Mandibular angle fractures account for approximately one-third of mandibular fractures and pose biomechanical challenges in surgical management. Although conventional miniplates are widely used, concerns regarding their strength in certain fracture patterns have led to evaluation of alternative fixation designs. This randomised clinical study compares single Y-shaped plate fixation with conventional miniplate fixation in mandibular angle fractures. Twenty patients were randomly allocated into two groups (2.0-mm Y-shaped plate or a 2.0-mm four-hole conventional miniplate with gap). Clinical and radiological outcomes were evaluated preoperatively and postoperatively using cone beam computed tomography. Parameters assessed included occlusion, maximum mouth opening, pain, neurosensory status, interfragmentary gap, and lower border distraction. Both fixation systems showed comparable outcomes in occlusion, mouth opening, and postoperative complications. The Y-plate group demonstrated higher bite force and more favourable radiological findings; however, these differences were not statistically significant. Both systems were clinically effective for mandibular angle fractures. Although the Y-shaped plate showed trends towards improved functional and radiological outcomes, no significant superiority over conventional miniplates was observed. It may, however, offer better biomechanical stability in complex fractures or where customised fixation is required.
Surgical removal of impacted mandibular third molars is a common procedure often associated with postoperative pain, swelling, and trismus. Corticosteroids such as dexamethasone are frequently used to mitigate these sequelae. This study aims to compare the efficacy of intra-masseteric (IM) and submucosal (SM) routes of dexamethasone administration in reducing postoperative discomfort. A prospective split-mouth randomized controlled trial was conducted on 15 patients with bilateral impacted mandibular third molars. Group A received 8 mg dexamethasone IM, and Group B received it SM, both 1 h preoperatively. Pain (VAS), swelling (thread method), and mouth opening (interincisal distance) were assessed on days 1, 3, 7, and 14 postoperatively. Both groups showed significant reduction in pain, swelling, and trismus over time. Although pain scores did not significantly differ between groups at any time point, Group B (SM) demonstrated significantly better mouth opening on days 1, 3, and 7 and significantly lower swelling scores on days 3, 7, and 14 compared to Group A (IM). No major complications were observed. Submucosal dexamethasone administration offers a simpler, effective, and minimally invasive alternative to the intra-masseteric route for managing postoperative sequelae in third molar surgery.
This randomized prospective clinical trial compared piezoelectric surgery (PS) and conventional rotary burs for lateral sinus floor elevation with simultaneous implant placement in patients with limited residual bone height (4–5 mm), focusing on intraoperative and postoperative outcomes. Twenty patients aged 30–60 years with 4–5 mm residual bone height in the posterior maxilla were randomized into two groups: Group I (PS) and Group II (rotary burs). Both underwent sinus lift with simultaneous implant placement and grafting. Intraoperative parameters included membrane perforation, surgical visibility, and duration. The success of dental implants is influenced by the timing of prosthetic loading. Therefore, it is of interest to compare the clinical and radiological outcomes of immediate loading (IL) versus conventional loading (CL) of dental implants supporting fixed prostheses. Hence, a total of 30 patients were randomly assigned to either IL or CL groups. Clinical parameters such as implant stability (measured by Periotest), peri-implant soft tissue health and radiographic marginal bone loss (MBL) were evaluated over a 12-month period. Results indicated that both groups demonstrated high implant survival rates (96.6% IL vs 100% CL), with no statistically significant differences in MBL at 3, 6 and 12 months (p > 0.05). However, IL provided faster functional rehabilitation and patient satisfaction. Thus, we show the clinical viability of immediate loading protocols in appropriately selected cases, without compromising implant stability or bone levels.
Aims and Objective: To Evaluate & Compare the Colour change & Surface Microhardness of Nanoenhanced composite resin after using whitening mouthwashes. Study Design And Settings: Forty disc-shaped specimens were prepared using two A2-shade nano-enhanced composites (FGM Vittra APS and Coltene Brilliant EverGlow). Specimens were stained in coffee to simulate extrinsic discoloration, followed by immersion in whitening mouthwashes to assess effects on color stability and surface microhardness. Materials and Methods: Specimens were divided into two groups (n=20 each) based on composite type. After 15 days of coffee staining, baseline color (CIE Lab*) and microhardness (Vickers test) were recorded. Each subgroup group was then immersed in Perfora Whitening or Activated Charcoal mouthwash for 24 hours. Post-immersion, color change (ΔE) and microhardness were reassessed. Statistical analysis used: Data were analyzed using SPSS. Normality was tested with Shapiro-Wilk. One-way ANOVA with Bonferroni and paired t-test were used; significance at p<0.05. Results: All the samples showed colour & surface microhardness to varying degrees. Whitening mouthwash caused greater discoloration; FGM vittra APS showed higher hardness than Coltene brilliant everglow with charcoal mouthwash preserving color stability best. Conclusions: The utilization of whitening mouthwash markedly diminished the color stability, especially in the context of FGM Vittra APS. The application of charcoal mouthwash resulted in reduced discoloration without adversely affecting hardness.
