The article that is proposed to you retraces some elements of Karlfried Graf Dürckheim's career, from his lived experience to a theorization that he developed as soon as he returned to Europe in 1947. It was on the basis of experience that he built the milestones of the initiatory therapy that he theorized as soon as he returned to Germany at the end of the Second World War. A professor of psychology and philosophy, he knew how to combine his academic knowledge with spirituality oriented by Rhenish mysticism. Similarly, he was inspired by the practice of Zen that he discovered during his years spent in Japan. His attention to sensoriality and his striking memories would guide his therapeutic work, his theorization and the support of the many people who came to work with him and his collaborators throughout his life. It is a journey in the footsteps of the "Sage of the Black Forest" that we are invited here, an initiatory journey as close as possible to what constituted the foundations of his thought and practice. Today, many disciples continue his work of transmission and accompany people on the way, in Rütte in Germany or in other countries in Europe, and especially one of his closest disciples, Jacques Castermane, in France in the Drôme.
In his role as psychiatrist, the head of a day hospital, in Mulhouse, a French city, Dr Fattah’s team included M. Senadla as Peer Help mediator. The author’s personal experience working with this former patient as a recovery agent gives an overview of the peer health mediator’s role, its benefits, and how it can be used to improve mental health care. This new management paradigm involving a stabilized patient could promote patient and family empowerment, leading to better social and professional rehabilitation of people suffering from psychiatric disorders.
Le trouble déficitaire de l’attention avec ou sans hyperactivité (TDAH) est souvent associé à des problèmes de sommeil aggravant le dysfonctionnement cognitivo-comportemental. Nous analysons le sommeil et la cognition des enfants TDAH recrutés dans notre unité de sommeil. Cette étude rétrospective analyse le sommeil de 40 enfants TDAH, non apnéiques, non traités, de 6–11 ans (moyenne 8,2), soit 36 garçons et 4 filles adressés à notre unité pour une nuit de polysomnographie (PSG). Le TDAH a été diagnostiqué après une évaluation clinique et un test neuropsychologique (Batterie Kitap). Les plaintes étaient : ronflements (75 %), fatigue (67 %), insomnie (45 %), sommeil agité (37 %), épisodes évocateurs d’une parasomnie (35 %). La PSG montrait une latence moyenne d’endormissement de 26 m ± 0,7 ; une efficience moyenne de 88 % ± 4, un temps total de sommeil de 541 m ± 27, des pourcentages moyens de sommeil profond et de Rem de 22 % et de 18 %. Soixante pour cent d’enfants remplissaient les critères diagnostiques d’un TDAH-forme mixte et 40 % de TDAH-forme inattentive. Quarante-cinq pour cent avaient un profil inattentif sévère : 5–6/6 subtests à la batterie Kitap étaient perturbés. Quarante-cinq pour cent ont été traités par méthylphénidate au terme du bilan. Cinquante pour cent des enfants avec un TDAH – forme inattentive et 29 % avec TDAH – forme-mixte ont initialement consulté pour les difficultés de sommeil. Les problèmes de sommeil pourraient toucher plus de garçons que de filles TDAH. Les troubles du sommeil et la fatigue peuvent être parmi les premières plaintes des enfants TDAH. Leur diagnostic précoce est un enjeu important.
Objectives: This study aimed to monitor respiratory tract outbreaks in nursing homes (NH) by applying standardized case definition criteria, pathogen identification and estimated mortality impact. Patients and methods: This longitudinal, observational study described NH outbreaks with temperature (T degrees), upper respiratory sign, lower respiratory sign (LRS), general status deterioration, general signs, and mortality. Viral examinations allowed for analysis of developing outbreaks based on positive, negative, or no tests (Flu+/Flu-/NT-Flu). Results: Forty-six influenza identification episodes (Flu+, 1067 patients), 14 Flu- (409 patients), and 18 NT-Flu (381 patients) were analyzed. Viral examinations were conducted mainly among residents with T degrees (84.8% [302/356]). A specific temperature pattern was observed in Flu+ outbreaks: 35.1% of infected residents with T degrees without LRS, 15.6% in Flu- episodes, and 17.1% vs. 29.1% in LRS without T degrees. A median temperature (MT) of >= 38.3 degrees C was observed in Flu+ outbreaks. MT analysis of the 18 NT-Flu episodes identified five outbreaks with high temperatures (MT >= 38.2 degrees C) and high mortality. Conversely, the 13 NT-Flu outbreaks with lower MT (<38.0 degrees C) were associated with lower total mortality. Similar clinical pictures led to closely comparable all-cause mortality impacts, particularly in Flu+, Flu-, and NT-Flu with MT of >= 38.2 degrees C. Conclusions: Validated sign/symptom monitoring highlighted some specificities of respiratory NH outbreaks and could be a complementary approach, taking into account common and atypical clinical pictures, assessing mortality and initiating virological investigations and infection control measures. Les effets anticholinergiques sont bien connus des prescripteurs, notamment en psychiatrie, à la fois comme des stratégies thérapeutiques pour les syndromes extrapyramidaux, mais également comme une source d’effets indésirables. Nous proposons ici une revue narrative de la littérature décrivant successivement : (i) la pharmacologie cholinergique et les effets anticholinergiques ; (ii) l’importance des effets anticholinergiques en psychiatrie ; (iii) les échelles anticholinergiques existantes avec la pertinence et les limites de leur usage en psychiatrie et ; enfin, (iv) une mise à jour de l’échelle d’imprégnation anticholinergique, développée pour la psychiatrie française. Les effets anticholinergiques peuvent se manifester autant au niveau périphérique, qu’au niveau central. Schématiquement, les effets anticholinergiques/antimuscariniques sont aisément attribués parmi les psychotropes : à certains antipsychotiques, aux traitements correcteurs qualifiés d’anticholinergiques, et aux antidépresseurs tricycliques. Afin d’évaluer la charge anticholinergique, des outils appelés des échelles d’évaluation des effets anticholinergiques ont été développés au cours des deux dernières décennies (13 échelles répertoriées en 2021), mais trois seulement bénéficient d’une utilisation réellement importante : l’Anticholinergic Drug Scale (ADS), l’Anticholinergic Risk Scale (ARS) et l’Anticholinergic Cognitive Burden scale (ACB). Ces échelles présentent plusieurs limites : une faible concordance entre elles, une difficulté à prédire les effets centraux en raison de classements ne différenciant pas, ou mal, la capacité des traitements à passer à la barrière hématoencéphalique, enfin, un manque de pondération de la puissance des effets anticholinergiques par la posologie. Nous proposons, à l’issue de cette revue, une mise à jour de l’échelle d’imprégnation anticholinergique développée spécifiquement pour la psychiatrie française.
