The Hospital El Salvador is a major hospital in San Salvador, El Salvador, and is planned to be the largest hospital in Latin America. The first phase was constructed between March and June 2020 as a conversion of the International Fair and Convention Center [es] (Centro Internacional de Ferias y Convenciones, CIFCO) and forms part of El Salvador's response to the COVID-19 pandemic, exclusively receiving COVID-19 patients. Originally intended to be temporary, it was announced in June 2020 that the hospital conversion would be made permanent. In its first phase, the hospital has 400 available beds, a number which was expected to increase to 2,000 total beds upon completion of phase 3 of construction. However, phase 3 was not opened as a hospital and instead served as a vaccination facility.
Steatotic liver disease (SLD), formerly named fatty liver disease, has a prevalence estimated at 30-38% in adults. Detection of SLD is important, since prompt initiation of treatment can stop disease progression, lead to a reduction in adverse outcomes, and reduce the economic burden associated with the disease. We report the development of a novel Deep Learning (DL) method for the prediction of SLD, which consists of transforming the input variables from tabular data into images, with the goal of using the pattern recognition power of DL models to reach the best prediction performance. The dataset used in this study includes registries from 2,999 patients. The data of each patient, originally represented as a vector, is converted into an image replicating each variable in rows and columns. Our DL models reach better results compared to those of traditional ML models at various levels of sensitivity and specificity. A sensitivity of 0.9497, a specificity of 0.6417, and an AUCROC of 0.8662 were reached with one DL model. We also achieved significantly better results relative to those obtained with the Hepatic Steatosis Index (HSI). Our DL models reach higher AUCROC values compared to those of the traditional ML models, and also with respect to those obtained with HSI.
La ideación suicida es un problema relevante en materia de salud pública a nivel mundial. Sin embargo, para medir este espectro son escasos los instrumentos que pueden utilizarse, sobre todo en el contexto salvadoreño. Por ello, el propósito de esta investigación fue comprobar las propiedades psicométricas de validez y confiabilidad de la Escala Paykel de Suicidio en una muestra salvadoreña. El estudio fue de tipo instrumental con un diseño transversal. Participaron 213 adultos salvadoreños a través de un muestreo de tipo bola de nieve, con un promedio de edad de 27 años. Los hallazgos revelaron por medio del Análisis Factorial Confirmatorio que la estructura unidimensional de la escala cuenta con un ajuste perfecto para medir la conducta suicida. Asimismo, los índices de confiabilidad por medio del alfa de Cronbach y omega de McDonald obtenidos fueron superiores a .80. Se concluyó que la Escala Paykel de Suicidio es un instrumento válido y confiable para ser utilizado en el contexto salvadoreño con fines investigativos y como una herramienta para los profesionales interesados en abordar las conductas suicidas.
ObjectiveTo report pharmacokinetics (PK), immunogenicity, clinical effect, and safety of intravenous (IV) golimumab in children with active polyarticular-course juvenile idiopathic arthritis (pcJIA) who participated in A Study to Evaluate the Pharmacokinetics, Efficacy and Safety of Intravenous Golimumab in Pediatric Participants With Active Polyarticular Course Juvenile Idiopathic Arthritis Despite Methotrexate Therapy (GO-VIVA)’s open-label, long-term extension (LTE) through week 252.MethodsGO-VIVA participants who continued IV golimumab (80 mg/m2every 8 weeks) after week 52 were included. PK and safety were assessed through week 244 (last dose) and week 252, respectively, and clinical response through week 116. Clinical outcomes included JIA–American College of Rheumatology (ACR) responses and clinical Juvenile Arthritis Disease Activity Score in 10 joints (cJADAS10). Binary outcomes used nonresponder imputation, and other descriptive analyses used observed data.ResultsOf 112/127 (88.2%) participants entering the LTE, 69 completed the week 252 visit. Median steady-state trough golimumab concentrations were generally maintained from week 52 through week 244 (range 0.3-0.6 μg/mL). Antigolimumab antibody rates were consistent through week 52 (39.2% [49/125]) and week 244 (44.8% [56/125]). Week 52 JIA-ACR 30/50/70/90 response rates (75.6% [96/127], 74% [94/127], 65.4% [83/127], and 48.8% [62/127], respectively) were generally maintained through week 116 (72.4% [92/127], 71.7% [91/127], 63.8% [81/127], and 50.4% [64/127], respectively), when the median cJADAS10 was 1.6 and 56.7% (72/127) of participants achieved cJADAS10 ≤ 5 (minimal disease activity). Rates (per 100 patient-years) of serious adverse events and serious infections through week 252 were 7.7 and 3.9, respectively.ConclusionGO-VIVA LTE participants experienced adequate PK exposure and stable safety and immunogenicity. The majority of participants experienced no more than minimal residual disease activity. Data suggest IV golimumab treatment provided durable clinical response through week 116, with an acceptable risk-benefit profile.
