This study aimed to assess the effectiveness of online emotional competencies (EC) training, and more specifically the relationships between adherence, confidence, and alliance and the effects of mental training on EC. A longitudinal four-wave measurement design (pre-intervention, post-knowledge development, post-abilities development, and post-disposition phase) was used in the present study. An experimental group (n = 20) received a specific EC training intervention, whereas the control group (n = 20) did not receive this intervention program. These two groups completed self-report questionnaires assessing emotional regulation and EC across four measurement times. Participants of the intervention group also completed a questionnaire measuring the adherence, confidence, alliance between practitioner and athlete. Results of repeated measure ANOVAs showed that the level of EC increased significantly in the intervention group but not in the control group. Moreover, correlation analyses highlighted significant relationships between the scores of adherence, confidence, alliance between the practitioner and the tennis player, and the scores difference of EC between before and after intervention. These outcomes suggest the possibility of improving EC with an online intervention. The online device did not affect the quality of the alliance between the practitioner and the intervention participant. Furthermore, the results showed that a high level of alliance, confidence, and adherence was associated with an improvement in emotional intelligence. This online program would serve psychologists and mental trainers to use an intervention to develop EC adapted to the constraints of the localisation of athletes.
Dupilumab est un anticorps monoclonal bloquant la sous-unité alpha du récepteur à l'interleukine 4. Disponible en France depuis novembre 2020 dans l'indication asthme sévère, il a démontré son efficacité à améliorer le contrôle de l'asthme, réduire le recours aux corticostéroïdes oraux (CSO) dans des essais de phase III. La France a été le premier pays à traiter des patients en vraie vie, grâce à une autorisation temporaire d'utilisation ouverte de novembre 2017 à mars 2018, au total 64 patients ont été traités, et leur suivi à 12 mois a montré un bénéfice clinique et fonctionnel superposable à celui des essais randomisés, et une hyperéosinophilie plus fréquente (25%) [1]. Objectifs: évaluer le maintien du traitement, l'efficacité à long terme et la tolérance du dupilumab en vraie vie dans la population Dupi-France à 5 ans de l'initiation thérapeutique. Nous avons mené une étude rétrospective multicentrique (13 centres) en vie réelle pour recueillir la persistance de prescription de dupilumab, et les données annuelles cliniques, biologiques et fonctionnelles des patients de la cohorte initiale DupiFrance encore traités à 12 mois de l'initiation ou perdus de vue (n = 59). Les données de suivi sont disponibles pour 37 patients (68%). 65% (24/37) sont toujours traités par dupilumab à 5 ans de l'initiation. 65% l'étaient à 4 ans, 73% à 3 ans et 81% à 2 ans. 5 patients ont été perdus de vue. Les motifs d'arrêt du dupilumab étaient un contrôle de l'asthme insuffisant (4/8 soit 50%), une réaction locale au site d'injection (1/8, 12,5%), une hyperéosinophilie asymptomatique à 3000/mm3 (1/8 12,5%) et une préférence des patients (2/8, 25%). Parmi les patients encore traités à 5 ans, le taux annualisé d'exacerbation d'asthme était 1,82,55% n'avaient présenté aucune exacerbation. Le score ACT moyen était de 18/25,12,5% étaient encore traités par CSO au long cours (dose moyenne 1,7 mg/j [0–20]). Parmi les 11 patients encore corticodépendants à 1 an de Dupilumab, 8 patients ont été sevrés de leur corticothérapie orale quotidienne dans les 4 années qui ont suivi. À 5 ans, l'éosinophilie moyenne était de 488/mm3 [0–1250]. 54% des patients avaient une éosinophilie > 500/mm3, 0% > 1500/mm3. Aucune symptomatologie n'a été rapportée à cette hyperéosinophilie. Nous présentons ici les premières données de suivi à long terme d'une population d'asthmatiques sévères traités par dupilumab en vraie vie, montrant une efficacité durable, un faible recours au switch et une bonne tolérance. L'éosinophilie persiste de façon prolongée sans s'accompagner de complication d'organe. Ces résultats encouragent l'utilisation du dupilumab et nécessitent d'être confirmés dans une population plus large.

ObjectiveAutoreactive B cells are responsible for antineutrophil cytoplasmic antibody (ANCA) production in ANCA‐associated vasculitis (AAV). Rituximab (RTX) depletes circulating B cells, including autoreactive B cells. We aimed to evaluate changes and associations with relapse of the circulating autoreactive B cell pool following therapeutic B cell depletion in AAV.MethodsSequential flow cytometry was performed on 148 samples of peripheral blood mononuclear cells from 23 patients with proteinase 3 (PR3)–ANCA–positive AAV who were treated with RTX for remission induction and monitored after stopping therapy during long‐term follow‐up in a prospective clinical trial. PR3 was used as a ligand to target autoreactive PR3‐specific (PR3+) B cells. B cell recurrence was considered as the first blood sample with ≥10 B cells/μl after RTX treatment.ResultsAt B cell recurrence, PR3+ B cell frequency among B cells was higher than baseline (P < 0.01). Within both PR3+ and total B cells, frequencies of transitional and naive subsets were higher at B cell recurrence than at baseline, while memory subsets were lower (P < 0.001 for all comparisons). At B cell recurrence, frequencies of B cells and subsets did not differ between patients who experienced relapse and patients who remained in remission. In contrast, the plasmablast frequency within the PR3+ B cell pool was higher in patients who experienced relapse and associated with a shorter time to relapse. Frequencies of PR3+ plasmablasts higher than baseline were more likely to be found in patients who experienced relapse within the following 12 months compared to those in sustained remission (P < 0.05).ConclusionThe composition of the autoreactive B cell pool varies significantly following RTX treatment in AAV, and early plasmablast enrichment within the autoreactive pool is associated with future relapses.
