Trotz ihrer Vorteile bei der Behandlung schwerer GERD-Symptome ist eine perkutane endoskopische Gastojejunostomie (PEG-J) mit zahlreichen Komplikationen verbunden und stellt sowohl für Kinder – von denen die meisten neurologische Beeinträchtigungen aufweisen – als auch für ihre Familien eine erhebliche Belastung dar. Ziel unserer Studie war es, die klinischen Erfahrungen mit der PEG‑J aus ärztlicher Sicht und aus Perspektive der Angehörigen darzustellen, die Effektivität sowie typische Komplikationen zu beschreiben und die Ergebnisse im Kontext der aktuellen Literatur zu diskutieren. Es wurden eine retrospektive Analyse von Patientendaten aus dem Zeitraum 2015 bis Ende 2024 sowie eine standardisierte Befragung der Familien durchgeführt. Alle Daten wurden anonymisiert erfasst und deskriptiv ausgewertet. Zwischen 2015 und 2024 erhielten 16 Patienten (10 Jungen und 6 Mädchen) aus unterschiedlichen Gründen in unserer Klinik eine PEG‑J. Renal replacement therapy (RRT) is frequently used in critically ill patients with acute kidney injury (AKI). Here, we provide guidelines for the management of RRT in critically ill patients on the intensive care unit (ICU). We convened a systemic literature research and a Delphi process with a bi-national multidisciplinary consensus panel including 22 clinicians of 12 different German-speaking societies (Germany and Austria) with expertise in RRT. This structured guideline process was the basis for the evidence-based statements and recommendations. We identified seven clinical areas needing guidance: (1) start, (2) modality (diffusion and convection), (3) continuous/ intermittent, (4) anticoagulation, (5) dose (6) pharmacotherapy, (7) stopping criteria. The consensus produced 73 statements and recommendations regarding key clinical areas, the most important 47 statements and recommendations are summarized in this overview. This evidence-based bi-national guideline should provide physicians with guidance for delivering best practice to critically ill patients with a dialysis-dependent AKI.
Das Milchpropfsyndrom („milk curd syndrome“, MCS) oder Laktobezoar (LB) ist eine seltene gastrointestinale Erkrankung, die v. a. Frühgeborene mit sehr niedrigem oder extrem niedrigem Geburtsgewicht betrifft. Typischerweise tritt sie im Zusammenhang mit Formulanahrung oder angereicherter Muttermilch auf. Während die meisten Fälle von LB den Magen betreffen, ist eine intestinale Lokalisation – insbesondere bei Reifgeborenen – extrem selten. Ein 4,5 Wochen alter reifer männlicher Säugling wurde mit einer irreponiblen rechtsseitigen Leistenhernie und Ileuszeichen vorgestellt. Die laparoskopische Reposition zeigte ein hämorrhagisch verändertes, jedoch vitales Darmsegment. Zwei Monate später wurde der Patient erneut mit Symptomen eines mechanischen Darmverschlusses aufgenommen. Die explorative Laparotomie ergab eine intraluminale Raumforderung im terminalen Ileum. Eine segmentale Resektion wurde durchgeführt, und die histologische Untersuchung bestätigte die Diagnose eines ilealen Laktobezoars. Die Entstehung von MCS wird auf multifaktorielle Ursachen zurückgeführt, darunter eine unreife gastrointestinale Funktion, Dehydratation sowie unterschiedliche Ernährungsfaktoren. Während gastrische Laktobezoare häufiger beschrieben sind, ist ein ilealer Befall nur selten berichtet – insbesondere bei Reifgeborenen. Dies unterstreicht die Bedeutung, bei klinischen Zeichen einer intestinalen Obstruktion auch einen ilealen Laktobezoar in die Differenzialdiagnose einzubeziehen – selbst außerhalb der typischen Risikogruppe. Der ileale Laktobezoar ist eine seltene, aber potenziell lebensbedrohliche Ursache für einen neonatalen Darmverschluss. Eine frühzeitige Diagnosestellung und chirurgische Therapie sind entscheidend für ein günstiges Outcome.
Background:Increased life expectancy is associated with increasing multimorbidity and polypharmacy, leading to a heightened risk of drug-drug interactions and adverse events, especially when multiple health care providers are involved. To address the urgent need for safer medication management in this population, tools such as medication plans (MP), electronic prescriptions (e-prescriptions), and clinical decision support systems (CDSS) offer valuable support. These instruments have the potential to enhance medication safety by providing physicians and pharmacists with a comprehensive overview of a patient's overall medication regimen and by assisting health care professionals in making informed prescribing decisions. Objective:This study aims to improve medication therapy safety by combining e-prescriptions, the use of claims data, MPs, CDSS, and interprofessional communication. To comprehensively evaluate this complex intervention, a holistic multiphase study will be conducted, examining (1) the effectiveness of the intervention and (2) health-economic and (3) implementation-related aspects. Methods:A multiphase study design is used. In the first phase, the intervention is implemented in selected outpatient practices (n=10) and pharmacies (n=10) in 2 regions in Germany as part of a cluster-randomized controlled trial to assess process-related outcomes. The primary outcome is the congruence between the MP and claims data. In phase 2, the intervention is scaled up in 3 regions and evaluated in a quasi-experimental study. The required sample size for the intervention group is 3528 patients, with a synthetic control group matched from existing claims data. The primary outcome is a combined end point of all-cause mortality and hospitalization within 3 months of an index prescription. Quantitative methods (descriptive, regression-based methods using claims data, calculation of the incremental cost-effectiveness ratio, and survey-based analyses of implementation-related aspects) and qualitative methods (interviews and focus groups to capture experiences of health care professionals and patients) are used. Results:In phase 1, a total of 187 patients were recruited (74 in the intervention group and 113 in the control group) by June 2025. Phase 2 is currently ongoing, with data collection continuing through December 31, 2025. Final analyses are planned by March 2027. Conclusions:Medication safety in polypharmacy remains a critical challenge in Germany. This study provides multiperspective evidence supporting the nationwide implementation of the eRIKA (e-prescription as an element of interprofessional care pathways for continuous medication therapy management [eRezept als Element interprofessioneller Versorgungspfade für kontinuierliche AMTS]) intervention.
