eye of the person was < 0.5 mm Hg. CONCLUSION The CLT can be used during slit-lamp examination and permits tonometry during ophthalmoscopy, ocular pulsation assessment at different intraocular pressures, and ophthalmodynamometry.
