A human biomonitoring study was conducted to assess the exposure of Hungarian children aged 8-11 years to ten phthalate esters (PEs) and DINCH between 2017 and 2018. In addition to collecting urine samples from 262 participants, a questionnaire was completed by the parents or legal guardians to identify potential determinants of exposure. The highest geometric mean concentration was observed for MiBP, followed by MBP, cx- MEHP, OH-MEHP and MEP. Three out of the four DINCH metabolites were detected in more than 90% of the samples. The comparison of the urinary concentrations measured in this study with those observed in the DEMOCOPHES study revealed a significant decreasing trend in all PE metabolites investigated in both studies between 2011/2012 and 2017/2018. Different approaches were used to assess the health risks associated with the exposure to PEs and DINCH. Our results highlighted that the hazard index (HI) values were higher than 1 in 17.6% of the children when the human biomonitoring guidance values were applied. In contrast, less than 3% of the children had HI values exceeding 1 when other sources of reference values were used. By applying a safety factor of 10 for the risk assessment, 17.6-91.6% of the children were characterized by HI values higher than 0.1, indicating the need for risk reduction measures. Overall, DnBP, DiBP and DEHP were identified as the main drivers of the mixture risk. Although PEs and DINCH are ubiquitous contaminants, there are still inconsistencies and gaps in our understanding of the determinants of exposure. The results of the multivariate regression analysis showed significant associations between PE or DINCH metabolite concentrations and certain individual characteristics, use of personal care products, home and school environment and food and beverages consumption 24 h prior to sample collection.
Phthalates are mainly used as plasticizers for polyvinyl chloride (PVC). Exposure to several phthalates is associated with different adverse effects most prominently on the development of reproductive functions. The HBM4EU Aligned Studies (2014-2021) have investigated current European exposure to ten phthalates (DEP, BBzP, DiBP, DnBP, DCHP, DnPeP, DEHP, DiNP, DiDP, DnOP) and the substitute DINCH to answer the open policy relevant questions which were defined by HBM4EU partner countries and EU institutions as the starting point of the programme. The exposure dataset includes similar to 5,600 children (6-11 years) and adolescents (12-18 years) from up to 12 countries per age group and covering the North, East, South and West European regions. Study data from participating studies were harmonised with respect to sample size and selection of participants, selection of biomarkers, and quality and comparability of analytical results to provide a comparable perspective of European exposure. Phthalate and DINCH exposure were deduced from urinary excretions of metabolites, where concentrations were expressed as their key descriptor geometric mean (GM) and 95th percentile (P95). This study aims at reporting current exposure levels and differences in these between European studies and regions, as well as comparisons to human biomonitoring guidance values (HBM-GVs). GMs for children were highest for Sigma DEHP metabolites (33.6 mu g/L), MiBP (26.6 mu g/L), and MEP (24.4 mu g/L) and lowest for Sigma DiDP metabolites (1.91 mu g/L) and Sigma DINCH metabolites (3.57 mu g/L). In adolescents highest GMs were found for MEP (43.3 mu g/L), Sigma DEHP metabolites (28.8 mu g/L), and MiBP (25.6 mu g/L) and lowest for Sigma DiDP metabolites (= 2.02 mu g/L) and Sigma DINCH metabolites (2.51 mu g/L). In addition, GMs and P95 stratified by European region, sex, household education level, and degree of urbanization are presented. Differences in average biomarker concentrations between sampling sites (data collections) ranged from factor 2 to 9. Compared to the European average, children in the sampling sites OCC (Denmark), InAirQ (Hungary), and SPECIMEn (The Netherlands) had the lowest concentrations across all metabolites and ESTEBAN (France), NAC II (Italy), and CROME (Greece) the highest. For adolescents, comparably higher metabolite concentrations were found in NEB II (Norway), PCB cohort (Slovakia), and ESTEBAN (France), and lower concentrations in POLAES (Poland), FLEHS IV (Belgium), and GerES V-sub (Germany). Multivariate analyses (Survey Generalized Linear Models) indicate compound-specific differences in average metabolite concentrations between the four European regions. Comparison of individual levels with HBM-GVs revealed highest rates of exceedances for DnBP and DiBP, with up to 3 and 5%, respectively, in children and adolescents. No exceedances were observed for DEP and DINCH. With our results we provide current, detailed, and comparable data on exposure to phthalates in children and - for the first time - in adolescents, and - for the first time - on DINCH in children and adolescents of all four regions of Europe which are particularly suited to inform exposure and risk assessment and answer open policy relevant questions.
As one of the core elements of the European Human Biomonitoring Initiative (HBM4EU) a human biomonitoring (HBM) survey was conducted in 23 countries to generate EU-wide comparable HBM data. This survey has built on existing HBM capacity in Europe by aligning national or regional HBM studies, referred to as the HBM4EU Aligned Studies. The HBM4EU Aligned Studies included a total of 10,795 participants of three age groups: (i) 3,576 children aged 6-12 years, (ii) 3,117 teenagers aged 12-18 years and (iii) 4,102 young adults aged 20-39 years. The participants were recruited between 2014 and 2021 in 11-12 countries per age group, geographically distributed across Europe. Depending on the age group, internal exposure to phthalates and the substitute DINCH, halogenated and organophosphorus flame retardants, per-and polyfluoroalkyl substances (PFASs), cadmium, bisphenols, polycyclic aromatic hydrocarbons (PAHs), arsenic species, acrylamide, mycotoxins (deoxynivalenol (total DON)), benzophenones and selected pesticides was assessed by measuring substance specific biomarkers subjected to stringent quality control programs for chemical analysis. For substance groups analyzed in different age groups higher average exposure levels were observed in the youngest age group, i.e., phthalates/DINCH in children versus teenagers, acrylamide and pesticides in children versus adults, benzo-phenones in teenagers versus adults. Many biomarkers in teenagers and adults varied significantly according to educational attainment, with higher exposure levels of bisphenols, phthalates, benzophenones, PAHs and acrylamide in participants (from households) with lower educational attainment, while teenagers from house-holds with higher educational attainment have higher exposure levels for PFASs and arsenic. In children, a social gradient was only observed for the non-specific pyrethroid metabolite 3-PBA and di-isodecyl phthalate (DiDP), with higher levels in children from households with higher educational attainment. Geographical variations were seen for all exposure biomarkers. For 15 biomarkers, the available health-based HBM guidance values were exceeded with highest exceedance rates for toxicologically relevant arsenic in teenagers (40%), 3-PBA in children (36%), and between 11 and 14% for total DON, Sigma (PFOA + PFNA + PFHxS + PFOS), bisphenol S and cadmium. The infrastructure and harmonized approach succeeded in obtaining comparable European wide internal exposure data for a prioritized set of 11 chemical groups. These data serve as a reference for comparison at the global level, provide a baseline to compare the efficacy of the European Commission's chemical strategy for sustainability and will give leverage to national policy makers for the implementation of targeted measures.
A jódhiány továbbra is jelentős közegészségügyi problémát jelent világszerte; különösen a várandósok és a kisgyermekek veszélyeztettek. Ugyanakkor mind az elégtelen, mind a túlzott jódbevitel is károsan befolyásolhatja a pajzsmirigy működését. A szervezet jódellátottságát a nemzetközi ajánlások alapján a vizelet jódkoncentráció (UIC) alapján lehet vizsgálni, ami nemcsak a jódhiány kimutatására, hanem a túlzott bevitel felderítésére is alkalmas módszer. Ezen ismeretek birtokában, rendszeres humán biomonitoring vizsgálatok (HBM) végzésével az adott populáció aktuális ellátottsági szintjéhez lehet igazítani a jódbevitel mértékét, így nemcsak a jódhiány, hanem a túlzott jódellátottság okozta betegségteher is megelőzhető. Az InAirQ projekt (2016-2019) keretében 9 magyarországi település 23 kiválasztott iskolájában, összesen 393 9-11 éves gyermektől gyűjtöttünk vizeletet egy alkalommal. A vizeletminták jód koncentrációját Sandell-Kolthoff reakción alapuló spektrofotometriás módszerrel határoztuk meg. A Jász-Nagykun-Szolnok megyei települések iskoláiban a vizelet mintavételekkel egy időben több ponton vett ivóvízminták jodid koncentrációját is mértük a Nemzeti Népegészségügyi Központ (NNK) laboratóriumában. Az analitikai méréseket kérdőíves felmérés egészítette ki, melynek célja a jódbevitel lehetséges forrásainak azonosítása volt. A karcagi és Karcag környéki településeken élő gyermekek vizeletének medián jód koncentrációja 629 µg/l (95% CI: 557-667); ez a WHO definíciója szerint túlzott jódellátottságot jelez, ugyanakkor az egyéb településeken élő, vizsgálatba bevont gyermekek esetében a medián vizelet jódkoncentráció 165,5 µg/l (95% CI: 148-186) volt, ami megfelelő jódellátottságra utal. A Karcagon és a környező településeken vett ivóvízmintákban átlagosan 540 µg/l jodid- koncentrációt mértünk, 580 µg/l mediánnal. A többi település esetében, az NNK 2017. évben végzett országos felmérése alapján 20 µg/l ivóvíz jodidion tartalommal számoltunk. Az átlagos vízfogyasztással súlyozott ivóvíz jodidion-koncentráció és UIC között szignifikáns pozitív összefüggést (R=0,83, p<0,0001) találtunk. Ezen eredmény és a kérdőívek elemzése alapján valószínűsíthető, hogy Karcagon és a környező településeken a magas jódtartalmú ivóvíz- fogyasztás következménye az itt élő gyermekek emelkedett vizeletjód koncentrációja. Az érintett településeken élő gyermekek esetében javasoljuk az étrendi jódbevitel mérséklését, valamint az esetleges jódpótlás megszüntetését. Meg kell vizsgálni továbbá az ivóvíz jodidkoncentráció csökkentésének a lehetőségét, míg az ország egyéb részén élő gyermekek esetében a jódozott só szélesebb körű felhasználását kell elérni. Az aktuális jódellátottsági helyzet értékelése érdekében továbbra is javasoljuk a rendszeres, egész országra kiterjedő szűrővizsgálatok, illetve HBM vizsgálatok végzését (iskolai szűrések, pajzsmirigy-, vizelet és vérvizsgálatok, várandósok, szoptató anyák vizsgálata stb.).
The indoor air quality (IAQ) was investigated in sixty-four primary school buildings in five Central European countries (Czech Republic, Hungary, Italy, Poland, and Slovenia). The concentration of volatile organic compounds, aldehydes, PM2.5 mass, carbon dioxide, radon, as well as physical parameters were investigated during the heating period of 2017/2018. Significant differences were identified for the majority of the investigated IAQ parameters across the countries. The median indoor/outdoor ratios varied considerably. A comprehensive evaluation of IAQ in terms of potential health effects and comfort perception was performed. Hazard quotient values were below the threshold value of 1 with one exception. In contrast, 31% of the school buildings were characterized by hazard index values higher than 1. The maximum cumulative ratio approach highlighted that the concern for non-carcinogenic health effects was either low or the health risk was driven by more substances. The median excess lifetime cancer risk values exceeded the acceptable value of 1 × 10-6 in the case of radon and formaldehyde. PM2.5 mass concentration values exceeded the 24 h and annual guideline values set by the World Health Organization in 56 and 85% of the cases, respectively. About 80% of the schools could not manage to comply with the recommended concentration value for carbon dioxide (1000 ppm).
A Nemzeti Népegészségügyi Központ konzorciumvezetőként részt vett az InAirQ elnevezésű nemzetközi projektben, melynek célja, többek között, a beltéri levegőminőség vizsgálata általános iskolákban, illetve olyan akciótervek kidolgozása, melyekkel a beltéri levegő minősége javítható. Egy kampány keretében összesen hatvannégy általános iskola egy-egy osztálytermét vizsgáltuk a Cseh Köztársaságban (n = 12), Magyarországon (n = 16), Olaszországban (n = 12), Lengyelországban (n = 12) és Szlovéniában (n = 12). A bel- és kültéri levegőminőség vizsgálatára irányuló mintavételeket a 2017/2018-as fűtési időszakban végeztük a kiválasztott iskolaépületeknél. A légszennyezők közül meghatároztuk számos aldehid és illékony szerves vegyület, a kisméretű aeroszol részecskék (PM2.5), a szén-dioxid és a radon koncentrációját. A felmérés ideje alatt néhány fizikai paraméter (hőmérséklet, relatív páratartalom) alakulását is nyomon követtük. Jelentős eltéréseket tapasztaltunk számos légszennyező anyag esetén a felmérésben résztvevő országok között. A benzol koncentrációja a vizsgált iskolák 37%-ában meghaladta az Európai Bizottság által, a kültéri környezetre meghatározott éves határértéket (5 μg/m3 ). Azonban a beltéri benzol koncentráció minden esetben a kültéri értékkel közel azonos volt, így kijelenthető, hogy a benzolnak nincs jelentős beltéri forrása. A formaldehid koncentrációja beltéren minden esetben meghaladta a kültéri értéket, azonban egyik esetben sem érte el az Egészségügyi Világszervezet (WHO) által javasolt irányértéket (100 μg/m3 ). A PM2.5 tömegkoncentráció az iskolák 56%-ában meghaladta a WHO által megállapított 24 órás irányértéket (25 μg/m3 ), illetve az iskolák 85%-ában az éves (10 μg/m3 ) irányértéket meghaladó értékeket mértünk. A vizsgált osztálytermekben a szén-dioxid koncentrációja több esetben is magas volt (az átlag meghaladta a 1000 ppm értéket), jelezve az elégtelen szellőztetést. A komfortérzetet befolyásoló paraméterek közül a relatív páratartalom sok esetben nagyon alacsony volt. A radon koncentrációja 100 Bq/m3 körül mozgott, kivéve néhány osztálytermet Magyarországon, Lengyelországban és Szlovéniában.

The National Public Health Center participated as a consortium leader in the international project called InAirQ, aiming to investigate the indoor air quality (IAQ) and to take actions to improve the indoor environment in primary school buildings. In total, sixty-four primary school buildings were investigated in the Czech Republic (n = 12), Hungary (n = 16), Italy (n = 12), Poland (n = 12), and Slovenia (n = 12). The air quality was investigated during the heating period of 2017/2018. The concentration of volatile organic compounds, aldehydes, PM2.5 mass, carbon dioxide, radon were investigated. Physical parameters (e.g. temperature, relative humidity) were also monitored. Significant differences were identified for the majority of the investigated IAQ parameters across the countries. The concentration of benzene was higher than 5 μg m-3, the annual limit value set for the ambient air by the European Commission, in 37% of the investigated school buildings. It must be noted that the concentration of benzene indoors was similar to that obtained for outdoors in all cases which indicates the lack of indoor sources. The concentration of formaldehyde was always higher indoors than outdoors, although the concentration of formaldehyde never reached the WHO guideline (100 μg m-3). The PM2.5 mass concentration values exceeded the 24-h and annual guideline values set by the World Health Organization in 56 and 85% of the cases, respectively. The carbon-dioxide concentration was high in the majority of the classrooms (the average value exceeded 1000 ppm), indicating the inappropriate ventilation. Among the parameters affecting the comfort of the students, the relative humidity was very low in many cases. The radon level was around or below 100 Bq m-3, except for some locations in Hungary, Poland, and Slovenia.
